[Autotransplantation of composit complexes of tissues for restoration of the combined bone-soft tissue post-traumatic defects of the wrist and the hand].

The experience of treatment of 27 patients, suffering the extended bone-soft tissue defects of the wrist and the hand was summarized. In 12 patients (1-st group) the bone plasty was performed, using nonvascularized fragment of os ilei wing with simultaneous excision of the damaged soft tissues and free transplantation of m. latissimus dorsi or m. serratus anterior flaps. In 15 patients (2-nd group) autotransplantation of m. serratus anterior flap, including the vascularized costal fragments in its content, was performed. The musculo-costal flap application had appeared a perspective procedure

Diagnosis and anatomic reconstruction of extensor carpi ulnaris subluxation.

PURPOSE: To evaluate the efficacy of dynamic ultrasound to diagnose extensor carpi ulnaris (ECU) subluxation and assess the results of a new technique for anatomic ECU tendon sheath reconstruction. METHODS: Dynamic ultrasound was used to confirm the diagnosis of ECU tendon subluxation in patients presenting with painful snapping of the ulnar wrist during supination and pronation. Twenty-one patients with persistent subluxation had reconstruction of the ECU tendon sheath using a new technique. In this procedure, the ECU tendon is elevated within its sheath dorsally and radially. The distal uln

Changes in impairment and function after static progressive splinting for stiffness after distal radius fracture.

The purpose of this study was to describe changes in range of motion, grip strength, and function in subjects treated with static progressive splinting for stiffness after distal radius fracture. A retrospective review was conducted on 25 patients; outcomes reviewed included wrist/forearm motion, grip strength and Disability of the Arm, Shoulder, and Hand (DASH) scores. The Wilcoxon-Signed Rank test was used to assess differences between pre-/postsplinting outcome measurements. Spearman correlation coefficients were calculated for the DASH scores with each of the other measurements. Wrist exte

Arthroscopic treatment of triangular fibrocartilage wrist injuries in the athlete.

BACKGROUND: Triangular fibrocartilage (TFC) injuries are an increasingly recognized cause of ulnar-sided wrist pain and can be particularly disabling in the competitive athlete. Previous studies show that arthroscopic debridement or repair can improve symptoms, but the results of arthroscopic treatment of TFC injuries in high-level athletes have not yet been reported. HYPOTHESIS: Arthroscopic debridement or repair of wrist TFC injury will allow a high rate of return to full function in the elite athlete. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Between 2001 and 2005, 16 com

Distal radius fractures in older patients: is anatomic reduction necessary?

UNLABELLED: The relationship between radiographic and functional outcomes in older patients with distal radius fractures is controversial. We explored this relationship by assessing the influence of radiographic displacement and fracture comminution on the functional outcomes of these fractures. We also asked whether operative intervention and demographic factors (age, gender, duration of followup) influenced outcome. We examined 53 patients older than 55 years with distal radius fractures with various functional assessments: range of motion (ROM) and strength measurements, three subjective su

Treatment of complex hand trauma using the distal ulnar and radial artery perforator-based flaps.

The clinical value of distal ulnar or radial artery adipofascial perforator flaps is shown in a series of 30 patients with severe hand and wrist injuries and major soft tissue defects requiring coverage. There were 22 men and 8 women, aged 16-73 years. The defects were dorsal and/or palmar, with or without transpalmar or transcarpal amputation, or amputation of the thumb and/or the digits. Tendon injuries have been treated primarily or secondarily, or reconstructed using silicon rods. In all cases, after surgical debridement of the wound, reconstruction of the defect was done using distal ulna

Short-term to medium-term outcomes of cemented Vitallium radial head prostheses after early excision for radial head fractures.

Studies show that the radial head is a significant stabilizer to valgus and varus stresses and external rotatory stability. We review the outcomes of patients who had radial head replacement in our institution. Six patients with Mason-Johnston type III or IV radial head fractures underwent radial head replacement and were evaluated by radiologic and clinical assessment. The American Shoulder and Elbow Surgeons score, DASH (Disabilities Arm, Shoulder and Hand) score, and Broberg and Morrey Performance Index were calculated. Average follow-up was 29.7 months. The Broberg and Morrey score was exc

Interventions for treating wrist fractures in children.

BACKGROUND: Approximately a third of all fractures in children occur at the wrist, usually from falling onto an outstretched hand. OBJECTIVES: We aimed to evaluate removable splintage versus plaster casts (requiring removal by a specialist) for undisplaced compression (buckle) fractures; cast length and position; and the role of surgical fixation for displaced wrist fractures in children. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (October 2007), the Cochrane Central Register of Controlled Trials (The Cochrane Library 2007, Issue 4), ME

The epidemiology of acute sports-related fractures in adults.

