Reverse forearm flap based on an "exteriorized" pedicle.

Soft tissue coverage after multiple digital amputations was achieved with a reverse forearm flap based on an "exteriorized" vascular pedicle. The length of exposed radial artery pedicle was calculated to permit full range of motion of the involved digits before flap division and inset. No complications of the exposed vascular pedicle were noted.

[Stable metal osteosynthesis in fractures of long tubular bones and cranio-cerebral trauma].

On the basis of an experience of application of modern methods of trans-osseous (123), intraosseous (67) and osseous (3) metallo-osteosynthesis and their combinations (30) in 159 victims (aged 4.5-86 years) with associated craniocerebral injury is drawn a conclusion on the expediency of application of different methods of metallo-osteosynthesis of long bones depending on a number of indications. In this case the severity of the craniocerebral injury and the victim state, the character and association of the bone fractures, the period of the traumatic disease are taken into consideration. A composite neuro-teno-cutaneous forearm flap in the one-stage reconstruction of a large defect of the soft tissue around the ankle.

The vascular anatomy of the radial forearm flap, incorporating the brachioradialis and palmaris tendons together with the superficial radial nerve in 20 fresh cadavers, is described. The radial artery in the cadaveric forearm was irrigated and injected with blue latex. The number and distribution of the cutaneous branches supplying the skin, brachioradialis tendon, palmaris tendon, and superficial radial nerve, were then documented in relation to the distance from the radiocarpal joint. The radial artery was found to provide adequate blood supply to the above structures. This flap has been use

Overview of osteoporosis.

Osteoporosis is a common age-related disorder manifested clinically by skeletal fractures, especially fractures of the vertebrae, hip, and distal forearm. The major cause of these fractures is low bone mass, although an increase in trauma due to falls in the elderly also contributes. There are multiple causes for the low bone mass which, in any given individual, may contribute differently to the development of the osteopenia. The most important groups of causes are failure to achieve adequate peak bone mass, slow bone loss due to processes relating to aging, the menopause in women, and a varie

The lateral thoracic region flap.

The lateral thoracic region flap is based at the level of the 3rd rib on multiple direct cutaneous perforators. It has an excellent vascularity with a dependable length/width ratio of 3:1. The donor area, which lies in a concealed region, can often be closed primarily. The flap is hairless and can be used anteriorly to cover raw areas over breast and axilla and posteriorly over scapula and chest wall. As a distant flap it can be used to resurface raw areas on the ipsilateral upper arm and forearm and contralateral hand. The successful use of the flap in 40 cases in this series confirms its rel

[Injuries caused by automobile dragging: a report of three cases].

Three cases of victims who were dragged a long distance by automobiles are reported. Injuries caused by such dragging are discussed. [Case 1] A 50-year-old man was found dead under an automobile after having been dragged for a distance of 530 m. Deep abrasions were found in the right temporal region, both scapular regions, the buttocks, and the heels. There were excoriations toward upper side in the right hypochondriac region along the rib arch, and multiple rib fractures with and without severe hemorrhaging. Alcohol levels were 3.54 mg/ml in the femoral venous blood, and 4.54 mg/ml in the uri

[Orthopedic treatment of the child with multiple injuries and its current progress].

We regularly use the Elastic Stable Intramedullary Nailing (ESIN) for children over seven years old with shaft limb fractures. This technique is especially adapted to pediatric traumatology, and is to be preferred to rigid osteosynthesis or osteotaxis in closed fractures. We use ESIN as a rule for femoral shaft fractures. Unstable fractures of shaft forearm, tibia and humerus are treated following this technique, as for multiple fractures of the lower limbs. Because of immediate and complete fracture stability. ESIN is especially useful in multiple trauma patients, where nursing facilities and

The use of a flexor carpi ulnaris muscle flap in the treatment of an infected nonunion of the proximal ulna. A case report.

Multiple efforts had failed to heal an infected nonunion of the proximal ulna. A flexor carpi ulnaris muscle pedicle flap was used to improve blood supply and soft-tissue coverage at the nonunion site. The muscle flap and application of the principles of nonunion treatment promoted bone healing and restoration of useful elbow function.

[A successful procedure in mitral valve rupture accompanied by rupture of the papillary muscle and the chordae tendinae following multiple injuries and blunt thoracic trauma].

In a considerable number of cases, blunt chest trauma also involves cardiac lesions including myocardial contusion or disruption pericardial effusion, or valve rupture. Definite cardiac trauma poses a challenge to everyone involved in intensive care, as early diagnosis and prompt treatment may be necessary to prevent a fatal outcome. We report a 32-year-old patient with fractures of the humerus, forearm, left clavicle and 2nd rib, and right ribs 4-6 after an 8 m fall. He was intubated in the emergency room because of arterial hypoxemia. Despite fluid administration the blood pressure deteriora

Single stage reconstruction of a large tibial defect using a free vascularised osteomyocutaneous ulnar transfer.

