Scaphoid Fracture Nonunion Due to Interposition of a Radial Styloid Fracture Fragment.

BACKGROUND: Radial styloid fractures (RSFs) and scaphoid fractures often occur together and are considered part of Mayfield stage 1 perilunate fracture dislocation. Proper management is crucial to maintain wrist stability. Interposition of an RSF within a scaphoid fracture is less common and can lead to nonunion. CASE DESCRIPTION: We report a case of a 35-year-old patient with nonunion of a scaphoid fracture following failed conservative management 2 months' postinjury. Imaging revealed increased flexion of the proximal pole due to the interposition of the RSF, which prevented proper approxim

Computer-aided three-dimensional analysis of carpal alignment in scaphoid nonunion advanced collapse wrists: A comparative study with scapholunate advanced collapse and healthy wrists.

OBJECTIVE: To analyze three-dimensional carpal malalignment and height in scaphoid non-union advanced collapse (SNAC) wrists. MATERIALS AND METHODS: Twelve cone-beam CT scans of SNAC wrists were analyzed using computer-aided software to define 3D carpal axes and measure intercarpal angles and carpal height ratio. Comparative data included 121 healthy wrists and 18 wrists with scapholunate advanced collapse (SLAC). RESULTS: SNAC wrists showed dorsal angulation of the lunate and even greater dorsal angulation of the triquetrum, resulting in an increased sagittal scapholunate angle and a decrea

Spontaneous Healing of a Scaphoid Nonunion Following an Ipsilateral Distal Radius Fracture: A Case Report.

CASE: A 55-year-old professional flutist developed a scaphoid waist nonunion that did not receive initial treatment. Six months later, she sustained an ipsilateral distal radius fracture for which cast immobilization was applied as conservative treatment. This approach facilitated healing of the distal radius fracture and serendipitously led to spontaneous healing of the scaphoid nonunion. CONCLUSION: This case illustrates an unusual healing response in a scaphoid nonunion, which typically requires surgical intervention. The spontaneous healing observed in this case following conservative tre

Functional outcome after percutaneous screw fixation of acute scaphoid fracture.

Scaphoid fractures are common in adults and are prone to complications such as nonunion and avascular necrosis due to the bone's retrograde blood supply. Hence, this study evaluated the prevalence of excellent wrist functional outcomes following percutaneous Herbert screw fixation for acute scaphoid fractures in a tertiary care hospital. A total of 67 patients were assessed using the Mayo wrist score four weeks postoperatively. Most patients were young males with high postoperative functional scores (mean 94.48 ± 9.42), indicating favorable recovery. Percutaneous fixation with a Herbert screw

Bilateral Traumatic Scaphoid Fracture Managed Surgically with Headless Compression Screw: A Case Report with Review Literature.

INTRODUCTION: The most frequent carpal bone fractures are scaphoid fractures, which frequently affect young people, particularly athletes. These fractures can be difficult to identify, and if left untreated, they can cause problems, including non-union. Although they are uncommon, high-energy trauma can result in bilateral scaphoid fractures, and related injuries can make treatment more difficult. CASE REPORT: A 20-year-old male who had fallen on an outstretched hand from a tractor appeared with pain, edema, and deformity in both wrists and the left elbow. Upon examination, the scaphoid compr

[Translated article] In vitro analysis of carpal kinematics during validation of a stabilized scaphoid hemiarthroplasty.

OBJECTIVE: Pseudoarthrosis and necrosis of the proximal pole of the scaphoid require complex treatment. If primary treatments fail, there are only techniques that sacrifice part of the mobility. We present the design and in vitro results of an anatomical partial prosthesis of the scaphoid bone. MATERIAL AND METHOD: The kinematics of the carpus are tested in vitro on six cadaveric forearms before and after prosthetic replacement, applying active loads on the main muscles. Pre- and post-intervention movements are recorded by Kinescan/IBV®, translating an angular value. RESULTS: After prostheti

Surgical outcomes in chronic perilunate dislocations: A systematic review.

Perilunate dislocations and perilunate fracture-dislocations are marked by a wide range of high energy trauma to the wrist. Chronic perilunate injuries are defined by a treatment delay of at least 6 weeks. This systematic review aims to compare the various operative techniques and outcomes for chronic perilunate injuries. A systematic review of literature on chronic perilunate dislocations and perilunate fracture-dislocations were performed according to Preferred Reporting Items for Systematic Reviews and Meta-analysis statement guidelines using PubMed, Embase, Scopus, and Web of Science. Excision of the Proximal Pole of the Scaphoid Affects the Midcarpal Joint.

BACKGROUND: Excision of the proximal pole of the scaphoid causes wrist instability. The often small proximal fragment can be challenging to retain, and multiple procedures are used to preserve the proximal pole. However, the mechanical implications of a proximal pole loss remain unclear. Possibly, under certain circumstances, the proximal pole can be excised without causing instability. PURPOSES: This study aimed to use a validated finite element model (FEM) of normal Type 1 and Type 2 wrists to examine the effect of removal of the proximal scaphoid pole on forces moving through the midcarpal

Four-corner arthrodesis in SNAC wrist: biomechanical and functional comparison of two fixation techniques.

