The scaphoid is the largest and most significant bone in the proximal carpal row and is essential for wrist biomechanics and stability. Its intricate anatomy and unique blood supply render it vulnerable to overlooked fractures and postfracture complications, often making for an exceptionally challenging radiologic appraisal. Radiographic diagnosis of a scaphoid fracture is particularly difficult because of the bone's complex geometric structure. Its numerous sulci, apices, ridges, and facets can resemble fractures, which may lead to unnecessary immobilization. A significant portion of the scap

Fractures of the thumb encompass a diverse spectrum of injury patterns, each requiring nuanced consideration of various treatment strategies. Associated conditions include injuries to the ulnar collateral ligament and radial collateral ligament of the metacarpophalangeal joint, which can significantly impact thumb stability and function. Although extra-articular fractures of the thumb metacarpal and phalanges are more tolerant of residual deformity due to compensatory movement of the carpometacarpal joint, intra-articular fractures, including Bennett and Rolando fractures, often benefit from s

AIMS: There is a lack of published evidence relating to the risk factors for nonunion seen in acute scaphoid fractures. We aimed to describe the risk factors for nonunion in adult patients (aged ≥ 16 years) treated with cast immobilization for acute scaphoid fractures.
METHODS: We conducted a single centre cohort study of patients with an acute scaphoid fracture and who were followed up with a CT scan to assess union. Data were gathered retrospectively. Outcomes were classified as union or nonunion based on CT scan follow-up. Descriptive and regression analyses were performed.
RESULTS: Of 50

BACKGROUND: Scaphoid fractures remain challenging because of the bone's limited vascularity, particularly at the proximal pole. Although surgical fixation represents the standard of care for displaced and high-risk fractures, pulsed electromagnetic field (PEMF) stimulation has been proposed as a noninvasive adjunctive therapy, with osteogenic, anti-inflammatory, bone-healing, and regenerative effects.
METHODS: This multicenter retrospective cohort study, conducted in accordance with the strengthening the reporting of observational studies in epidemiology (STROBE) guidelines, evaluated PEMF st

INTRODUCTION: Scaphoid is the most common carpal to be fractured (60%), caused by fall on outstretched hand, and proximal pole fractures are difficult to manage due to the high risk of non-union and osteonecrosis due to retrograde blood supply. Conservative management of proximal pole fractures leads to high rates of non-union; hence, different surgical techniques have been devised to address bone loss and optimize vascularity at fracture site to promote healing, such as vascularized bone grafting (VBG) and non-VBG. We report the use of proximal hamate as a replacement arthroplasty in the cont

Arthrodesis with a locking plate is widely used for thumb carpometacarpal osteoarthritis. However, complications such as nonunion and plate fracture remain a concern. Although clinical studies suggest that an oblique screw reduces the complication rate, its mechanical effect and an optimal insertion method remain unclear. We aimed to quantify the oblique screw's biomechanical effect with finite element (FE) analysis. A FE model of thumb arthrodesis was constructed using a CT-based bone model and three plate designs. For each plate, models with and without an oblique screw were tested under thr

PURPOSE: Decreased Medicare reimbursement rates have been observed in many surgical subspecialties, which can negatively impact access and sustainability of care. While several studies report decreased reimbursement for hand surgery procedures, none have accounted for changes in case volume.
METHODS: The CMS Physician and Other Practitioners database was used to extract all procedure codes billed to Medicare Part B by hand surgeons. The Medicare Physician Fee Schedule database was used to collect reimbursement information, and reimbursement trends were weighted by procedure volume.

BACKGROUND: Scaphoid nonunion remains a challenging condition due to its tenuous vascularity and complex wrist biomechanics, often resulting in deformity, instability, and progressive degenerative changes. The modified Fisk-Fernandez technique combines structural bone grafting with stable fixation to address both biological and mechanical factors.
METHODS: This retrospective cohort study included 82 patients with scaphoid waist nonunion treated between 2017 and 2026. Clinical outcomes were assessed using QuickDASH and Visual Analog Scale (VAS) scores, while radiological evaluation included sc

INTRODUCTION: Owing to the scaphoid's retrograde blood supply, fractures can compromise perfusion to the proximal pole, making it vulnerable to avascular necrosis (AVN) or irreversible bone death. Despite their differing prognoses, both conditions are evaluated using the same diagnostic methods. This review aims to determine the most reliable method of distinguishing between AVN and irreversible bone death, considering factors such as magnetic resonance imaging (MRI), computed tomography (CT), intraoperative findings, and histology.
METHODS: A comprehensive literature search was conducted unt

BACKGROUND: Accessory carpal bones around the scaphoid are uncommon anatomical variants that may mimic fractures, non-unions, or other pathological entities on wrist imaging. Despite more than a century of published reports, the literature remains limited by inconsistent terminology, unclear anatomical definitions, and variable reporting quality. This review aimed to synthesize the available evidence on accessory bones around the scaphoid and to clarify their prevalence, anatomical characteristics, imaging features, clinical presentation, and management.
METHODS: We performed a combined syste

The success of arthroscopically assisted scaphoid nonunion surgery depends on multiple factors, most importantly the time between injury and surgery. The optimal bone grafting option for a particular scaphoid nonunion case depends on multiple factors including scaphoid bone loss and concurrent wrist pathology. Autograft and allograft are both considered reasonable bone grafting options depending on the details of the case.

