Treatment and Return to Sport in Athletes With Scaphoid Fractures: A Narrative Review.

Scaphoid fractures are the most common fracture of the carpal bone and frequently occur in athletic populations due to acute trauma or repetitive wrist loading. Management of these injuries in athletes is challenging because of the need to balance reliable fracture healing with timely return to sport. This narrative review summarizes current literature regarding treatment strategies, healing outcomes, and return-to-sport timelines in athletes with scaphoid fractures. We conducted a literature search using PubMed, Scopus, and CINAHL between July 2025 and January 2026. A total of 862 records wer

Outcomes of volar locking plate fixation and bone grafting for scaphoid fracture non-unions.

BACKGROUND: Scaphoid fractures are the most common carpal bone fractures and are particularly prone to non-union due to their distinctive blood supply1. If left untreated, scaphoid non-union may result in carpal collapse and severe degenerative arthritis, leading to significant impairment of hand function. Delayed or missed diagnoses increase the risk of non-union and subsequent osteoarthritis. Open reduction and internal fixation (ORIF) with bone grafting have become the standard approach to prevent the progression of arthritis resulting from non-union2. However, the optimal fixation method r

Is arthroscopic-assisted treatment applicable for the treatment of pediatric scaphoid nonunion?

Pediatric scaphoid nonunion is uncommon. Immobilization is initially attempted; however, it causes delays in both intervention and healing. Therefore, surgical intervention has gained broader acceptance and has been implemented in recent years. Among these, arthroscopic management has emerged as a contemporary technique. This study evaluated the clinical outcomes of pediatric scaphoid nonunion treated with arthroscopic-assisted bone grafting and Kirschner (K) wire fixation. Six pediatric patients with scaphoid nonunion treated with arthroscopic bone grafting and K-wire fixation between Novembe

Effects of a GLP-1 agonist and anti-diabetic therapies on pheochromocytoma/paraganglioma growth.

BACKGROUND: A high proportion of patients with pheochromocytomas and paragangliomas (PPGLs) show glycemic dysregulation. However, studies on the effects of anti-diabetic drugs on PPGL tumor growth are still lacking. METHODS: We evaluated the effects of glucagon-like peptide 1 receptor agonist (GLP-1RA) semaglutide, metformin and insulin on fresh patient-derived PPGL primary cultures (n=54), including 8 cluster 1 tumors and 18 cluster 2/3 tumors (n=4 tumors with RET pathogenic variants [PVs]). We correlated the primary culture data with clinical data from 67 patients enrolled in the prospectiv

Periscaphoid and Lunate Dislocation With an Intact Rotated, Scapholunate Interosseous Ligament: A Case Report.

Perilunate disassociation injuries are rare and may be missed in the acute setting. Prompt identification and management are critical to prevent late complications such as arthritis and instability. These injuries can present several patterns with soft tissue disruption, fracture, or a combination of both. In this report, we present a unique pattern of a periscaphoid, perilunte injury with an intact scapholunate interosseous ligament and describe our treatment strategy and outcome.

Evidence-Based Management of Common Hand and Wrist Injuries: Tips, Pearls, and Controversies.

General orthopaedic surgeons should have up-to-date information on the most common types of hand and wrist injuries, including distal radius fractures, scaphoid fractures, proximal interphalangeal joint injuries, scapholunate and perilunate injuries, nerve lacerations, common pediatric upper extremity injuries, and metacarpal and phalangeal fractures. These topics represent a broad swath of the typical pathologies the common hand surgeon will face in their practice. Providing the most up-to-date evidence and rationale for current treatments should help clinicians identify current themes in the

"Bullseye" Fluoroscopic View to Assess Center Scaphoid Screw Placement: Surgical Technique.

