BACKGROUND: Perilunate injuries are rare high-energy wrist injuries whose open treatment carries a risk of capsular stiffness and carpal devascularisation. Arthroscopically assisted reduction with temporary fixation aims to limit this morbidity, and temporary trans-articular headless compression screws may provide more rigid scapholunate and lunotriquetral control than Kirschner wires. We asked what the mid-term clinical, radiographic and patient-reported outcomes of this technique are, using the contralateral wrist as a paired control, and whether they differ by injury pattern.

BACKGROUND: Unstable scaphoid waist nonunion disrupts wrist biomechanics through scaphoid shortening, humpback deformity, and dorsal intercalated segment instability (DISI), predisposing to degenerative collapse. Structural bone grafting aims to restore scaphoid length and alignment in addition to achieving union. This study evaluated radiological and functional outcomes following tricortical iliac crest wedge grafting for unstable scaphoid waist nonunion.
METHODS: A retrospective analysis was conducted on 40 adult patients with unstable scaphoid waist nonunion treated using a volar tricortic

Background: Metacarpal neck fractures account for approximately half of all hand fractures; however, there is no consensus on which surgical approach should be used for these fractures. This study aimed to compare the clinical and radiological outcomes of open reduction internal fixation (ORIF) and closed reduction percutaneous pinning (CRPP) for fifth metacarpal neck fractures. Methods: A total of 71 patients who underwent ORIF or CRPP for fifth metacarpal neck fractures between 2020 and 2024 were retrospectively evaluated. Patients were divided into two groups: ORIF (n = 31) and Kirschner (K

Carpal fractures are common orthopaedic injuries, with the scaphoid being the most frequently affected bone. Hamate fractures are uncommon and easily missed due to their subtle clinical presentation and difficulty in detection on standard radiographs, often being misdiagnosed as soft tissue trauma. Simultaneous involvement of the scaphoid and hamate is uncommon, and progression to dual non-union is even more so. We report a case of concurrent scaphoid and hook of hamate non-union to highlight the importance of early recognition and appropriate imaging to prevent delayed diagnosis and long-term

Finger pulp infections commonly occur due to bacteria entering through wounds. Foreign body-related pulp infections are uncommon and can present atypically. In this rare case, infection occurred due to a retained cement bead, which was surgically implanted for treating a compound, comminuted distal phalanx fracture with bone loss. A 22-year-old woman presented with persistent swelling, dull pain, and discharge from the pulp of the right middle finger for three months, unresponsive to empirical antibiotic therapy. Examination revealed localized swelling, tenderness with a puncture wound, and pu

BACKGROUND: Treatment of scaphoid nonunion with a vascularized bone graft based on the 1,2‑intercompartmental supraretinacular artery (1,2‑ICSRA) involves complex wrist anatomy. Traditional teaching methods rely mainly on anatomical atlases and two‑dimensional (2D) computed tomography (CT) images, making it difficult for students to mentally reconstruct three‑dimensional (3D) spatial relationships. Three‑dimensional digital anatomy technology can provide visual 3D structural information that clearly illustrates the spatial relationships among anatomical structures, thereby potentially improvin

Osteoid osteoma (OO) of the scaphoid is an uncommon entity, particularly in pediatric patients, where diagnosis is often delayed due to its rarity and overlapping presentation with more common wrist pathologies. We report the case of a pediatric athlete with persistent wrist pain initially diagnosed as a scaphoid fracture. Imaging progression from radiographs to MRI suggested fracture, but confirmatory CT revealed a 5-mm radiolucent lesion with central mineralization, consistent with OO. Surgical excision via volar approach was performed due to intraosseous lesion location and proximity to art

BACKGROUND: Scaphoid non-union with humpback deformity is commonly managed with volar structural bone grafting. This meta-analysis compares outcomes of distal radius bone graft (DRBG) versus iliac crest bone graft (ICBG).
METHODS: Following PRISMA guidelines (PROSPERO CRD420251106177), MEDLINE, Embase, ClinicalTrials, and SPORTDiscus were searched to March 1, 2026. Comparative adult studies of scaphoid non-union with humpback deformity treated via volar DRBG or ICBG with headless compression screw fixation were included. Primary analysis pooled vascularized and non-vascularized grafts; subgro

Benign tumors of the carpal bones are uncommon, and their optimal treatment is still a topic of debate due to the condition's rarity. While there is consensus on treating enchondromas with pathological fractures to alleviate symptoms, the best approach for removing these benign tumors remains uncertain. With the growing popularity of wrist arthroscopy for various wrist pathologies, we present our technique of arthroscopic enucleation, bone grafting, and fixation of scaphoid enchondroma. Our method showed favorable outcomes at the one-year follow-up. We aim to describe our technique, including

