Saccharomyces cerevisiae isolated in a degloving baker's machine injury, a contamination or infection - a case report.

Deep tissue and bloodstream yeast infections are associated with poor outcomes due to rapid progression, antifungal resistance, and immune evasion. Differentiating between wound contamination and true fungal infection remains a clinical challenge, with limited guidance on the use of prophylactic antifungal therapy in this setting. This article presents a rare case of an immunocompetent patient who sustained a degloving hand injury and radius fracture from a bread kneading machine, with subsequent deep tissue cultures growing Saccharomyces cerevisiae. The case underscores the potential risk of

Intra-operative Technique for Managing Paediatric Distal Forearm Fractures.

Paediatric distal forearm fractures are a common injury in children and adolescents, often requiring surgical intervention. Traditional techniques involve forceful manipulation and hyper-exaggerating the defect to overcome haematoma resistance, increasing the risk of complications including iatrogenic displacement, neurovascular injury, and growth plate damage. This technical note describes an alternative intra-operative reduction technique aimed at addressing these limitations. A minimally invasive approach with a small incision allows for precise identification and gentle evacuation of the h

Radiographic assessment of medial elbow stability.

BACKGROUND: Diagnosing simple valgus instability of the elbow currently involves time- and cost-intensive imaging modalities such as magnetic resonance imaging or magnetic resonance arthrography. Previous studies have demonstrated that stress radiography represents an alternative diagnostic tool for such conditions. The aim of this study was to investigate whether standardized valgus stress radiography can identify soft-tissue lesions of the medial elbow. METHODS: A telos stress device (telos GAIII/E; telos Arzt- und Krankenhausbedarf GmbH, Woelfersheim-Bernstadt, Germany) was used to apply 5

Defining technical competence in olecranon fracture fixation: an international Delphi consensus on tension band wiring assessment.

BACKGROUND: Tension band wiring (TBW) is a widely used technique for displaced olecranon fractures but carries a substantial risk of complications, often due to technical errors. Despite being a designated core competency in orthopedic curricula, no standardized, procedure-specific tool exists to assess technical performance in TBW. This study aimed to establish international expert consensus on assessment parameters for evaluating surgical competence in TBW of simple transverse olecranon fractures. METHODS: A modified 4-round Delphi process was conducted with 98 orthopedic trauma surgeons, p

Pigtail assisted tracking of guide catheter during transradial coronary angioplasty on forearm complications: prospective randomized trial.

OBJECTIVES: During transradial percutaneous intervention (PCI), sharp edge of guide catheter tip may produce "razor blade effect" leading to radial vascular injury. Pigtail assisted tracking (PAT) technique has been described to overcome it, but has not been systematically evaluated. We prospectively evaluated the safety and efficacy of PAT technique in transradial PCI. METHODS: Consecutive patients undergoing transradial-PCI were randomized into two groups (Group 1: transradial navigation using PAT technique, Group 2: transradial navigation without PAT technique). Patients were observed for

An implant positioning score for tension band wiring in olecranon fractures.

INTRODUCTION: Tension band wiring (TBW) is one of the most commonly used methods of fixation for displaced olecranon fractures. This study aimed to assess whether implant positioning (IMPO) of TBW for olecranon fractures was associated with complications. METHODS: This was a multicentre cohort study. Eligible patients were retrieved from the hospitals' administrative databases using diagnosis codes for elbow and olecranon fractures. The patients' healthcare files were reviewed for demographics and complications. A major complication was defined as any reoperation within eight weeks or deep in

Influence of elastic stable intramedullary nailing on fracture reduction of paediatric diaphyseal forearm fractures.

BACKGROUND: Diaphyseal fractures of both forearm bones (both-bone forearm fractures) are common pediatric injuries. When surgical treatment is indicated, elastic stable intramedullary nailing is widely used due to its minimally invasive nature and favorable outcomes. Nail design may vary – straight, pre-contoured, or a combination of both – however, the influence of these configurations on fracture reduction has not been clearly explored. The aim of this study was to investigate postoperative radiological outcomes after stabilization of both-bone forearm fractures with either two non-contoured

Patient Satisfaction and Perioperative Outcomes of Wide-Awake Local Anesthesia No Tourniquet Versus Supraclavicular Peripheral Nerve Block in Elective Hand and Forearm Surgery: A Prospective Comparative Study.