INTRODUCTION: Scaphoid nonunion advanced collapse (SNAC) is a debilitating condition that significantly impacts wrist function, pain levels, and return-to-work outcomes. Surgical fixation method may influence long-term functional results. This study aimed to compare the complications and functional outcomes of four-corner arthrodesis using either a CarpalFix screw system or a dorsal locked circular plate. MATERIALS AND METHODS: Patients diagnosed with SNAC wrist who underwent four-corner arthrodesis between 2016 and 2022. A total of 47 patients were included: 25 treated with dorsal circular p

Novel Treatment of Post-Traumatic Scaphoid Osteonecrosis in a Pediatric Patient Utilizing Temporary Scaphocapitate Fixation: A Case Report.

Avascular necrosis or osteonecrosis of the scaphoid following fracture is extremely rare in the pediatric population with no concrete treatment protocol established in the literature. In this case report, we present an 11-year-old boy with a scaphoid waist fracture nonunion complicated by proximal pole osteonecrosis. Due to the severe fragmentation of the scaphoid, fixation across the nonunion site was not feasible, and we instead used temporary scaphocapitate fixation. The patient ultimately had radiographically confirmed bony union and full wrist range of motion at 3 months post-op.

Challenges and complications of trans-osseous carpal tenodesis for scapholunate ligament tears.

Scapholunate (SL) instability remains a challenging condition with significant possibility of implications for wrist function and long-term outcomes. This review explores the limitations and complications of current trans-osseous scapholunate ligament (SLL) reconstruction techniques, focusing on isometric reconstruction challenges and complications, for example osteonecrosis, tunnel fractures, graft failure, and iatrogenic extrinsic ligament injuries. Through biomechanical analysis and clinical case reviews, we demonstrate that the conventional three-ligament tenodesis (3LT) scaphoid tunnel ha

Combined Vascularised Bone Graft Techniques for Displaced Coronal Fracture in Kienböck Disease: A Case Report.

Surgical treatment of Kienböck disease remains controversial and depends on patient age, activity and disease stage. We report an 18-year-old female badminton player who presented with wrist pain, limited motion and decreased grip strength. Imaging revealed a displaced coronal lunate fracture, with the palmar fragment locked against the volar rim of the radius, consistent with Lichtman Stage IIIC and Bain grade 2b. MRI showed low signal intensity, suggesting compromised vascularity. Arthroscopy confirmed intact cartilage except at the fracture site. Lunate reconstruction using the Kuhlmann tec

Deep Learning in Scaphoid Fracture Detection and Healing Prediction: A Systematic Review of Artificial Intelligence Applications in Hand Surgery.

Scaphoid fractures are the most common carpal bone injuries and continue to pose diagnostic and therapeutic challenges due to their tendency for delayed union or non-union. Interpretation variability and delayed detection remain key contributors to missed injuries and adverse outcomes. Deep learning (DL) models, particularly convolutional neural networks (CNNs), have shown strong performance in musculoskeletal imaging and offer potential to improve diagnostic accuracy and prognostication in hand surgery. A systematic review of PubMed, Embase, and PMC was conducted covering January 2015 to Marc

An Introspection on Vascularized bone transfers for Avascular Necrosis of Bones of Femoral Head and Scaphoid.

INTRODUCTION: Avascular necrosis of bone, due to various etiologies, is not an uncommon skeletal pathology seen in clinical practice. Treatment of these conditions includes treating the resultant pain, stiffness, and limitations of movement surgically with bone grafts both vascularized and non-vascularized and arthroplasty in most advanced stages. MATERIALS AND METHODS: In a duration of 2 years, five patients were treated for such pathologies using vascularized bones (pedicled or free). All of them were subjected to radioisotope (Technetium 99m) scan postoperatively to evaluate the viability

Prevalence of the Os Supranaviculare: A Systematic Review with Meta-Analysis.

Background/Objectives: The os supranaviculare (OSSN), also known as os talonaviculare dorsale, astragalo-scaphoid ossicle, or Pirie's bone, is a small extra bone that was first described in 1921 by A.H. Pirie and is located at the top front part of the navicular bone or talonavicular joint. The knowledge regarding the epidemiology of the OSSN is scarcely established, as its prevalence remains unknown and varies significantly among multiple studies. This meta-analysis aims to clarify and systematically summarize all available data on the characteristics and prevalence of the OSSN. Methods: Four

Use of the trans-scaphoid portal for the arthroscopic treatment of scaphoid nonunion in the middle third with flexion deformity: technique description and patient series.

Nonunion is one of the main complications associated with scaphoid fractures. Over the past decades, the use of arthroscopy in the treatment of scaphoid nonunion has proven to be a valuable technique, even in cases presenting deformity and carpal instability. This approach facilitates rapid consolidation while minimizing damage to the joint capsule and ligaments, thereby preserving the already compromised vascular supply of the scaphoid. Due to their simpler anatomy, dorsal portals are commonly employed in wrist arthroscopy, reducing the risk of injury to neurovascular structures during joint
