Single-sequence low-field 3D T1-weighted MRI for scaphoid fracture detection compared to radiography: prospective diagnostic accuracy pilot study.

PURPOSE: To determine the diagnostic accuracy of a novel high-resolution 3D T1-weighted gradient-echo sequence in low-field MRI for detecting scaphoid fractures compared with radiography. METHODS: In this prospective single-center study, patients with suspected scaphoid injury underwent wrist radiography and 0.31 T extremity MRI including a 4-min 3D gradient-echo sequence (3D SST1). Three musculoskeletal radiologists independently evaluated radiographs and the isolated 3D sequence in separate sessions, rating fracture presence, displacement/stability, diagnostic confidence (five-point Likert

Rerupture after flexor tendon repair of the hand and wrist: a retrospective risk factor analysis.

INTRODUCTION: Rerupture after primary flexor tendon repair of the hand and wrist is a serious complication that can substantially impair hand function and restrict activities of daily living, occupational performance, and recreation. Identifying factors associated with rerupture is essential to optimize surgical techniques and postoperative rehabilitation strategies and to improve patient outcomes. METHODS: A retrospective observational cohort study was conducted including patients who underwent surgical repair of flexor tendon injuries of the hand and wrist at a single institution between 201

External Fixation and Adjuvant Pins Versus Volar Locking Plates in Unstable Distal Radius Fractures: A Randomized Controlled Study with a 10-Year Follow-Up.

PURPOSE: To investigate if volar locking plates (VLPs) are superior to external fixation (EF) with adjuvant pins in unstable distal radius fractures after 10 years of follow-up. METHODS: From 2007 to 2009, 111 unstable distal radius fractures were randomized to either EF or VLP. The patients' mean age was 54 years (20-84). At 10 years, 86 patients (77%) were assessed (VLP 43, EF 43). Eighty-one patients (VLP 42, EF 39) had a clinical and radiological examination. Five patients (EF 4, VLP 1) refused to attend the examination, but returned the Disabilities of the Arm, Shoulder, and Hand (QuickD

Surgical versus non-surgical treatment of intra-articular comminuted distal radius fractures (AO 23-C2/C3) is associated with better patient-reported outcomes: an instrumental variable analysis using a national Swedish cohort.

BACKGROUND: The surgical rate for distal radius fractures is steadily rising despite limited evidence of its benefits over non-surgical treatment. Using a natural experimental approach, we aimed to compare patient-reported outcomes following surgical versus non-surgical treatment of distal radius fractures. METHODS: Registered in the Swedish Fracture Register by 36 Swedish hospitals from 2013 to 2018, we included a cohort of 13,356 fractures on 13,031 patients aged 18 years or older with distal radius fractures Arbeitsgemeinschaft fur Osteosynthesefragen (AO) 23-A2.1-2, A3, and C1-C3. Sustainable copper-doped lignin-derived nanobiochar for high-performance hydrogel strain sensors and self-healing flexible supercapacitors.

Hydrogel-based strain sensors (HSSs) hold great promise for flexible and wearable electronics owing to their low modulus, excellent biocompatibility, and strong adhesiveness. However, conventional HSSs typically rely on costly, nonrenewable, and poorly biocompatible conductive fillers. To address this challenge, this study proposes a "hydrothermal coordination‑carbonization" strategy. By doping copper ions into industrial by-product lignosulfonate (LS) molecules to regulate their aggregate structure, highly conductive copper nanoparticle-doped hydroxylated LS-derived nanobiochar (Cu@HLSC) is s

Self-Assembling Biohybrid Bands of Büngner for Axonal Regeneration in the Peripheral Nervous System.

There is broad consensus that successful repair of severe peripheral nerve injuries requires recreating key structural and cellular features of the natural regenerative process, particularly the action of Bands of Büngner (BoB), longitudinal Schwann cell (SC) structures that guide regenerating axons. Current biomaterial-based strategies have shown limited efficacy, in part because they do not sufficiently reproduce the anisotropic and cellular microenvironment established by BoB, resulting in disorganized axonal growth and reduced regenerative efficiency across long gaps. To address this limit

Optimising the utility of patient-reported outcome measurements during recovery after orthopaedic extremity fractures: protocol for a multicentre prospective cohort study.

INTRODUCTION: Recovery after extremity fracture is influenced by injury-related impairments, functional capacity, symptom severity and psychosocial well-being, emphasising the need for patient-reported outcome measures (PROMs). Despite increasing adoption of Patient-Reported Outcomes Measurement Information System (PROMIS) and legacy PROMs in orthopaedic research, substantial knowledge gaps remain regarding their measurement properties, comparability and clinical utility, particularly in fracture populations. Our aim is to develop meaningful guidance to clinicians regarding the use of PROMs in

What Is the Interversion Reliability and Agreement Between the Decision Tree Patient-rated Wrist Evaluation and the Full-length Version?

BACKGROUND: A frequently used strategy to improve patient-reported outcome measure (PROM) response rates and reduce patients' burden with PROM completion is item reduction. In this context, a shortened decision tree version of the Patient-Rated Wrist Evaluation (DT-PRWE) was developed, reducing the number of items from 15 to 5. The DT-PRWE demonstrated excellent psychometric properties in simulated data; however, its psychometric properties have not yet been evaluated in a real-world clinical setting. QUESTIONS/PURPOSES: (1) What is the interversion reliability and agreement between the DT-PR

Artificial intelligence software to help detect fractures on X-rays in urgent care: An Early Value Assessment.

