BACKGROUND: Emergency free flap reconstruction (EFFR) involves flap coverage performed during initial operative intervention or within 24 h after traumatic injury. Although EFFR offers notable advantages, standardized criteria for its application are not well established. This manuscript proposes a treatment framework outlining three primary indications for EFFR, including the use of viable tissue from spare parts.
METHODS: We retrospectively analyzed 14 patients (15 flaps) with traumatic limb-threatening injuries treated with EFFR across two academic centers from 2016 to 2024. Data collected

Since war injuries are not predictable, each patient presents as a new and unique case. Before deciding on amputation for upper extremity injuries, it is necessary to evaluate all limb-sparing vascular interventions that will keep the amputation level as distal as possible, in order to improve the patient's quality of life and enable the use of more functional prostheses. While amputation may be a faster and more life-saving option for a surgeon, limb-sparing and mobile joint-preserving surgical procedures should be the preferred choice, provided the patient's overall condition permits. Background: Brachial artery injuries are common in upper limb trauma and require timely repair to prevent ischemia and limb loss. In developing countries like India, delayed presentation is frequent due to limited access to specialised care, inadequate infrastructure and delays in multidisciplinary coordination, complicating timely revascularisation. This study aimed to evaluate limb salvage outcomes and factors influencing prognosis in patients undergoing delayed repair of traumatic brachial artery injuries. Methods: This prospective study included 30 male patients (ages 18-58 years) with tra

Upper limb amputation leads to significant physical and psychosocial burden; however, it remains underrepresented in global epidemiological and rehabilitation studies. This study aimed to systematically review the global epidemiological data on upper limb amputation, focusing on its prevalence, incidence, anatomical levels, and etiology. According to the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines, a comprehensive literature search was conducted using PubMed, Embase, Cochrane Library, and Regional Information Sharing Systems (up to May 2023). The treatment of traumatic major upper limb amputation is complex and of great urgency. Loss of time often represents a majorrestriction for replantation. Thus, logistical and infrastructural developments, such as the expansion of specialised hand trauma centres, are crucial for optimizing delivery of care. Surveillance represents the fundament for a proper, demand-adapted implementation of such therapeutical improvements. However, a comprehensive database for surveillance of these injuries does currently not exist in Germany or Europe. In this study quality reports of German hospitals from 20

Although there have been numerous reports of major replantation of upper extremity amputations, limited numbers of above-elbow amputation replantation have been reported. We present the technical details of two successful replantations of forequarter amputations in a nine-year-old girl and a three-year-old boy. In both cases, the forequarter was amputated due to avulsion traction injuries resulting in amputation including the entire upper limb, while the integrity of the scapula and parascapular muscles was maintained, with no injury to the glenohumeral joint. Replantation was performed, invol

STUDY DESIGN: Retrospective cohort study INTRODUCTION: Service members who have sustained traumatic amputations are typically young and otherwise healthy. Beyond standard care, these individuals desire long, highly active, and relatively pain-free lifestyle, whether that is returning to active duty or transitioning to civilian life. Development of overuse musculoskeletal conditions could have a significant influence on quality of life for Service members with traumatic upper limb amputation.
PURPOSE OF THE STUDY: Compare one-year incidence of overuse musculoskeletal injuries in Service member

BACKGROUND RESULTS: of replantation surgery following upper extremity traumatic amputation are extensively described in the literature, with success rates varying from 57 to 100 percent. The purpose of this study was to evaluate replantation success rate at a Level I trauma center over a 17-year period and to assess definable factors contributing to these results.
METHODS: A retrospective review of all digit and hand replantations at a Level I trauma center was performed using CPT codes from 2001 through 2018. Descriptive analyses, Mann-Whitney test, Kruskal-Wallis test, and logistic regressi

People with proximal transhumeral amputation usually choose prosthesis as an alternative to improve occupational performance, but frequently presenting difficulties in the distal control, poor elevation to reach some object, and tolerance to use in ADL and day time, added to frustration and high perception of disability. This report described the results in the first experience with one patient who suffered a work accident in Chile in a long-term following up 7 years after discharge, who take the choice for osteointegrated prosthesis training for amputation at the proximal transhumeral level a

