BACKGROUND: Inferior pole patellar fractures are difficult to treat because fixation must restore extensor continuity while minimizing hardware-related complications. This study compared the clinical outcomes of tension band wiring (TBW) and suture anchor fixation (SAF).
METHODS: We retrospectively reviewed 45 adult patients with inferior pole patellar fractures treated between September 2020 and September 2023. Twenty-nine patients underwent TBW and 16 underwent SAF. The primary outcomes were fracture union time, postoperative complications, maximum knee flexion at 12 months, and Bostman pat

The effects of percutaneous spinal cord epidural stimulation (SCES) on motor control have yet to be elucidated in persons with spinal cord injury (SCI). We examined the effects of percutaneous SCES on the central and peripheral nervous systems after SCI. We determined the effect of SCES combined with exoskeletal assisted walking (EAW) and resistance training (RT) on motor performance as assessed by the 10 m-walking test, electromyography outcomes and spasticity measurement. Four chronic (6-24 years post-injury) motor complete (C8-T11 AIS A and B) persons with SCI underwent permanent implantati

INTRODUCTION: Patellar tendon rupture is an uncommon but disabling injury, often resulting from eccentric quadriceps contraction or trauma. Chronic ruptures (>3 weeks) present a reconstructive challenge due to patellar migration, scarring, and tendon retraction, making primary repair inadequate. Various graft options exist; autologous semitendinosus provides a strong, biologically compatible solution with minimal morbidity.
CASE REPORT: A 42-year-old man presented 8 weeks post-injury with left knee instability and inability to extend the leg following a fall. Examination revealed patella alta

PURPOSE: Knee dislocations (KDs) can be limb-threatening injuries that may require a temporising knee-spanning external fixator (KSEF) for stabilisation. Precise indications for this commonly utilised invasive immobilisation technique remain controversial and poorly defined. The purpose of this study was to establish consensus-driven indications for temporising KSEF use in the initial management of KDs.
METHODS: A working group of fellowship-trained orthopaedic surgeons generated clinical scenarios reflecting commonly debated indications for temporising KSEF application. Utilising a modified

Traumatic mallet hallux, an avulsion injury of the extensor hallucis longus tendon or its bony insertion, is rare and often underdiagnosed, with no established treatment guidelines. We report two cases illustrating distinct management approaches based on fragment size and interphalangeal (IP) joint congruency. The first case - a nondisplaced avulsion fracture involving 30% of the articular surface - was successfully treated conservatively with extension splinting, achieving full recovery at three months. The second case - a 50% articular fracture with volar subluxation - required open reductio

OBJECTIVE: To investigate whether early postoperative shear wave elastography (SWE) parameters-specifically absolute stiffness and novel elasticity ratios reflecting regional stiffness gradients-can serve as early biomechanical proxies to predict 12-week functional outcomes.
METHODS: This prospective study included 65 patients (81 tendons) who underwent extensor tendon surgical repair. Conventional ultrasound and SWE were performed one week postoperatively. Absolute shear wave velocities (SWV) were measured at the distal tendon (ROI-1), suture site (ROI-2), and proximal tendon (ROI-3). Elasti

BACKGROUND: Preoperative and postoperative asymmetries in knee kinematics and kinetics, particularly in vertical ground-reaction force (vGRF), have been linked to long-term functional outcomes after anterior cruciate ligament (ACL) reconstruction. Gait asymmetry may indicate altered neuromuscular control and compensatory cocontraction strategies, which can compromise joint integrity and delay recovery.
PURPOSE: To determine whether preoperative vGRF asymmetry could predict postoperative knee extensor strength symmetry and neuromuscular recovery 1 year after ACL reconstruction.
STUDY DESIGN:

PURPOSE: Cross-sectional studies indicate that sprint performance is strongly associated with leg-muscle strength, however there is no consensus on which muscles to strengthen to maximize running speed. The purpose of this study was to quantify and rank the sensitivity of maximum sprinting speed to changes in the strengths of individual leg muscles.
METHODS: A full-body musculoskeletal model was combined with dynamic optimization theory to predict the effects of muscle strengthening on maximum sprinting speed.
RESULTS: Maximum sprinting speed was most sensitive to a change in hip muscle stre