OBJECTIVE: To determine the incidence and epidemiology of acute sports-related fractures in adults. DESIGN: Retrospective analysis of a prospectively collected database containing all in-patient and out-patient fractures in a defined patient population in 2000. SETTING: Orthopaedic Trauma Unit. RESULTS: There were 5953 fractures in 2000 of which 761 (12.8%) were caused in sporting accidents. The overall incidence was 142/10(5) with 261/10(5) in males and 35/10(5) in females. The mean age was 25.6 years. 41 sports caused the fractures but 10 sports accounted for 86.8% of fractures. In 40 spo

Wrist mobilization following volar plate fixation of fractures of the distal part of the radius.

BACKGROUND: Plate fixation of the distal part of the radius is believed to improve wrist motion by allowing earlier exercises. We performed a clinical trial comparing mobilization of the wrist joint within two weeks (early motion) or at six weeks (late motion) after volar plate fixation of a fracture of the distal part of the radius in order to test the null hypothesis that there are no differences in the flexion-extension arc three and six months after surgery. METHODS: Sixty patients with an isolated fracture of the distal part of the radius that was treated with a single, fixed-angle volar

Delayed radial nerve palsy due to entrapment of the nerve in the callus of a distal third humerus fracture.

A 45-year-old male patient was referred for management of radial nerve palsy. His past medical history revealed that he had been injured in a car accident and broken his left humerus 4 months ago. Primary stabilization of the fracture has been achieved by the application of a long-arm plaster cast. His medical reports displayed that he had experienced no clinical signs of radial nerve palsy at that time. After the cast was removed, he noticed that he could not extend his wrist. Surgical exploration revealed that the radial nerve was encased inside the callus. He had probably not realized the w

Reliability and validity of the German version of "the Patient-rated Wrist Evaluation (PRWE)" as an outcome measure of wrist pain and disability in patients with acute distal radius fractures.

The aim was to test the reliability and validity of the German version of the Patient-rated Wrist Evaluation (PRWE) for patients with acute distal radius fractures. To estimate the reliability and construct validity, 44 patients completed a questionnaire booklet containing the German PRWE, the Short Form-36 (SF-36), and the Disability of the Arm, Shoulder, and Hand (DASH) four to six weeks after the fracture, and the PRWE again seven days later. For reliability, the intraclass correlation coefficient (ICC) was 0.94 for the PRWE total score. Its internal consistency was 0.89 (Cronbach's alpha).

Management of intra-articular metacarpal base fractures of the second through fifth metacarpals.

Intra-articular fractures of the second through fifth metacarpal bases are uncommon injuries but can result in serious morbidity if improperly managed. These injuries usually occur because of forced flexion of the wrist with simultaneous extension of the arm, as occurs with a punch or a fall. As there are few large series of reports for these injuries, there is no consensus in the current literature on the most appropriate treatment for them. Whereas some authors have reported successful results after closed reduction, many recommend open reduction with internal fixation to ensure the integrit

Treatment of intra-articular fractures of the distal radius: fluoroscopic or arthroscopic reduction?

In a randomised prospective study, 20 patients with intra-articular fractures of the distal radius underwent arthroscopically- and fluoroscopically-assisted reduction and external fixation plus percutaneous pinning. Another group of 20 patients with the same fracture characteristics underwent fluoroscopically-assisted reduction alone and external fixation plus percutaneous pinning. The patients were evaluated clinically and radiologically at follow-up of 24 months. The Disabilities of the Arm, Shoulder, and Hand (DASH) questionnaire and modified Mayo wrist score were used at 3, 9, 12 and 24 mo

Cartilage-retaining wafer resection osteotomy of the distal ulna.

UNLABELLED: Ulnar-sided wrist pain resulting from ulnar impaction is common. We describe a new cartilage-retaining wafer resection osteotomy designed to keep the cartilage intact and decompress the ulnocarpal articulation without requiring internal fixation. We retrospectively reviewed seven patients with ulnar impaction who had the procedure. The minimum followup was 14 months (mean, 30 months; range, 14-38 months). The mean change in ulnar variance was -1.29 mm. Patients showed radiographic healing by a mean of 11 weeks. Our preliminary results suggest the cartilage-retaining wafer resection

Reproducibility of sonographic measurements of the median nerve.

OBJECTIVE: The purpose of this study was to evaluate the reproducibility of median nerve cross-sectional area (CSA) measurements using the indirect method (ellipsoid formula) and the direct or tracing method. METHODS: The median nerve CSA was measured in 22 wrists of patients with carpal tunnel syndrome by means of high-frequency sonography. Measurements were made at the level of the pisiform bone by a standardized sonographic examination protocol. Two observers with different levels of experience performed the measurements, independently and blinded. An analysis of reliability was carried ou
