Anatomic course of the superficial branch of the radial nerve in the wrist and its location in relation to wrist arthroscopy portals: a cadaveric study.

PURPOSE: The aim of this study was to assess the course of the superficial branch of the radial nerve (SBRN) at the level of the wrist and its branches in relation to wrist arthroscopy portals. METHODS: Dissections were performed on 11 hands from 6 cadavers in the section starting from the point where the SBRN begins to emerge and ending at the terminal branches of the dorsal hand. The distribution of the SBRN, the distance from the superficial branch to the dorsal portals used in wrist arthroscopy, and the distance from the superficial branch to the anatomic determinants (styloid process of

A survey of self-mutilation from forensic medicine viewpoint.

OBJECTIVES: Currently little research exists examining self-mutilation (SM) in samples of forensic referrals. The present study provides a comprehensive review on the frequency, etiology, and morphologic characteristics of self-inflicted injuries in a sample of outpatients' forensic referrals. METHODS: In a prospective cross-sectional study, during 3 years, we examined 9874 outpatients' forensic referrals and found 1248 SM cases in Ghouchan (an urban and suburban area of Iran). RESULTS: Based on forensic medical examinations, it was found that 12.6% of all outpatients' forensic referrals had

Vascularized nerve grafts and vascularized fascia for upper extremity nerve reconstruction.

Since 1976, experimental and clinical studies have suggested the superiority of vascularized nerve grafts. In this study, a 27-year experience of the senior author is presented regarding vascularized nerve grafts and fascia for complex upper extremity nerve reconstruction. The factors influencing outcomes as well as a comparison with conventional nerve grafts is presented. Since 1981, 21 vascularized nerve grafts, other than vascularized ulnar nerve, were used for reconstruction of nerve injuries in the upper extremity. Indications were prolonged denervation time, failure of the previously use

Using risk factors, myoelectric signal, and finger tremor to distinguish computer users with and without musculoskeletal symptoms.

Most of previous studies use questionnaire to assess risk factors for cumulative trauma disorders of the upper extremity (CTDUE) for computer workers. Few studies combine both physical examination and questionnaire to assess musculoskeletal symptoms. Fifteen symptomatic and 15 non-symptomatic computer users were recruited. Both of them were asked to perform a repetitive tapping task (200 taps/min) as the fatigue task. Tremor of the index finger and surface electromyography (EMG) of the flexor digitorum superficial (FDS) were collected prior and after the tapping task. Muscle strength and range

Retrospective study on suicidal cases by sharp force injuries.

A total of 65 suicidal cases due to sharp force injuries (cut and/or stab wounds) were investigated. Suicide by sharp force injuries accounted for 2.5% of all suicides in our prefecture during 1995-2005. The 65 victims were composed of 49 males and 16 females, and the age range of 50-70 years was most common. A history of psychiatric disease was found in 11 victims, and depression was the most common disease followed by schizophrenia. Of 65 cases, 8 victims had a history of previously attempting suicide. In 41 cases, a suicide note or will was found. Forty-six cases had pleural sharp force inj

[Restore hand's function after electric injuries at the wrist by a free composite flap].

OBJECTIVE: To introduce a new free composite flap for the treatment of electric injuries of hands, which can repair the skin, blood vessels, tendons and nerves injuries in just one operation. It may improve the prognosis of electric hand injuries. METHODS: 5 patients with electric injuries at wrist were treated by the free composite flap. The procedure is followed: The composite flap was harvested from medial lateral crural skin flap. Its blood supply was from tibial posterior vessels. The perforans arteries to flap and the branches to plantaris were preserved, and the nerve suralis and tendo

[The frequency of vascular variations and complications with the use of the posterior interosseous artery flap].

