BACKGROUND: Distal biceps tendon ruptures significantly impair forearm flexion and supination strength, particularly in active individuals. Suspensory fixation using cortical button devices is a widely accepted surgical approach. Adjustable-loop techniques allow maximal tendon-to-bone apposition, while fixed-loop methods are simpler but may leave a gap. Biomechanical differences between these techniques specific to the distal biceps tendon remain poorly investigated, especially given the unique loading patterns of the elbow compared to other joints.
METHODS: Finite element analysis was perfor

Distal radius fractures (DRFs) are among the most common upper extremity fractures encountered in clinical practice. Closed reduction followed by external immobilization remains a fundamental treatment approach; however, the procedure is highly operator-dependent and lacks standardization. This article presents a step-by-step protocol for manual reduction and four-splint immobilization using volar, dorsal, radial, and ulnar splints for acute DRFs. The protocol encompasses patient selection, anesthesia administration, patient positioning, sustained longitudinal traction with countertraction, re

Distal radius fracture (DRF) usually adopts static fixation method, which often leads to complications such as abnormal fracture healing and pressure injury. The reason lies in the lack of real-time regulation of fixed pressure and biomechanical feedback, which affects the functional recovery. Our team designed an intelligent and adaptive orthopedic equipment. Based on the patient's personalized three-dimensional (3D) printing architecture, the equipment system realizes light and accurate adaptation, and embeds a series of thin film pressure sensors in key anatomical parts to continuously quan

Microsurgical replantation of simultaneous bilateral upper extremity amputations is a complex procedure requiring a highly coordinated multidisciplinary team. Functional outcomes depend on anatomical level of injury, duration of ischemia, extent of crushing, degree of contamination, and quality of postoperative rehabilitation. We report a 45-year-old male industrial worker who sustained complete simultaneous amputations through the distal third of the right forearm and at the left wrist following an industrial paper-cutting machine accident. Total ischemia time was 8 h. Two microsurgical teams

INTRODUCTION AND IMPORTANCE: Combat-related injuries to the upper limb pose significant reconstructive challenges, particularly in resource-limited, conflict-zone settings. Innovative strategies are essential to preserve hand function, and the choice of an optimal reconstruction remains complex. Temporary wound dressings must foster healing and protect vital structures during the interval before definitive closure. This report evaluates the effectiveness of a tensor fascia lata (TFL) graft combined with negative-pressure wound therapy (NPWT) as a temporary dressing for complex wounds in auster

BACKGROUND: Side-by-side all-terrain vehicle (ATV) crashes produce high-energy upper-extremity trauma requiring complex reconstruction. Despite the increasing prevalence of these injuries, characterization of injury patterns, management, and outcomes remains limited.
METHODS: A multicenter retrospective review was performed across 9 level I trauma centers from 2019 to 2023. Adult patients with upper-extremity injuries from side-by-side ATV crashes who were evaluated by the hand surgery service were identified through a trauma registry and manual chart review. Demographics, injury characterist

BACKGROUND: The pull-in suture technique is a method of primary tendon repair that involves pulling the distal stump of the ruptured tendon into the proximal muscle belly and suturing it to the intact muscle fascia for avulsion injuries at the musculotendinous junctions associated with severe open forearm trauma. The aim of this study is to report the clinical outcomes of the pull-in suture technique for musculotendinous avulsion injuries associated with severe open forearm trauma and to evaluate its effectiveness.
METHODS: This study included patients who underwent pull-in suture repair for

BACKGROUND: Distal radius fractures are among the most common surgically treated upper-extremity fractures, and volar locking plate fixation is the predominant operative technique. Although perioperative antibiotic prophylaxis is widely used in orthopaedic surgery, procedure-specific evidence for closed distal radius fractures remains limited. Contemporary data regarding postoperative infection rates and antibiotic prophylaxis practice following volar locking plate fixation are scarce. The aim of this study was to describe postoperative infection rates following volar locking plate fixation of

BACKGROUND: Extensive upper extremity soft-tissue defects with exposed vital structures present a significant reconstructive challenge, particularly when microsurgical options are unavailable. Traditional distant pedicled abdominal flaps require a wide skin tube to protect the vascular pedicle, resulting in substantial donor-site morbidity and risks of pedicle compression. This study introduces a modified technique utilizing a hybrid combination of artificial dermis and local skin flap to create a hybrid-enveloped pedicle, aiming to minimize donor-site sacrifice and improve postoperative manag

INTRODUCTION AND IMPORTANCE: Monteggia equivalent fractures, a spectrum of proximal ulnar and radial injuries extending beyond classic Monteggia patterns, are rare and diagnostically challenging, particularly in adults. Delayed presentation further complicates management and increases the risk of long-term morbidity. This report details a case series of two adult patients with Type I Monteggia Equivalent fractures, one with a delayed presentation, successfully treated with open reduction and internal fixation (ORIF) of the radial fracture. We aim to elucidate effective diagnostic and treatment

CASE: A 50-year-old man developed radioulnar synostosis and presented with restricted forearm rotation, 15 months after sustaining an undisplaced fracture of the ulna. Radiographs and computed tomography revealed a mature bony bridge between the forearm bones. Surgical excision of the heterotopic bone was performed, and bone wax was applied to mitigate hemorrhage and prevent recurrence. Postoperatively, the patient achieved near-complete pronation and supination and no recurrence was found at the end of 2 years of follow-up.
CONCLUSION: Radioulnar synostosis is an infrequent complication asso

This case illustrates the complex challenges of managing forearm trauma complicated by bone loss after implant removal. A long-standing radial shaft plate, integrated into cortical bone with overgrowth, had to be removed to treat a new peri-prosthetic fracture. Removal caused massive cortical loss, leaving poor bone stock for fixation. Dual plating was performed to bridge the defect and stabilize the distal radius fracture, but mechanical failure prompted a staged revision. A deliberately delayed second operation allowed time for biological preparation. Definitive reconstruction included corre

PURPOSE: Nonsteroidal anti-inflammatory drugs (NSAIDs) are commonly used for pain management following distal radius fracture (DRF) surgery, yet their effects on postoperative complications remain unclear. This study evaluated the association between perioperative prescriptive NSAID use and surgical and medical complications following DRF surgery.
METHODS: A retrospective cohort study using the TriNetX database identified adult patients who underwent DRF surgery between 2012 and 2024. Patients were categorized based on NSAID prescription within 1 day before to 1 month after surgery. Propensit

This article presents our experience in managing a case of a persistent infected nonunion of the proximal ulna that failed to heal after a free vascularized fibular graft. Free vascularized bone grafting is one of the best treatments for infected nonunion, but multiple factors must be addressed to achieve bone union. It is crucial to target septic, biological, and mechanical factors systematically using a step-by-step approach, even when faced with a worst-case scenario after a challenging surgery such as a free vascularized fibular graft.

BACKGROUND: Operant up-conditioning of the motor evoked potential (MEP) elicited by transcranial magnetic stimulation can increase corticospinal excitability for the ankle dorsiflexor and improve its activation in individuals with foot drop due to chronic incomplete spinal cord injury (SCI). It is unknown whether MEP up-conditioning can work similarly in the upper limb (UL).
OBJECTIVES: As the first step in testing the possibility of MEP up-conditioning enhancing UL neurorehabilitation, we examined the effects of forearm extensor MEP up-conditioning in 6 adults with chronic incomplete cervica
