Twenty-five patients with documented Bennett's Fractures were reviewed five to sixteen years following injury (Mean 9.6 years). The majority of fractures affected the dominant hand and occurred in males. Twenty-two patients were managed conservatively by plaster immobilisation. At review ten patients were asymptomatic and only two had significant symptoms. Examination revealed loss of movement in the trapeziometacarpal joint of the thumb in twenty-one cases. Infection has not been considered in this article with each individual implant. The incidence is low indeed. From an experience of 120 fractures of the base of the first metacarpal bone, the authors evaluated the functional results of in 40 patients followed-up. Most of these patients had been treated by Bunden's method of longitudinal pinning. Nine patients on long-term hemodialysis with dialysis encephalopathy were studied, with sex matched control subjects for eight of the patients. Comminuted fractures of the base of the first metacarpal are difficult to stabilize with internal fixation or with plaster casts. Use of the external mini-fixator can provide good results. A case of Ewing's Sarcoma of the thumb is described. Four years after a fracture of the first metacarpal, followed by a series of repeated traumata to the area, a tumour was discovered. The frequency fo secondary displacement in fractures of the distal end of the radius is very high and varies from 40 to 60% in various series. This results from the void in the epiphysis after reduction due to crushing of the cancellous bone. Secondary displacement can be easily avoided by the bipolar fixation method. Osteogenesis imperfecta is often regarded as a form of osteoporosis. In many cases, particularly those in whom the first fracture occurs outside the neonatal period, bones that have not been fractured may appear radiologically normal. In a group of 24 adults with osteogenesis imperfecta the thickness of the metacarpal cortex was normal but their bones were often slender. Osteoporosis is probably not an inevitable feature of such cases, and some of the radiological abnormalities reported may be the results of previous fractures and their treatment.