INTRODUCTION/OBJECTIVES: Patellofemoral pain (PFP) is frequently reported following anterior cruciate ligament reconstruction (ACLR). The tibiofemoral rotation angle (TFRA) may influence patellofemoral joint biomechanics as it forms part of the knee extensor mechanism. This study aimed to compare TFRA and other related parameters before and after anatomic single-bundle (SB) ACLR, and to evaluate the relationship between post-operative TFRA, PFP, and knee function.
METHODS: Sixty-five patients who underwent anatomic SB-ACLR between 2012 and 2023 were retrospectively reviewed. TFRA, tibial tube

BACKGROUND: Hand injuries are common traumatic conditions that often require specialized surgical care. Differences in hospital structures and levels of specialization may influence the timeliness and complexity of treatment. This study evaluates quality of care across different hospital types, focusing on time-to-skin-incision and surgical management of finger and hand injuries.
MATERIALS AND METHODS: This retrospective multicenter study is based on data from the HandTraumaRegister of the German Society for Hand Surgery (HTR DGH). A total of 16,726 surgically treated cases with documented fi

BACKGROUND: Scapular dyskinesis (SD) is a prevalent dysfunction among overhead athletes. Although the kinetic chain emphasizes the functional linkage of the core and shoulder musculature, limited evidence exists regarding the endurance and electromyographic (EMG) characteristics of core muscles in athletes with SD. This study aimed to compare core muscle endurance and EMG activity during closed kinetic chain tasks between overhead athletes with and without SD.
METHODS: Thirty female overhead athletes were divided into two groups: SD (n = 15) and without SD (n = 15), diagnosed using the Kibler

BACKGROUND: Extensor mechanism disruption following total knee arthroplasty (TKA) significantly impairs knee function and quality of life. Extensor mechanism reconstruction (EMR) is effective but carries risks, including post-operative falls due to persistent quadriceps weakness and extensor lag. We sought out to answer the following four questions: What is the incidence of traumatic events following EMR after TKA using different grafting and reconstruction techniques? Are there significant differences in the rate of traumatic events based on the type of graft used (allograft vs. mesh graft) i

Multiligament knee injuries (MLKIs) are rare but severe injuries involving bicruciate or collateral ligament disruption, frequently associated with knee dislocation, fractures, and neurovascular compromise. Vascular injury occurs in a mean of approximately 18% of cases and may be present despite palpable pulses; an ankle-brachial index (ABI) < 0.9 demonstrates high sensitivity for arterial injury. Peroneal nerve injury occurs in approximately 10-40% of cases. Early recognition and structured evaluation are critical. Serial vascular examinations, selective CT angiography, and careful neurologic

BACKGROUND: Release of the gluteus maximus tendon during a total hip arthroplasty (THA) can lead to gluteal dysfunction. Sciatic nerve injury, while rare, remains a serious complication of THA. During deep hip flexion, release of the tendon increases the distance between the femoral neck and the sciatic nerve, potentially reducing the risk of nerve injury. The functional consequences of such a release have yet to be investigated. The aim of this study was to evaluate the effects of gluteus maximus tendon release on the development of gluteal dysfunction using clinical, radiological, and kinema

INTRODUCTION: Bilateral patellar tendon rupture is an uncommon injury, particularly in young, healthy individuals without established risk factors for tendinopathy. Most reported cases involve systemic disease or medication-related tendon compromise. We present a rare case of simultaneous bilateral patellar tendon rupture in a healthy, athletic male following an indirect mechanism of injury.
CASE PRESENTATION: A 33-year-old man sustained acute bilateral knee injuries while playing recreational rugby. He reported an audible "pop" in both knees, immediate pain, and inability to extend either le

CASE: A 53-year-old man sustained a severe open distal radius and ulna fracture with approximately 7-cm osteoarticular bone loss, extensive extensor musculotendinous junction avulsions, and radial nerve disruption. Acute 3-cm forearm shortening restored tendon tension and reduced defect size, enabling primary tendon reconstruction with pull-in sutures and nerve repair. Early staged free vascularized fibular grafting was performed. Despite postoperative Pseudomonas aeruginosa infection requiring implant removal and local antibiotics, solid union was achieved with preserved forearm rotation at 2

Knee dislocations and multiligament knee injuries represent a significant clinical and surgical challenge, particularly when associated with patellar tendon rupture, a rare combination with a high functional impact that may initially go unrecognized. Management of these injuries requires meticulous planning to restore anatomy, joint stability, and extensor mechanism function in order to optimize long-term functional outcomes. We report the case of a patient who sustained a knee dislocation associated with patellar tendon rupture, which was surgically managed using a two-stage approach. Current
