The treatment of traumatic major upper limb amputation is complex and of great urgency. Loss of time often represents a majorrestriction for replantation. Thus, logistical and infrastructural developments, such as the expansion of specialised hand trauma centres, are crucial for optimizing delivery of care. Surveillance represents the fundament for a proper, demand-adapted implementation of such therapeutical improvements. However, a comprehensive database for surveillance of these injuries does currently not exist in Germany or Europe. In this study quality reports of German hospitals from 20

BACKGROUND RESULTS: of replantation surgery following upper extremity traumatic amputation are extensively described in the literature, with success rates varying from 57 to 100 percent. The purpose of this study was to evaluate replantation success rate at a Level I trauma center over a 17-year period and to assess definable factors contributing to these results.
METHODS: A retrospective review of all digit and hand replantations at a Level I trauma center was performed using CPT codes from 2001 through 2018. Descriptive analyses, Mann-Whitney test, Kruskal-Wallis test, and logistic regressi

BACKGROUND: Upper extremity amputations are usually not life-threatening, but they negatively affect the life quality of the victim. In addition to the functional disabilities of upper extremity amputation, disfigurements frequently cause psychological and social debilitations.
METHODS: Between 2007-2015, fourteen cases were admitted to emergency with total major amputation of the upper extremity. All cases were male (22-45 years of age. Mean age: 29.6). Replantation was applied to all except three cases with multileveled crush injuries.
RESULTS: All replantations were successful. Additional

BACKGROUND: Respiratory management in patients with esophagobronchial fistulae is challenging since positive pressure ventilation (PPV) may not be feasible due to air leaks and possible risks for regurgitation and aspiration of gastric contents. We and others have previously reported that spontaneous respiration may be one of the good options of respiratory management during general anesthesia in those patients. However, adverse events associated with this respiratory strategy have not been reported previously. We experienced a 77-year-old male patient who suffered unexpected impairment of oxy

BACKGROUND: Little literature currently exists on reconstructive strategies following successful upper extremity replantation. The authors hypothesized that the type of secondary surgery would vary predictably depending on mechanism and amputation level.
METHODS: The authors performed a retrospective review of upper extremity replantations performed at their institution between 2003 and 2012. The mean follow-up period was 3 years. Patient, injury, and surgical demographics, as well as replantation survival rates and secondary surgical procedures, were recorded.
RESULTS: Forty-five upper extr

INTRODUCTION: Micro- or macroreplantation is classified depending on the level of amputation, distal or proximal to the wrist. This study was performed to review our experience in macroreplantation of the upper extremity with special attention to technical considerations and outcomes.
MATERIALS AND METHODS: Between January 1990 and December 2010, 11 patients with a complete amputation of the upper extremity proximal to the wrist were referred for replantations to our department. The patients, one woman and ten men, had a mean age of 43.4 ± 18.2 years (range 19-76 years). There were two elbow,

OBJECTIVES: The aim of this study was to evaluate long-term clinical and functional results of major upper extremity replantations.
METHODS: We retrospectively evaluated 26 male patients (mean age 27 years; range 3 to 69 years) who underwent major upper extremity replantations and had a mean follow-up of 11.3 years (range 5 to 19 years). The levels of the replantations were transmetacarpal (n=6), wrist (n=4), forearm (n=5), elbow (n=4), and arm (n=7). Amputations were of clean-cut, crush, and avulsion types in seven, eight, and 11 patients, respectively. Secondary operations were performed in

During the period 1996-2005, in General and Hand Surgery Department in Szczecin, 47 patients, 45 males and 2 females, with a me an age of 42 years, with total (72%) and subtotal (28%) amputations within the metacarpus and more proximal were operated. 35 replants survived, however 10 (21%) necrotized, in 2 cases (5%) blood flow was not re-established during the operations. The study presents results of assessment of objective parameters of 19 patients who received replantation of amputated extremities at the level of metacarpus - 5 cases, wrist - 9, forearm - 3, elbow - 1 and arm - 1 case. We performed a prospective study in 186 patients with a minimum follow-up of two years in order to attempt to predict successful major re-implantation of the upper limb. There were 137 men and 49 women with 24 amputations of the palm, 75 of the wrist, 50 of the forearm, 9 disarticulations through the elbow, and 28 amputations through the upper arm. The degree of injury to the amputated segment and the stump were good predictors of the rate of success and the final outcome. Adequate preservation, contraction of the muscle in the amputated part after stimulation, the level of injury and a smokin

From May 1986 to December 1992, we performed 36 major limb replantations in 35 patients; 30 were complete and 6 were incomplete amputations. Twenty-eight of these patients were male. Their ages ranged from 5 to 81 years, the average being 22 years. The levels of injury were 13 hemihand, 8 wrist, 5 lower forearm, 5 upper forearm, and 5 above the elbow amputation. The degrees of injury were 8 guillotine, 14 local crush, 9 diffuse crush, and 5 avulsion amputation. There were a partial loss of digits in 4 hemihand amputations and one failure in an above-elbow amputation--a limb survival rate of 95

Seven patients with a complete transhumeral limb amputation had their limb replanted. In all seven limbs the mechanism of injury was avulsion, and the ischemic time was relatively prolonged, with a range of 10 to 14 hours. The five patients with surviving limbs achieved useful elbow control. Of these patients, two achieved useful distal function to the wrist and hand and one had a below elbow amputation. All but one patient required multiple secondary operative procedures, with an average of 2.8 procedures for those patients with surviving limbs. We did not encounter any significant life-threa
