To identify risk factors for forearm shaft refractures in paediatric patients treated with flexible nails in situ and explore effective management strategies. From January 2022 to April 2024, paediatric patients with diaphyseal radius and ulna fractures treated with flexible nails were retrospectively reviewed. Patients without refractures were assigned to group A, and those with refractures and flexible nails in situ to group B. Demographics, injury mechanisms, operative details, complications, and time to radiographic union were analyzed. Fifty-six paediatric patients with 105 forearm fractu

BACKGROUND: Medial epicondyle fractures account for a substantial proportion of pediatric elbow injuries, but the determinants of functional outcome after operative fixation remain incompletely defined. This study sought to describe functional and radiographic outcomes after open reduction and internal fixation (ORIF) of pediatric medial epicondyle fractures and to identify clinical, injury-related, and treatment-related parameters associated with these outcomes.
METHODS: We retrospectively reviewed consecutive patients 16 years or younger who underwent ORIF for isolated medial epicondyle fra

BACKGROUND: Paediatric firearm injuries are rare in civilian settings but pose significant challenges due to high-energy trauma, small anatomical compartments, and limited physiological reserve. Shotgun wounds, in particular, produce complex skeletal and soft tissue destruction requiring staged multidisciplinary care.
CASE PRESENTATION: We report a 3-year-old girl with an accidental close range shotgun injury to the right forearm, presenting with grossly contaminated wounds, soft tissue loss, and comminuted fractures of the shaft of radius and ulna. Emergent debridement and elbow-spanning ext

PURPOSE: All-terrain vehicle (ATV)-related upper-extremity injuries are commonly seen and treated in the emergency room. Despite existing research on patterns of injuries in ATV-related trauma, there has not been a comprehensive study on patterns of upper-extremity injuries from ATV use. The aim of this study was to examine the incidence, trends, and characteristics of ATV-related distal upper-extremity injuries across the United States.
METHODS: The National Electronic Injury Surveillance System database was utilized to investigate distal upper-extremity injuries specifically related to the

In resource-limited settings such as rural Nepal, the lack of intra-operative fluoroscopy often forces clinicians to choose between suboptimal conservative management or referral to distant centres. Our case series presents the feasibility of the titanium elastic nailing system for displaced childhood both-bone forearm fractures using tactile feedback and portable radiography instead of a C-arm. While the rate of open reduction was higher than in a fluoroscopy-guided series (2/9 for radius, 4/9 for ulna), all cases achieved stable intramedullary placement and clinical union. This approach offe

BACKGROUND AND PURPOSE: There is no universal consensus on evaluating the quality of pediatric fracture care. While U.S. News ranks hospitals based on surgical timing and procedural benchmarks, additional metrics such as complication rates, reoperations, and patient-reported outcomes could provide a more comprehensive assessment. We aimed to evaluate pediatric fracture care outcomes at a Level I trauma center, emphasizing standardization and transparency in benchmarking.
METHODS: This retrospective study reviewed all fractures diagnosed at HUS New Children's Hospital, Helsinki from 2018 to 20

An adult lesser anteater (Tamandua tetradactyla), weighing 8.4 kg, was presented for surgical repair of a distal ulnar fracture following a road accident. The aim of this case report was to describe the use of an ultrasound-guided regional anesthetic block in this species, specifically an axillary approach to the brachial plexus, as part of a multimodal analgesic protocol. Premedication included dexmedetomidine (5 µg/kg), ketamine hydrochloride (2 mg/kg), midazolam (0.2 mg/kg), and morphine sulfate (0.2 mg/kg), administered intramuscularly in a single syringe. Anesthetic induction was achieved

Dog bites are a frequent cause of extremity trauma and pose a substantial risk of infection and complications, particularly when bone exposure is present. We present the case of a 58-year-old male with no comorbidities who arrived at the emergency department after being attacked by a dog while cycling, sustaining multiple bites to the left lower extremity and right forearm. On physical examination, the patient exhibited complex wounds measuring up to 10 × 5 cm, with muscular and tibial bone exposure in the left leg but no associated fracture, and additional forearm lesions with muscle exposure