BACKGROUND: Traumatic upper limb amputations proximal to the carpus are devastating injuries. Existing literature on outcomes following replantation is limited. Our objective was to perform a scoping review of (1) functional outcomes; (2) return to work data; and (3) secondary surgeries required following proximal to carpus replantation.
METHODS: A literature search of MEDLINE, EMBASE, Cochrane, and CINAHL was performed according to PRISMA guidelines. All studies reporting on functional outcomes, return to work, or secondary surgeries following replant of traumatic proximal to carpus amputati

BACKGROUND: Upper extremity amputations are usually not life-threatening, but they negatively affect the life quality of the victim. In addition to the functional disabilities of upper extremity amputation, disfigurements frequently cause psychological and social debilitations.
METHODS: Between 2007-2015, fourteen cases were admitted to emergency with total major amputation of the upper extremity. All cases were male (22-45 years of age. Mean age: 29.6). Replantation was applied to all except three cases with multileveled crush injuries.
RESULTS: All replantations were successful. Additional

Amputations associated with trauma carry significant morbidity in addition to risk of limb loss due to tissue damage, contamination, and risk of infection. The timing of formal amputation closure in this situation varies among surgeons, with some allowing the wound to remain open for some time after debridement, whereas others perform formal amputation and closure at the index operation if debridement is adequate. The presence of infection after closure of an amputation has the potential to result in a higher amputation (i.e., a below-knee amputation to an above-knee amputation). Our goal was

BACKGROUND: Although the distal targets have been lost in proximal upper limb amputees, the neural signals for intuitive hand and arm function are still available and thus can be incorporated into more useful prosthetic function using targeted muscle reinnervation technique. In this article, the authors present their outcomes and range of indications in addition to experiences and pitfalls after 30 targeted muscle reinnervation cases at above-elbow and shoulder disarticulation level of amputation.
METHODS: Thirty patients with above-elbow or shoulder disarticulation amputations were enrolled

BACKGROUND: Theoretically, patients with only one functional arm secondary to contralateral amputation or paralysis will subject their only functional upper extremity to increased loads. This could become an issue after reverse shoulder arthroplasty (RSA). However, there are no reported data on the implant survival or function for patients with a nonfunctional contralateral upper extremity.
OBJECTIVE: To report the outcomes of RSA in patients with contralateral upper extremity amputation or paralysis.
DESIGN: Retrospective case series.
SETTING: Tertiary university hospital.

BACKGROUND: Traumatic arm amputations can be treated with replantation or surgical formalization of the stump with or without subsequent prosthetic fitting. In the literature, many authors suggest the superiority of replantation. This systematic review compared available literature to analyze whether replantation is functionally and psychologically more profitable than formalization and prosthetic fitting in patients with traumatic arm amputation.
METHODS: Functional outcome and satisfaction levels were recorded of patients with amputation levels below elbow, through elbow, and above elbow.

Sarcoma development has been associated with genetics, irradiation, viral infections, and immunodeficiency. Reports of sarcomas arising in the setting of prior trauma, as in burn scars or fracture sites, are rare. We report a case of a leiomyosarcoma arising in an arm that had previously been replanted at the level of the elbow joint following traumatic amputation when the patient was eight years old. He presented twenty-four years later with a 10.8 cm mass in the replanted arm located on the volar forearm. The tumor was completely resected and pathology examination showed a high-grade, subfas

BACKGROUND AND AIM: Despite undergoing complex brachial plexus, surgical reconstructions, and rehabilitation, some patients request an elective amputation. This study evaluates the role of elective amputation after brachial plexus injury.
METHODS: A retrospective chart review was performed for all the 2140 patients with brachial plexus injuries treated with elective amputation between 1999 and 2012 at a single institution. Analysis was conducted on the potential predisposing factors for amputation, amputation level, and postamputation complications. Patients were evaluated using pre- and post

BACKGROUND: Respiratory management in patients with esophagobronchial fistulae is challenging since positive pressure ventilation (PPV) may not be feasible due to air leaks and possible risks for regurgitation and aspiration of gastric contents. We and others have previously reported that spontaneous respiration may be one of the good options of respiratory management during general anesthesia in those patients. However, adverse events associated with this respiratory strategy have not been reported previously. We experienced a 77-year-old male patient who suffered unexpected impairment of oxy
