Spontaneous Closed Extensor Tendon Ruptures in the Nonrheumatoid Hand and Wrist.

Extensor tendon disruptions are frequently seen in emergency rooms and hand surgery offices. Traumatic open injuries are more familiar, but spontaneous closed extensor tendon ruptures represent an important clinical entity. Often occurring without any prodromal warning, they can surprise patients and their providers alike. Spontaneous extensor tendon ruptures often occur in the setting of systemic medical conditions or hand and wrist pathology, with the mechanisms that lead to rupture often dependent upon the underlying comorbidities. Remaining knowledge of the factors that place patients at r

Hospital Resource Utilization and Associated Costs of Common Hand Fractures in a Large Contemporary Cohort.

OBJECTIVES: Hand and wrist injuries comprise roughly 25% of emergency department visits and generate major costs. Growing incidence and surgical treatment highlight the need to understand resource use and economic impact. METHODS: We retrospectively analyzed Dutch adults (2017-2022) with phalangeal, metacarpal, or carpal fractures or finger dislocations in a level-2 trauma center. Resource use and treatments were recorded, costs calculated as volume × unit cost, and differences assessed with t tests, analysis of variance, and regression with bootstrap. RESULTS: A total of 8453 patients were

Have Commonly Utilised Patient-Reported Outcome Measures Been Validated for the Japanese Language and Culture Amongst Patients with Distal Radius Fractures? A Systematic Review.

Background: Patient-reported outcome measures (PROMs) are questionnaires used to evaluate a patient's functional status, examine outcomes and inform clinical decisions. PROMs have been primarily developed with English-speaking populations and should be validated in the patient population in whom they are being applied. As present, the validity of commonly utilised distal radius fracture (DRF) PROMs in patients who speak Japanese is unknown. The purpose was to evaluate the validity and psychometric properties of common DRF PROMs in the Japanese population. Methods: A systematic review was condu

Scaphoid Fractures: How to Treat Symptomatic Patients with a Normal X-Ray and When Should We Operate on Acute Fractures?

The scaphoid is the most injured carpal bone, with an incidence of 12 per 100,000 per annum in the United Kingdom (UK). Fractures of the scaphoid can be challenging to diagnose, both clinically and radiographically, and have a propensity to fail to unite or develop necrosis. There is debate regarding whether to manage acute fractures conservatively or offer early surgical fixation. The poor predictive value of initial examination and plain imaging not only results in missed fractures but also, conversely, in a fivefold overtreatment, which has subsequent significant socioeconomic consequences.

Diagnostic Use of Four-Dimensional Computed Tomography in Scapholunate Ligament Tear Detection.

Background: Wrist arthroscopy is widely used to assess the intactness of ligaments, including the scapholunate ligament (SLL), as it allows direct visualisation and palpation. However, arthroscopy is invasive and costly, while traditional imaging modalities, such as X-rays and MRI, often miss ligament injuries due to their static nature. A promising alternative is four-dimensional computed tomography (4DCT), which allows dynamic, non-invasive visualisation of wrist motion in real time. This study evaluates the diagnostic potential of 4DCT for detecting SLL tears by comparing its findings with

Kirschner Wire Skeletal Fixation in the Hand and Wrist: Risk of Infection and Predictors of Progression to Major Complications.

PURPOSE: Kirschner wire (K-wire) infections continue to be a challenge in hand surgery. Our purpose was to quantify the frequency of K-wire site infections of the hand and wrist, to tabulate major and minor infectious manifestations, to identify factors that may predispose a patient to major complications, and to generate an updated algorithm for prognostication and treatment of K-wire site infections. METHODS: Patients treated with K-wires in the fingers, hand, and wrist who developed infections between 2018 and 2024 at our institution were identified. Information regarding patient factors,

Reliability of Radiographic Parameters in the Evaluation of Scapholunate Dissociation.

