Relative Metacarpal Shortening as a Radiographic Measure of Fourth and Fifth Carpometacarpal Fracture Dislocation.

PURPOSE: Fourth and fifth carpometacarpal fracture dislocations may be missed on plain x-ray. The purpose of this study was to describe a radiographic measurement, the relative metacarpal shortening (RMS), to evaluate for fourth and fifth carpometacarpal fracture dislocations on posteroanterior radiographs. We hypothesize that the RMS will increase in patients with fourth and fifth carpometacarpal fracture dislocations relative to controls. METHODS: A retrospective review identified posteroanterior hand radiographs of skeletally mature patients with fourth and/or fifth carpometacarpal fractur

Age-related changes in bone mineral density of the scaphoid and distal radius: A quantitative computed tomography study.

BACKGROUND: Scaphoid fractures predominantly occur in young males, while distal radius fractures occur in elderly females, despite similar injury mechanisms. We hypothesized that age-related bone mineral density (BMD) changes differ between these bones, explaining their distinct fracture patterns. This study compared age-related BMD changes between the scaphoid and distal radius using quantitative computed tomography (qCT). METHODS: We analyzed 167 cases (110 males, 57 females; mean age 45.2 ± 18.1 years) who underwent qCT including forearm and carpal bones. Exclusion criteria included fractu

Biomechanical Properties of the Extensor Mechanism Following Retrograde Intramedullary Nailing for Metacarpal Fractures.

PURPOSE: Retrograde percutaneous placement of headless compression screws or threaded intramedullary nails for metacarpal fracture fixation may injure the extensor mechanism. Alternatively, mini-open, tendon-sparing approaches may be used, with instrumentation placed through the sagittal band or a longitudinal split of the tendon. Our study evaluated extensor mechanism biomechanical properties after using different techniques for retrograde threaded intramedullary nail insertion. METHODS: Sixty-four cadaveric fingers were divided into a control group and three testing groups based on the foll

Forensic Evaluation of Multiple Gunshot Injuries: A Case Report.

Firearm injuries, particularly those caused by shotguns, present complex clinical and forensic challenges due to the variable nature of wound patterns and retained projectiles. Shotguns inflict varying injury patterns based on range and ammunition type, while high-velocity bullets cause extensive cavitation injuries, especially in dense or fluid-filled organs. Understanding firearm mechanisms, wound ballistics, and associated forensic evidence like gunpowder residue and blood patterns is essential for accurate forensic investigation, trauma management, and preventing long-term complications fr

Variation in definitions of scaphoid fracture on MRI scans for suspected fracture: a systematic review.

We conducted a systematic review of the definition of a scaphoid fracture on MRI scans, examining 58 studies of patients with suspected scaphoid fractures. Nearly half the studies did not report any definition or measurement of fracture. Among those that did, 29 out of 33 studies (88%) described a linear fracture shape, 13 out of 33 studies (39%) reported the extent of the fracture across the scaphoid and none specified the fracture line orientation. Of the 25 studies describing fractures using at least one of the shape and extent groupings, 12 (48%) regarded a linear signal through the cortex

More Amputations and Open Fractures: Pediatric Utility Task Vehicle (UTV) Injuries Are More Severe Than All-terrain Vehicle (ATV) Injuries.

BACKGROUND: Utility task vehicles (UTVs) differ from all-terrain vehicles (ATVs) in that they are heavier, have larger engines, and have rollover protective structures. Despite their growing popularity for recreational use, there is emerging evidence that mutilating injuries may be higher in UTVs than ATVs, but this has not been evaluated in a pediatric-only population. This study seeks to determine differences in injury characteristics between vehicle types. METHODS: A retrospective cohort study was performed at a level 1 pediatric trauma center, evaluating all patients <18 years old who pre

Closed reduction of multiple MCP joint dislocations.

We report a young man in his 20s who fell from the roof of his house and sustained injury to his left hand. Physical examination and radiographic evaluation revealed closed dislocations of the second, third, and fourth metacarpophalangeal (MCP) joint with fractures of the bases of the third and fourth metacarpals and the base of the proximal phalanx of the middle finger. The injury was managed with closed reduction and splintage with an uneventful outcome. Multiple MCP joint dislocations are rare, and there are only a few reports of closed reduction with a good outcome.

Trapeziectomy and Ligament Reconstruction Using a Reinforced Half-Slip of the Extensor Carpi Radialis Longus Tendon for Thumb Carpometacarpal Osteoarthritis.

BACKGROUND: Carpometacarpal (CM) joint osteoarthritis of the thumb is characterized by pain and impaired thumb function in terms of pinch and range of motion. Here, we newly adopted a trapeziectomy with a graft-augmented ligament reconstruction procedure, in which the reconstructed ligament is reinforced using a half-slip extensor carpi radialis longus (ECRL) tendon. METHODS: From 2015 to 2022, 101 hands of 95 patients with CM joint osteoarthritis of the thumb underwent trapeziectomy with graft-augmented ligament reconstruction using the half-slip ECRL tendon to reconstruct the ligament. Staged Reconstruction and Salvage of a Small Finger With a Corticocancellous Distal Radius Autograft: A Case Report.

Finger reconstruction in the aftermath of severe trauma poses significant challenges in restorative surgery, particularly when addressing extensive soft tissue damage, comminuted fractures, and simultaneous neurovascular complications. This study presents a case of a patient with a complicated proximal phalangeal fracture who pursued digit salvage. A 31-year-old Hispanic woman sustained severe injury to her right fifth digit during an all-terrain vehicle (ATV) rollover resulting in an exposed, comminuted fracture of the proximal phalanx with extensive structural and neurovascular compromise. Management Strategies of Fingertip Injuries: A Case Series.

Fingertip injuries are among the most frequently encountered hand traumas in clinical settings and often result from occupational, recreational, or domestic incidents. These injuries range from simple lacerations and avulsions to more complex presentations involving severe tissue loss, exposed bone, fractures, and nail bed disruptions. Given the intricate anatomy of the fingertip, comprising soft tissue, bone, nerves, and the nail unit, these injuries can significantly compromise hand function, sensation, dexterity, and aesthetic appearance. Prompt and appropriate management is crucial to mini

Outcome of Non-thumb Metacarpal Shaft Fractures Treated by Two Different Techniques - K Wire and Herbert Screw Fixation: A Comparative Single Centre Study.

INTRODUCTION: Metacarpal fractures are one of the most commonly encountered musculoskeletal injuries in the hand often caused by road traffic accidents, trivial fall or workplace injuries. It can lead to severe disability of the hand if left untreated or improperly treated. About 88% of all metacarpal fractures are non-thumb metacarpals, with the fifth finger being most frequently affected. Hence, achieving optimal function will be the primary goal, though it remains challenging. MATERIALS AND METHODS: A prospective study done at Government Thoothukudi Medical College for a period of 3 years

A unique case of exploded hand syndrome with hand ischemia and multiple phalangeal neck fractures of all fingers: Case report.

INTRODUCTION: Although exploded hand syndrome is well described in the literature, only a few cases have been reported. A sudden compressive force to the hand leads to extrusion of the intrinsic muscles through burst lacerations in the skin. This case report has been reported in line with the SCARE checklist. CASE REPORT: A 45-year-old male industrial worker sustained a crush injury to his right hand when a compressor plate hit the hand at the factory. Besides extrusion of the intrinsic muscles and multiple metacarpal fractures, two unique features were seen: fractures of the phalangeal necks
