Effect of AI-assisted radiographic fracture detection on paediatric emergency department length of stay through fracture exclusion: A before-after cohort study.

BACKGROUND: Artificial intelligence-assisted radiographic fracture detection tools (AI-RFDT) have the potential to support confident fracture exclusion by physicians in the paediatric emergency department (ED), where presentations for suspected fractures substantially outnumber confirmed fractures. This study aimed to evaluate whether implementation of AI-RFDT in a paediatric ED is associated with reduced length of stay (LOS) among patients presenting with suspected fractures, and to determine whether any observed effect is specifically mediated through the fracture-exclusion pathway. PATIENT

Pediatric Upper Extremity Compartment Syndrome: An Analysis of Inpatient Outcomes.

Pediatric acute compartment syndrome (PACS) results from increased pressure within a fibro-osseous space, reducing local limb perfusion. It can be traumatic or non-traumatic, often due to fractures, burns, or iatrogenic compression. PACS is challenging to diagnose in children due to insidious onset. While lower extremity PACS is well studied, upper extremity PACS remains underexplored. This study analyzes characteristics and inpatient outcomes of pediatric upper extremity PACS. The National Inpatient Sample (2015 - 2019) was queried for patients under 18 diagnosed with ACS. ICD-10 codes identi

Delayed Radial Nerve Recovery After Ballistic vs. Blunt Humeral Shaft Fractures: A Time-to-Recovery Analysis of Nerves in Continuity.

Background/Objectives: Radial nerve palsy complicates 8-16% of humeral shaft fractures, and because more than 90% of primary palsies recover spontaneously, the clinically decisive question is not whether a nerve will recover but when. That timeline has been defined almost entirely from blunt-mechanism cohorts, yet is routinely applied to ballistic injury. No study has directly compared radial nerve recovery between gunshot wound (GSW) and blunt trauma. We compared time to full radial nerve recovery between mechanisms within a cohort of radial nerve injuries with continuity either operatively c

Effect of tendon-to-bone gap on adjustable- versus fixed-loop biceps tendon fixation.

BACKGROUND: Distal biceps tendon ruptures significantly impair forearm flexion and supination strength, particularly in active individuals. Suspensory fixation using cortical button devices is a widely accepted surgical approach. Adjustable-loop techniques allow maximal tendon-to-bone apposition, while fixed-loop methods are simpler but may leave a gap. Biomechanical differences between these techniques specific to the distal biceps tendon remain poorly investigated, especially given the unique loading patterns of the elbow compared to other joints. METHODS: Finite element analysis was perfor

A Step-by-Step Protocol for Manual Reduction and Splint Immobilization of Distal Radius Fractures.

Distal radius fractures (DRFs) are among the most common upper extremity fractures encountered in clinical practice. Closed reduction followed by external immobilization remains a fundamental treatment approach; however, the procedure is highly operator-dependent and lacks standardization. This article presents a step-by-step protocol for manual reduction and four-splint immobilization using volar, dorsal, radial, and ulnar splints for acute DRFs. The protocol encompasses patient selection, anesthesia administration, patient positioning, sustained longitudinal traction with countertraction, re

[A Proof-of-Concept Study of Intelligent Chinese Medicine Splint for Distal Radius Fracture].

Distal radius fracture (DRF) usually adopts static fixation method, which often leads to complications such as abnormal fracture healing and pressure injury. The reason lies in the lack of real-time regulation of fixed pressure and biomechanical feedback, which affects the functional recovery. Our team designed an intelligent and adaptive orthopedic equipment. Based on the patient's personalized three-dimensional (3D) printing architecture, the equipment system realizes light and accurate adaptation, and embeds a series of thin film pressure sensors in key anatomical parts to continuously quan

Outcomes of simultaneous microsurgical replantation of complete amputations at the distal third of the right forearm and left wrist: a case report and 22-month follow-up.

Microsurgical replantation of simultaneous bilateral upper extremity amputations is a complex procedure requiring a highly coordinated multidisciplinary team. Functional outcomes depend on anatomical level of injury, duration of ischemia, extent of crushing, degree of contamination, and quality of postoperative rehabilitation. We report a 45-year-old male industrial worker who sustained complete simultaneous amputations through the distal third of the right forearm and at the left wrist following an industrial paper-cutting machine accident. Total ischemia time was 8 h. Two microsurgical teams

The role of tensor fascia lata (TFL) graft in wound treatment: a case report.

INTRODUCTION AND IMPORTANCE: Combat-related injuries to the upper limb pose significant reconstructive challenges, particularly in resource-limited, conflict-zone settings. Innovative strategies are essential to preserve hand function, and the choice of an optimal reconstruction remains complex. Temporary wound dressings must foster healing and protect vital structures during the interval before definitive closure. This report evaluates the effectiveness of a tensor fascia lata (TFL) graft combined with negative-pressure wound therapy (NPWT) as a temporary dressing for complex wounds in auster

Postoperative infection following volar locking plate fixation of closed distal radius fractures: a retrospective cohort study of infection rates and antibiotic prophylaxis practice.

BACKGROUND: Distal radius fractures are among the most common surgically treated upper-extremity fractures, and volar locking plate fixation is the predominant operative technique. Although perioperative antibiotic prophylaxis is widely used in orthopaedic surgery, procedure-specific evidence for closed distal radius fractures remains limited. Contemporary data regarding postoperative infection rates and antibiotic prophylaxis practice following volar locking plate fixation are scarce. The aim of this study was to describe postoperative infection rates following volar locking plate fixation of

Hybrid-Enveloped Pedicle of Distant Abdominal Flap for Reconstruction of Extensive Upper Extremity Defects.

BACKGROUND: Extensive upper extremity soft-tissue defects with exposed vital structures present a significant reconstructive challenge, particularly when microsurgical options are unavailable. Traditional distant pedicled abdominal flaps require a wide skin tube to protect the vascular pedicle, resulting in substantial donor-site morbidity and risks of pedicle compression. This study introduces a modified technique utilizing a hybrid combination of artificial dermis and local skin flap to create a hybrid-enveloped pedicle, aiming to minimize donor-site sacrifice and improve postoperative manag