We present the case of a young man with 13 cm of bone and soft-tissue loss in the tibia and a severe traction injury of the brachial plexus. A free vascularised composite transfer of the bone and soft tissues of the ulnar side of the forearm was undertaken to reconstruct the defect. Bony union was achieved after a year and was followed by complete functional recovery of the lower limb.

[Plate osteosynthesis in humeral shaft fractures. Indications and results].

Non operative management of humeral shaft fractures is well recognized as the standard of care for uncomplicated injuries. Operative treatment of humeral fractures may be performed when limited indications are present as in patients with multiple trauma including ipsilateral forearm injuries, arterial injury or primary radial nerve palsy. 18 patients with humeral shaft fractures underwent open reduction and internal fixation (ORIF) using the AO plating technique at the Kantonsspital Chur from 1980 to 1986. Follow-up was available for 17 patients of whom 16 suffered from multiple injury trauma.

[Treatment of patients with multiple and associated fractures of the long bones].

The authors have studied 266 patients with multiple and associated injuries, 31 of whom were performed a one-stage osteosynthesis in fractures of the bones of the femur (36), the crus (23), the humerus (5) and the forearm (3). Before 1985 the main method of surgical treatment was open intramedullary osteosynthesis with Küntscher-type nails with drilling out of the medullary canal. This method was applied in operating 21 patients with fractures of the femur, 5 patients with fractures of the bones of the crus and 2 patients with fractures of the humerus. Tubed free flap combined with split-rib-graft for reconstruction of multiple oral cavity defects. A case report.

The successful use of a conventional bone graft for mandibular reconstruction in combination with a free forearm flap is described. Two separate intraoral mucosal defects, one in the mandible and one in the palate, were covered, using the same flap and partial tubing of the flap. Three weeks post-flap-transfer the flap was successfully divided. A safe reliable and versatile technique combining nonvascularized bone graft and free flap for simultaneous reconstruction of mandibular and intraoral mucosal defects is described. Simultaneous coverage of multiple intraoral defects with tubing of the f

Bone scintigraphy in post-traumatic reflex dystrophy.

Twenty-five patients presenting consecutively with persistent pain and tenderness, edema, and vasomotor instability in an extremity following trauma to the same extremity were clinically diagnosed as having early post-traumatic reflex dystrophy (PRD). All underwent scintigraphy with 99mTechnetium methylene diphosphonate before institution of prednisolone treatment. The time lag between the occurrence of the initial trauma and scintigraphic imaging averaged 11.5 weeks. Of 25 scintigrams, 24 showed multiple periarticular accentuation in the affected extremity. Scintigraphy is a useful clinical a

Strengthening of the partially paralyzed shoulder girdle by multiple muscle-tendon transfers.

Strength and stability of the shoulder girdle affect function of the entire upper extremity. Isolated muscle weakness may be compensated for by adjacent muscles; however, profound weakness of the deltoid muscle, for example, affects not only placement of the extremity, but also strength and endurance of the forearm and hand. Orthotic substitutes for weakened muscles are not successful. If the muscle weakness is nonprogressive and stable, then surgical treatment will improve glenohumeral stability, increase endurance, and provide more uniform strength for many activities of daily living.

Blast injuries from glass bottles containing dry ice.

We present three cases (two incidents) of severe blast injury from the explosion of covered glass bottles containing dry ice. The first patient sustained lacerations of the face, left eye, right wrist and forearm, and abdomen with protrusion of the small bowel. The second sustained a deep laceration to her anterior neck. The third sustained multiple lacerations to her lower extremities and one to the labia majora. All three patients were taken to the operating room for surgical exploration and repair and were discharged without complications.

Orthopaedic treatment of paediatric multiple trauma patients. A new technique.

In multiple trauma, skeletal injuries are generally responsible for long hospital stays, with concomitant nursing inconveniences and for most of the sequelae. A new operating technique has been perfected, especially adapted to paediatric traumatology: stable elastic rodding, precise and less aggressive technique. It avoids plastering and traction systems, and rapidly restores sufficient independence for a return to school. It is especially useful in multiple trauma patients, where nursing facilities and easy transport may be very helpful. The hospital regularly uses this technique for patients

Toe transfer to the vessels of the reversed forearm flap.

A 23-year-old man had a severe degloving injury of his hand, amputation of multiple fingers, and devascularization of the thumb. Primary care consisted of successful revascularization of the thumb and skin grafting of the hand. Secondary soft tissue reconstruction of the hand was done with a reverse forearm flap. This was followed by second-toe transfer to the hand, with the recipient vessels, the radial artery, and the venae comitantes of the reverse forearm flap.
