The Clinical Utility of Strength Measures in Predicting Patient Progression Following ACLR.

BACKGROUND: Prognostic indicators of lack of strength progression or poor self-reported function may alter rehabilitation decision-making following anterior cruciate ligament reconstruction (ACLR). The torque-velocity relationship is a noninvasive measure of muscle function that is altered following ACLR, but its prognostic value has not been explored. PURPOSE: To compare the torque-velocity relationship of knee extensors (quadriceps) and flexors (hamstrings) between the ACLR and uninvolved limbs and to determine whether the torque-velocity relationship was prognostic of subsequent achievemen

Effects of repeated tDCS on lower limb muscle explosive strength and executive function in male collegiate volleyball players: a randomized controlled trial.

OBJECTIVE: Volleyball requires superior lower limb explosive strength and rapid executive function for optimal performance and injury risk reduction, yet the efficacy of repeated transcranial direct current stimulation (tDCS) in enhancing these volleyball-specific physical and cognitive capacities remains unclear. This study aimed to investigate the effects of five consecutive days of tDCS on lower limb explosive strength and executive function in male collegiate volleyball players. METHODS: Thirty-two participants were randomized to receive real tDCS (anodal stimulation over M1, 2 mA for 20

Augmented Repair of an Acute Midsubstance Patellar Tendon Rupture Using Suture Anchors, InternalBrace™, and Achilles Tendon Allograft: A Case Report.

Patellar tendon (PT) ruptures are rare but disabling injuries that result in loss of active knee extension and significant functional impairment. We present the case of a 56-year-old male who sustained a complete midsubstance PT rupture after a direct blow to the knee. MRI confirmed the injury, and surgical management included end-to-end repair with suture anchors, augmentation with an InternalBrace™ (Arthrex Inc, Naples, USA), circumferential Achilles tendon allograft reinforcement, and repair of the medial and lateral patellofemoral capsule. Postoperative imaging confirmed restoration of pat

Salvage of composite soft tissue and extensor mechanism defects of the knee using free anterolateral thigh flaps with vascularized fascia lata.

Composite soft tissue reconstruction of the knee poses a great challenge. At our institution, we have employed free anterolateral thigh (ALT) flaps with vascularized fascia lata (vFL) for reinforcing or reconstructing extensor mechanism. Herein, we evaluated complications and outcomes in these patients. A retrospective chart review was performed, including individuals who underwent soft tissue free flap reconstruction of the knee from 01/01/2015 to 12/31/2022 at our institution. We analyzed each patient's clinical history, peri-, and postoperative details. Analysis was performed using Excel. T

Failure modes of mechanically aligned versus kinematically aligned total knee arthroplasty.

BACKGROUND: While mechanical alignment (MA) has long been considered the "gold standard" technique when performing total knee arthroplasty (TKA), kinematically aligned (KA) TKA has been gaining recent popularity. This technique aims to restore the pre-arthritic knee anatomy and alignment, with studies demonstrating excellent functional and biomechanical outcomes when compared to MA TKA. The primary aim of this study is to compare reasons for reoperation in MA versus KA primary TKA and understand if the reasons for re-operation differ between the two techniques. METHODS: This is a retrospectiv

Major Quadriceps Gap After Neglected Patellar Sleeve Fracture in a Child: A Case Report.

CASE: A 14-year-old boy presented with a neglected patellar sleeve fracture, resulting in a 19 cm gap between the retracted quadriceps and patella. A stepwise surgical reconstruction combining the "Chambat Sardine Can" tensioning technique, tibial tubercle transfer, rectus femoris release, and allograft bridging was performed. CONCLUSION: This is the first reported case of such a massive quadriceps retraction. The adaptable surgical strategy, tailored to defect severity, led to excellent functional outcomes and may be applied in both pediatric and adult chronic extensor mechanism ruptures.

Extensor apparatus of the knee: anatomy and injury patterns.

The extensor mechanism of the knee is essential for maintaining normal daily function and for activities such as walking and athletic performance. It comprises the quadriceps muscles and tendon, patella and peripatellar fat pads, patellofemoral joint and retinacula, patellar tendon, and tibial tuberosity. Disruption of any component can result in pain, instability, or loss of active extension. Owing to its superficial location and continuous biomechanical loading, it is particularly vulnerable to acute trauma, repetitive microtrauma, chronic degeneration, and traction apophysitis in the pediat

Quadriceps Tendon Rupture While Rehabilitating from a Patellar Tendon Repair: A Case Report.

CASE: We present the case of a 27-year-old man who sustained a quadriceps tendon rupture within 3 months after surgical repair of an ipsilateral patellar tendon rupture repaired with the transosseous technique. At 1-year follow-up, the patient demonstrated outcome measures indicating an excellent functional recovery. CONCLUSION: This case demonstrated a rare injury pattern treated with surgical repair and a structured rehabilitation program that led to a functional recovery. The case underscores the mechanical, technical, and compliance-related factors that contributed to this catastrophic se

Standing NanoNeedle Arthroscopy of the Distal Interphalangeal Joint for Removal of Osteochondral Fragments of Distal P2 and the Extensor Process in a Horse.