Percutaneous headless screw fixation is an effective, minimally invasive treatment option for select individuals with nondisplaced or minimally displaced scaphoid waist fractures. While both volar and dorsal approaches have been described, fixation rigidity is optimized by screw placement down the center axis of the scaphoid. This is crucial, as the central axis placement of the screw allows for more stable fracture fixation. Intraoperative fluoroscopic confirmation is critical to ensure optimal center axis or near-central axis placement of both the guide pin and ultimately the final headess s

Distal radius morphometry of volar curvature along with scaphoid and lunate facet inclinations and ulnar variance in the Anatolian population.

This study evaluates the morphometric characteristics of the volar distal radius, specifically volar curvature, scaphoid and lunate facet inclinations, and ulnar variance, in the Anatolian population. By identifying these population specific morphological variations, the study seeks to support improved anatomical compatibility of volar implants and enhance surgical planning for distal radius fracture fixation. Morphometric measurements were analyzed retrospectively on three-dimensional computed tomography images of 103 intact distal radii. Standardized sagittal sections were created through th

Effects of Image Degradation on Deep Neural Network Classification of Scaphoid Fracture Radiographs: Comparison Study of Different Noise Types.

BACKGROUND: Deep learning models have shown strong potential for automated fracture detection in medical images. However, their robustness under varying image quality remains uncertain, particularly for small and subtle fractures, such as scaphoid fractures. Understanding how different types of image perturbations affect model performance is crucial for ensuring reliable deployment in clinical practice. OBJECTIVE: This study aimed to evaluate the robustness of a deep learning model trained to detect scaphoid fractures in radiographs when exposed to various image perturbations. We sought to id

Patient-Related Factors Associated With Scaphoid Proximal Pole Avascular Necrosis Healing.

BACKGROUND: Avascular necrosis (AVN) of the proximal pole is a well-known complication of scaphoid fractures. Avascular necrosis is poorly understood, including the transition from ischemia to necrosis, optimal treatment, and why some AVN heals but others do not. The primary purpose of this study is to evaluate patient-related factors that are associated with healing outcomes in individuals with proximal pole AVN following scaphoid fractures. METHODS: This is a retrospective review of all patients diagnosed with scaphoid proximal pole AVN secondary to a fracture from 2018 to 2024 in a single

Cartilage damage in patients with scapholunate lesions: arthroscopic prevalence, location and associated clinical factors.

INTRODUCTION: Scapholunate ligament (SLL) injuries can lead to instability and osteoarthritis through stages of scapholunate dissociation and scapholunate advanced collapse. Although cartilage damage is traditionally seen as a late finding, recent evidence suggests that it may occur earlier. This study aimed to assess the prevalence and distribution of cartilage damage in patients with isolated SLL lesions confirmed by arthroscopy and to explore associated sociodemographic and clinical factors. METHODS: A retrospective cohort study was conducted in patients with arthroscopically confirmed SLL

Scaphoid Fractures: How to Treat Symptomatic Patients with a Normal X-Ray and When Should We Operate on Acute Fractures?

The scaphoid is the most injured carpal bone, with an incidence of 12 per 100,000 per annum in the United Kingdom (UK). Fractures of the scaphoid can be challenging to diagnose, both clinically and radiographically, and have a propensity to fail to unite or develop necrosis. There is debate regarding whether to manage acute fractures conservatively or offer early surgical fixation. The poor predictive value of initial examination and plain imaging not only results in missed fractures but also, conversely, in a fivefold overtreatment, which has subsequent significant socioeconomic consequences.

Finite Element Analysis of Dual Screw Configurations for Enhanced Stability in Scaphoid Fracture Fixation.

PURPOSE: Internal fixation with a headless compression screw has become the gold standard for the surgical treatment of scaphoid fractures. Some authors reported on the theoretical benefit of dual screw fixation. Our aim was to investigate the optimal dual screw configuration for fixing scaphoid fractures with regard to biomechanical stability. METHODS: In a previously established finite element model of the scaphoid, a fracture plane at the scaphoid waist was modeled. Two 1.7 mm cannulated, partially threaded compression screws were positioned in parallel. The distance between the screws and
