Laryngohyoid fractures after agonal falls: not always a certain sign of strangulation.

Haemorrhagic fractures of the thyroid cartilage and hyoid bone are frequently observed in cases of strangulation and often regarded as evidence for an assault against the neck. In contrast, two cases of laryngohyoid fractures after agonal falls in prone position are presented to draw attention to alternative causes of these injuries with special regard to practical medicolegal casework. A 45-year-old man collapsed at a fairground and died after unsuccessful resuscitation. He showed excoriations at his elbows and right knee, a crush injury at the mentum and his mandibular front teeth were knock

One-stage reconstruction of both the biceps brachii and triceps brachii tendons using a free anterolateral thigh flap with a fascial flap.

An avulsed elbow with severe degloving and crush injury was reconstructed in a 66-year-old man. The biceps brachii was completely transected, and the triceps brachii tendon was partly transected. In addition, the skin around the elbow had become totally necrotic. Four weeks after the injury, a free anterolateral thigh flap with a fascial flap was transplanted to reconstruct both the biceps brachii and triceps brachii tendons simultaneously. Six months after the initial injury, the range of elbow motion had recovered to almost the same level as that before injury.

Traumatic major muscle loss in the upper extremity: reconstruction using functioning free muscle transplantation.

Traumatic crushing injuries of the muscle compartments will occasionally cause major muscle or muscle-group loss with concomitant irreparable nerve injuries. Those cases with or without the availability of local muscle transfers form another indication for functioning free muscle transplantation (FFMT). The authors report their experience with 20 FFMTs in 14 patients with severe muscle loss from 1986 to 1996. They were selected because all their major muscle loss was primarily due to muscle avulsion, avascularity, and necrosis, but not secondarily due to nerve injury or ischemic injury. The mo

[The problems in replantation of limbs amputated through the arm region].

The problems in replantation of arms were analysed on the ground of nine such replantations performed in Center of Replantations of Limbs in Trzebnica in the same number of patients (7 men, 2 children, 1 woman) during the seven year period 1993-2000. The range of age of the patients was from 12 to 62 years (35.5 on average). Among the problems discussed were such as qualification for the operation, operative technique (limb shortening, perfusion of vessels, half-open anastomosis of veins, neurotisation of remaining nerves) evaluation scale. Eight of the amputations were the results of crush-av

Case report: Myofascial pain syndrome: a double crush-like appearance.

OBJECTIVE: To discuss the case of a patient with myofascial pain syndrome that appeared similar to double crush syndrome. CLINICAL FEATURES: This is a case report of a 45-year old female who presents to the private practice clinic with the same signs and symptoms of carpal tunnel syndrome, 1 year post-carpaltunnel surgery. The history is consistent with injuries that result in double crush syndrome and thoracic outlet syndrome. The patient history includes a motor vehicle accident (MVA) in 1963 that resulted in a prolonged hospital stay and fracture of a cervical vertebral (C4) body. She frac

Isolated posttraumatic posterior dislocation of the radial head in an adult.

Isolated posterior dislocation of the radial head without concomitant ulnar fracture or humeroulnar subluxation is a rare injury with few reports in the literature. We describe a case of a 27-year-old man who sustained a traction injury to the right elbow and crush injury to the forearm with subsequent posterior dislocation of the radial head. The dislocation was treated by manual reduction under anesthesia, surgical repair of the annular ligament, and subsequent rehabilitation for 6 weeks in a range-of-motion brace with the forearm in pronation. To our knowledge, this is the only case to be t

[Reconstruction with vein graft interposition and pedicled muscle flap in a patient with above.elbow partial arm amputation].

It is difficult to decide replantation or revascularization in partial or total amputations proximal to the wrist. Important point is the expected functional result and/ or protection of sensibility. Crush type partial or total amputations have poor prognosis. In addition to the vascular injury there are also broad cutaneous and muscular injury which have a potential risk for necrosis and infection. And this may threat the patient's life. In this case report, as two major nerves seemed intact, arterial reconstruction by a vein graft 22 cm. long and protection of important structures such as ve

Traction avulsion amputation of the major upper limb: a proposed new classification, guidelines for acute management, and strategies for secondary reconstruction.

Major replantation of a traction avulsion amputation is undertaken with the goal of not only the reestablishment of circulation, but also functional outcome. This type of amputation is characterized by different levels of soft-tissue divisions involving crushing, traction, and avulsion injuries to various structures. Between 1985 and 1998, 27 cases were referred for secondary reconstruction following amputation of the upper extremity involving both arm and forearm. Replantation was performed by at least 12 qualified plastic surgeons using different approaches and management, resulting in diffe

Cervical myeloradiculopathy with entrapment neuropathy: a study based on the double-crush concept.

In an attempt to formulate a standardised approach to the diagnosis and management of patients with the double-crush syndrome, we reviewed 65 surgical cases (39 men and 26 women) with cervical myeloradiculopathy associated with entrapment neuropathy in the arm by examining the clinical features, results of nerve conduction velocity studies and distribution of the vertebral levels of compression. Fifty-three patients (average age, 43 years) presented with a carpal tunnel syndrome at the wrist and cervical lesions (41 with spondylosis and 12 with ossified posterior longitudinal ligament), while

Distribution of primary motor nerve branches and terminal nerve entry points to the forearm muscles.