BACKGROUND: Scaphoid fractures are common carpal injuries among adults, though their optimal management may differ based on patient demographics. While these fractures often result from high-energy trauma in younger patients, low-energy mechanisms predominate in older adults, who are frequently treated nonoperatively due to concerns regarding bone quality and healing potential. Recent studies suggest that factors other than age more strongly influence patient outcomes, indicating older adults may benefit from surgical fixation over conservative management. This study evaluated outcomes followi

BACKGROUND: Condylar stress fracture of the third metacarpal bone (MC3) is a common catastrophic injury in Thoroughbred racehorses and is associated with parasagittal groove (PSG) subchondral osteolysis. Standing computed tomography (sCT) imaging enables sensitive identification of this fatigue-induced early subchondral bone injury (SBI), but there is no objective method for identifying racehorses at heightened risk of condylar stress fracture.
OBJECTIVES: To estimate PSG first principal strain in elite Thoroughbred racehorses that have undergone subjective risk assessment using sCT fetlock s

OBJECTIVE: Scaphoid nonunion remains a challenging problem due to limited vascularity and carpal malalignment. Although autologous bone grafting remains the standard of care, donor-site morbidity and technical demands can be substantial. NOVOSIS, a composite of recombinant human bone morphogenetic protein-2 (rhBMP-2) and a hydroxyapatite (HA) carrier, has shown osteoinductive and osteoconductive potential in other orthopedic applications, but its clinical efficacy in scaphoid nonunion has not been established.
METHODS: Seven consecutive patients with scaphoid waist nonunion underwent fixation

BACKGROUND: No international consensus regarding optimal non-surgical or surgical treatment of metacarpal fractures exists. Surgical outcome using patient-reported outcome measurements (PROMs) from national registers with large surgically treated population have rarely been reported. We present demographics and patient-reported outcomes in relation to fixation methods for surgically treated metacarpal fractures, using data from the Swedish National Quality Registry for Hand Surgery (HAKIR).
METHODS: In a retrospective analysis of a prospective collected dataset, patients aged ≥ 18 years with

OBJECTIVES: To establish evidence-based consensus statements on imaging of scaphoid fractures.
MATERIALS AND METHODS: Nineteen hand surgeons formulated a preliminary list of eleven questions on imaging of scaphoid fractures. Based on this preliminary list, radiologists crafted statements considering literature and their clinical experience, then refined them through an iterative Delphi process to revise the questions and statements. A maximum of three Delphi rounds was scheduled until group consensus was achieved for an individual statement, whichever arose first. Twenty-eight radiologists dr

PURPOSE: Scaphoid fractures are often complicated by nonunion, but incidence, risk factors, and management have not been well-characterized for pediatric patients.
METHODS: Patients aged <18 years with scaphoid fracture were identified in the 2015 to 2023 PearlDiver M170 database. Patient and fracture characteristics, imaging, and surgeries were extracted. Predictive factors for nonunion were characterized with multivariable analyses. Among patients with nonunion, characteristics of those receiving surgical and nonsurgical management were compared with multivariable analyses.
RESULTS: Of 26,

Scaphoid fractures are common and often progress to nonunion due to the unique vascularity. When nonunion occurs, patients are at risk of developing carpal collapse and arthritis if not managed appropriately with traditional surgical management. However, open surgical management puts the fragile vascular supply and extrinsic soft tissue stabilizers of the carpus at risk, compromising healing and clinical outcomes. Arthroscopy allows for the preservation of these structures and improved visualization of fracture debridement, reduction, and alignment. We describe an arthroscopic-assisted reducti

AIMS: The Scaphoid Waist Internal Fixation for Fractures Trial (SWIFFT) reported no notable difference in functional outcomes between surgical fixation and cast immobilization for minimally displaced scaphoid waist fractures in UK settings. We aimed to emulate SWIFFT using Chinese hospital electronic health record (EHR) data to evaluate whether similar treatment effects are observed in a population with different risk profiles and to identify subgroups that may benefit differentially from surgery.
METHODS: This target trial emulation used a clone-censor-weight design with inverse probability

PURPOSE: Scaphoid fractures (SF) in adolescents differ from adult fractures in fracture pattern, healing capacity, and skeletal maturity. Limited data exist on factors affecting union and healing time in this population. This study evaluated clinical, radiological, and treatment-related factors associated with union and healing time in adolescent SF.
METHODS: This study included 77 adolescents (aged 10-18 years) treated for SF. Chronological age, bone age, fracture characteristics, treatment modality, union status, and healing time were recorded. Fractures were classified by anatomical locati

BACKGROUND: When treated early with cast immobilization, adolescent scaphoid fractures have excellent outcomes; however, nonunion may occur, particularly in patients with a delayed presentation. The purpose of this study is to evaluate the radiographic outcomes and to determine factors associated with time till union in adolescent scaphoid fracture nonunions treated with open reduction and internal fixation (ORIF) with distal radius cancellous autograft interposition.
METHODS: Children with scaphoid fracture nonunions treated at a single center were identified through an electronic database f
