Monteggia fracture-dislocation is an inherently unstable injury in the elbow region, characterized by a fracture of the proximal ulna associated with dislocation of the radial head, often with high rates of postoperative complications. Some variants of this injury involve a combination with a fracture of the radial head, which further complicates the scenario, often requiring multiple surgical approaches, increasing soft tissue damage, and promoting a higher risk of complications. The objective of this study is to report the case of a patient undergoing surgical intervention through a posterio

Recent observational studies have indicated that psychiatric disorders were associated with risk of bone mineral density (BMD) reduction. But the causal relationship between neuroticism and BMD remained unclear. By using public genome-wide association study data, a 2-sample bidirectional Mendelian randomization (MR) study was performed to investigate the causal relationship between neuroticism and BMD (heel BMD, forearm BMD, femoral neck BMD, lumbar spine BMD, and total body BMD). Inverse-variance weighted, weighted median, and MR-Egger were used to assess the causal effects. Multiple sensitiv

Upper extremity trauma is one of the most frequent surgical emergencies encountered in hospital trauma units. The complexity of injuries to the hand and forearm is due to the convergence of multiple anatomical structures within a relatively small and compact area, all of which are essential for the proper function of the extremity. Among these, neurovascular injuries are particularly significant. While these lesions are rarely associated with high mortality, inadequate or delayed management often results in severe dysfunction. In this paper, we present a series of cases involving complex forea

INTRODUCTION: Forearm lacerations could lead to disastrous disabilities, nerve injuries led by the ulna nerve cause severe hand deformity and disfunction with little options to repair.
CASE PRESENTATION: A young male suffered a distal ulna nerve injury and underwent multiple surgeries over two years. Zancolli-Lasso procedure was done, and major improvement was obtained.
DISCUSSION: A comprehensive approach for claw hand is essential, supple mobile joints and the Bouvier maneuver are the most decisive factors, literature shows that the right procedure will work for the right patient.
CONCLUS

BACKGROUND: Fall-related head impact is the leading cause of traumatic brain injury in older adults. There is limited understanding of factors related to fall-related head impact. This investigation examined characteristics of upper limb movements during standing-height falls and examined their association with fall-related head impact in older adults at risk for falls.
METHODS: Older adults (n = 29) at risk for fall-related injuries underwent experimentally induced falls in multiple directions (backwards and sideways). To characterize the upper limb movements and their association with head

For individuals with motor complete spinal cord injury (SCI), previous works have shown that spared motor neurons below the injury level can still be voluntarily controlled. In this study, we investigated the behavior of these neurons after SCI by analyzing neural and spatial properties of individual motor units using high-density surface electromyography (HDsEMG) and ultrasound imaging. The dataset for this study is based on motor unit data from our previous work (Oliveira et al. Brain 147: 3583-3595, 2024). Eight participants with chronic motor complete SCI and twelve uninjured controls atte

PURPOSE: Complex injuries of the hand with finger amputation can result in important functional impairment. If the amputated fingers cannot be replanted, the developed sequelae should be further evaluated. Sometimes, in the presence of a few remaining local vascular resources, the use of the flow-through technique can help in solving the problem.
METHODS: This study retrospectively reviewed four cases that underwent single or multiple fingers' reconstruction with toe transfers by using true flow-through or flow-through conduit flaps. A thorough description of the two methods is done, and the

BACKGROUND: Compartment syndrome commonly occurs in patients with forearm and lower leg fractures. Compartment syndromes of the gluteal and thigh muscles are less common. It is imperative that compartment syndrome be diagnosed and treated with fasciotomy as soon as possible. However, there are few reports on the diagnosis and treatment strategies for compartment syndromes that occur simultaneously in multiple anatomical regions or in the ipsilateral gluteal region and thigh.
CASE PRESENTATION: We report on a 76-year-old man who was obliquely crushed under a tree extending from the right forea

BACKGROUND: The forearm flexor-pronator muscles act as a dynamic elbow stabilizer against elbow valgus load during baseball pitching. The elasticity of these muscles increases with pitching. However, it is unclear whether increased muscle elasticity is associated with greater elbow valgus torque during pitching. This study aimed to determine the relationship between the elasticity of the forearm flexor-pronator muscles and elbow valgus torque during pitching in young baseball pitchers.
METHODS: We recruited 124 young baseball pitchers aged 9 to 12 years. The exclusion criteria included curren