OBJECTIVES: We evaluated vascular problems in patients who were planned to receive posterior interosseous artery (PIA) flap for soft tissue defects in the dorsal hand, thumb, and the first web. METHODS: The study included 25 male patients (mean age 27 years; range 12 to 54 years) in whom reconstruction was planned with the use of the reverse PIA flap. The defects were in the following localizations: the dorsal hand (n=15), the first web (n=5), hypothenar region (n=3), and the palmar aspect of the wrist (n=2). The frequency of anatomic variations and complications were evaluated. RESULTS: The

[Severe infections within the upper extremity--analysis of the causes and methods of treatment].

INTRODUCTION: Infections within the upper extremity can be a cause of serious problems when not treated early and adequately. Apparently looking innocuous at the beginning, infection can spread through synovial sheath of the tendons from finger to the hand, wrist and forearm finally developing phlegmone of the upper limb. The objective of the study was to review the methods of treatment of deep infections of the upper limb and to analyse the causes of severity. PATIENTS AND METHODS: Thirty-seven patients with deep infections within upper limb were identified in the Department over the period

Anatomy of the ulnar artery as it relates to the transverse carpal ligament.

The anatomic relationship between the ulnar artery and transverse carpal ligament (TCL) as an aid in planning for minimally invasive carpal tunnel surgery was investigated. The anatomic course of the ulnar artery and its branches toward the TCL and the location of the median nerve were determined in 24 fresh cadaver hands perfused with a silicone compound. The ulnar artery coursed from 7 mm ulnar to 2 mm radial to the hook of hamate. The average distance between the superficial palmar arch and the distal margin of the TCL was 12 mm as measured along the flexor tendon of the ring finger. The lo

Practical anatomy of the carpal tunnel.

The carpal tunnel is most narrow at the level of the hook of the hamate. The median nerve is the most superficial structure. It has specific relationships to surrounding structures within the carpal tunnel to the ulnar bursa, flexor tendons, and endoscopic devices placed inside the canal. The importance of the ring finger axis is stressed. Knowledge of topographical landmarks that mark the borders of the carpal tunnel, the hook of the hamate, superficial arch, and thenar branch of the median nerve ensure appropriate incision placement for endoscopic as well as open carpal tunnel release surger

Anatomic relationships of an endoscopic carpal tunnel device to surrounding structures.

Anatomic relationships of an endoscopic carpal tunnel device to surrounding soft tissue structures along the ring finger and the long-ring interspace axis were investigated in 28 adult cadaver hands. The average distance from the center of the device to the median nerve in the carpal tunnel averaged 3.3 mm in the ring finger axis and 2.5 mm in the long-ring interspace axis. The average distance from the distal edge of the transverse carpal ligament to the superficial palmar arch was 4.8 mm in the ring finger axis and 5.5 mm in the long-ring interspace axis. These and other more subtle anatomic

Endoscopic carpal tunnel release: a cadaveric study.

Five surgeons performed endoscopic (Dyonics) carpal tunnel releases on 24 fresh cadaver wrists. In 50% of the specimens, transection of the transverse carpal ligament was incomplete. The average amount of incomplete release was 31% (range, 0% to 53%). Three types of incomplete release were noted: (1) release of Guyon's canal release--one, (2) incomplete distal ligament release--five, and (3) incomplete central or superficial release--six. Other technical errors were noted in nine of 24 specimens. There were no nerve lacerations. Endoscopic carpal tunnel release is either a technically demandin

Management of bleeding and associated complications of hemophilia in the hand and forearm.

Hemorrhage into the hand and forearm in hemophiliacs can be devastating if not treated early and properly. Of 200 hemophiliacs followed from 1969 to 1976, thirty-four had lesions of the hand and forearm. Hemarthrosis, the most common musculo-skeletal manifestation of hemophilia, occurred in the wrist and hand on only ten occasions. Superficial hemorrhage in the seven cases seen was not followed by significant sequelae. Bleeding into the anterior muscles and into the volar aspect of the wrist, on the other hand, was complicated by contracture, neuropathy, or both in six cases. Early diagnosis,
