Spatial distribution of HD-EMG improves identification of task and force in patients with incomplete spinal cord injury.

BACKGROUND: Recent studies show that spatial distribution of High Density surface EMG maps (HD-EMG) improves the identification of tasks and their corresponding contraction levels. However, in patients with incomplete spinal cord injury (iSCI), some nerves that control muscles are damaged, leaving some muscle parts without an innervation. Therefore, HD-EMG maps in patients with iSCI are affected by the injury and they can be different for every patient. The objective of this study is to investigate the spatial distribution of intensity in HD-EMG recordings to distinguish co-activation patterns

The role of elective amputation in patients with traumatic brachial plexus injury.

BACKGROUND AND AIM: Despite undergoing complex brachial plexus, surgical reconstructions, and rehabilitation, some patients request an elective amputation. This study evaluates the role of elective amputation after brachial plexus injury. METHODS: A retrospective chart review was performed for all the 2140 patients with brachial plexus injuries treated with elective amputation between 1999 and 2012 at a single institution. Analysis was conducted on the potential predisposing factors for amputation, amputation level, and postamputation complications. Patients were evaluated using pre- and post

Complications of hinged external fixation compared with cross-pinning of the elbow for acute and subacute instability.

BACKGROUND: Elbows that are unstable after injury or reconstructive surgery often are stabilized using external fixation or cross-pinning of the joint supplemented by cast immobilization. The superiority of one approach or the other remains a matter of debate. QUESTIONS/PURPOSES: We compared patients treated with external fixation or cross-pinning in terms of (1) adverse events, (2) Broberg and Morrey scores, and (3) ROM. METHODS: Between 1998 and 2010, 19 patients (19 elbows) had hinged external fixation and 10 patients (11 elbows) cross-pinning and casting for subacute or acute posttraumat

A systematic review of outcomes reporting for brachial plexus reconstruction.

PURPOSE: To better understand the manner in which outcomes are reported after brachial plexus reconstruction, we conducted a systematic review of the scientific literature. METHODS: We included English-language articles describing treatment of brachial plexus injuries to restore motor function of the shoulder, elbow, forearm, and/or wrist with nerve repair, nerve graft, and/or nerve transfer. We recorded the anatomical location of injury, the treatment used, and the manner in which motor function, active and passive range of motion, pain, quality of life, function or disability, patient satis

The anatomical basis for anterior interosseous nerve palsy secondary to supracondylar humerus fractures in children.

INTRODUCTION: Various studies have found that 6.6 to 31% of supracondylar elbow fractures in children have nerve-related complications. One-third of these are cases of anterior interosseous nerve (AIN) palsy that usually result in a deficit of active thumb and index flexion. The goal of this cadaver study was to describe the course of the AIN to achieve a better understanding of how it may get injured. MATERIALS AND METHODS: On 35 cadaver specimens, the median nerve and its collateral branches destined to muscles were dissected at the elbow and forearm levels. The distance at which the variou

Clinical outcomes after chronic distal biceps reconstruction with allografts.

BACKGROUND: Chronic ruptures of the distal biceps are often complicated by tendon retraction and fibrosis, precluding primary repair. Reconstruction with allograft augmentation has been proposed as an alternative for cases not amenable to primary repair. PURPOSE: To investigate the clinical outcomes of late distal biceps reconstruction using allograft tissue. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A total of 20 patients who underwent distal biceps reconstruction with allograft tissue between May 2007 and May 2012 were identified. Charts were retrospectively reviewed for p

The variable communicating branches between musculocutaneous and median nerves: a morphological study with clinical implications.

BACKGROUND: The course of the brachial plexus, its relations with surrounding structures and unique primary and secondary divisions result in its wide range of anatomical variations. Most of these variations were detected during anatomical dissections and studies. It has been found that 53% of studied brachial plexuses contained variations. The communication between musculocutaneous and median nerves is the most common variation of infraclavicular part of brachial plexus. METHODS: During gross anatomical dissections of peripheral nerves, we observed neuronatomical variations in upper limbs of

Collagen conduit versus microsurgical neurorrhaphy: 2-year follow-up of a prospective, blinded clinical and electrophysiological multicenter randomized, controlled trial.

PURPOSE: To compare repair of acute lacerations of mixed sensory-motor nerves in humans using a collagen tube versus conventional repair. METHODS: In a prospective randomized trial, we repaired the ulnar or the median nerve with a collagen nerve conduit or with conventional microsurgical techniques. We enrolled 43 patients with 44 nerve lacerations. We performed electrophysiological tests and hand function using a standardized clinical evaluation instrument, the Rosen scoring system, after 12 and 24 months. RESULTS: Operation time using the collagen conduit was significantly shorter than for

Simple clinical test to detect deltoid muscle dysfunction causing weakness of abduction--"akimbo" test.