BACKGROUND: In horses, standing arthroscopy offers a valuable alternative to general anesthesia for the reduction of anesthetic-associated risks, and provides improved visualization in joints where access is facilitated by weightbearing. These techniques are particularly advantageous for patients with increased anesthetic risk or financial limitations. To the authors' knowledge, the removal of osteochondral fragments from the distal interphalangeal joint in a standing, sedated horse using small-diameter arthroscopic equipment has not been previously reported. CASE PRESENTATION: A 7-year-old A

Limb Strength and Power Asymmetries in Professional Team Sport Athletes at Return-to-Sport Testing Following ACL Reconstruction.

Background and Objectives: Present assessment methods have not effectively mitigated the risk of recurrent anterior cruciate ligament (ACL) injury following reconstruction (ACLR), suggesting that critical neuromuscular deficits may be underdiagnosed. This study aimed to compare limb asymmetries across strength, concentric and eccentric power, and deceleration metrics during return-to-sport (RTS) testing in professional athletes post-ACLR. Materials and Methods: Forty-four participants (33 males, 11 females; age 22.5 ± 5.8 years, body mass 75.9 ± 13.0 kg, height 180.5 ± 8.38 cm) (mean ± SD) wit

Precision anterior cruciate ligament (ACL) reconstruction in high-level athletes.

Anterior cruciate ligament (ACL) tear is a common injury in high-level athletes and has a significant impact on a professional career. Precision management is necessary for successful return to sports (RTS) and return to a competitive level, referred to as return to play (RTP). ACL reconstruction (ACLR) is considered the gold standard and restores anteroposterior and rotatory knee stability to facilitate a successful comeback in the elite athlete's sport. Despite an overall high RTS and RTP rate in elite athletes, performance and career longevity can be significantly impacted. Management of AC

Differences in lower-limb alignment and muscle strength between the sexes during distance-specific single-leg hop tasks.

[Purpose] This study examined the differences in lower-limb alignment and muscle strength between sexes during distance-specific single-leg hop (SLH) landings. SLH tasks are widely used in rehabilitation and return-to-sport decision-making; however, standardized progression criteria for hop distance are lacking. [Participants and Methods] A total of 59 healthy university participants performed SLH landings at distances set to 30, 50, and 80% of their height. The lower-limb alignment variables and isometric muscle strength were measured. Lower-limb kinematic variables were obtained using three-

Hand trauma associated with non-professional animal slaughter during Eid al-Adha: an emergency and hand surgery experience.

BACKGROUND: Eid al-Adha is an important religious holidays celebrated annually in Muslim communities, during which the ritual of animal sacrifice is performed. The demand for butchery services rises significantly during this period; however, due to the limited availability of professional butchers, many individuals undertake the slaughter themselves. This practice is associated with an increased incidence of traumatic injuries, particularly involving the hands and upper extremities. This study aims to characterize the pattern of hand and upper limb injuries associated with non-professional ani

Comparative outcomes of trans-osseous tunnels versus suture anchors reinsertion for the treatment of acute quadriceps tendon rupture.

OBJECTIVE: Quadriceps tendon rupture is a rare disabling injury, predominantly affecting older males. Prompt surgical intervention is essential to restore knee extensor mechanism. Patellar trans-osseous tunnels technique is considered the treatment of choice, however recently suture anchors technique is suggested as a comparative method to restore the integrity of the quadriceps tendon. This study aimed to compare patient-reported outcomes, operative times, and complication rates between trans-osseous tunnel and suture anchor fixation techniques for acute quadriceps tendon rupture. METHODS: A

Limb asymmetry in swimmers and its correlation with sprint performance.

BACKGROUND: Inter-limb strength asymmetry is well-established as an injury risk factor, yet its relationship with athletic performance remains controversial and context-dependent. In swimming-a bilateral cyclical sport theoretically requiring symmetrical force production-the characteristics and performance implications of strength asymmetry remain insufficiently understood, particularly regarding contraction velocity and sex. OBJECTIVE: This study aimed to: (1) characterize shoulder and hip isokinetic strength asymmetry in competitive swimmers at 60°/s and 180°/s; (2) explore sex differences

Sport-Specific Considerations in ACL Reconstruction: Diagnostic Evaluation and Graft Selection.

Knee biomechanical demands vary across different sports due to sport- and position-specific patterns of muscle recruitment. To return to performance, athletes must adequately restore knee kinematics to regain control over the same sport mechanics that led to the initial anterior cruciate ligament (ACL) injury. ACL graft selection should therefore minimize donor site morbidity and support sport-specific demands. This study aims to address the available evidence and guide surgical graft choice in athletes. A literature search of PubMed, MEDLINE, Scopus, and Web of Science (up to September 2025)
