Current Trends in Scaphoid Fracture Management: An Update.

Scaphoid fractures are a common but frequently missed wrist injury. Patients classically present following a fall onto an outstretched hand, with pain and tenderness in the anatomical snuffbox. When missed, scaphoid fractures can have serious complications such as avascular necrosis of the scaphoid and scaphoid non-union advanced collapse. The British Society for Surgery of the Hand has recently published guidelines on the management of suspected scaphoid fractures. Initial management involves radiographs and immobilisation with referral to a specialist within 7 days. Further management includ

Simple Excision of Proximal Scaphoid Nonunion: A Viable Option When the Scapholunate Ligament Is Intact.

PURPOSE: Because of the biological, anatomical, and biomechanical characteristics of the scaphoid, treating some proximal pole nonunions remains challenging. The location of the nonunion (proximal or distal to the dorsal apex) may influence its natural history. Based on this concept, patients with a proximal pole nonunion located proximal to the dorsal apex and maintaining an intact insertion of the dorsal scapholunate ligament in the distal fragment of the scaphoid may benefit from simple excision of the proximal pole. This study evaluated the clinical and functional outcomes of this treatmen

Comparing Conservative and Surgical Treatment of Acute Nondisplaced Fractures of the Scaphoid Based on Fracture Location: A Systematic Review and Meta-Analysis.

INTRODUCTION: Treatment of nondisplaced scaphoid fractures with either nonsurgical or surgical methods ideally involves shared decision making. To date, no meta-analysis has explicitly reported differences in healing rates between fracture locations and treatment choices, making this conversation challenging. We aimed to report the nonunion risk in nondisplaced scaphoid fractures when stratified by both treatment type and fracture location. METHODS: We searched PubMed, SCOPUS, CINAHL, SportsDiscus, HAND , Journal of Hand Surgery , and Plastic and Reconstructive Surgery for outcome studies of

Percutaneous Screw Fixation of Scaphoid Nonunion Without Bone Grafting.

BACKGROUND: This study evaluated the results of minimally invasive headless compression screw (HCS) fixation for treatment of scaphoid delayed union or nonunion. A minimally invasive approach has several potential benefits, including preservation of an already tenuous scaphoid blood supply. It was hypothesized that minimally invasive HCS fixation results in healing of scaphoid fracture with delayed union via increased fracture compression and biomechanical stability, while preserving soft tissue envelope and blood supply. METHODS: This retrospective review considered patients diagnosed with a

Scaphoid waist fractures in the presence of carpal coalitions: a consistent biomechanical weak point?

Carpal coalitions may alter intercarpal kinematics and load transfer. Reported coalition-associated scaphoid fractures show an exclusive predilection for the scaphoid waist, raising the possibility of a non-random biomechanical mechanism with focal stress concentration. These injuries have been associated with delayed union and nonunion. This pattern may justify CT-based assessment and consideration of a lower threshold for early surgical stabilization, including percutaneous screw fixation even in selected nondisplaced waist fractures. However, the currently available evidence is limited to c

Arthroscopically assisted temporary trans-articular headless screw fixation for perilunate injuries: A retrospective study.

BACKGROUND: Perilunate injuries are rare high-energy wrist injuries whose open treatment carries a risk of capsular stiffness and carpal devascularisation. Arthroscopically assisted reduction with temporary fixation aims to limit this morbidity, and temporary trans-articular headless compression screws may provide more rigid scapholunate and lunotriquetral control than Kirschner wires. We asked what the mid-term clinical, radiographic and patient-reported outcomes of this technique are, using the contralateral wrist as a paired control, and whether they differ by injury pattern. METHODS: Retr

Tricortical iliac crest wedge grafting with headless screw fixation for unstable scaphoid waist nonunion: Union rates and two-year functional outcomes.

BACKGROUND: Unstable scaphoid waist nonunion disrupts wrist biomechanics through scaphoid shortening, humpback deformity, and dorsal intercalated segment instability (DISI), predisposing to degenerative collapse. Structural bone grafting aims to restore scaphoid length and alignment in addition to achieving union. This study evaluated radiological and functional outcomes following tricortical iliac crest wedge grafting for unstable scaphoid waist nonunion. METHODS: A retrospective analysis was conducted on 40 adult patients with unstable scaphoid waist nonunion treated using a volar tricortic

Hooked on Non-union: A Rare Case of Concurrent Scaphoid and Hook of Hamate Non-union.

Carpal fractures are common orthopaedic injuries, with the scaphoid being the most frequently affected bone. Hamate fractures are uncommon and easily missed due to their subtle clinical presentation and difficulty in detection on standard radiographs, often being misdiagnosed as soft tissue trauma. Simultaneous involvement of the scaphoid and hamate is uncommon, and progression to dual non-union is even more so. We report a case of concurrent scaphoid and hook of hamate non-union to highlight the importance of early recognition and appropriate imaging to prevent delayed diagnosis and long-term

Enhancing the clinical understanding of scaphoid nonunion treatment with a 1,2-ICSRA vascularized graft through 3D digital anatomy: a randomized controlled trial for medical students.

BACKGROUND: Treatment of scaphoid nonunion with a vascularized bone graft based on the 1,2‑intercompartmental supraretinacular artery (1,2‑ICSRA) involves complex wrist anatomy. Traditional teaching methods rely mainly on anatomical atlases and two‑dimensional (2D) computed tomography (CT) images, making it difficult for students to mentally reconstruct three‑dimensional (3D) spatial relationships. Three‑dimensional digital anatomy technology can provide visual 3D structural information that clearly illustrates the spatial relationships among anatomical structures, thereby potentially improvin

Scaphoid Osteoid Osteoma in a Pediatric Patient: A Case Report and Review of Diagnostic Pitfalls and Management Strategies.