PURPOSE: Accurate and reliable radiographic measurement is essential to diagnose and evaluate surgical treatment outcomes for scapholunate dissociation (SLD). Although published standards for measuring radiographic parameters have demonstrated excellent interrater and intrarater reliability in normal patients, we were unable to find reliability data for patients with SLD. We hypothesized that six radiographic parameters for SLD would have good reliability in normal and injured patients. METHODS: Wrist radiographs of patients with SLD and a control group of uninjured patients seen at our insti

[Arthroscopic stabilization of the distal radioulnar joint and treatment of TFCC lesions].

Arthroscopic treatment of TFCC lesions and stabilization of the DRUJ offer a differentiated range of findings-oriented procedures. Theses include central smoothing/partial resection via peripheral capsular sutures to foveal refixation and, in cases of irrepairable damage, anatomical reconstruction of the DRUJ-stabilizing radioulnar ligaments. The Palmer and Atzei classifications are widely used in everyday clinical practice and offer a structured approach to diagnosis and indication, with foveal integrity being the central criterion for DRUJ stability. Medical history, clinical examination, im

Fracture distribution in cross-country skiing accidents: an observational study from the Swedish Fracture Register.

BACKGROUND AND PURPOSE: Cross-country skiing (CCS) is a popular winter activity in Sweden, but little is known about what type of orthopaedic injuries are sustained following accidents in the general population. The purpose of this study was to describe the fracture distribution in accidents during cross-country skiing. METHODS: In this registry-based observational study, all patients who sustained one or more fractures in CCS accidents registered in the Swedish Fracture Register (SFR) with injury date between January 1, 2015 and June 30, 2022 were included. Fractures were classified using the

Arthroscopic debridement with microfragmented adipose tissue for wrist and hand chondral lesions in professional athletes.

BACKGROUND: Focal chondral lesions of the wrist and hand are an underrecognized source of pain and functional limitation in athletes. Their management remains challenging due to the limited healing capacity of hyaline cartilage, the biomechanical demands of small joints, and the lack of standardized treatment algorithms. Arthroscopy allows accurate diagnosis and targeted treatment with minimal morbidity, while biologic augmentation using autologous adipose-derived products has emerged as a potential adjunct to improve clinical outcomes. OBJECTIVE: To evaluate the clinical outcomes of arthrosc

Provision of Video Education Instead of an In-person Visit for Pediatric Buckle Fractures: A Randomized Controlled Trial.

INTRODUCTION: Pediatric buckle fractures are common. These are often referred to as orthopaedic surgery offices, despite evidence that specialty evaluation is unnecessary. Here, we sought to determine if the provision of an educational video instead of an in-person visit with an orthopaedic provider would be non-inferior for patient-reported outcomes and demonstrate improvement in other outcomes, such as patient, caregiver, and provider time spent on caring for this injury. METHODS: This was a single-center, prospective, randomized controlled, non-inferiority trial of 41 patients with distal

Uncommon Encounter: Carpometacarpal Dislocation of 2nd-5th Digits without Concomitant Fractures - A Rare Case Report.

INTRODUCTION: Carpometacarpal (CMC) joint dislocations represent <1% of all injuries to the hand and wrist regions. Up to 70% of CMC dislocations are missed or misdiagnosed due to severe swelling and overlapping of bones on the radiograph of the wrist. CASE REPORT: We report the case of a 36-year-old male patient, following a road traffic accident, who complained of pain and swelling in his left hand. On plain radiographs, 2nd-5th CMC joints dislocation without any fracture was found, confirmed on computed tomography scan . Closed reduction with manual traction and slab application was attemp

Hierarchically Oriented Carbon Nanotubes/Poly(vinyl Alcohol) Composite Conductive Hydrogels for Flexible Strain Sensors.

Hydrogels have been widely used in flexible electronics due to their water content similar to that of muscles and other biological tissues. However, it was a challenge to significantly improve mechanical robustness and electrical conductivity owing to their trade-off. To address this, a synergistic method of freeze casting and hot pressing was proposed to create a multiscale hierarchical structure in the hydrogel. In detail, the mixture of poly(vinyl alcohol) (PVA) and cellulose nanofibers (CNFs) with dispersed carbon nanotubes (CNTs) was freeze-cast and freeze-dried, then hot-pressed to enhan
