BACKGROUND: Throwing-related shoulder and elbow injuries are common among collegiate baseball pitchers; however, prospective evidence on scapulothoracic (ST) joint function as a risk factor is limited. This study prospectively investigated whether ST joint stability was associated with throwing-related injuries.
METHODS: Twenty-seven collegiate pitchers were prospectively categorized into injury and noninjury groups during the 12-month follow-up. Scapular and trunk positional coordinates were recorded using a 3D scanner during preseason screening under 2 conditions: (1) 90° scapularplane abdu

BACKGROUND: Olecranon fractures are common injuries and may be indicated for open reduction and internal fixation (ORIF). Surgical techniques include plates and screws, intramedullary screw, Kirschner wire with tension band (KWTB), and all-suture fixation. The purpose of this study was to compare surgical and patient-reported outcomes between these fixation techniques.
METHODS: Adult patients who underwent surgery were retrospectively queried (January 2016-December 2020). Demographics, comorbidities, fracture characteristics, surgical details, elbow range of motion (ROM), incidence and time t

While the relationship between radial head fractures (RHF) and coronoid process fractures (CPF) is biomechanically established, the exact frequency of CPF in RHF patients without focus on elbow dislocations is underexplored. To better estimate the likelihood of CPF, the analysis of typical fracture constellations is useful. The aim of this study was therefore to analyze the correlation between RHF severity and the presence and type of CPF. This retrospective study analyzed 356 RHF patients, evaluating the prevalence and correlation of CPF using CT and intraoperative data. Only cases with confi

INTRODUCTION: Olecranon fractures are common injuries, especially in the elderly population, in whom conventional osteosynthesis with tension band wiring (TBW) or plating (PF) is associated with high reoperation rates for hardware removal due to intolerance (up to 80% of surgically treated patients in the TBW group and 25% in those treated with PF). High-strength transosseous suture (TBS) has been proposed as an alternative to reduce these complications associated with conventional techniques, although few studies with long-term follow-up exist. The aim of this study was to assess, in the long

BACKGROUND: Few studies have investigated the contributing factors for pediatric radial neck fractures. This study aimed to analyze the radiographic and clinical factors that influence the treatment outcomes in pediatric radial neck fracture.
METHODS: This retrospective study included the 112 pediatric patients with radial neck fracture. On elbow radiographs, angulation, displacement, and carrying angle were measured, and fractures were classified using the modified Judet classification. Clinical outcomes were assessed using the Flynn criteria based on the elbow ROM and carrying angle. Univar

Upper extremity deep venous thrombosis (UEDVT) is an uncommon clinical entity in adolescents, most often linked to central venous catheters or hypercoagulable states. Trauma-induced UEDVT is rare and challenging to diagnose due to nonspecific clinical signs that can mimic infection or soft tissue injury. We report an 18-year-old female who developed post-traumatic swelling and tenderness of the right forearm after being beaten by her school teacher. Initial non-contrast CT revealed subcutaneous edema without fracture. Subsequent CT angiography identified right brachial vein thrombosis with pro

BACKGROUND: The application of intraoperative tourniquets is a fundamental component of distal radius fracture surgery, yet it carries a risk of significant complications. The British Orthopaedic Association (BOA) has published safety guidelines (BOA Standards for Trauma (BOAST)) to standardise practice, but adherence is often suboptimal. This quality improvement project aimed to evaluate and improve compliance with these guidelines in a real-world clinical setting.
METHODS: A two-cycle retrospective audit was conducted at a major trauma centre in the UK. The first cycle (January to April 202

CASE: A 53-year-old man sustained a severe open distal radius and ulna fracture with approximately 7-cm osteoarticular bone loss, extensive extensor musculotendinous junction avulsions, and radial nerve disruption. Acute 3-cm forearm shortening restored tendon tension and reduced defect size, enabling primary tendon reconstruction with pull-in sutures and nerve repair. Early staged free vascularized fibular grafting was performed. Despite postoperative Pseudomonas aeruginosa infection requiring implant removal and local antibiotics, solid union was achieved with preserved forearm rotation at 2

BACKGROUND: Despite the clinical relevance of forearm fractures and malunions and the impact of a functional limitation, the link between forearm malalignment and limited pronation and supination remains poorly understood and still relies on anatomical alignment expressed as angulation. Using recently developed technologies, mechanisms that limit function can be automatically detected by modeling individual forearm kinematics using three-dimensional (3D) bone models of the radius and ulna.
QUESTIONS/PURPOSES: We evaluated the accuracy of a personalized 3D kinematic model to identify limitatio