Background/Objectives: Wide Awake Local Anesthesia No Tourniquet (WALANT) and ultrasound-guided peripheral nerve blocks (PNBs) are increasingly used alternatives to general anesthesia for hand and forearm surgery. While WALANT is commonly perceived as a time-efficient and resource-sparing technique, comparative data regarding patient satisfaction, perioperative pain, and time-related outcomes remain inconsistent. This study aimed to compare WALANT and ultrasound-guided supraclavicular peripheral nerve block techniques with respect to patient satisfaction, perioperative pain, time-related param

Analgesic and Hemodynamic Effects of Preoperative Ultrasound-Guided Brachial Plexus Block in Radius Fracture Surgery: A Propensity-Matched Cohort Study.

Background and Objectives: Optimal pain control after radius fracture surgery is critical for recovery and reducing opioid exposure. While brachial plexus block (BPB) offers analgesic benefits, its additive effect alongside general anesthesia remains underexplored. Materials and Methods: We conducted a retrospective cohort study of adults undergoing open reduction and internal fixation for radius fractures under general anesthesia between July 2020 and September 2025. Patients receiving preoperative ultrasound-guided BPB were matched 1:1 to those without BPB using propensity score matching. Pa

Long-term implant retention after impacted elastic stable intramedullary nailing in pediatric diaphyseal forearm fractures: a retrospective cohort study.

BACKGROUND AND PURPOSE: In elastic stable intramedullary nailing (ESIN) for forearm fractures, nail protrusion may cause irritation-related implant removal. The primary aim was to evaluate implant retention after nail impaction, and secondarily to assess secondary procedures, complications, and protrusion as a predictor of removal. METHODS: We analyzed a retrospective cohort of children with diaphyseal forearm fractures treated with impacted ESIN between 2017 and 2024. Implant retention was defined as no nail removal at record review. Secondary procedures were unplanned operations, while comp

Current perspectives on 3D-printed casts.

OBJECTIVE: Aim: Fractures of the distal radius and ulna are among the most common skeletal injuries and are traditionally treated with cast immobilization. However, conventional casting techniques have significant limitations. In recent years, three-dimensional (3D) printing technology has emerged as a promising alternative in the production of personalized post-traumatic stabilization devices. The aim of this review was to present current information and perspectives on the development of 3D-printed casts, with particular emphasis on the diversity of designs, material properties, and clinical

Non-surgical casting versus surgical reduction for children with severely displaced distal radial fractures (the CRAFFT Study): a multicentre, randomised, controlled non-inferiority trial and economic evaluation.

BACKGROUND: Severely displaced distal radial fractures are among the most common and controversial injuries in children. Despite observational evidence of reliable remodelling with growth in younger children, their alarming radiographic appearance-particularly when completely displaced (off-ended)-has driven routine surgical reduction and fixation. The Children's Radius Acute Fracture Fixation Trial (CRAFFT) aimed to evaluate the clinical and cost-effectiveness of surgical reduction compared with non-surgical casting. METHODS: This pragmatic, multicentre, randomised, non-inferiority trial inc

Restoring rotational balance of the paediatric forearm: lessons from post-traumatic malunion.

INTRODUCTION: Post-traumatic malunion of paediatric forearm fractures may lead to impairment of pronation and supination. Treatment decisions are traditionally based on radiographic alignment, although functional limitations do not always correlate with deformity on plain radiographs. Understanding the interaction between bony deformity, soft tissue contracture and growth related remodelling is essential for restoring rotational balance of the forearm.Current concepts:Impaired forearm rotation may result from angular or rotational malalignment, loss of the radial bow or soft tissue contracture
