Anatomical Variation of the Superficial Palmar Branch of the Radial Artery: A Cadaveric Case Report.

Understanding anatomical variations in hand vasculature is critical for hand and vascular surgery. This case report describes rare unilateral variations in the course and branching of the superficial palmar branch of the radial artery (SPRA) in the hand of a 100-year-old Caucasian female donor. The forearm, wrist, and hand were dissected bilaterally to demonstrate the musculature and neurovasculature. Two anatomical variations were observed in the right hand of the donor body. The first variant was the SPRA coursing superficial to the thenar muscles after emerging from the radial artery rather

Severe Spaghetti Wrist Injury With Complete Laceration of Median Nerve, Radial and Ulnar Arteries, and Multiple Flexor Tendons: A Case Report and Literature Review.

Spaghetti wrist injuries are complex distal forearm lacerations involving flexor tendons, nerves, and vessels, associated with significant functional morbidity. We present the case of a 64-year-old right-handed man with a deep cut wound to his left forearm and clinical signs of distal ischemia, complete loss of digital flexion, and palmar anesthesia. An urgent examination was performed, identifying sections of the radial and ulnar arteries, median nerve, and injuries to the palmaris longus, flexor carpi ulnaris, flexor carpi radialis, superficial and deep flexor tendons of the fingers, and fle

Buried Versus Exposed K-Wires in Hand Fracture Fixation: A Meta-Analysis of Outcomes.

A key decision in orthopaedic surgery is whether to leave Kirschner wires (K-wires) protruding through the skin or to cut and bury them subcutaneously. This choice may affect infection risk, need for secondary procedures, patient comfort and cost. However, the evidence comparing buried versus exposed K-wires has been conflicting, leaving surgeons with no clear consensus. We performed a meta-analysis to quantitatively compare outcomes of buried versus exposed K-wires in phalangeal, metacarpal and distal radius fractures. We followed the Preferred Reporting Items for Systematic Reviews and Meta-

Dorsal Triangular Fibrocartilage Complex Arthroscopic Repair.

BACKGROUND: Dorsal triangular fibrocartilage complex (TFCC) is a newer entity of its family, which was not included in the Palmer classification. We believe that tear of the dorsal TFCC can cause pain and disability. Literature on this type of tear is limited, in addition, outcomes post-repair, especially arthroscopically, are scarce. PURPOSES: This article defines dorsal TFCC anatomy and investigates the clinical and functional outcomes post-TFCC dorsal tear arthroscopic repair. MATERIALS AND METHODS: An 18-year retrospective review of 32 patients with arthroscopic repair of TFCC dorsal tea

The Lateral Antebrachial Neurocutaneous Flap: Case Series.

BACKGROUND: The neurocutaneous flap was developed based on the understanding that every superficial cutaneous nerve includes vessels, also known as vasa nervorum or paraneural vessels, which run around and inside the nerves and deliver blood to the skin above. A cutaneous perforator connected to paraneural vessels that vascularize the skin and nerves is referred to as a neurocutaneous perforator. The lateral antebrachial cutaneous nerve (LACN), the most dependable blood supply from the primary underlying veins of the neurocutaneous flap, is the subject of this study. METHODS: We reviewed the

Keloidal dermatofibroma: Clinicopathological comparison of 52 cases with a series of 2077 other dermatofibromas.

Dermatofibroma is a common benign skin lesion with a contested etiology: some believe it is a neoplasm while others propose minor injuries initiate it. Many dermatofibroma variants have been described, including keloidal dermatofibroma, which is unusual by bearing keloidal collagen. Keloidal dermatofibroma was first described in 1998 and only 15 cases have been reported. Since keloids are driven by skin injuries, the existence of keloidal dermatofibroma has been suggested to support the injury hypothesis of dermatofibroma etiology. To better understand keloidal dermatofibroma characteristics a

Complications associated with open reduction and internal fixation for adult distal humerus fractures: a multicenter retrospective study.

BACKGROUND: Open reduction and plate fixation are the preferred treatment options for most distal humerus fractures in adults. However, it is often challenging for orthopedic surgeons because of the complex anatomy and the difficulty in achieving stable fixation. This multicenter study aimed to analyze the complication types and rates of patients with distal humerus fractures treated with open reduction and plate fixation, and compare the results with those found in the literature. In addition, we describe the clinical outcomes. METHODS: This retrospective multicenter study was conducted betw

A unique bilateral accessory forearm flexor muscle.

Muscular and neurovascular variations in the upper extremity are of utmost clinical significance. Here we report a unique bilateral accessory muscle in the forearm and palm of an 89-year-old male cadaver. The accessory muscle presented two bellies on the right side, one in the forearm, innervated by the anterior interosseous nerve, and the other in the palm, innervated by a branch of the median nerve. A long tendon interconnected the two bellies. On the left side, the muscle had a single belly in the palm, which began at the end of a long tendon that extended from the forearm. However, on both

Palmar Creases and Their Implication on Carpal Tunnel Surgery.

