Spinal cord injury (SCI) causes muscle weakness or paralysis, with functional deficits partly due to motor neuron damage near or below the injury level. Understanding how damage affects the morphology and function of motor neurons and the muscle fibers they innervate (i.e., motor units [MUs]) is important for characterizing motor pathophysiology. We assessed MU electrophysiological properties in muscles innervated near the injury zone (anconeus: C7; extensor indicis [EI]: C8-T1) in motor-incomplete cervical SCI and controls. The EI of 14 SCI participants (48.2 ± 14.6 years; 1 female; C2:1, C4:

Purpose: Floating knee (FK) injuries are complex high-energy traumas associated with poor functional outcomes. This study aimed to identify independent predictors of functional prognosis and to develop a novel, multidimensional scoring system to predict long-term functional outcomes. Methods: A retrospective analysis was performed on 182 adult patients with ipsilateral femur and tibia fractures treated between January 2010 and December 2023. Functional outcomes were assessed using the Karlström-Olerud criteria and dichotomized as excellent-good versus fair-poor. Variables significant in univar

Background and Objectives: Anterior cruciate ligament (ACL) reconstruction (ACLR) is widely performed to restore knee stability and facilitate return to activity. However, recovery of muscle strength, balance, functional performance, and patient-reported outcomes does not occur uniformly over time. The longitudinal recovery trajectory across various functional areas during the first year after ACLR remains insufficiently characterized. Materials and Methods: We included 107 patients who underwent isolated unilateral ACLR using a hamstring autograft. Isokinetic knee extensor and flexor strength

BackgroundThe knee is one of the most common areas to suffer injuries or be affected by surgery. Physiotherapy rehabilitation was shown to support recovery, but evidence guiding optimal rehabilitation practices is limited. To recommend appropriate exercises, it is essential to understand the musculoskeletal requirements involved in both physiotherapy and activities of daily living (ADLs).ObjectiveThis study aimed to evaluate and compare the knee joint kinematics, joint forces and muscle activity in knee flexors and extensors during selected rehabilitation exercises and ADLs.MethodsKinematic an

INTRODUCTION: Extensor pollicis longus (EPL) tendon rupture is an uncommon but serious complication following nondisplaced or minimally displaced distal radius fractures.
CASE PRESENTATION: We present two cases of delayed EPL tendon rupture. A 61-year-old woman and a 41-year-old man both developed inability to extend the thumb several weeks after conservative treatment of minimally displaced distal radius fractures. Intraoperative findings revealed poor tendon quality and significant retraction. Both patients were managed with palmaris longus tendon graft using the Pulvertaft weave technique.

BACKGROUND: Periprosthetic patellar fractures (PPPFs) following total knee arthroplasty (TKA) remain a challenging complication with high rates of fixation failure and functional impairment. Optimal surgical management for type II PPPF remains unclear. This study compared outcomes between transtibial tension band wiring (TTTBW) and heavy suture fixation (HSF).
METHODS: A retrospective review identified 18 patients with type II PPPF treated between 2020 and 2023 (HSF, n = 12; TTTBW, n = 6). Primary outcomes included implant survival and extensor mechanism function, assessed by extension lag, f

BACKGROUND: The height of the foot arch is highly related to knee joint function. Knee joint strength is a significant contributing factor to knee joint injuries, while existing research has not yet clearly established the correlation between arch height and knee joint strength.
OBJECTIVE: This study aims to investigate the influence of arch height on isokinetic knee muscle strength at varying angular velocities, thereby providing a potential scientific basis for the prevention and rehabilitation of associated conditions.
DESIGN: Cross-sectional study.
PARTICIPANTS: Sixty healthy young adul

OBJECTIVE: To compare the effects of initiating a rehabilitation program with open kinetic chain (OKC) versus closed kinetic chain (CKC) exercises on clinical outcomes in degenerative meniscal tears (DMT).
DESIGN: Fifty-six patients with DMT were randomized to an 8-week one-on-one synchronous telerehabilitation program. For the first 2 weeks, rehabilitation was initiated with either OKC or CKC exercises. From weeks 3 to 8, both groups performed strengthening exercises targeting their assigned kinetic chain alongside identical functional exercises. The primary outcome was the Knee Injury and O

INTRODUCTION: Chronic anterolateral ankle pain in young athletes is commonly attributed to ligament sprains, stress injuries, or tendinopathies. However, uncommon underlying pathologies may occasionally be responsible for persistent symptoms. Calcaneonavicular coalition is a congenital condition that can lead to chronic ankle pain and restricted hindfoot motion. The coexistence of calcaneonavicular coalition with a superficial ankle lipoma is extremely rare.
CASE REPORT: An 18-year-old female cricket athlete presented with gradually progressive swelling and pain over the anterolateral aspect

