Isokinetic dynamometry represents the gold standard measure of strength, but there is a lack of consensus regarding data collection. The aim of this study was to develop a comprehensive protocol considering contractile speed and modality, and to use this model to generate normative data for recreational athletes. Twenty male and 20 female, non-sport-specific but recreationally active participants completed isokinetic trials at 60, 180 and 300°/sec in eccentric and concentric modes for the knee flexors and extensors. Males generated significantly greater peak torque in extension and flexion, ac

BACKGROUND: The posterior interosseous flap is commonly based on the posterior interosseous artery (PIA), but anatomical variability and intraoperative compromise may challenge its reliability. Alternative microvascular networks may contribute to flap survival beyond named axial vessels.
CASE: A 68-year-old woman presented with a dorsal hand defect with exposed extensor tendons following a grade V extravasation injury. A posterior interosseous artery (PIA)-based flap design was initially planned for coverage. Intraoperatively, however, the PIA pedicle could not be clearly identified. The flap

Hallux motion disturbance after lateral closing-wedge high tibial osteotomy (LCW-HTO) is uncommon, and tendon-related complications following fibular osteotomy remain poorly recognized. We describe a case of extension-type hallux checkrein deformity caused by presumed extensor hallucis longus (EHL) tethering after LCW-HTO with fibular osteotomy and discuss its diagnostic and surgical implications. A man in his 40s developed abnormal hallux extension during ankle plantarflexion 6 months after LCW-HTO with concomitant fibular osteotomy. The deformity interfered with gait and shoe wear. Physical

INTRODUCTION: Tendon injuries are prevalent musculoskeletal conditions in clinical settings, affecting various regions such as the rotator cuff tendons, forearm flexor/extensor tendons, finger flexor tendons, and Achilles tendon. While surgery is an effective treatment, it often fails to balance intrinsic and extrinsic healing processes, leading to abnormal cell activation, proliferation, and migration, resulting in collagen deposition at the injury site. This pathological fibrosis causes severe peritendinous adhesions, posing a therapeutic challenge. Traditional nonsteroidal anti-inflammatory

There were no reports of chronic extensor digitorum communis (EDC) III tendon rupture in zone 5. We report a case of chronic EDC III tendon rupture in zone 5 reconstructed with a bridge tendon graft using palmaris longus tendon. End-to-end tendon repair seemed difficult for chronic EDC tendon rupture, and it was feared that direct repair would result in residual extension contracture of the finger. A 47-year-old right-handed male underwent a bridge tendon graft using the palmaris longus tendon for a chronic EDC III tendon rupture in zone 5 six weeks after the injury. Six months after the surge

INTRODUCTION: Cervical spine musculature contributes to stability and load carrying capacity of the human head-neck. As muscle morphologies influence segmental neck biomechanics and physiological responses to operational activities, their accurate description in computational musculoskeletal models is needed to better estimate osteoligamentous column loads, and determine the mechanisms of internal load transfer, and segmental and local component load sharing. Cervical muscle morphologies specific to fighter jet pilot populations have not been included in the current musculoskeletal injury mode

PURPOSE: To answer three clinically oriented questions after acute patellar tendon repair: how often and how quickly patients return to sport (RTS), which patient factors are associated with RTS, and whether repair technique is associated with RTS under a common surgical and rehabilitation protocol.
METHODS: Single-centre retrospective cohort of 46 consecutive adults treated for acute complete patellar tendon rupture at a public university hospital (2014-2022). Repairs were performed by a single surgeon using either suture anchors (n = 25) or transosseous tunnels (n = 21); technique availabil

Simultaneous bilateral quadriceps tendon rupture is an exceptionally rare injury of the knee extensor mechanism and may present a diagnostic challenge, particularly after low-energy trauma. Prompt recognition is essential to restore knee extension and ambulatory function. We report the case of a 73-year-old man who presented to the Emergency Department after falling while descending stairs and landing directly onto both knees. He developed immediate bilateral knee pain and inability to ambulate. Clinical examination revealed palpable suprapatellar defects bilaterally, inability to perform stra

INTRODUCTION: Bilateral tibial tuberosity avulsion fractures (TTAF) are among the rarest and most significant injuries affecting the extensor mechanism in skeletally immature patients. Successful treatment mandates acute anatomical reduction and stable internal fixation that accounts for high quadriceps tensile forces and the risk of physeal growth arrest.
CASE REPORT: A 14-year-old male sustained an acute, displaced bilateral TTAF (Modified Watson-Jones/Ogden Type II) after a vigorous, simultaneous jump and landing. The patient presented with partial bilateral loss of active knee extension.