AIMS: Articular impaction in olecranon fractures has been under-recognized. This study aimed to evaluate the clinical implications of articular impaction, and to identify factors associated with poor postoperative outcomes following open reduction and internal fixation (ORIF).
METHODS: Patients who underwent ORIF for olecranon fractures were retrospectively reviewed. Articular impaction was assessed using preoperative CT scans, and articular impaction reduction quality was categorized based on residual depression. Functional outcomes were assessed using the Mayo Elbow Performance Score (MEPS)

BACKGROUND: Closed reduction of distal radius fractures is painful, and current analgesic strategies may be inadequate. Ultrasound-guided lateral infraclavicular brachial plexus block may offer complete analgesia and muscle relaxation, potentially improving patient comfort and reduction quality. However, benefits and challenges regarding anesthetic agents for this procedure remain unclear.
METHODS: In this randomized, controlled, blinded, noninferiority trial, 63 adults with distal radius fractures requiring closed reduction received a lateral infraclavicular block with either 30 mL of ropiva

INTRODUCTION: Self-inflicted upper extremity amputation is a rare injury which leaves surgeons with unique operative considerations. Although psychiatric decompensation is traditionally considered a contraindication to replantation, available evidence is limited to isolated case reports and small series. Hence, we sought to describe the injury characteristics, psychiatric comorbidities, operative decision-making, and clinical outcomes of patients with self-inflicted upper extremity amputations.
METHODS: A retrospective review of traumatic upper extremity amputations and replantations was perf

BACKGROUND: Valgus stress on the elbow during pitching can exceed the tensile strength of the ulnar collateral ligament and is the primary cause of throwing-related elbow injuries. The forearm flexor-pronator muscles act as dynamic stabilizers against elbow valgus stress. However, the temporal changes in activity during repetitive pitching and their relationship with valgus stabilization remain unclear. This study aimed to investigate changes in the activity of the forearm flexor-pronator muscles and medial elbow joint gapping during repetitive pitching, and to examine their association.

Traumatic injuries to the forearm often involve damage to the radial or ulnar arteries. In isolated single vessel injuries whereby the hand remains perfused, the surgeon is presented with a dilemma: to repair or ligate? This systematic review aims to compare the outcomes between patients who underwent surgical repair or ligation of vessels (PROSPERO: CRD42022311323). A systematic electronic search of Medline, Embase, CINAHL, PubMed, and Cochrane CENTRAL databases was conducted (March 11, 2022). All surgical comparative studies in adults sustaining a single forearm vessel injury with a minimum

The term "floating elbow" describes a rare injury pattern in children involving ipsilateral fractures of the humerus and forearm bones. These high-energy injuries are frequently associated with neurovascular compromise and carry a risk of compartment syndrome. We report an 11-year-old boy who sustained a fall from height, resulting in a left supracondylar humerus fracture (Gartland type III) and ipsilateral distal radius and ulna fractures. Closed reduction and percutaneous K-wire fixation were performed for the supracondylar fracture, and open reduction with K-wire fixation was done for the d

Background and Objectives: Pediatric forearm fractures are among the most common childhood injuries. COVID-19-related societal restrictions, including school closures and suspension of sports activities, altered children's daily routines and may have influenced injury patterns. This study aimed to evaluate whether periods of stricter COVID-19 restrictions were associated with changes in the incidence of pediatric distal and diaphyseal forearm fractures after accounting for seasonal variation and long-term temporal trends. Materials and Methods: This retrospective observational time-series stud

Distal leg reconstruction is particularly challenging due to limited local options. This challenge is accentuated when there is a concurrent tendon rupture and threatened hardware, making soft tissue coverage crucial for preserving limb function. This report describes a 37-year-old male with a 5 × 8 cm wound on the distal leg, including a 7.5 cm loss of tibialis anterior tendon (TAT) with underlying tibial hardware. A 5 × 9 cm composite left radial forearm free flap (FF), including flexor carpi radialis and palmaris longus tendons, was used to reconstruct the TAT and cover the tibial hardware.

Terrible triad injuries of the elbow consist of elbow dislocation associated with fractures of the radial head and coronoid process and are characterized by significant instability. Variants that include an associated olecranon fracture are rare, and data regarding long-term functional outcomes and patient-reported outcome measures remain limited. We report the case of a 74-year-old female patient who sustained a terrible triad injury of the elbow associated with an ipsilateral olecranon fracture following a low-energy fall. This injury pattern has occasionally been referred to as a "terrible

INTRODUCTION: distal biceps tendon rupture is an uncommon injury, with an incidence of two to three cases per 100,000 people annually. It predominantly affects men between the ages of 40 and 60, commonly in the dominant arm and during eccentric contractions. Surgical treatment is preferred as it allows better restoration of supination and flexion strength. Proper anatomical positioning has been shown to optimize forearm supination strength.
CLINICAL CASE: a 45-year-old male developed an injury to the posterior interosseous nerve secondary to improper positioning of the distal biceps tendon du