Osteoid osteoma (OO) of the scaphoid is an uncommon entity, particularly in pediatric patients, where diagnosis is often delayed due to its rarity and overlapping presentation with more common wrist pathologies. We report the case of a pediatric athlete with persistent wrist pain initially diagnosed as a scaphoid fracture. Imaging progression from radiographs to MRI suggested fracture, but confirmatory CT revealed a 5-mm radiolucent lesion with central mineralization, consistent with OO. Surgical excision via volar approach was performed due to intraosseous lesion location and proximity to art

Comparison of volar distal radius and iliac crest bone grafts in the treatment of scaphoid non-union with humpback deformity: a systematic review and meta-analysis.

BACKGROUND: Scaphoid non-union with humpback deformity is commonly managed with volar structural bone grafting. This meta-analysis compares outcomes of distal radius bone graft (DRBG) versus iliac crest bone graft (ICBG). METHODS: Following PRISMA guidelines (PROSPERO CRD420251106177), MEDLINE, Embase, ClinicalTrials, and SPORTDiscus were searched to March 1, 2026. Comparative adult studies of scaphoid non-union with humpback deformity treated via volar DRBG or ICBG with headless compression screw fixation were included. Primary analysis pooled vascularized and non-vascularized grafts; subgro

Arthroscopic Curettage and Bone Grafting of Scaphoid Enchondroma.

Benign tumors of the carpal bones are uncommon, and their optimal treatment is still a topic of debate due to the condition's rarity. While there is consensus on treating enchondromas with pathological fractures to alleviate symptoms, the best approach for removing these benign tumors remains uncertain. With the growing popularity of wrist arthroscopy for various wrist pathologies, we present our technique of arthroscopic enucleation, bone grafting, and fixation of scaphoid enchondroma. Our method showed favorable outcomes at the one-year follow-up. We aim to describe our technique, including

Senile Scaphoids: Outcomes of Surgical Fixation for Scaphoid Fractures in an Elderly Population.

BACKGROUND: Scaphoid fractures are common carpal injuries among adults, though their optimal management may differ based on patient demographics. While these fractures often result from high-energy trauma in younger patients, low-energy mechanisms predominate in older adults, who are frequently treated nonoperatively due to concerns regarding bone quality and healing potential. Recent studies suggest that factors other than age more strongly influence patient outcomes, indicating older adults may benefit from surgical fixation over conservative management. This study evaluated outcomes followi

Clinical and Radiologic Outcomes of NOVOSIS (rhBMP-2 With Hydroxyapatite Carrier) Grafting for Scaphoid Waist Nonunion: A Preliminary Clinical Study.

OBJECTIVE: Scaphoid nonunion remains a challenging problem due to limited vascularity and carpal malalignment. Although autologous bone grafting remains the standard of care, donor-site morbidity and technical demands can be substantial. NOVOSIS, a composite of recombinant human bone morphogenetic protein-2 (rhBMP-2) and a hydroxyapatite (HA) carrier, has shown osteoinductive and osteoconductive potential in other orthopedic applications, but its clinical efficacy in scaphoid nonunion has not been established. METHODS: Seven consecutive patients with scaphoid waist nonunion underwent fixation

Interdisciplinary consensus statements on imaging of scaphoid fractures.

OBJECTIVES: To establish evidence-based consensus statements on imaging of scaphoid fractures. MATERIALS AND METHODS: Nineteen hand surgeons formulated a preliminary list of eleven questions on imaging of scaphoid fractures. Based on this preliminary list, radiologists crafted statements considering literature and their clinical experience, then refined them through an iterative Delphi process to revise the questions and statements. A maximum of three Delphi rounds was scheduled until group consensus was achieved for an individual statement, whichever arose first. Twenty-eight radiologists dr

Incidence, Risk Factors, Management, and Nonunion Rates of Pediatric Scaphoid Fractures: A National Database Study.

PURPOSE: Scaphoid fractures are often complicated by nonunion, but incidence, risk factors, and management have not been well-characterized for pediatric patients. METHODS: Patients aged <18 years with scaphoid fracture were identified in the 2015 to 2023 PearlDiver M170 database. Patient and fracture characteristics, imaging, and surgeries were extracted. Predictive factors for nonunion were characterized with multivariable analyses. Among patients with nonunion, characteristics of those receiving surgical and nonsurgical management were compared with multivariable analyses. RESULTS: Of 26,

Scaphoid Nonunion Arthroscopic-Assisted Reduction and Percutaneous Fixation With Distal Radius Autologous Bone Graft.

Scaphoid fractures are common and often progress to nonunion due to the unique vascularity. When nonunion occurs, patients are at risk of developing carpal collapse and arthritis if not managed appropriately with traditional surgical management. However, open surgical management puts the fragile vascular supply and extrinsic soft tissue stabilizers of the carpus at risk, compromising healing and clinical outcomes. Arthroscopy allows for the preservation of these structures and improved visualization of fracture debridement, reduction, and alignment. We describe an arthroscopic-assisted reducti

Surgical Fixation Versus Cast Immobilization for Adults With Bicortical Scaphoid Fractures: A Target Trial Emulation of the SWIFFT Trial.

AIMS: The Scaphoid Waist Internal Fixation for Fractures Trial (SWIFFT) reported no notable difference in functional outcomes between surgical fixation and cast immobilization for minimally displaced scaphoid waist fractures in UK settings. We aimed to emulate SWIFFT using Chinese hospital electronic health record (EHR) data to evaluate whether similar treatment effects are observed in a population with different risk profiles and to identify subgroups that may benefit differentially from surgery. METHODS: This target trial emulation used a clone-censor-weight design with inverse probability
