Optimization of blood sampling strategy to avoid spurious PTH elevation in patients with forearm parathyroid autotransplantation and arteriovenous fistula.

Accurate monitoring of parathyroid hormone (PTH) is critical for assessing surgical outcomes in patients with stage 5 chronic kidney disease (CKD), yet preanalytical factors such as sampling location are often neglected. We report a 52-year-old male with CKD5 who underwent total parathyroidectomy with autotransplantation (TPTX+AT) in the right forearm and arteriovenous fistula (AVF) creation in the left forearm. Routine PTH measurement showed an extremely elevated level (>530 pmol/L) that was inconsistent with his stable clinical condition. PTH was measured using a fully automated chemilumines

"Reconstruction of segmental defect of flexor tendons of the wrist and hand using extensor digitorum longus".

Plastic and hand surgeons rely on traditional tendon grafts, but severe bilateral multi-level trauma may render these unusable. We report a 30-year-old man with extensive forearm loss reconstructed using the entire extensor digitorum longus tendon, nerve and vein grafts, and an ALT flap, demonstrating EDL's versatility and minimal donor morbidity.

Complications of obesity in children and youths with type 1 diabetes mellitus.

In the clinical practice, radial shaft may be exposed via two approaches, namely the posterolateral Thompson and volar (anterior) Henry approaches. A feared complication of both of them is the injury to the deep branch of the radial nerve. No consensus has been reached, yet, as to which of the two approaches is more beneficial for the proximal half of radius. According to our anatomical studies and clinical experience, Thompson approach is safe only in fractures of the middle and distal thirds of the radial shaft, but highly risky in fractures of its proximal third. Henry approach may be used

Peripheral IV Catheter-Associated Upper Extremity Deep Vein Thrombosis in a Patient with Psychiatric Illness: A Case Report and Literature Review.

BACKGROUND: Upper-extremity deep vein thrombosis (UEDVT) is relatively uncommon accounting for roughly 4-10% of all deep vein thromboses but its incidence is rising, largely due to the widespread utilization of intravenous catheters. While central venous catheters are the typical culprits, peripheral IV cannulas are not benign and may provoke thrombosis via local endothelial trauma and venous stasis, especially in individuals with systemic prothrombotic predispositions. CASE PRESENTATION: We report a 44-year-old man with bipolar affective disorder on haloperidol, promethazine, and carbamazepi

Coverage of elbow and forearm soft tissue defects with the posterior ulnar recurrent artery perforator flap (PURAP): an anatomical study.

INTRODUCTION: Covering soft tissue defects from the elbow and forearm is challenging for the plastic surgeon. The posterior ulnar recurrent artery perforator flap is a fasciocutaneous perforator flap vascularized by the perforators emerging from the posterior ulnar recurrent artery. It has multiple functional and aesthetic advantages but has not yet been well studied. This work aimed to examine the number, caliber, and topography of the posterior ulnar recurrent artery's perforators. METHODS: Perforator mapping was performed by blue latex injection on 20 fresh cadavers' upper extremities. The

Superficial Arterial Variants of the Upper Limb: Clinical Implications of High-Origin Ulnar and Radial Arteries Detected by Ultrasound and Anatomy Study.

Background: Arterial variations in the upper limb, although infrequent, carry critical clinical implications. The presence of superficial ulnar and radial arteries, especially when originating from high levels, increases the risk of iatrogenic injury, misdiagnosis, and surgical complications. To confirm and describe, through ultrasound and anatomical dissection, the presence of a high-origin superficial ulnar artery and a superficial radial artery in a cadaver, highlighting their anatomical trajectory and clinical relevance. Methods: A cross-sectional ultrasound and anatomical study was conduc

Topographic anatomical landmarks for targeted nerve infiltration in distal radius fracture surgery.

The study focuses on the increasing incidence of distal radius fractures, particularly in individuals under 15 and over 50, and the critical need for effective perioperative pain management strategies. Distal radius fractures often require surgical intervention and pain control, essential to enhancing recovery and minimizing complications like chronic regional pain syndrome (CRPS). Standard analgesic options include hematoma blocks, Intravenous regional anesthesia (IVRA), regional nerve blocks, sedation, and general anesthesia. This research proposes a nerve block approach targeting the anteri

New Ultrasonographic Parameter for Diagnosing Carpal Tunnel Syndrome: The Median Nerve-to-Ulnar Artery Cross-Sectional Area Ratio.

Background and Objectives: Carpal Tunnel Syndrome (CTS) is the most common entrapment neuropathy. Ultrasonography is a widely used diagnostic method for CTS, and the median-to-ulnar nerve cross-sectional area ratio (MUR) is a well-known parameter. However, the ulnar nerve may be affected by various conditions of the median nerve; therefore, we considered finding further parameters. The aim of the study is to identify the correlation between the median nerve-to-ulnar artery cross-sectional area ratio (MUAR) and existing ultrasonographic parameters used as diagnostic indicators of CTS. Materials

On a Hunt for the "True" Septocutaneous Perforator: A Histology Cross-Section Study.

