Radial head subluxation, commonly known as nursemaid's elbow, is a frequent upper extremity injury in young children and typically presents with refusal to use the affected arm without focal distal motor dysfunction. We report an atypical presentation in a previously healthy two-year-and-10-month-old girl who was brought to the emergency department after an unwitnessed event with no clearly identified mechanism of trauma. She held her right elbow flexed against her chest in a posture suggestive of radial head subluxation, and additionally refused to let go of her wrist and demonstrated a wrist

CASE: A 49-year-old woman presented after a leash-pulling injury with ring and small finger proximal phalanx fractures. Pain and supination of the long finger metacarpophalangeal (MCP) joint were noted without collateral ligament injury on examination or imaging. Surgical exploration noted isolated capsular avulsion, which was repaired with restoration of alignment and full range of motion with relief of pain and instability at 1-year follow-up.
CONCLUSION: This is a unique case of isolated finger MCP capsular injury causing rotatory instability. Surgeons should consider exploration and repai

BACKGROUND AND PURPOSE: Interphalangeal joint arthrodesis of the hand is a common procedure, but data on outcomes and complications is limited. We aimed to determine the non-union rate, evaluate risk factors for non-union, and investigate the frequency and distribution of complications across different fixation methods.
METHODS: We retrospectively reviewed all interphalangeal joint arthrodeses performed between 2014 and 2022. Union and non-union were defined clinically and/or radiographically. Potential risk factors for non-union were evaluated by logistic regression with cluster-robust stand

UNLABELLED: Seymour fractures are pediatric distal phalanx physeal injuries of the fingers and toes associated with disruption of the nail bed. Despite increasing recognition, they are still frequently misdiagnosed, leading to delayed treatment and preventable morbidity. This review synthesizes contemporary literature on the definition, clinical and radiographic diagnosis, differential diagnoses, management, and outcomes of Seymour fractures. Diagnosis relies on maintaining a high index of suspicion in skeletally immature patients presenting with distal interphalangeal (DIP) joint flexion, ble

BACKGROUND: Post-traumatic osteoarthritis of the proximal interphalangeal joint (PIPJ) substantially impairs hand function, and vascularized joint transfer (VJT) can be used to restore range of motion (ROM). However, malunion, particularly dorsal angulation of the proximal phalanx (P1), can occur postoperatively. We aimed to analyze the causes and impact of malunited P1 on surgical outcomes in VJT cases and provide insights regarding configuration and corrective osteotomy.
METHODS: The authors retrospectively reviewed 61 VJT cases from 2008 to 2022, with the primary indication being PIPJ oste

Stüve-Wiedemann Syndrome (SWS) is a rare autosomal recessive skeletal dysplasia caused by variations in the leukemia inhibitory factor receptor (LIFR) gene. Due to its high rate of mortality in early life, current literature is largely limited to children with SWS. We present a case report of a 35-year-old female with SWS, one of the few documented adults with SWS. She was diagnosed at age 34 after clinical exome sequencing identified homozygous pathogenic variants in LIFR at c.756dup (p.Lys253*), a known pathogenic variant associated with SWS. Her past medical history consists of feeding and

Assessment of fracture healing in carpal bones is commonly performed using computed tomography (CT). However, CT may not accurately reflect bone viability. Magnetic resonance imaging (MRI) is more sensitive in detecting early changes in bone marrow and vascularity. A diagnostic challenge arises when CT suggests fracture union while MRI demonstrates features of avascular necrosis (AVN). We conducted a retrospective case series of three patients with prior carpal bone fractures (one scaphoid and two lunate) who presented with persistent or new-onset wrist pain following apparent fracture healing

BACKGROUND: Scaphoid nonunion is a rare but challenging condition in the pediatric population. While acute scaphoid fractures in children typically heal well with cast immobilization, nonunion can result in persistent pain and functional impairment. The medial femoral condyle (MFC) flap offers potential advantages for reconstruction by providing a robust blood supply and structural support. However, its application in skeletally immature patients is limited by concerns over damaging the open distal femoral physis.
METHODS: We present 2 cases of skeletally immature patients with scaphoid nonun

Trapeziometacarpal (TMC) joint arthroplasty has become an established and effective treatment option for advanced trapeziometacarpal osteoarthritis. Prosthetic replacement can also be employed in revision procedures following failed surgeries involving the trapezium or the base of the first metacarpal. Revision arthroplasty is technically demanding, as altered anatomical conditions, scar tissue, and compromised bone quality may complicate implantation. Patients and Surgical Techniques Based on specific cases, we present indications and techniques for revision arthroplasty of the TMC joint, inc

A 31-year-old man presented with an acute, ligamentous perilunate dislocation and paresthesia in the median nerve distribution after a dirt bike accident and underwent closed reduction. The following morning, he was treated with carpal tunnel release and open reduction and internal fixation using a 4-anchor suture tape internal bracing treatment technique through a dorsal carpal approach. The construct consisted of a series of internal braces dunked into four carpal anchors to reinforce/recreate the scapholunate interosseous ligament, lunotriquetral interosseous ligament, and dorsal intercarpa

INTRODUCTION: Scaphoid fractures still have high rate of nonunion, mainly due to the tenuous blood supply and only primary bone healing. Rigid fixation facilitates this kind of bone healing and is considered as vital as protection of the blood supply. The optimal implant (screw) location should meet the requirements for researcher's biomechanical findings on fracture stability.
MATERIALS AND METHODS: Raw CT-scanned data of eight volunteers' wrists were imported into Mimics. The 3D scaphoid was segmented out and calculated, then opened in Geomagic Studio. Four fracture planes (proximal, obliqu

BACKGROUND: Cereals are among the major foods that account for food hypersensitivity reactions. Salt-soluble proteins appear to be the most important allergens contributing to the asthmatic response. In contrast, very limited information is available regarding cereal allergens responsible for allergic reactions after ingestion of cereal proteins.
OBJECTIVE: The aim of this study was to evaluate the allergenic reactivity of ingested and inhaled cereal allergens in different ages, in order to investigate if the response to different allergens would depend on the sensitization route.
METHODS: W

Flexor pollicis longus (FPL) tendon rupture is an uncommon complication following volar locking plate fixation for distal radius fractures and is most often attributed to attritional wear from hardware contact. While most reported cases occur within the first few years after open reduction and internal fixation (ORIF), rupture occurring a decade or more after surgery remains exceedingly rare. A 48-year-old, left-hand-dominant male field engineer presented with a two-week history of inability to flex the interphalangeal (IP) joint of his left thumb. The symptoms began acutely while lifting heav

INTRODUCTION: National Institute of Clinical Effectiveness (NICE) guidance mandated early MRI scans for suspected scaphoid fractures in 2016. In national surveys, only 15% of NHS Emergency Departments achieve this. Missed fractures may require bone grafting surgery for non-union. NHS litigation costs are high. Conversely, over-treatment harms patients' lifestyle and income.
METHODS: With leadership across Orthopaedics, Radiology and the Emergency Department, we co-created a rapid MRI pathway for suspected scaphoid fractures at site A. We defined the technical specification for a 5 min MRI sca
