Assessing the efficacy of ultrasound-guided platelet-rich plasma on nerve regeneration and functional outcomes in forearm peripheral nerve injuries: a retrospective observational study.

BACKGROUND: This study evaluates the efficacy of ultrasound-guided platelet-rich plasma (PRP) treatment in promoting nerve regeneration and improving functional outcomes in patients with peripheral nerve injuries (PNI) of the forearm. METHODS: A retrospective analysis was conducted on 183 patients diagnosed with unilateral forearm median or ulnar nerve injuries, classified as grade II or III according to the Sunderland system. Patients were divided into a conventional rehabilitation group (n = 105) and a PRP group (n = 78), which received additional ultrasound-guided PRP injections. Primary o

A cost-effective 3D-printed cement spacer reconstruction in a rare case of calcaneal Ewing sarcoma: a case report and surgical technique.

BACKGROUND: Ewing sarcoma (ES) accounts for 15% of primary bone tumors and is the second most common type seen in children and young adults between ages 5 and 20, with a male-to-female ratio of 1.5 to 1. Around one-third of patients have metastatic disease at diagnosis, while involvement of the foot is rare, accounting for only 5% of cases. For patients without metastases, treatment typically includes neoadjuvant chemotherapy, limb salvage surgery when possible, and adjuvant chemotherapy. Recent advances in 3D printing enable customized, patient-specific reconstruction in complex anatomical si

Self-inflicted Thoraco-abdominal stabbing followed by bathtub drowning: Reconstruction of the terminal mechanism in a complex suicide.

Complex suicide involving sharp-force injury and drowning may closely mimic homicide and requires careful reconstruction of the terminal mechanism. We report an anonymized case of a 27-year-old man found prone in a bathtub containing water mixed with blood after emergency entry into his apartment. Blood traces were documented in several rooms. Autopsy identified 18 stab-incised wounds of the precordial and epigastric regions, hesitation-type superficial injuries, old linear forearm scars, and no defensive injuries. Seven wounds penetrated the thoracic cavity; however, the limited right ventric

Right forearm wound and soft tissue abscess complicated with sepsis due to Streptococcus suis: a case report.

BACKGROUND: Streptococcus suis (S. suis) is an opportunistic zoonotic bacterial pathogen that is typically transmitted to humans through injured skin or mucous membranes, or through consumption of undercooked contaminated pork. It primarily causes meningitis, septicemia, arthritis, and endocarditis. Reports describing wound infection as the predominant clinical manifestation of S. suis infection are rare. We report a case of S. suis wound infection complicated by sepsis. CASE PRESENTATION: A 49-year-old woman presented to the emergency department with swelling and pain in her right forearm fo

Mycobacterium marinum Infection: A Clinical Case Report and 5-Year Systematic Literature Review (2021-2025).

We report a case of cutaneous Mycobacterium marinum infection in a 66-year-old male patient with traumatic aquatic exposure, manifesting as multiple red nodules and plaques (1-2 cm in diameter) showing typical sporotrichoid (lymphocutaneous) distribution extending from the extensor knuckle of the right index finger to the right forearm, accompanied by partial rupture and purulent exudation. Histopathological examination of skin biopsy specimens demonstrated infectious granuloma formation, and combined bacterial culture, hsp65 PCR and next-generation sequencing (NGS) confirmed M. marinum as the

Upper-Extremity Compartment Syndrome: Comparison of Substance-Related Found Down and Acute Trauma Mechanisms.

PURPOSE: Upper-extremity compartment syndrome can result in functional deficits, loss of limb, and loss of life. Although most commonly caused by trauma, in recent years, opioid and substance-related overdose has led to a rise in patients with "found down" compartment syndrome. Our purpose was to compare clinical presentations and outcomes in patients with upper-extremity compartment syndrome caused by a substance-related found down mechanism to those caused by an acute trauma mechanism. METHODS: A retrospective chart review was performed to confirm a diagnosis of upper-extremity compartment

Forearm Compartment Release with Incidental Soft Tissue Sarcoma.

Compartment syndrome is often trauma-related, but rare cases may arise from soft tissue sarcomas (STS) increasing pressure within muscle compartments. This case emphasizes the need to consider atypical causes of compartment syndrome in older adults. A 95-year-old woman with multiple health conditions presented with severe pain, swelling, and finger immobility following a minor twisting injury to her forearm. Examination findings suggested compartment syndrome, prompting an urgent fasciotomy to relieve pressure. Intraoperatively, a large mass was found within the volar compartment. Pathology id

Osteogenesis imperfecta type V: About a clinical case.

