Bilateral stress fracture of the ulna in an adult weightlifter: a case report.

Bilateral stress fractures of the ulna are extremely rare and have only been described in non-adults. In this case report we present a young adult weightlifter who reported bilateral stress fractures of both ulnae after increasing his training frequency and load. Stress fractures of the ulnae are examples of bone stress injuries due to repetitive muscle pull. The diagnosis can be made by radiography, isotope bone scan, CT or MRI. Thorough training analysis and adaptation is essential to treat this rare injury. Return to full activity can take up to three months.

Two surgical approaches to fracture malunion repair.

Two birds were presented with malunion fractures. The first was a young toco toucan (Ramphastos toco) with malunion of the tarsometatarsus that was treated by an opening-wedge corrective osteotomy and an acrylic-pin external skeletal fixator (type II) to stabilize the osteotomy. The second bird was an adult southern caracara (Caracara plancus) with radial and ulnar malunion that was treated by closing-wedge osteotomies. Stabilization of the osteotomy sites was accomplished through a bone plate fixed cranially on the ulna with 6 cortical screws and an interfragmentary single wire in radius. In

Biomechanical stability of different fixation constructs for ORIF of radial neck fractures.

Radial head and neck fractures are common and at times require operative fixation. There is no consensus on the ideal fixation construct for unstable radial neck fractures. Using 7 fresh frozen cadaveric radii, fractures of the radial neck were created 2 cm from the articular surface. The fractures were stabilized with 5 different commonly used constructs: crossed K-wires; a 2.4-mm T-plate using screws in the head (T-plate and nonlocked screw construct); a T-plate using a screw and locked buttress pin in the head (T-plate and locked buttress pin construct); a T-plate with an interfragmentary s

[Reconstruction of large through-and-through palate defects with folded free forearm flap].

OBJECTIVE: To explore the reconstruction method of large through-and-through palate defects. METHODS: From 2003 to 2006, 7 cases of large through-and-through palate defects were reconstructed with vascularized folded free forearm flap. 8 flaps were used, including 7 free forearm flaps and 1 pectoralis major myocutaneous flap. RESULTS: All the tissue flaps survived well except one flap necrosis because of arterial thrombosis. The appearance of reconstructed palate was acceptable, and the functions of swallowing and speech were normal or almost normal. CONCLUSIONS: It is feasible and effectiv

[Method of compression osteosynthesis in treatment of fractures of the ulnar process in children].

The authors made a comparison of results of treatment by different techniques used in the clinic including an original technique of compression osteosynthesis. Due to this method the development of the joint against the background of immobilization began earlier and the extremity function recovered 1.8 times quicker than when using other methods.

[Continuous skeletal traction in multifragmentary fractures of radius and ulna combined with cast].

INTRODUCTION: Fractures of the distal third of the radius are frequently encountered in emergency services. Until recently considered as benign, however, they represent a group of numerous therapeutic problems, as debated in anatomical, radiological and clinical studies. OBJECTIVE: To emphasize the importance of recovering the radial distance and the articular congruence in fractures of radius treated with a cast. HYPOTHESIS: To demonstrate that ligamentotaxis obtained with the application of the Cole-Obletz technique is a good method for treatment of distal radius fractures that have a comp

[Considerations regarding the treatment of the algodystrophy using the acupuncture].

MATERIAL AND METHOD: A team made up of 14 subjects with algodystrophy (AD) was studied at the level of the distal zone of one superior member result of the radius and/or ulna fracture, which needed an immobilization device for 30-40 days, using acupuncture. RESULTS: The results were compared with those obtained at a witness team treated by electro-physical recovery (ultrasound-therapy, ultrasound-phoresis with hydrocortisone, Bernard diadynamic currents and physiotherapy). Taking into account that AD is the local expression of the General Syndrome of Adaptation, the acupuncture method was use

Flexible nailing of fractures in children using stainless steel Kirschner wires.

Elastic titanium nails are widely used for the established procedure of Elastic Stable Intramedullary Nailing (ESIN), but are costly. Thirty-five diaphyseal fractures in various long bones in children were treated by flexible nailing using stainless steel Kirschner (K) wires. The results were comparable with reports by other authors using titanium nails and stainless steel K wires. Cost-effective K wires and instrumentation are easily available. A stainless steel K wire is 92% cheaper than a Nancy nail and 84% cheaper than a Synthes titanium elastic nail. K wires can be used for flexible naili

Monteggia and Monteggia-like-lesions: Classification, Indication, and Techniques in Operative Treatment.

Monteggia fractures consist of an ulna fracture accompanied by radial head dislocation. Such fractures are easily overlooked due to the prominence of the ulna fracture. Earlier studies have reported on the results of treating Monteggia fractures in children and adults even though this type of fracture is different in these two patient populations. As such they should be considered as separate entities due to the different injury pattern, the prognosis, and the preferred method of treatment. For a good postoperative result, an early detection of the Monteggia dislocation, an efficient operative

Operative treatment of distal radius fractures.

