The Enduro motorcyclist's wrist and other overuse injuries in competitive Enduro motorcyclists: a prospective study.

According to public opinion, Enduro motorcycling is a dangerous sport. Little is known about overuse injuries in connection with this sport. The study consisted of two phases. Phase 1 was planned as a prospective feasibility study for phase 2. The latter was initiated as a prospective investigation, comprising a random sample of the participants of phase 1. Enduro motorcyclists were interviewed with a prepared questionnaire. Clinical examinations were performed immediately before, and after, a major Enduro race, including pain assessment in the hand/wrist and the forearm using the visual analo

Reconstruction of a 4-cm human median nerve gap by including an autogenous nerve slice in a bioabsorbable nerve conduit: case report.

Nerve gaps up to 3 cm in length can be reconstructed in the human with a bioabsorbable nerve conduit; however, nerve gaps greater than 3 cm have not been reconstructible in animal models unless a neurotrophic factor, like cultured Schwann cells, was added to the nerve conduit. It is the purpose of this article to present the result of human nerve regeneration across a 4-cm nerve gap in the median nerve at the wrist when a slice of autogenous normal nerve was placed into the middle of a bioabsorbable nerve conduit.

Effect of wrist posture on carpal tunnel pressure while typing.

Long weekly hours of keyboard use may lead to or aggravate carpal tunnel syndrome. The effects of typing on fluid pressure in the carpal tunnel, a possible mediator of carpal tunnel syndrome, are unknown. Twenty healthy subjects participated in a laboratory study to investigate the effects of typing at different wrist postures on carpal tunnel pressure of the right hand. Changes in wrist flexion/extension angle (p = 0.01) and radial/ulnar deviation angle (p = 0.03) independently altered carpal tunnel pressure; wrist deviations in extension or radial deviation were associated with an increase i

The ultrasonographic wrist-to-forearm median nerve area ratio in carpal tunnel syndrome.

OBJECTIVE: Peripheral nerve ultrasound is an emerging tool in the diagnosis of carpal tunnel syndrome (CTS). Although numerous publications have cited an increased median nerve area at the wrist to be the diagnostic of CTS, there has been considerable variability in the published normal values for this measurement. Our objective is to collect data on the wrist-to-forearm ratio (WFR) of median nerve area in patients with CTS and healthy controls. METHODS: Patients with electrodiagnostically proven CTS underwent ultrasonography of the median nerve at the wrist and forearm. The median nerve area

Controversies in carpal tunnel syndrome.

Compression of the median nerve at the wrist (carpal tunnel syndrome) is the most common compression neuropathy of the upper extremity. Surgical decompression has become one of the most frequently performed peripheral nerve procedures. Despite the popularity of the procedure, and the volume of clinical and basic science information available, differences of opinion exist as to diagnosis and management. This review discusses several of these controversies and presents both recent and historical data that have led to some of these disagreements.

The quality and strength of evidence for etiology: example of carpal tunnel syndrome.

PURPOSE: The purpose of this investigation was to evaluate the quality and strength of scientific evidence supporting an etiologic relationship between a disease and a proposed risk factor using a scoring system based on the Bradford Hill criteria for causal association. METHODS: A quantitative score based on the Bradford Hill criteria (qBHs) was used to evaluate 117 articles presenting original data regarding the etiology of carpal tunnel syndrome: 33 (28%) that evaluated biological (structural or genetic) risk factors, 51 (44%) that evaluated occupational (environment or activity-related) r

Diffusion tensor imaging and tractography of the median nerve in carpal tunnel syndrome: preliminary results.

The purpose was to demonstrate the feasibility of in vivo diffusion tensor imaging (DTI) and tractography of the human median nerve with a 1.5-T MR scanner and to assess potential differences in diffusion between healthy volunteers and patients suffering from carpal tunnel syndrome. The median nerve was examined in 13 patients and 13 healthy volunteers with MR DTI and tractography using a 1.5-T MRI scanner with a dedicated wrist coil. T1-weighted images were performed for anatomical correlation. Mean fractional anisotropy (FA) and mean apparent diffusion coefficient (ADC) values were quantifie

Association of occupational and non-occupational risk factors with the prevalence of work related carpal tunnel syndrome.

PURPOSE: Several occupational and personal risk factors cause the development of carpal tunnel syndrome (CTS). The purpose of the study was to evaluate both non-occupational and occupational factors associated with CTS in industrial workers. METHODS: A cross sectional study was designed with 400 industrial workers (77% male, 23% female) randomly selected. Workers' upper extremities were examined and related signs and symptoms were assessed. Questionnaires about personal and occupational risk factors were completed and suspicious cases were referred for NCV (nerve conduction velocity) testing

[Acute carpal tunnel syndrome in a patient with Marfan syndrome].

