PURPOSE: Bennett fractures are inherently unstable intra-articular fractures of the first metacarpal base and are subject to multiple deforming forces. Suboptimal treatment can lead to fracture displacement, malunion, and post-traumatic arthritis of the carpometacarpal joint. The purpose of this study was to evaluate three fixation techniques for the stabilization of Bennett fractures. Our hypothesis was that screw fixation would be stiffer than the other methods.
METHODS: Thirty fresh frozen cadaver hands were obtained. Bennett fractures were created by a fellowship-trained hand surgeon. Spe

OBJECTIVES: Fractures and dislocations of the bases of the fourth and fifth metacarpal bones are rare, accounting for <1 % of all hand injuries. If left untreated or inadequately fixed, there is a risk of persistent pain, functional impairment, and post-traumatic osteoarthritis. While common surgical approaches such as arthrodesis or suspension arthroplasty have been described, data on alternative treatment options remain limited.
CASE PRESENTATION: This case report presents the first documented use of minced cartilage for treating post-traumatic CMC V osteoarthritis. A 17-year-old patient ex

A 28-year-old woman presented with a large lytic lesion and pathologic fracture of the right first metacarpal. Imaging demonstrated an expansile, destructive process with cortical breakthrough, and open biopsy confirmed the diagnosis of a giant cell tumor. Given the locally aggressive lesion and minimal remaining bone stock, the entire first metacarpal was excised. Definitive reconstruction was performed using a pedicled reverse radial forearm osteocutaneous flap, incorporating a vascularized segment of radius to recreate the thumb metacarpal. Same-stage metacarpophalangeal arthrodesis also wa

PURPOSE: Retrograde intramedullary headless compression screw (IMHS) fixation offers potential advantages over traditional surgical treatment of metacarpal shaft fractures. The purpose of this study was to compare the rates of recovery of short-term functional outcomes for displaced metacarpal shaft fractures treated nonsurgically and those treated using a minimally invasive intramedullary screw technique. We hypothesized that IMHS fixation would lead to earlier return of function with equivalent outcomes at final follow-up.
METHODS: We identified patients between 2017 and 2022 with a displac

INTRODUCTION: Hand fractures are associated with significant morbidity. Current management standards often result in prolonged immobilization, stiffness, and delayed return to functional use. Intramedullary (IM) compression screws offer minimal soft tissue disruption and early postoperative active motion. In this study, we describe our outcomes after intraosseous fracture fixation using IM cannulated headless screws for a multitude of fracture patterns.
METHODS: This study is a retrospective review of patients who underwent IM screw placement for fixation of metacarpal and phalangeal fracture

BACKGROUND: Metacarpal fracture fixation using the retrograde intramedullary screw technique can be performed through two different approaches. The mini-open approach requires greater soft tissue dissection but allows for direct visualization of the metacarpal head compared with the percutaneous approach. Our aim was to determine which approach resulted in optimal screw position.
METHODS: Eighty-one consecutive patients that underwent intramedullary screw fixation for metacarpal fractures from 2016 to 2021 were identified. Patients were treated by 4 fellowship-trained orthopedic hand surgeons

Intra-articular fractures of the base of the first metacarpal (Bennet fractures) are prone to dislocation and require surgical reduction and fixation to prevent secondary degenerative joint disease and chronic dysfunction. Therefore, a prompt diagnosis is necessary, mostly achieved by conventional roentgenograms. We report the case of a 62-year-old man in whom a Bennet fracture was highly suspected on ultrasound (US) examination realized after a fall. Standard radiographs, obtained after US to confirm the diagnosis, were interpreted as normal. A computed tomography was then performed showing a

CASES: Two competitive athletes with isolated comminuted trapezium fractures, sustained during American football and rugby, presented with pain and swelling of the first metacarpal base. Computed tomography confirmed isolated comminuted trapezium fractures. Arthroscopic reduction of displacement with screw fixation below the articular surface was performed through a small incision. Both patients returned to their preinjury sports levels within 2 months postoperatively.
CONCLUSION: Arthroscopic treatment of isolated comminuted trapezium fractures achieved accurate reduction of articular surfac

