BACKGROUND: The proximal interphalangeal (PIP) joint is critical for hand function, contributing substantially to grasping and overall dexterity1-3. Injuries to this joint, often caused by axial loading, can lead to various fracture patterns and dislocations, resulting in long-term complications such as pain, stiffness, reduced range of motion, arthritis, and instability1,2. Traditional surgical approaches for unstable dorsal PIP joint fracture-dislocation include dorsal block pinning, open reduction and internal fixation (ORIF), hemi-hamate or hemi-capitate arthroplasty, and external fixation

Traumatic segmental bone defects of the hand are uncommon injuries that may require complex reconstruction to restore skeletal stability and function. Calcium sulfate is widely used as a biodegradable bone void filler and local antibiotic carrier, although its use during the acute management of traumatic metacarpal defects has been rarely reported. We present the case of a 55-year-old man who sustained a severe open injury to his dominant hand caused by a radial saw, resulting in dissociation of the first and second rays, segmental bone loss involving approximately 40% of the first metacarpal

BACKGROUND: Proximal pole scaphoid fractures complicated by nonunion and osteonecrosis present a challenging scenario with few good reconstructive options, especially when the proximal fragment is small or fragmented1-4. The use of an ipsilateral proximal hamate osteochondral autograft has emerged as a promising technique for reconstructing the proximal scaphoid pole, with the potential to restore carpal mechanics while avoiding the morbidity associated with free vascularized bone grafts or distant autograft sites.
DESCRIPTION: The procedure involves replacing the diseased proximal scaphoid w

BACKGROUND: Methamphetamine use in orthopedic trauma carries an indeterminate risk, with prior studies suggesting increased rates of deep vein thrombosis and elevated mortality. Upper extremity fractures (UEFs) have distinct mechanisms of injury and operative risk. This study evaluates outcomes following operative management of UEFs in patients with and without documented methamphetamine use.
MATERIALS AND METHODS: A retrospective chart review was conducted of adults undergoing surgical fixation of UEFs (distal radius/ulna to clavicle; phalangeal, metacarpal, and carpal fractures were exclude

BACKGROUND AND PURPOSE: Spiral or oblique metacarpal shaft fractures (MSFs) can be treated nonoperatively with unrestricted mobilisation; however, this approach leads to bone shortening upon healing. While an association between bone shortening and impaired hand function is suspected, empirical clinical evidence is lacking, and differing measurement methods make comparisons between studies difficult.
METHODS: This study is based on a midterm follow-up (mean 4.5 years) of 33 patients with a single spiral or oblique MSF from our previous randomised controlled trial. The original study and the p

Introduction Injuries sustained by professional athletes can severely impact seasonal performance and future abilities. The aim of this study was to characterize the impact of phalanx, thumb, metacarpal, and carpal injuries on the performance of athletes in the National Football League (NFL). Analyzing outcomes for football players is significant because the functional demands of each position vary widely. Methods NFL players who sustained phalanx, thumb, metacarpal, or carpal fractures from the 2012-2013 to 2022-2023 seasons were identified using the public database Pro Sports Transactions. P

Scaphoid fractures are the most common fracture of the carpal bone and frequently occur in athletic populations due to acute trauma or repetitive wrist loading. Management of these injuries in athletes is challenging because of the need to balance reliable fracture healing with timely return to sport. This narrative review summarizes current literature regarding treatment strategies, healing outcomes, and return-to-sport timelines in athletes with scaphoid fractures. We conducted a literature search using PubMed, Scopus, and CINAHL between July 2025 and January 2026. A total of 862 records wer

BACKGROUND: Scaphoid fractures are the most common carpal bone fractures and are particularly prone to non-union due to their distinctive blood supply1. If left untreated, scaphoid non-union may result in carpal collapse and severe degenerative arthritis, leading to significant impairment of hand function. Delayed or missed diagnoses increase the risk of non-union and subsequent osteoarthritis. Open reduction and internal fixation (ORIF) with bone grafting have become the standard approach to prevent the progression of arthritis resulting from non-union2. However, the optimal fixation method r

Pediatric scaphoid nonunion is uncommon. Immobilization is initially attempted; however, it causes delays in both intervention and healing. Therefore, surgical intervention has gained broader acceptance and has been implemented in recent years. Among these, arthroscopic management has emerged as a contemporary technique. This study evaluated the clinical outcomes of pediatric scaphoid nonunion treated with arthroscopic-assisted bone grafting and Kirschner (K) wire fixation. Six pediatric patients with scaphoid nonunion treated with arthroscopic bone grafting and K-wire fixation between Novembe

This case report describes the successful multistage reconstruction of a severely mangled hand in a 15-year-old boy following a firecracker explosion. The injury resulted in multiple digit amputations, complex fractures, extensive soft tissue loss, and damage to critical anatomic structures, sparing only the little finger. Initial management focused on debridement, skeletal stabilization, and soft tissue coverage, followed by a carefully planned reconstructive pathway. Key surgical interventions included a medial plantar free flap for glabrous, sensate coverage of the thumb, a second toe trans

PURPOSE: Phalangeal fractures of the hand are the second most common upper-extremity fracture in the United States. Numerous studies have reported on the biomechanical efficacy of intramedullary (IM) hand nail fixation of the phalanges and increased utilization of IM hand nails. During IM fixation, the nail often penetrates the articular cartilage during insertion. This study seeks to quantify the percentage of cartilage involvement created by anterograde and retrograde intramedullary fixation of proximal and middle phalangeal fractures with 3.0 mm and 3.5 mm IM threaded hand nails.
METHODS:

PURPOSE: Intramedullary screws in single- or double-screw configurations are increasingly used for proximal phalanx (P1) fractures. Our purpose was to assess the biomechanical rotational stability of antegrade one- versus two-screw constructs in the fixation of simulated transverse mid-diaphyseal P1 fractures.
METHODS: We used matched pairs of fresh-frozen cadaver upper extremities. We harvested the P1s of all fingers and made transverse mid-diaphyseal osteotomies. Across each matched pair, we alternated between one- and two-screw antegrade fixation constructs on matched P1s. We placed one-sc

Regional anesthesia has been proven useful in hand surgery after trauma, most commonly via brachial plexus blocks (BPB), with widely established benefits. Distal nerve blocks at the elbow are used much less frequently. A 15-year-old boy was admitted because of traumatic amputation of the 4th digit on his right hand. Difficult intubation was suspected. History was indicative of obstructive sleep apnea, and nil per os status was confirmed, with no apparent risk factors for regurgitation. Ultrasound-guided blocks of the median, radial, and ulnar nerves at the elbow were performed with 2% Lidocain