Acute carpal tunnel syndrome (ACTS) is rare and is mostly the result of fractures of the distal radius or the carpal bones. This paper gives the first report of an ACTS following contusion of the wrist as the result of an extensive haematoma of the flexor tendon sheath, which did not appear until 50 hours after the injury was sustained but then developed rapidly. The patient suffers from Marfan syndrome. This disease is associated with pathologic changes to the major vessels, and especially the aorta, and of the smaller peripheral vessels. It is assumed that the haematoma arose from an aneurys

Carpal tunnel syndrome: matching minimally invasive surgical techniques. Technical note.

In the past few years, several different minimally invasive surgical techniques have been proposed to decompress the median nerve at the wrist. Use of these techniques has become widespread due to fewer local complications, faster functional recovery, and reduced surgical time. In this paper the authors compare 3 different minimally invasive surgical techniques used at their institution in the past 13 years. Between January 1994 and January 2007, 891 patients underwent 1272 surgeries at the authors' institution for carpal tunnel syndrome (CTS), for which a minimally invasive technique was used

Acute carpal tunnel syndrome.

Carpal tunnel syndrome is considered the most common of the chronic compressive neuropathies. Its cause is generally unknown. Acute carpal tunnel syndrome, which is much less common, is more often directly related to fractures and fracture-dislocations about the wrist, hemorrhagic conditions, and vascular disorders involving the wrist. Many rare and unusual causes have been described, including chronic conditions that may be associated with acute carpal tunnel syndrome, such as rheumatologic disorders and anomalous anatomy. In contrast to the more common chronic idiopathic form, the acute form

Alteration of proximal conduction velocity at distal nerve injury in carpal tunnel syndrome: demyelinating versus axonal change.

The objective of this study was to determine the cause of median forearm motor conduction velocity (FMCV) slowing in patients with carpal tunnel syndrome, due to either focal conduction abnormality over wrist or retrograde conduction slowing, and to decide whether the slowing is related to severity of compression or not. Fifty carpal tunnel syndrome patients confirmed by conventional nerve conduction study with abnormal electromyography of the abductor pollicis brevis muscle were group 1, and 100 with normal electromyography, group 2. One hundred volunteers served as controls. In addition to c

Effects of wrist splinting for Carpal Tunnel syndrome and motor nerve conduction measurements.

BACKGROUND: Carpal tunnel syndrome (CTS) is one of the most common disease among the entrapment neuropathies. Wrist splinting has been conventionally used for the CTS treatment. The purposes of this study were to assess the efficacy of wrist splinting for CTS, and to evaluate the value of the motor nerve conduction measurement as a prognostic indicator for CTS. METHODS: Two hundred and fourteen hands with CTS were treated by wrist splinting, and reviewed after a mean follow up of seven months. Severity of symptoms were minimal lesions in 177 hands, intermediate lesions in 33 hands, and severe

Endoscopic revision of carpal tunnel release.

BACKGROUND: Open revision has been the standard approach for treatment of recurrent carpal tunnel syndrome. The authors hypothesized that endoscopic revision would yield results comparable to those with open revision. METHODS: Forty-one patients with unilateral recurrence were analyzed prospectively before and after endoscopic revision for a period of 1 year. All had clinical signs or symptoms, a positive response to a steroid injection, and electrodiagnostic findings consistent with carpal tunnel syndrome after primary open release and had failed to improve after an average of 16 months. Case study: musicians' playing-related injuries.

Professional and student musicians are at high risk of acquiring a playing-related injury at some point in their careers. Yet, specialized healthcare for musicians is scarce and expensive for most self-employed musicians. Diagnosing these injuries is challenging, and simply taking a break from an activity that has caused physical problems does not address the ergonomic and biomechanic causes of the problem. Under these circumstances, it is not surprising that musicians are reluctant to seek care, and when they do, lack trust in the care that is provided to them. This article is a case presenta

Anatomic variations of the median nerve in carpal tunnel release.

Carpal tunnel syndrome (CTS) is a well-known clinical entity. Release of the transverse carpal ligament is considered to be the treatment of choice. Both open and endoscopic release of the transverse carpal ligament in CTS has yielded satisfactory results. Although these procedures are very common in surgical practice, inadequate release and intraoperative damage to neural elements are very frustrating complication for both the patient and the surgeon. The purpose of this study was to demonstrate incidental intraoperative findings of variations of the standard median nerve anatomy. We obtained

Carpal tunnel syndrome: a case-control study evaluating its relationship with body mass index and hand and wrist measurements.

This case-control study investigated the associations between the body mass index (BMI), hand and wrist measurements and carpal tunnel syndrome (CTS). The hands and wrists of 50 patients with CTS and 50 age- and sex-matched controls were measured. The right and left wrist indices (wrist depth/wrist width) were significantly greater in CTS patients (mean = 0.71. SD = 0.04) than in the controls (mean = 0.69 SD = 0.04). The hand index (hand length/palm width) and BMI were not significantly different in the two groups. The hand, but not the wrist, index was found to correlate with the BMI. These r
