Can Knee Extensor Loading Deficits Be Diagnosed Using Variables from Force Plates during Squats Post-ACLr?

UNLABELLED: The persistence of knee extensor moment (KEM) deficits across rehabilitation post-ACL reconstruction may be attributed to difficulty detecting and addressing deficits clinically. During squats, large KEM deficits are present along with much smaller differences in joint or segment angles, making them difficult to detect visually. Data outputs from force plates and video technology may improve clinical identification. PURPOSE: Investigate the accuracy of three approaches to estimate KEM deficits using outputs from the force plate and two-dimensional (2D) position data to identify in

Asymmetrical Patellofemoral Joint Loading Persists Through Return to Sport After Anterior Cruciate Ligament Reconstruction in Adolescent Athletes.

CONTEXT: Patellofemoral joint loading after anterior cruciate ligament reconstruction (ACLR) remains poorly understood, despite the high prevalence of patellofemoral joint complications following surgery. With an observational cohort design, we compared patellofemoral joint stress (PFJS) and reaction force (PFJRF) between surgical and nonsurgical limbs during a double-limb squat at 12 weeks postsurgery and return to sport (RTS), with sex-based comparisons. METHODS: Forty-seven athletes (21 males and 26 females) aged 12-25 years performed a body weight double-limb squatting task at 12 weeks po

Evaluation of sport-related concussion factors at baseline in a cohort of collegiate rugby union players.

Sport-related concussion (SRC) is a growing concern in Rugby Union (RU) and SRC's relationship to intrinsic and extrinsic factors is unknown. A descriptive cross-sectional design was used to describe the relationship between these factors in collegiate RU players. Intrinsic and extrinsic factors included: age, weight, height, position of play (POP), previous SRCs, previous neck injuries; cervical flexor and extensor endurance; cervical anterolateral flexion strength (CAFS); and joint position error (JPE). Descriptive statistics and frequency distributions were calculated. Inferential statistic

Physical Characteristics of Fast Roping in British Elite Law Enforcement Officers.

Elite law enforcement and special forces operators around the world have a unique skill set, including some risky methods of entry into a scene of operation. With fast roping being actively utilized by this population, it is important to gain an understanding of the physical demands of the task. Thirty-seven Law Enforcement Officers (LEOs) completed five 20 ft fast rope descents onto a force platform, three in standard uniform (without kit) and two with the additional weight of operational kit (with kit). Additionally, 12 LEOs were also fitted with electromyography on their dominant arm. Landi

Sonographic Evaluation of Tendinous Mallet Finger to Estimate the Extent of Extension Lag.

BACKGROUND: Although several reports have addressed tendinous mallet finger (TMF), they have not identified which method best informs surgeons about the extent of residual extension lag in these patients. We aimed to assess the association between sonographic classification of acute TMF and the degree of extension lag remaining at the final follow-up. METHODS: The inclusion criterion was acute TMF with symptom onset within 2 weeks. Thirty-eight patients (23 male and 15 female) participated, with a mean follow-up of 17.1 months (range, 12.3-23.5 months). Range of motion, including extension la

The impact of age on muscle and kinematic responses following an unpredictable forward slip while walking.

BACKGROUND: Slips lead to many falls in older adults. This study aimed to determine the impact of age on muscle and kinematic responses following an unpredictable forward slip while walking. METHODS: A cross-sectional study was conducted with 127 adults (25 young and 102 older) exposed to a single unpredictable forward slip, induced at foot-strike while walking. Bilateral lower limb muscle activity was measured via electromyography and gait kinematics were measured via motion capture. FINDINGS: Compared to young, older adults displayed greater relative muscle activity and higher relative cen

Hamstring Tendon vs. Bone-Patellar Tendon-Bone Grafts for Primary Anterior Cruciate Ligament Reconstruction in Football (Soccer) Players: A Systematic Review and Meta-Analysis.

Anterior cruciate ligament (ACL) rupture is a common and career-impacting injury among football players, often requiring surgical reconstruction. The two most commonly used grafts, hamstring tendon (HT) and bone-patellar tendon-bone (BPTB), have both shown favorable outcomes, but the optimal choice remains debated. This systematic review aimed to evaluate and compare outcomes of ACL reconstruction using HT vs. BPTB autografts specifically in football players. A systematic literature search was conducted across PubMed, Cochrane Library, and MEDLINE databases from inception to January 2025. Elig

How current extensor tendon repair practices differ from expert recommendations.