Osteogenesis imperfecta (OI) is a rare inherited connective tissue disorder. It is characterized by short stature, fragility and decreased bone mass, which leads to multiple and recurrent fractures after low-energy trauma, which generates susceptibility to long bone deformity and vertebral compression. There are several types of OI, with types I to IV, in which the COL1A1 and COL1A2 genes are affected, being the most frequent. In recent years, the discovery of new forms of OI has led to research into the pathways critical aspects of bone metabolism, with new genes involved being identified. Th

Coverage of elbow and forearm soft tissue defects with the posterior ulnar recurrent artery perforator flap (PURAP): an anatomical study.

INTRODUCTION: Covering soft tissue defects from the elbow and forearm is challenging for the plastic surgeon. The posterior ulnar recurrent artery perforator flap is a fasciocutaneous perforator flap vascularized by the perforators emerging from the posterior ulnar recurrent artery. It has multiple functional and aesthetic advantages but has not yet been well studied. This work aimed to examine the number, caliber, and topography of the posterior ulnar recurrent artery's perforators. METHODS: Perforator mapping was performed by blue latex injection on 20 fresh cadavers' upper extremities. The

From injury to reconstruction, utilizing staged technique in a self-inflicted facial gunshot wound: a case report.

BACKGROUND: Facial gunshot injuries are rare and complex, with mortality and complication rates of up to 15% and 30%, respectively. Management of facial gunshot wounds has proven to be challenging for surgeons because the treatment requires multidisciplinary evaluation and planning. We present a rare case of a gunshot wound to the face that resulted in extensive deformities, requiring multiple reconstructive procedures. CASE PRESENTATION: A 19-year-old male of South Asian (Pakistani) origin presented to the emergency department with a 3-day-old gunshot wound to the floor of the mouth, causing

Biodegradable (PLGA) Implants in Pediatric Trauma: A Brief Review.

Background/Objectives: Biodegradable implants have emerged as a promising alternative to traditional metallic fixation devices in pediatric orthopedic surgery. Avoiding implant removal is especially advantageous in children, who would otherwise require a second operation with additional anesthetic and surgical risks. This study reviews the current use of poly(lactic-co-glycolic acid) (PLGA) implants in pediatric fracture fixation and evaluates how they address limitations associated with traditional hardware. Methods: A narrative review was conducted summarizing current evidence, clinical expe

A Rare Case of Ipsilateral Radial Head, Neck, and Shaft Fracture With Intact Ulna in a Young Adult.

Fractures involving multiple levels of the radius are rare and often associated with ulnar involvement or distal radius injury. We report a previously undocumented injury pattern: a comminuted ipsilateral fracture of the radial head, neck, and shaft, with an intact ulna, in a 28-year-old male following road traffic trauma. The classification system of the Arbeitsgemeinschaft für Osteosynthesefragen (AO), also known as the Working Group for Osteosynthesis Questions, does not include this type of fracture configuration. The patient underwent open reduction and internal fixation using a headless

A complex suicide with over 250 sharp force injuries and falling from height.

Suicides with a high number of sharp force injuries are extremely uncommon, in particular in combination with another suicide method. In these cases, the differentiation between homicidal and suicidal injuries is complicated and a thorough investigation and autopsy is needed. We report about a 57-year-old woman with a history of severe depression who died due to her blunt trauma from falling from height after self-infliction of multiple sharp force injuries. Autopsy revealed at least 41 injuries on her throat, 120 on her thorax and abdomen, 81 on her right forearm and 8 in her right palm. In a

Biomechanical risk factors for medial elbow valgus stress in baseball pitchers: the role of Forearm Whip Angle.

BACKGROUND: Medial elbow injuries, particularly ulnar collateral ligament injuries, are a major concern in youth and adolescent pitchers. Elbow varus torque (EVT) is commonly used as an index of medial valgus load. We examined whether the Forearm Whip Angle (FWA)-defined as the sagittal-plane inclination of the throwing-forearm segment relative to the horizontal-relates independently to EVT and which phase-specific lower-extremity/trunk motions determine FWA. METHODS: Seventeen healthy high-school pitchers were analyzed with 3-dimensional motion capture. Across the pitching cycle (0%-100%, 1%