OBJECTIVES: Utility terrain vehicles (UTVs), equipped with roll cages and safety harnesses, are increasingly popular recreational vehicles. Unlike all-terrain vehicles (ATVs) which lack these features and have well-documented injury patterns, UTV injury patterns are not represented well in literature. This study aims to characterize these distinct injury patterns and their impact.
METHODS: A retrospective chart review from a Level 1 trauma center was conducted, including all UTV- and ATV-related injuries between 2018 and 2022. Statistical analyses utilized Chi-square, t-tests, and Mann-Whitne

BACKGROUND: When transplanting a superficial circumflex iliac artery perforator (SCIP) flap to the forearm, the small diameter of the superficial circumflex iliac artery (SCIA) complicates end-to-end anastomosis with the radial or ulnar artery. The small diameter of the anterior interosseous artery (AIA) may make it suitable for end-to-end anastomosis with the SCIA; however, no such cases have been reported to date. We assessed AIA diameter using computed tomography angiography (CTA) and analyzed six cases of SCIP flap transfer to the forearm using the AIA as the recipient vessel.
METHODS: In

Background Pediatric forearm fractures account for approximately 40% of childhood skeletal injuries, with ulnar fractures being particularly prevalent. Titanium Elastic Nailing (TEN) offers an effective minimally invasive option for these injuries; however, the optimal nail entry point - antegrade (olecranon) versus retrograde (distal ulna) - remains debated. This study compared functional and radiological outcomes, complication profiles, and implant-related morbidity between the two approaches. Methods A retrospective comparative study was conducted from May 2023 to May 2025 at the R.L. Jalap

BACKGROUND: Epidemiological data on injuries and fractures in children under two years of age is limited, thus, we aimed to explore the incidence, patterns and mechanisms.
METHODS: Retrospective, population-based cross-sectional study including children under two, seen at the A&E department due to a trauma, or having a radiograph due to birth trauma. All radiographs were re-reviewed.
RESULTS: Four hundred thirty children (49.8% female), mean age 14.7 months (SD 6.8), were included, of whom four had a high-energy trauma and 42 were birth-related. Of 388 children (50.3% female) (mean age 16.3

Peripherally inserted central catheters (PICCs) were introduced as an alternative to centrally inserted central catheters (CICCs) inserted into the jugular vein to provide central venous access. In head and neck reconstructive surgery, PICCs are preferred over CICCs because they do not obstruct surgical access, such as during neck dissection. Additionally, PICCs offer a safer option than peripheral access for the infusion of vesicants, irritants, hyperosmolar solutions, antibiotics, prolonged parenteral nutrition, and chemotherapy agents. We present a case of perforation of the internal jugula

BACKGROUND: Nerve transfers are increasingly used to restore upper limb function in infants and toddlers with brachial plexus birth injuries (BPBI). However, standardized, pediatric-specific rehabilitation protocols remain lacking. Adult-focused approaches are often developmentally inappropriate, and the absence of tailored rehabilitation may contribute to high failure rates, particularly for complex transfers.
PURPOSE: To present a structured, milestone-based rehabilitation protocol adapted from the Donor Activation Focused Rehabilitation Approach (DAFRA) for infants and toddlers undergoing

BACKGROUND: Pediatric upper extremity fractures are common and frequently require surgical intervention. Traditional fixation methods using metallic implants, such as Kirschner wires and elastic stable intramedullary nails, offer reliable mechanical stability but necessitate secondary procedures for hardware removal. Polymer-based resorbable implants offer an advantage by eliminating the need for secondary procedures for hardware removal while maintaining efficacy. This systematic review evaluates the safety, efficacy, and clinical outcomes of resorbable compared to non-resorbable implants in

Total ear reconstruction in old avulsion injuries can be extremely challenging for reconstructive surgeons due to regional scarring. This rules out the use of local flaps. In this case report, a young woman presented to us with a seven-year-old total amputation of the left ear, which was initially reconstructed with a Medpor implant and occipito-parietal fascial flap. The implant had to be later explanted due to infection. The authors decided to use a pre-laminated radial free flap with a costal cartilage graft framework for ear reconstruction.