INTRODUCTION: Neglected patellar tendon ruptures are uncommon injuries that result in disruption of the knee extensor mechanism. Their diagnosis is often challenging, and their management remains technically demanding, frequently necessitating advanced reconstructive procedures.
CASE REPORT: We report three cases of neglected patellar tendon ruptures in patients over 40 years of age. Diagnosis was based on clinical examination and imaging findings. All patients underwent surgical repair of the tendon reinforced with a protective steel wire frame. Postoperatively, immobilization was achieved w

Modern clinical practice has become increasingly reliant on imaging modalities to diagnose the extent of soft tissue injuries. We describe and then validate a novel bedside clinical test, the acute lower limb activation mechanism test (ALAM), that can be used to identify an extensor mechanism rupture in those unable to straight leg raise (SLR). Patients who presented with an acute knee injury and were unable to SLR were identified over a 6-year period. Patients meeting the inclusion criteria were examined utilizing the ALAM test and diagnosis subsequently confirmed with ultrasound, magnetic re

Isolated management of metacarpophalangeal (MCP) joint injuries or sagittal band disruptions often leads to incomplete symptom resolution and suboptimal functional outcomes, particularly in complex trauma. We present a combined surgical technique integrating MCP joint arthroscopy with open sagittal hood repair for traumatic extensor mechanism instability and intra-articular pathology. This paper describes a stepwise approach to address both intra-articular and soft tissue components of MCP joint injuries. Arthroscopy facilitates direct visualization, synovectomy, debridement, and capsular shri

In precision sports like archery, muscle control, fatigue reduction, and efficient movement execution are essential to an athlete’s performance. This study aimed to (i) analyze variations in muscle tone, stiffness, elasticity, creep, and relaxation at baseline, after archery shooting, and at a final post-session time point obtained after a standardized 30-min rest period followed by a manual-therapy session, and (ii) examine the relationship between acute fatigue after 100 shots and the magnitude of subsequent post-session change scores (not treatment effects). Twenty experienced archery athle

Simultaneous bilateral patellar tendon rupture is an exceedingly rare injury, often linked to systemic comorbidities. This report presents the case of a 31-year-old active male with a history of hypothyroidism, asthma, and chronic anabolic steroid use who sustained bilateral ruptures while running. He underwent urgent surgical repair using intratendinous sutures augmented with a double-bundle ipsilateral gracilis autograft. Postoperatively, a structured rehabilitation protocol was followed. At the 1-year follow-up, the patient achieved an excellent clinical outcome, with an IKDC score of 92.3

Extensor hallucis longus (EHL) tendon rupture is an uncommon injury that usually results from trauma and often requires surgical intervention. In the subacute phase, primary tendon repair becomes challenging because of tendon retraction, and tendon transfer or graft reconstruction is often necessary. Anatomical variations of the EHL, particularly the extensor hallucis capsularis (EHC), may influence tendon retraction and reparability, although their clinical significance has rarely been reported. We present a rare case of subacute EHL rupture in which primary end-to-end repair was feasible owi

BACKGROUND: Diagnosing simple valgus instability of the elbow currently involves time- and cost-intensive imaging modalities such as magnetic resonance imaging or magnetic resonance arthrography. Previous studies have demonstrated that stress radiography represents an alternative diagnostic tool for such conditions. The aim of this study was to investigate whether standardized valgus stress radiography can identify soft-tissue lesions of the medial elbow.
METHODS: A telos stress device (telos GAIII/E; telos Arzt- und Krankenhausbedarf GmbH, Woelfersheim-Bernstadt, Germany) was used to apply 5

The aging process is associated with many functional decrements. It is important to consider the additive acute functional decreases that may occur following eccentric exercise-induced muscle damage (EIMD). This study aimed to examine the effect of age on muscle damage metrics following EIMD. Ten younger (age=20±2 years, BMI=22.49±3.97 kg/m2) and six older (age=68±2 years, BMI=24.45±2.82 kg/m2) females participated in this study. Participants performed a maximal knee extensor EIMD protocol. Additionally, participants completed various assessments before, immediately after, 24-, 48-, and 72-hou

BACKGROUND: The knee arthroplasty terrible triad (TT) is a combination of periprosthetic joint infection, extensor mechanism insufficiency, and soft tissue defect. The TT is a limb-threatening condition that has historically poor functional results and high probability of amputation or fusion. The rotational gastrocnemius-Achilles rotational flap (GARF) holds promise as native soft tissue to provide coverage and muscular power to assist with knee extension. We hypothesized that application of the GARF in conjunction with two-stage revision improves functional and limb-salvage outcomes compared