INTRODUCTION: This study aimed to understand the current real-world status of extensor tendon repairs with data through an international survey, thus providing information to guide continuing education, design of courses and global standardization efforts. METHODS: Ten hand surgeons were surveyed from each of five countries in America, Asia and Europe. The survey participants - all practising hand surgeons - were asked to answer multiple choice questions regarding their extensor tendon repair surgery and therapy. RESULTS: The survey revealed remarkable discrepancies or 'distances' between th

Hand reconstruction after a high-pressure injection injury: review of biophysics and prognostic classification.

High-pressure injection injuries are rare but potentially devastating, often presenting with deceptively minor skin wounds that conceal extensive internal damage. This case highlights the Pressure, Anatomy, Content and Time of presentation classification system to standardise injury assessment and staged surgical management of an upper limb injury from a sealant gun: initial debridement and pedicled groin flap coverage, followed by extensor tendon reconstruction using a three-tailed fascia lata graft. Postoperative rehabilitation yielded full range of motion, restoration of grip strength from

Gore-Tex Membrane Augmentation for Treatment of Acute Patellar Tendon Rupture in a Soccer Player - A Case Report.

INTRODUCTION: Patellar tendon rupture is a serious injury affecting the knee extensors and often requires surgical repair with graft augmentation, which lengthen return to sports not sooner than 6 months. CASE REPORT: We, hereby, describe a case of an active male presenting with sudden knee pain and inability to flex the knee during a soccer match. Clinical examination and imaging were positive for complete patellar tendon rupture. Surgical treatment consisted of tendon repair and side-to-side augmentation with Gore-Tex membrane. Weight-bearing was allowed from day 1, knee flexion after 1 mon

Anatomic safety profile of a novel mini external fixator for proximal crescentic osteotomy in hallux valgus correction: a cadaveric study.

PURPOSE: The proximal crescentic osteotomy is an effective procedure for hallux valgus correction, but stable fixation remains challenging. Our previous biomechanical study suggested that the MEF may offer comparable or greater stability than a specific cannulated screw fixation construct in a cadaveric model. This study aims to evaluate the anatomical safety of the MEF by investigating its relationship with critical neurovascular structures and tendons. METHODS: An anatomical dissection study was performed on ten foot cadaveric specimens. The MEF was applied using five mini-Schanz pins insert

Proximal ulna corticocancellous autograft for the repair of metacarpal defect following ballistic hand injury.

Metacarpal defects following ballistic hand injuries are a devastating complication that can compromise hand function. Autologous bone grafting is the gold standard treatment of these osseous defects, most commonly utilizing the anterior iliac crest as the donor site. Alternatively, use of corticocancellous bone graft from the proximal ulna has been described, although with mixed clinical outcomes. Additionally, the upper limit of defect that can be repaired with this technique remains unknown. Herein we described the case a patient who sustained a partial extensor tendon injury and significan

Metacarpal Malunion Correction With Crescentic Osteotomy.

Fractures of the metacarpals account for 18% to 44% of all hand fractures, with malunion being the most common complication. Sagittal plane malunion may result in extensor lag, bone shortening, loss of knuckle contour, and prominent dorsal or palmar bony deformity. In this paper, we describe a novel crescentic osteotomy technique of the border metacarpals stabilized with intramedullary screw fixation for the correction of extra-articular malunions. We report 3 cases of border metacarpal malunion treated with this method. At a median follow-up of 9 months (mean: 12.3 mo), patients achieved exce

Tendon versus nerve transfers - balancing hand function in upper extremity high nerve injuries.

IntroductionHigh peripheral nerve injuries disrupt the balance between flexor and extensor muscle groups and result in significant impairment of strength, coordination and dexterity. Primary nerve repair is widely accepted as the optimal treatment following transection injury, but outcomes after high-level nerve reconstruction remain unpredictable. Prolonged denervation commonly leads to irreversible muscle atrophy, limiting the potential for meaningful distal functional recovery despite technically successful nerve repair.Reconstructive strategies:Different surgical strategies are available t

Exercise during 14 days of head down tilt bedrest attenuates motor unit impairments in older humans.

Head-down tilt bedrest (HDBR) models the effects of mechanical unloading on neuromuscular function. The efficacy of exercise in preserving motor unit (MU) function in older adults during HDBR remains unclear. This study investigated the effects of 14-day HDBR on MU properties in older adults and the protective role of exercise. Fifteen participants aged 55-65 years were randomized to Control group (n = 7, passive mobilization only) or Exercise group (n = 8, daily mixed resistance and aerobic training) during 14 days of strict HDBR. Knee extensor strength and leg lean mass were measured, and in