BACKGROUND: A thorough knowledge of normal and variant anatomy of the wrist and hand is fundamental to avoiding complications during carpal tunnel release. The purpose of this study was to document variations of the surface anatomy of the hand to identify a safe zone in which the initial carpal tunnel incision could be placed. The safe zone was identified as the distance between the radial side of hook of hamate and the ulnar edge of the origin of the motor branch of the median nerve (MBMN). METHODS: Kaplan's cardinal line and other superficial markers were used to estimate the size of the sa

Outcomes of an aseptic technique for Kirschner wire percutaneous pinning in the hand and wrist.

INTRODUCTION: Kirschner wires (K-wires) have been shown to effectively stabilize fractures of the hand and wrist, but are associated with high infection rates, which may limit its use. Previous literature has attributed the risk of infection to many different aspects of a fixation technique. However, we introduce an approach to percutaneous K-wire pinning to mitigate the risk of infection. METHODS: Patients undergoing K-wire fixation procedures of the hand and wrist were retrospectively queried. All cases were performed under the same principles of our technique. None of the K-wires were buri

Sonography of tendon pathology in the hand and wrist.

Traumatic and non-traumatic tendon lesions are common at the wrist and hand. For the diagnosis, therapy management, and long-term prognosis of tendon lesions, a detailed understanding of the complex anatomy and knowledge of typical injury patterns is crucial for both radiologists and clinicians. Improvements in high-resolution ultrasound are producing high-quality images of the superficial tendinous and peritendinous structures. Thus, ultrasound is a valuable first-choice tool for visualizing traumatic, inflammatory, and degenerative conditions of the extensor and flexor tendons, particularly

Clinical Outcome for Surgical Treatment of Traumatic Neuroma With a Processed Nerve Allograft: Results of a Small Prospective Case Series.

Processed nerve allografts are used increasingly in the treatment of traumatic neuroma in small sensory nerves. The goal of the present study was to investigate the use of an allograft after different intervals between injury and repair and to analyze results, not only for the success of pain relief, but also for potential recovery of sensation in time. Four patients with painful neuroma in small sensory nerves in the lower extremity were surgically treated with a decellularized allograft. Patients were followed prospectively for at least 1 y. Clinical outcome was assessed using the Likert sca

The importance of radial multiplanar reconstructions for assessment of triangular fibrocartilage complex injury in CT arthrography of the wrist.

BACKGROUND: Triangular fibrocartilage complex (TFCC) lesions commonly cause ulnar-sided wrist pain and instability of the distal radioulnar joint. Due to its triangular shape, discontinuity of the TFCC is oftentimes difficult to visualize in radiological standard planes. Radial multiplanar reconstructions (MPR) may have the potential to simplify diagnosis in CT wrist arthrography. The objective of this study was to assess diagnostic advantages provided by radial MPR over standard planes for TFCC lesions in CT arthrography. METHODS: One hundred six patients (49 women, 57 men; mean age 44.2 ± 1

Anatomy of the palmar cutaneous branch of the median nerve: A review.

The palmar cutaneous branch of the median nerve (PCBm) supplies afferent innervation to the volar aspect of the hand. It consistently originates from the radial side of the median nerve, travels in relation to the tendons of the palmaris longus and flexor carpi radialis muscles, and courses superficially through fascial planes to reach the surface of the palm. Because it is at risk of injury in numerous operations, this review serves to provide a summary of anatomical findings regarding the PCBm across various studies to aid orthopedists and other clinicians in anticipating the location of the

Reliability of ultrasound speckle tracking with singular value decomposition for quantifying displacement in the carpal tunnel.

Inhibited movement patterns of carpal tunnel structures have been found in carpal tunnel syndrome (CTS) patients. Motion analysis on ultrasound images allows us to non-invasively study the (relative) movement of carpal tunnel structures and recently a speckle tracking method using singular value decomposition (SVD) has been proposed to optimize this tracking. This study aims to assess the reliability of longitudinal speckle tracking with SVD in both healthy volunteers and patients with CTS. Images from sixteen healthy volunteers and twenty-two CTS patients were used. Ultrasound clips of the th

Landmark Technique for a Wrist Block.

Effective anesthesia of the hand and wrist has many uses inside and outside the operating room. In the emergency department or fracture clinic, a wrist block may be used for closed reductions of dislocations and fractures or for effective inspection and treatment of wounds. In the operating room, surgery may be carried out under a wrist block alone or a wrist block may be used as an adjunct to general anesthesia as the block is an opiate-sparing option to facilitate outpatient surgery and to provide many hours of postoperative analgesia, particularly if administered prior to the commencement o