The incidence of distal radius fractures is increasing together with the average age of population. Intra-articular incongruity is the most probable cause of unsatisfactory outcome of distal radius fractures in younger and more active patients. Thus, the main goal in the treatment of distal radius fractures should be restoration of articular congruence. A computed tomography (CT) is recommended to help surgeon in preoperative planning in the treatment of comminuted intra-articular fractures. New implants have been designed to provide stable enough fixation for early mobilisation after surgery

Osteochondral flap avulsion fracture in a child with forearm compartment syndrome.

Traumatic elbow dislocations account for approximately 3% to 6% of childhood upper extremity injuries, and 41% of these dislocations are concomitant injuries around the elbow. Most of these injuries are associated with a fall on an outstretched hand. Compartment syndrome is one recognized complication after elbow dislocation and distal humeral and radial neck fracture. Compartment syndrome in the forearm is a devastating complication in children with forearm, elbow, and supracondylar fractures. Compartment syndrome occurs as the result of hypoxic damage caused by interruption of the circulatio

Rare case of upper arm compartment syndrome following biceps tendon rupture.

We present a case of upper arm compartment syndrome following a biceps tendon rupture in a 77-year old man on warfarin sodium. Compartment syndrome is common in the forearm and leg, but rare in the upper arm with only a handful of cases reported in the literature. Our patient's anticoagulated state predisposed him to the development of compartment syndrome. To the best of our knowledge there has been only one other case reported in the literature of upper arm compartment syndrome following biceps tendon rupture in a patient on warfarin sodium. Compartment syndrome of the upper arm is a rare oc

An unusual case of elbow dislocation.

The annual incidence of elbow dislocation is 6 to 8 cases per 100,000 in the United States; these dislocations represent 11% to 28% of all elbow injuries. The frequency of elbow dislocation is second to that of shoulder dislocation. Adult elbow dislocations are classified by the direction of displacement and associated fractures. They can be anterior, posterior, lateral, or divergent, with the most common type being posterior displacement of both the radius and ulna in relation to the distal humerus. This case report presents a rare case of a twisting dislocation of the elbow joint without any

The Elastic Bridge Plating of the Forearm Fracture: A Prospective Study.

Rigid plate osteosynthesis with compression is still the treatment of choice for forearm fractures to gain anatomic reposition, provide proper rotation and avoid a bridging callus. Due to necessary operative dissection there is a serious risk for infection and malunion. Based on good clinical results with elastic bridge plating at femur, humerus and tibia, this technique was also started to be used for forearm fractures in our clinic in 1995. In a prospective study, 86 of 124 consecutive patients at the age of 35.2 ± 14.7 years with 129 diaphyseal fractures of the radius or ulna (AO: 37 type A

Management of brachial plexus injury in adults.

Brachial plexus injury in adults is commonly caused by motorcycle accidents. Surgical management consists of nerve repair and nerve grafting for extraforaminal nerve root or trunk injury, and of neurotization or nerve transfer for nerve roots avulsion. In general, the results regarding restoration of shoulder and elbow function are good but reinnervation of the forearm muscles is less than safisfactory in respect to restoration of hand function. Functioning free muscle transfer in combination with selective nerve transfer is a reasonable alternative surgical procedure. Key words: Brachial plex

Bilateral combined fractures of the scaphoid and distal radius in a 13-year-old male.

The scaphoid is the most common of the carpal bones to fracture; upper limb fractures are associated in 5-10% of cases, and around 2% of the concomitant fractures are of the distal radius. One case of bilateral carpal scaphoid fractures associated with bilateral distal radius fractures is presented. Radiographs showed displaced Salter-Harris Type II radius fracture and nondisplaced transverse midscaphoid fracture on both sides. Conservative treatment with scaphoid casts on both arms was inconvenient for the patient, but gave complete union of the fractures and functionally, painless full range

Lymphedema of the hand and forearm following fracture of the distal radius.

Lymphedema of the hand following a fracture of the distal radius is a disabling, but rare complication. Although the pathogenesis of this mechanism is poorly understood, extensive review of the literature suggests that an underlying cause of this lymphedema is psychogenic in nature. After numerous therapeutic modalities including intense physiotherapy, sympathetic nerve blocks and hospitalization, our patient still continued to persist with this debilitating disease. This article presents a case of Colles fracture complicated by nonpitting edema in a 62-year-old woman in whom psychogenic cause

Compartment Syndrome of the Arm After Cable-Wakeboard Accident.

A compartment syndrome is an increased tissue pressure within a closed osteofascial compartment. This compromises blood flow to the muscles and nerves within that compartment, which -if not treated adequately in an early stage-results in permanent tissue and nerve damage. It most frequently occurs in the lower leg, but can also occur elsewhere when muscles are enclosed in tight fascial compartments, such as the forearm and hand. In this report a patient is described who developed an acute compartment syndrome of the arm after a cable-wakeboard accident in which his arm was strangulated. Cable-

Acquired boneless forearm as a complication of traditional bone setting.

An 8-year-old girl sustained closed fracture of the right ulna 10 weeks prior to presentation. She was taken to a traditional bone setter who applied a tight splint. The patient reported pain, but the splint was not removed. A week after application of the splint, a foul odor was detected and removal of the splint showed extensive exposure of the forearm bones. Above elbow amputation was rejected by the patient's parents when she was taken to hospital, where she was admitted for 8 weeks by a second traditional bone setter. A trained nurse applied herbal concoctions and dressed the wound daily