BACKGROUND: Artificial intelligence algorithms have been developed to support clinicians in diagnosing fractures, with the intention to improve the diagnostic accuracy of clinicians reviewing X-rays. The purpose of this rapid early value assessment was to identify the existing evidence base for the technology and to assess whether there was a prima facie case for the technology to represent positive outcomes for patients and a value-for-money investment for people in the National Health Service. METHODS: This early value assessment assessed the potential value of the use of artificial intelli

The Unified Classification System applied to the management of periprosthetic fractures after Motec® total wrist arthroplasty.

INTRODUCTION: As the use of total wrist replacements increases there is an inevitable increase in periprosthetic fractures, as has been observed in other joint replacements. Lower limb arthroplasty surgeons have adopted the Unified Classification System to help classify and manage periprosthetic fractures around hip and knee replacements. In this paper, we demonstrate how the same classification can be used for fractures around wrist replacements. METHODS: A retrospective review of prospectively collected data was performed, analysing all periprosthetic fractures treated in four arthroplasty

Trends of E-Scooter-Related Pediatric Orthopaedic Injuries: 20 Years of Nationally Representative Data.

INTRODUCTION: The rise in popularity of battery-powered 2-wheeled electric scooters (e-scooters) has fostered safety concerns, particularly because of a lack of universally recommended safety precautions. Orthopaedic injuries are common in the pediatric population, yet little is known about the trends of e-scooter-related orthopaedic injuries in this population. The aim of this study was to investigate the national trends in orthopaedic injuries among children operating e-scooters over a twenty-year period. METHODS: The National Electronic Surveillance System, a publicly available database of

Pediatric Both-Bone Forearm Fracture with Flexor Digitorum Profundus Entrapment Causing Pseudo-Volkmann Contracture: A Case Report.

CASE: A 10-year-old boy presented with mechanical flexion contractures of the little, ring, and middle fingers after a both-bone forearm fracture surgery. Despite initial rehabilitation, he was unable to extend the fingers actively or passively in a neutral or extended wrist position, although extension was possible with wrist flexion, suggesting tendon entrapment. Surgical exploration revealed flexor digitorum profundus (FDP) entrapment at the radial fracture site. Surgical release provided immediate resolution of finger contractures. CONCLUSION: This case demonstrates that FDP entrapment ca

Modified retrograde ESIN for diametaphyseal distal radius fractures in children: a retrospective cohort study.

BACKGROUND: Diametaphyseal distal radius fractures in children represent a therapeutic challenge because conventional elastic stable intramedullary nailing (ESIN) may provide insufficient distal fragment control, while alternative fixation methods are more invasive. This study evaluates a modified retrograde ESIN technique with a 90° distal bend ("crutch-bend technique") designed to improve distal fragment support and alignment. METHODS: In this retrospective single-center study, 43 pediatric patients with diametaphyseal distal radius fractures treated between 2022 and 2025 were analyzed. Pat

Restoring balance in the distal radioulnar joint: advancements in minimally invasive surgical approaches.

Distal radioulnar joint (DRUJ) balance is essential for forearm rotation, load transmission, and overall wrist function. Traumatic injuries - including foveal triangular fibrocartilage complex (TFCC) tears, distal radius malunions and disruption of the distal oblique bundle - represent the most common causes of symptomatic DRUJ instability. Accurate differentiation between physiological laxity and clinically relevant instability remains challenging owing to limited reliability of physical examination and the subtle nature of soft-tissue pathology. Comprehensive assessment requires detailed ana

Volar Tilt as a Predictor in Dorsal Proximal Interphalangeal Joint Fracture-Dislocations.

Background: Restoring the volar lip of the middle phalanx is key to regaining concentric joint gliding after dorsal proximal interphalangeal (PIP) joint fracture-dislocations, but no reliable radiographic assessment currently exists. This study introduces volar tilt as a new radiographic measure. Methods: We evaluated 27 dorsal PIP fracture-dislocations in 25 patients, analysing clinical outcomes, osteoarthritis (OA) and volar tilt on lateral radiographs at injury, after extension block pinning and at 16-year follow-up. Results: Initial volar tilt averaged 22°, improving to 12° after extension

The Efficacy of Hook Plates Around the Elbow: A Stable Fixation.

Hook plates are low-profile implants commonly used for olecranon fractures and osteotomies, offering stable fixation especially in complex or osteoporotic fractures. This study evaluates the clinical and functional outcomes of hook plate fixation around the elbow. We retrospectively analyzed 20 patients (mean age 52.7 years) treated between 2016 and 2023 for olecranon or distal humerus fractures using hook plates alone or combined with other implants. Inclusion criteria were patients aged 16 years or older with at least 12 months of follow-up. Exclusion criteria included ASA ≥ IV, open fractur

The impact of immediate surgical intervention versus conservative treatment on functional recovery and quality of life in older adults with distal radius fractures.

Distal radius fractures are among the most prevalent fractures, accounting for approximately 17% of all fractures treated. Given the high prevalence of these fractures, particularly in the older adults population, determining the optimal treatment approach is of utmost clinical importance. While previous research has highlighted both the advantages and drawbacks of surgical and conservative treatment strategies, there remains a lack of consensus regarding which approach yields superior outcomes in terms of functional recovery and quality of life (QoL). This study was designed to determine whet

A Tactile Trans-Flexor Carpi Radialis Surface Landmark Technique for Median Nerve Local Injection: A Cadaveric Study.

PURPOSE: Median nerve (MN) block under local anesthesia at the wrist is commonly performed but can be limited by anatomic variation, particularly absence of the palmaris longus tendon. This study investigates a novel trans-flexor carpi radialis (trans-FCR) technique for MN anesthesia that uses the flexor carpi radialis (FCR) tendon as a consistent surface landmark. METHODS: Fourteen fresh frozen upper extremity cadaver specimens were used. A 25-gauge needle was inserted through the FCR sheath approximately 4 cm proximal to the wrist crease in a transverse fashion, angled approximately 0° to t