Boutonniere deformity results from damage to the central band and structures stabilizing the finger's extensor apparatus, leading to flexion of the proximal interphalangeal (PIP) joint and hyperextension of the distal interphalangeal (DIP) joint. Untreated or delayed diagnosis of boutonniere deformity results in progression of both deformity and degenerative changes in the PIP joint. In advanced, chronic deformities (Burton IV), surgical treatment includes arthrodesis or PIP joint replacement. In the presented case, a 29-year-old right-handed man presented with a persistent boutonniere deformi

Background and Clinical Significance: Scaphoid fractures and nonunion are uncommon in skeletally immature patients. Pediatric nonunion most often follows a missed or delayed diagnosis or failure of conservative treatment. Inferior-pole nonunion is particularly uncommon, and evidence guiding graft selection in children is limited. We report a case in which preoperative and intraoperative assessment of fragment viability supported the use of a non-vascularized graft. Case presentation: A 12-year-old boy sustained a right inferior-pole scaphoid fracture after falling onto an outstretched hand. Th

Scaphoid nonunion is a common complication of missed or inadequately treated scaphoid fractures and is generally considered an indication for surgical treatment because of the risk of progressive carpal collapse and degenerative arthritis. Spontaneous union of an established scaphoid nonunion is exceptionally rare, particularly in the presence of radiological findings suggestive of proximal pole vascular compromise. We report the case of a 23-year-old right-handed manual worker with a chronic scaphoid nonunion. The patient sustained an initial wrist injury in 2017 that was treated conservative

Volkmann ischemic contracture is a severe and largely preventable complication of untreated or delayed compartment syndrome. In children, it is most commonly associated with supracondylar humerus fractures, especially when external compression from casts or splints is not promptly recognized. We report the case of a six-and-a-half-year-old right-handed girl who developed severe left upper-limb dysfunction after a closed supracondylar humerus fracture treated in a rural setting with tight traditional splint immobilization, locally known as "Jbira." No fasciotomy was performed during the acute p

Pelvic digit is a rare congenital anomaly characterized by a finger-like bony structure near the pelvic bone, often discovered incidentally on imaging. Although typically asymptomatic, it may mimic other pathologies when symptoms arise. We report two cases: Case A involved a 14-year-old boy with post-traumatic foot pain, where imaging revealed a pelvic digit initially suspected to be an avulsion fracture; and Case B concerned a 40-year-old man with unrelated back pain, in whom the pelvic digit was found incidentally during imaging workup. In both cases, computed tomography and magnetic resonan

Open distal phalanx fractures with nail-bed injury require prompt treatment of both the fracture and the nail-unit soft tissues. Kirschner-wire fixation is commonly used when an unstable fracture requires stabilization, but operating-room access and fluoroscopy may be delayed or unavailable in high-volume or resource-constrained emergency settings. Published series suggest that hypodermic-needle fixation can be an alternative in selected distal phalanx fractures, although the evidence remains limited. A 49-year-old man with hearing impairment sustained a door-crush injury to the dominant left

The role of demographic factors and disease in altering bone mineral density remains a topic of increasing importance for our aging population. As a technique for obtaining detailed insights into bone density, quantitative computed tomography, where detailed estimates of local bone density can be obtained from computed tomography scan data, has gained popularity in recent years. This approach is particularly useful for obtaining measurements opportunistically from clinical scans, however given the large amount of data generated, processing and drawing insights from the results can be challengi

INTRODUCTION: Avascular necrosis (AVN) of the proximal pole of the scaphoid is a well-recognized complication of scaphoid fractures, particularly when diagnosis or treatment is delayed. Due to the scaphoid's retrograde blood supply, the proximal pole is highly susceptible to ischemia, often necessitating surgical intervention to prevent long-term complications such as scaphoid nonunion advanced collapse and wrist osteoarthritis.
CASE PRESENTATION: We report a rare case of a 23-year-old male who sustained a scaphoid fracture that was initially misdiagnosed as a wrist sprain. The patient presen