BACKGROUND: Modern trends in reconstructive surgery involve the use of free perforator flaps to reduce the donor site morbidity. The course of perforator vessels has a great anatomic variability and demands detailed knowledge of the anatomical relationships and the variability of the course of the perforators. The numerous modifications to perforator nomenclature proposed by various authors resulted in confusion rather than simplification. In our study, we focused on the hypothesis that a septocutaneous perforator traverses from the given source vessel to the deep fascia adherent to but not to

Iliac crest bone graft with radial forearm flap for thumb reconstruction: A case report.

UNLABELLED: Traumatic thumb amputation is a serious injury that requires replantation or reconstruction. Toe-to-thumb transfers method have great survival and patient satisfaction in thumb reconstruction. Alternative method like Iliac Crest Bone Graft (ICBG) with flaps may help surgeons achieve maximum results. A 32-year-old male presented with occupational traumatic right thumb amputation. After initial debridement and K-wire installation, the thumb became necrotic. An ICBG with radial forearm flap was performed after the patient denied a toe-to-thumb transfer. Follow-up demonstrated viable f

Anatomy of the Lateral Antebrachial Cutaneous Nerve: Landmarks for Procedures in the Cubital Fossa.

PURPOSE: The research outlines anatomical landmarks that may help surgeons in identifying the lateral antebrachial cutaneous nerve (LABCN) to minimize nerve damage during procedures in the cubital fossa. METHODS: Twenty-eight fresh cadaveric upper extremities were dissected. The course of the LABCN was followed from the emerging point at the biceps brachii tendon (BT) to the mid-forearm. The nerve's relationships with the BT, lateral epicondyle (LE), antebrachial vein, and brachioradialis (BR) muscle were measured and documented. RESULTS: The LABCN emerged lateral to the BT in all specimens

Microsurgical Reconstruction of the Columella.

BACKGROUND: Traditionally, the columella can be difficult to reconstruct because of its unique contours, paucity of adjacent soft tissues, and tenuous vascularity. When local or regional tissues are unavailable, microsurgical transfer can provide a means for reconstruction. In this retrospective review, the authors report their experience with microsurgical reconstruction of the columella. METHODS: Seventeen patients were enrolled in this study and divided into two groups: group 1, isolated columella defects; and group 2, defects of the columella and portions of adjacent soft tissues. Median artery persistence in coexistence with a bifid median nerve and interconnections of the median with the ulnar nerve and vice versa.

PURPOSE: This report describes a bilateral persistent median artery (PMA) originating from the ulnar artery and terminating at different levels of the upper limb. The PMA coexisted with a bilateral bifid median nerve (MN) and two bilateral interconnections (ICs characterized with the symbol -) of the MN with the ulnar nerve (UN) (MN-UN) and a unilateral reverse IC (UN-MN). Emphasis was given to the artery's developmental background. METHODS: The PMA was identified in an 80-year-old formalin-embalmed donated male cadaver. RESULTS: The right-sided PMA terminated at the wrist, posterior to the

Management of Traumatic Radial and Ulnar Artery Injuries and Risk Factors for Amputation.

INTRODUCTION: Traumatic injuries to the radial and/or ulnar arteries represent a subset of arterial injuries. In the absence of injury to both forearm arteries, treatment was historically ligation if perfusion was maintained to the hand via the uninjured vessels or adequate collateral vessels. We sought to determine management of traumatic forearm arterial injuries in 2019 and to identify risk factors for major upper extremity amputation. METHODS: The American College of Surgeons Trauma Quality Improvement Program database was queried by International Classification of Diseases 10 code for pa

Median neuropathy after multiple punctures of the forearm for catheterization: A case report.

BACKGROUND: Neuropathy may occur at some sites after catheterization for close examination of cardiac disease. Although the radial artery is considered a relatively uncom-plicated site for catheterization, the radial artery and median nerve are in relatively close proximity, with the risk of median nerve injury depending on the angle of puncture. The purpose of this study was to report the outcomes of surgery performed for conservative therapy-resistant median neuropathy following forearm catheterization. CASE SUMMARY: A 50-year-old woman experienced palsy from the right thumb to the radial s

Ulnar-Sided Upper Extremity Traumatic Wounds: What Should We Expect to Find?

Background: Upper limb traumatic injuries have a significant impact on social and professional life; however, there is still a paucity of studies focusing on the injuries of the ulnar border of the forearm, wrist and hand. Methods: We designed a retrospective single-blinded study, including all patients with deep traumatic wounds affecting the ulnar side of the forearm, wrist or hand, that received surgical treatment from 2006 until 2016. A characterisation of the sample, assessment of concomitant injuries and clinical outcomes, as well as neurological and functional evaluation were performed.

Association of Cord Ferritin Levels with Brain Stem Evoked Response Audiometry in Newborns.

Iron is an important nutrient and it plays a pivotal role in myelin formation and neurotransmitter synthesis, thus contributing to normal neurological activity. Ferritin is a reliable indicator of the tissue iron stores and in-utero stores can be well measured by cord ferritin levels. The objective of this study was to evaluate the effects of umbilical cord ferritin levels (CFL) on the brain stem evoked response audiometry (BERA) This prospective observational study was conducted in a tertiary care centre of North India with a sample size of 100 inborn neonates. After evaluation of the umbilic
