BACKGROUND: The origin of the flexor pollicis longus muscle can vary, causing compression of the anterior interosseous nerve in the forearm. The topographic relationship of the flexor pollicis longus to the median and anterior interosseous nerves must be considered when these nerves are decompressed. The anterior interosseous nerve is a motor nerve supplying the flexor pollicis longus, the flexor digitorum profundus, and the pronator quadratus. Unusual etiologic conditions can occasionally cause isolated neuropathy of the flexor pollicis longus branch from the anterior interosseous nerve.

The superficial radial vein at the lateral edge of the inferior third of the forearm and of the wrist has strong anatomical relationship with the sensory superficial branch of the radial nerve. At this level, any venous puncture may be responsible for a lesion of this superficial part of the radial nerve. We report two cases of dysesthesia in the radial territory after this kind of puncture. This risk should not be neglected and venous puncture should be avoided in this area.

The lateral antebrachial cutaneous nerve (LACN) is the terminal sensory branch of the musculocutaneous nerve supplying the lateral aspect of forearm. Because of its close proximity to the biceps brachii tendon (BBT), the lateral epicondyle (LE), and the cephalic vein (CV), surgery and venipuncture in the cubital fossa can injure the LACN. The superficial branch of the radial nerve (SBRN) is highly vulnerable to trauma and iatrogenic injury. INTRODUCTION: Avulsion and crush injuries constitute a particularly difficult problem due to extensive damage of vessels and nerves. In cases where a crush is the dominating injury factor causing complex fractures of forearm and carpal bones, shortening of the extremity is necessary for primary vessel and nerve reconstruction. This article reports a patient who had a total degloving of the hand, caused by a roller injury, up to the distal forearm level. The authors replanted the degloved skin using arteriovenous anastomosis of the radial artery at the wrist to the cephalic vein in the degloved skin in an end-to-side manner. To enhance the survival of the replanted skin, it was deepithelialized and buried in an abdominal pocket created specifically for this purpose. This paper presents the experiences of the thumb reconstruction with osteofasciocutaneous reverse flap (OFCR flap). In the period between 1987 and 2000 the OFCR flap was used in 15 patients. The youngest of them was 18 and the oldest was 38 years of age. The average age was 25.4. All the patients had posttraumatic amputations. Ring avulsion replantation is a technically challenging procedure with a very low success rate. Because the zone of arterial injury is more extensive than what it appears to be in such avulsion amputations, a technique was developed to debride the avulsed digital artery for a long distance, extending well into the digital pulp, thereby creating healthy arterial ends to be bridged by one or two segments of long venous grafts. UNLABELLED: The cephalic vein of the forearm is often used for IV catheters because of its ease of access for peripheral venous cannulation. But its close relation to the sensory branch of the radial nerve sometimes causes it to be damaged when the vein is cannulated. Current experience in the management of upper-extremity arterial injury in a Level I trauma center between 1992 and 1994 is reported. Arterial trauma was seen in 21 of 643 (3.3%) patients admitted with upper-extremity injury. The mechanism of injury was penetrating in 15 of 21 and blunt in 6 cases. Patient characteristics were: 18 of 21 male, mean age 28, left upper extremity 12 of 21, and 4 patients in shock. Preoperative angiography was performed in 12 of 21 cases (5 of 6 blunt and 7 of 15 penetrating). Involved arteries included: brachial (10), axillary (5), radial (3), and subclavian (3).