BACKGROUND: Orthopaedic injuries are a major contributor to morbidity and mortality in low-income settings, driven by rising urbanisation and inadequate road safety measures. This study evaluates the cost-effectiveness of treatment for four common orthopaedic injuries in Malawi: femoral shaft fractures, patellar fractures, humeral shaft fractures and paediatric supracondylar humeral fractures (PSCHF).
METHODS: We developed a dynamic state-transition model to simulate fatal and non-fatal outcomes from a healthcare perspective from 2021 to 2031, informed by cost, epidemiological and population

Severe peripheral nerve injuries result in incomplete recovery despite neurorrhaphy. Microsurgical suturing is technically demanding, time-intensive, and may produce variable fascicular alignment. Nerve Tape is an FDA-approved sutureless device enabling rapid, reproducible nerve coaptation. This study compared Nerve Tape with epineurial microsuturing following common peroneal nerve transection in Yucatan minipigs. Over 12 months, both groups demonstrated reinnervation of the tibialis anterior and extensor digitorum brevis, representing proximal and distal muscle targets, respectively. Tibialis

PURPOSE: Extensor pollicis longus (EPL) tendon ruptures are a rare but recognized complication following distal radius fractures (DRFs) with several proposed mechanisms for their pathophysiology. Existing literature on the incidence of EPL ruptures has largely been limited to small, single-center studies. The purpose of this study was to define the incidence of EPL ruptures following operatively and non-operatively treated DRFs using a large, multicenter database.
METHODS: A retrospective cohort analysis was performed using the TriNetX database. Patients aged 18 to 90 years who sustained a di

INTRODUCTION: The triceps brachii muscle (TBM) is the main muscle of the posterior aspect of the arm, occupying most of the extensor compartment. Proximally, it is composed of three heads: long, lateral, and medial, and presents a single tendon. Its insertion in the olecranon region remains controversial. Magnetic resonance imaging (MRI) assessment and detailed knowledge of this insertion may assist in orthopedic surgical reconstructions.
OBJECTIVE: To evaluate the anatomical aspect of the insertion of the triceps brachii tendon (TBT) into the olecranon regarding tendon insertion.
METHODS: I

INTRODUCTION: Displaced fractures of the base of the proximal phalanx (PP) remain a challenging surgical problem. Regardless of the osteosynthesis material chosen (K-wires, screws, plates), when open reduction is required, the surgical approaches typically used are either dorsal or lateral. These approaches inevitably cause surgical trauma to the extensor apparatus which, via the intrinsic system, envelops the dorsal and lateral surfaces of PP at this level. To preserve the extensor apparatus, the authors propose a palmar approach combined with anterior plate fixation.

BACKGROUND/PURPOSE: Sports injury induces peripheral and central neurophysiological alterations that influence motor control and recovery. The mixed-nerve silent period (SP), a transient suppression of voluntary muscle activity after electrical stimulation, reflects multilevel neural inhibitory mechanisms involving spinal and cortical pathways. This study aimed to elucidate central nervous system (CNS) excitability during muscle output regulation and explore neurophysiological characteristics in healthy athletes and contrast with findings from an athlete with delayed return to play.

We report three adolescent boys with combined tibial tubercle fracture and patellar tendon rupture. All presented with pain, swelling, and inability to actively extend the knee. Initial radiographs demonstrated tibial tubercle fractures, and complete fragment rotation was present in two cases. All patients underwent tibial tubercle fixation and patellar tendon repair. At final follow-up, all achieved fracture union, full active knee extension, and return to their pre-injury or higher activity level. Complete fragment rotation may represent an early radiographic clue to associated patellar tend

Acute anterior tibialis tendon (ATT) rupture is a rare injury that may go unrecognized in the acute trauma setting, particularly when it co-occurs with a closed tibial shaft fracture. We report a case of complete ATT rupture in a 25-year-old male following a motorcycle accident causing a distal tibial shaft fracture. The rupture was identified intraoperatively; during distal locking screw placement, tendon-like fibers were observed through the surgical incision. Given this unexpected finding, the incision was extended to allow further exploration. Careful dissection revealed a complete rupture

BACKGROUND: Multiligament knee injuries involving the posterior cruciate ligament (PCL), posterolateral corner (PLC), and lateral collateral ligament (LCL) are rare but complex pathologies. Historically managed with open surgery, these injuries are increasingly being addressed using minimally invasive techniques, which provide anatomical reconstruction and less surgical morbidity.
INDICATIONS: We report the case of a 24-year-old male farmer who sustained a high-energy multiligament knee injury after a motorcycle accident. Six months after the accident, he presented with persistent daily insta

Hip-Hop dance is physically demanding, yet epidemiological data in female dancers remain limited. This mixed observational cohort study described musculoskeletal injury and physical complaint burden, evaluated core endurance and balance, compared female Hip-Hop dancers with healthy non-dancer controls, and examined associations with prospective complaints. Adult female Hip-Hop dancers (n = 31) and controls (n = 34) completed the McGill test battery, Unipedal Stance Test, and modified Star Excursion Balance Test. Dancers also reported prior-year injuries and musculoskeletal complaints and were

PURPOSE: Since the UK Armed Forces opened all combat roles to women in 2018, more female soldiers are carrying heavier loads for longer. This study investigated associations between aerobic capacity, strength, and lean mass and the demands of load carriage in women, and associations between load carriage demands and muscle fatigability.
METHODS: Thirty-nine women (28 ± 4 y) had maximal rate of oxygen uptake (V̇O2max) and lean mass assessed before completing load carriage (12.8 km in 120 min carrying 20 kg). Oxygen uptake (%V̇O2max), heart rate (HR), and ratings of perceived exertion (RPE) wer