BACKGROUND: The information available on innervation pattern of the human forearm muscles in standard anatomy texts, although adequate for routine procedures, is not detailed enough for surgical reconstruction in complex injuries of the limb and for paralytic conditions of the forearm from peripheral nerve and spinal cord injuries. METHODS: The innervation pattern in 10 cadaveric forearms was studied. The contributions of the main nerve trunks to each forearm muscle was examined. The location and number of the primary motor nerve branching points and of the terminal nerve entry points to each

Major limb replantation: a Thai experience.

From May 1986 to December 1992, we performed 36 major limb replantations in 35 patients; 30 were complete and 6 were incomplete amputations. Twenty-eight of these patients were male. Their ages ranged from 5 to 81 years, the average being 22 years. The levels of injury were 13 hemihand, 8 wrist, 5 lower forearm, 5 upper forearm, and 5 above the elbow amputation. The degrees of injury were 8 guillotine, 14 local crush, 9 diffuse crush, and 5 avulsion amputation. There were a partial loss of digits in 4 hemihand amputations and one failure in an above-elbow amputation--a limb survival rate of 95

[Surgical therapy of radial head fracture--results of osteosynthesis and resection treatment].

Between 1982 and 1993 we performed internal fixation or resection in 95 adult patients for radial head fracture. It was possible to follow up 68 patients for an average of 2.1 years. In 53 cases the initial injury was an isolated radial head fracture, which was treated by internal fixation in 41 and resection in 12 cases. Good results were obtained in 80% of patients. A satisfactory outcome was associated with young age, little initial fragment dislocation, internal fixation with screw + K-wire, and early postoperative physiotherapy. Primary resection for crush fractures of the radial head did

Nerve problems and compartment syndromes in the hand, wrist, and forearm.

Most sports-related peripheral neuropathies occurring from the elbow distally are compressive in nature. These result from overuse or overload principles superimposed on normal or variant anatomy. Tensile injury occurs less often and is usually associated with the extremes of the throwing motion, i.e., cocking and follow-through phases of throwing. Neurogenic syndromes are usually incomplete, indicating the absence of severe motor or sensory deficits, but typically with subjective complaints of pain or vague sensory disturbance. As a result, nerve injuries are frequently overlooked as a source

Early upper-extremity prosthetic fit in patients with burns.

Patients with upper-extremity amputations necessitated by burn injury have frequently faced delays in prosthetic fit. At the Regional Medical Center, Memphis, Tennessee, seven patients required amputations because of burns. These injuries were electrical in four cases, thermal and crush in one case, thermal in one case, and steam and crush in one case. Five patients had below-elbow amputations, one had a bilateral below-elbow amputation, and one had a bilateral above-elbow amputation. All patients were fitted with prostheses within 30 days of the last definitive surgery on the amputated extrem

[Transpalmar transplantation of a severed hand to the opposite side: a case report].

A man of 20 got both his upper limbs badly injured by an accident on May 28, 1989. Crush of all the 4 fingers of right hand, traumatic amputation through the left arm with severe damage of soft tissue and burst fracture of bones around left elbow. And so, the chance of providing him a useful hand could only be transpalmar transplantation of the undamaged left hand to the opposite side. This was done urgently to reconstruct a new hand, a hand with its fingers in a reversed order. The wound healed up nicely, the transplanted fore-hand was well taken and the new hand functions satisfactorily, tho

Electrical fascicle identification as an adjunct to nerve repair.

In view of the tragic consequences of median nerve and/or ulnar nerve paralysis and the biologic problems inherent in nerve healing, it seems logical that the surgeon should make a serious effort to control those problems that lie within his or her power: to identify and align those principal sensory and motor pathways in the severed nerve ends. I find that this is feasible. For over 15 years, I have used the electrical fascicle identification method, repairing 49 major upper extremity nerves. (A single radial nerve case was lost to follow-up.) As with any new technique, problems have been ide

Anterior submuscular transposition of the ulnar nerves by the Learmonth technique.

Thirty-eight patients with progressive posttraumatic ulnar neuropathy at the elbow underwent anterior submuscular transposition of their nerves. Multiple mechanical causes of neuropathy were identified. Fourteen patients had undergone previous surgery for ulnar neuropathy, while 24 had not. Postsurgical follow-up averaged 23.1 months. The operative technique is described and illustrated in detail. Complications attributable to surgery were minimal. No absolute prognostic factors could be identified, and even those patients with significant muscular atrophy or time delay before operation were g

Management of severe forearm injuries.

A review of principles and an operative guideline for repair of severely mutilating injuries to the forearm have been set forth. These concepts and their application have been illustrated in a series of clinical cases. The following key concepts have developed from these clinical experiences: 1. The surgeon must evaluate each case based upon the potential for return of sensation and function. One cannot justify the statement that a replanted arm is always superior to a prosthesis, even if its only purpose is cosmetic. 2. Care must be taken in the emergency room to evaluate the entire patient,

The problems in replantation of limbs amputated through the upper arm region.

We performed three replantations at the upper arm: two patients had severely crushed arms and one patient was at advanced age (50 years). One replanted arm was reamputated because of severe neuralgia 1 year, 3 months after replantation. In the other two cases, sensation was recovered to fingertips and powerful motion of the elbow joint was gained. However, pinch or grasp was impossible because of insufficient reinnervation to the extensor muscles. In replantation at the proximal region, recovery of motor function generally tends to be poor especially in a crush injury or in the case of advance