BACKGROUND: In emergency presentations, it is not uncommon for patients to present with imaging requests of multiple body regions to detect concurrent injury. While current literature explores diagnostic efficacy of adjacent imaging for forearm fractures, there is limited research on its effectiveness across all extremities. This paper explores the diagnostic yield of X-ray adjacent imaging of the upper and lower limb in paediatric patients.
METHODS: A retrospective audit was performed using information available on radiology request forms from paediatric patients (age <18 years) that had mul

Patella fractures are not typically considered osteoporotic fractures. We compared bone mineral density (BMD) and microstructure in elderly women from a multiethnic population-based study in New York City with any history of a patella fracture (n = 27) to those without historical fracture (n = 384) and those with an adult fragility forearm fracture (n = 28) using dual energy x-ray absorptiometry (DXA) and high resolution peripheral quantitative computed tomography (HR-pQCT). Compared to those without fracture, women with patella fracture had 6.5% lower areal BMD (aBMD) by DXA only at the total

Forearm fractures involving both bones are prevalent among pediatric patients. Elastic stable intramedullary nailing (ESIN) is an excellent option for forearm fractures that require surgical stabilization in children and adolescents. Proximal third fractures can be particularly challenging to reduce using closed techniques, and multiple unsuccessful attempts at nail insertion can increase the risk of compartment syndrome. In this article, we present a novel technique called Shanz-assisted closed reduction (SACR), which involves using a 4-mm Shanz to facilitate alignment and nail insertion whil

BACKGROUND: Upper limb (UL) amputation is disabling. ULs are necessary for many domains of life1, and few effective motor and sensory replacements are accessible2. Approximately 41,000 people in the United States have UL amputation proximal to the fingers3, two-thirds of (all) traumatic amputations are UL4, and 80% of UL amputations are performed for trauma-related etiologies5. Socket prosthesis (SP) abandonment remains high because of the lack of sensation, limited prosthesis control, perceived weight, and difficulty comfortably wearing the SP6. Gustilo type IIIB open forearm fractures associated with avulsion injuries of multiple extensor tendons are difficult to reconstruct. Not only are bones, nerves, blood vessels, and soft tissues injured, but also tendons directly related to hand function. A 74-year-old man sustained an injury to his dominant right hand after being hit by a heavy pulley. The patient was diagnosed with a Gustilo type IIIB open forearm fracture, and multiple extensor tendons were avulsed from the musculotendinous junction. On the day of injury, the radius was fixed using a volar locking plate, and the ulnar head w

A man attacked by a pet zebra suffers multiple bite wounds to his right arm and generalized trauma after being dragged to the ground and pinned by the animal. He was transported to the emergency department (ED) by the local emergency medical service (EMS) with a tourniquet in place for nearly 60 min. The man's injuries included extensive soft tissue damage, an open fracture and dislocation of his right elbow, and a soft tissue injury to his right ear. His clinical course was complicated by severe necrosis of the soft tissue of the right forearm requiring transhumeral amputation just above the

The triangular fibrocartilaginous complex is made of multiple components, of which the palmar and dorsal radioulnar ligaments play an important role in distal radioulnar joint stability. The ulnar wrist ligaments may be injured during forearm and wrist trauma. There are several aspects of triangular fibrocartilaginous complex management that are still open to debate. The aim of the present study was to review the current concepts and discuss emerging trends to better elucidate and treat this important ligament complex.Level of evidence: V.

BACKGROUND: Foreign bodies that migrate into the heart may include medical devices dislodged from their original location or, rarely, external particles (shrapnel and other foreign bodies) that penetrate the vein, remain intraluminal, and migrate via the venous blood flow to the right heart. Most reported entry sites of these external foreign bodies were in the torso, thigh, or neck; none of them penetrated through a distal extremity of the body. We report a case where shrapnel was found in the right ventricle (RV) following penetrating injury to the hand.
CASE SUMMARY: An otherwise healthy 2
