[Isolated abscess of the thoracic and abdominal wall: an exceptional form of tuberculosis].

Isolated thoracic parietal involvement is a very rare form of tuberculosis and multifocal localization is exceptional. It often poses a diagnostic problem with parietal tumours requiring recourse to surgical biopsy. We report a case of tuberculous abscess localized to the anterior and superior part of the chest wall with a second abdominal localization but without any pulmonary involvement. The patient was a woman of 56 years presenting with a one-year history of a swelling to the right of the sternum accompanied after several months by a second swelling to the right side of the abdomen. The r

[Tuberculous pneumothorax. Retrospective study of 23 cases in Tunisia].

The tubercular pneumothorax is a rare and severe form of tuberculosis which persists in Tunisia. We reviewed retrospectively our experience at the Pneumology-Allergology Department of the Sousse Hospital with 23 cases of tubercular pneumothorax observed between 1985 and 2003. The suggestive symptom, pneumothorax, always occurred within a context poor general health and fever. The chest roentgengraph showed pneumothorax (n=8) or hydropneumothorax (n=15), and pulmonary lesions associated in 82.5% of patients. Bacteriological proof was obtained in 21 cases; in the two other patients, histological

[Rare case of primary sinonasal tuberculosis].

INTRODUCTION: Tuberculosis, considering entry of infection, most frequent concerns the lungs. Rarely we observed extrapulmonary cases of tuberculosis within the bones, joints, urogenital organs and lymph nodes as well as within the head and neck region--larynx, middle ear, pharynx, tonsils, tongue, parotid glands and central nervous system. Sinonasal tuberculosis, either primary or secondary to pulmonary infection, is very rare, but should be considered in the differential diagnosis of sinonasal granulomas. The diagnosis of the head and neck tuberculosis is based on the medical patientis exami

[Case of cavitary coccidioidomycosis with fungus balls in the apices of both lungs].

Pulmonary cavitary coccidioidomycosis with a fungus ball was observed in a immunocompetent case. A 32-year-old Japanese man visited Arizona to play golf. After 1 month he consulted a local hospital complaining of a prolonged cough and hematopysis. The laboratory examination revealed eosinophillia and chest radiograph showed 2 cavitary lesions, surrounded by small nodules in the apices of both lungs. Pulmonary tuberculosis was suspected and treated with 4 antituberculosis drugs for 3 months. However, the cavities enlarged and he was admitted to our hospital for further examination and treatment

Tuberculosis-like lesions arising from the use of Freund's complete adjuvant in an owl monkey (Aotus sp.).

An apparently normal, non-tuberculin-reacting, splenectomized owl monkey presented tuberculosis-like lesions of the lung at necropsy. Histological and bacteriological examination failed to demonstrate the presence of acid-fast organisms. Retrospective inquiry showed the animal had been inoculated using complete Freund's adjuvant during a malaria vaccine trial. Lesions observed were compatible with lipid embolism of the adjuvant in the lungs.

[Isolated tuberculosis of the sternum. Two cases].

INTRODUCTION: Among the extra-pulmonary localizations, isolated tuberculosis of the sternum is rare and difficult to diagnose. OBSERVATIONS: A 30 year-old man presented with tumefaction of the sternum that had developed over the past two months and a 52 year-old woman had developed a painful tumefaction of the sternum over the past 4 months. In both cases, the histopathological examination of a biopsy confirmed the diagnosis of tuberculosis. Anti-bacterial therapy permitted the favourable progression of the disease. DISCUSSION: Such diagnosis is rare. Contamination can occur through contigui

[Various approaches to tuberculosis diagnosis in patients with hemoblastosis].

AIM: To characterize diagnosis of tuberculosis in hemoblastosis patients. MATERIAL AND METHODS: Diagnosis of active tuberculosis in 2.6% from 2123 hemoblastosis patients admitted to Hematological Research Center in 1990-1999 shows that such patients can be referred to high risk group in relation to tuberculosis infection. Methods and terms of tuberculosis diagnosis in hemoblastosis patients are analysed. RESULTS: Bacteriological and histological tests were positive in 27.8% examinees with hemoblastosis. In extrapulmonary tuberculosis location, histological diagnosis was positive in 60%. Espe

[Tuberculosis in hemodialysis patients in Tunisia].

BACKGROUND: Because of the immunity depression, patients with chronic renal failure undergoing haemodialysis are at increased risk for developing infections, Mycobacterium tuberculosis (MTb) in particular. OBJECTIVE: To evaluate, in a prospective longitudinal study over 36 months period, the frequency of Tb among these patients and underline the diagnostic difficulties. PATIENTS AND METHODS: Sixty dialysis patients were interested. The Tb assessment comprised: a questionnaire, a meticulous clinical examination, a chest X-ray, a tuberculin skin testing, as well as MT bacilli screening in biol

[Diagnosis and treatment of tuberculous lymphadenitis of the neck].

The incidence of extrapulmonary tuberculosis is rising. The patients are predominantly immigrants and HIV-infected persons. Tuberculous lymphadenitis of the neck is the most common presentation, in the Netherlands about 200 patients a year. Fine needle aspiration with auramine/Ziehl-Neelsen stain investigation, culture and cytological examination is the diagnostic procedure of choice. If this fails to be conclusive excision biopsy is the next step. If the diagnosis is suspected on clinical, epidemiological, laboratory and bacteriological (presence of acid-fast bacilli) or cytological/histologi

Colonofiberscopic diagnosis of intestinal tuberculosis.

The purpose of this study was to assess the predictive value of colonofiberscopy in the diagnosis of intestinal tuberculosis. The records of more than 14,000 colonofiberscopic examinations performed at the National Taiwan University Hospital from 1971 to 1995 were studied retrospectively. Colonofiberscopic diagnosis of intestinal tuberculosis required at least one of the following: circumferential ulcers, a single transverse axis ulcer with uneven base and elevated or nodular margin surrounded by nodular and hyperemic mucosa, or pseudopolyps. Sixty cases were diagnosed as intestinal tuberculos

Tuberculous pericarditis.

A patient with a 5-month history of pericardial effusion and unsuccessful steroid treatment was found at pericardial exploration to have thickened pericardium adhered to the heart. After anterior pericardiectomy, histologic examination revealed severe granulomatous pericarditis resulting from infection with Mycobacterium tuberculosis. Despite the pericardiectomy and antituberculous therapy, the patient continued to have symptoms, including bilateral pleural effusions, 4 days after discharge from the hospital. After a second exploration (after echocardiography revealed decreased left ventricula

[Diagnostic problems in pediatric tuberculosis: a description of two cases].

This paper describes two cases of tuberculosis (tbc) in children in whom tbc was misdiagnosed and who were initially treated with antibiotics (and corticosteroids) as for non-specific lower respiratory tract infections. Chest radiograms, good response to antituberculosis drugs (in both children), bacteriologic and histologic examinations and a history of family contact with infectious tbc (in case I) confirmed the diagnosis of tbc. We suggest that the most important factor in rapidly diagnosing tuberculosis is a high index of suspicion in children with respiratory tract infections.