INTRODUCTION: Full range of motion and powerful shoulder abduction can be performed without any deltoid muscle function by the supplemental action of the supraspinatus and biceps brachii muscles. For evaluation of deltoid muscle function, we need to negate these actions, which can be done with the "akimbo test," in which patients place their hands on the iliac crest with abduction in the coronal plane and internal rotation of the shoulder joint while simultaneously flexing the elbow joint and pronating the forearm. METHODS: We examined the akimbo test in five patients with incomplete upper-ty

[Cubital tunnel syndrome and the scratch test to determine the surgical procedure].

BACKGROUND: Neuropathy due to ulnar nerve compression at the elbow level is the second most frequent neuropathy. The scratch collapse test is useful to diagnose compression neuropathies. This test helps us rank compression sites and decide the type of treatment to use. METHODS: From May to July 2011, 34 patients, mostly females, were preoperatively analyzed with this test. Ethyl chloride was also used to show other compression sites. RESULTS: The main compression site was found to be at the level of Osborne's ligament, contrary to the idea that it was located at the medial epicondyle. Anothe

Review of the dimensions of the median nerve and carpal tunnel using sonography in asymptomatic adults.

OBJECTIVE: The study aims to establish the normal range of all sonologic measurements of carpal tunnel (CT) structures in an asymptomatic population. METHODS: Sonological evaluation of 150 wrists in 75 asymptomatic adults was performed. The cross-sectional area (CSA) of the median nerve at four levels, the flattening ratio (FR) at three levels, the antero-posterior (AP) diameter of the CT and the distance of the transverse carpal ligament (TCL) from the trapezium-hamate (TmH) line were measured. RESULTS: The mean (standard deviation (SD)) CSA of the median nerve at the distal forearm, CT inl

[The usefulness of ultrasonography in the diagnosis of carpal tunnel syndrome--a review].

Ultrasonography has had relatively long history (approximately 20 years) in the diagnosing carpal tunnel syndrome, but as late as in last decade has gained greater popularity and has been applied in the clinic. Numerous studies revealed that the most accurate parameter indicating the compression of the median nerve in the carpal tunnel is the cross sectional area of the nerve at the inlet level. However, contrary to the nerve conduction studies, sonographic measurements are characterised by wide range of normal, physiological (a mean of 4.8 to 9.7 mm2), pathological, indicating compression of

Arm rotated medially with supination - the ARMS variant: description of its surgical correction.

BACKGROUND: Patients who have suffered obstetric brachial plexus injury (OBPI) have a high incidence of musculoskeletal complications stemming from the initial nerve injury. The presence of muscle imbalances and contractures leads to typical bony changes affecting the shoulder, including the SHEAR (Scapular Hypoplasia, Elevation and Rotation) deformity. The SHEAR deformity commonly occurs in conjunction with Medial Rotation Contracture (MRC) of the arm. OBPI also causes muscle imbalances at the level of the forearm, that lead to a fixed supination deformity (SD) in a small number of patients.

Surgical treatment of traumatic pediatric humeral diaphyseal fractures with titanium elastic nails.

BACKGROUND: Nearly all pediatric humeral shaft fractures can be successfully treated with closed methods. Some patients, however, require internal fixation either because of an inability to maintain an adequate reduction, significant soft tissue injury, or concomitant fractures. METHODS: This is a retrospective review of all traumatic humeral shaft fractures treated at our hospitals between 1999 and 2006. Thirteen pediatric patients ranging in age from 4.8 to 16.7 years (mean age 12.0 years) were treated surgically with titanium elastic nails (TENs). Relative surgical indications included ope

Induction of periostin-like factor and periostin in forearm muscle, tendon, and nerve in an animal model of work-related musculoskeletal disorder.

Work-related musculoskeletal disorders (WMSDs), also known as repetitive strain injuries of the upper extremity, frequently cause disability and impairment of the upper extremities. Histopathological changes including excess collagen deposition around myofibers, cell necrosis, inflammatory cell infiltration, and increased cytokine expression result from eccentric exercise, forced lengthening, exertion-induced injury, and repetitive strain-induced injury of muscles. Repetitive tasks have also been shown to result in tendon and neural injuries, with subsequent chronic inflammatory responses, fol

Prevalence of carpal tunnel syndrome in pregnant women.

Carpal tunnel syndrome (CTS) is a frequent complication of pregnancy, with a prevalence reported as high as 62%. The most typical symptoms are numbness and tingling in the thumb, index finger, middle finger, and radial half of the ring finger. Other common manifestations include burning dysesthetic wrist pain, as well as the loss of grip strength and dexterity. Proximal radiation along the volar forearm, medial arm, and shoulder, while not as common, is not unusual. Symptoms are often worse at night and can be exacerbated by forceful activity and extreme wrist positions. It can be diagnosed to