Periosteal sleeve fractures, or avulsions of cartilage and/or periosteum with or without an osseous fragment in skeletally immature individuals, are notoriously easy to miss and a high index of suspicion is necessary for accurate diagnosis and treatment. While periosteal sleeve avulsion fractures are classically reported in the patella, they have also been reported in the shoulder, clavicle, and elsewhere in the knee. However, no published reports exist for a periosteal sleeve avulsion fracture in the hand. This case details the first reported instance of such an injury involving a thumb metac

BACKGROUND: Bennett's fracture, a fracture-dislocation of the base of the first metacarpal, poses significant challenges due to the unique biomechanics of the thumb's carpometacarpal (CMC) joint. Effective management is critical to restoring thumb function and preventing long-term complications such as arthritis and instability.
OBJECTIVE: This article provides a comprehensive overview of Bennett's fracture, including its mechanism of injury, diagnostic considerations, and management strategies, with a focus on conservative and surgical options.
METHODS: A detailed review of the current lite

Rolando fractures are comminuted, multifragmented, complete intra-articular fractures at the base of the thumb metacarpal. A review of the literature and our clinical practice shows that it can be treated in many ways. Patients with a non-displaced fracture require immobilization of the thumb. Nonoperative treatment is rare. Displaced fractures should be treated surgically. The primary goal of Rolando fracture treatment is to achieve anatomical reduction, ensuring that the fractured bones realign in their original anatomical position. As described in the article, the procedure of implanting an

INTRODUCTION: The percutaneous pinning method described by Iselin is one of the techniques used for treating Bennett fractures at the base of the thumb metacarpal. There is little published data on the medium- and long-term outcomes of this treatment, with most studies having a mean follow-up of 4 years. The primary objective of this study was to evaluate the functional and radiological outcomes of percutaneous pinning by Iselin's method with a mean follow-up of 6 years. The secondary objective was to look for evidence of radiographic carpometacarpal (CMC) arthritis.

BACKGROUND The thumb is crucial for the aesthetic and functional aspects of the upper extremity. A crushed thumb injury can be particularly challenging, especially for individuals with high demands. Currently, there is no consensus on the best approach for treating a crushed thumb. The scapular flap, vascularized by the subscapular artery system, shows great potential as a free flap option. This report aims to highlight the use of a triple flap procedure to preserve the function of the crushed thumb. Introduction Understanding the incidence and epidemiology can inform clinicians and policymakers about the population's needs. Our study reports on upper limb fractures treated at a major trauma center over 7.5 years. Methods We collected data on fracture locations, age, gender, Charlson Comorbidity index (CCI), and treatment options of all upper limb fractures treated at a Level I Trauma Centre from January 1, 2015 to June 30, 2022. Humerus, radius, and ulna fractures were each classified as proximal, diaphyseal, and distal. Results About 9,915 patients sustained 12,790 fractures, given an o

BACKGROUND: Isolated, highly comminuted trapezium fractures are extremely rare. While they can be initially diagnosed with plain radiographs, a computed tomography scan of the wrist will more clearly reveal the fracture details and aid in appropriate preoperative planning. Restoration of the fracture with a congruent reduction of the adjacent joints is mandatory for a favorable prognosis.
CASE DESCRIPTION: A case of a highly comminuted, biarticular, trapezium fracture in a young male patient that was treated operatively with open reduction and internal fixation (ORIF) with a cannulated minisc

INTRODUCTION: Closed hand fractures represent a significant proportion of emergency department attendances, result in substantial health service utilisation and have a detrimental effect on quality of life. Increasingly, hand therapists in the United Kingdom provide first line fracture treatment. However, the knowledge and skills required to work in such an extended scope capacity have not been elucidated or standardised. This literature review synthesises and reports evidence for the knowledge requisite of clinicians to make evidence-based treatment decisions for patients with hand fractures.

Repetitive instances of sudden injuries to the first metacarpal bone can affect thumb movement. These injuries typically occur after vertical impact to the thumb. The treatment for these injuries should focus on restoring the structure and biomechanics of the trapeziometacarpal joint, with surgery being recommended for optimal results. Bennett's fracture involves the bifurcation of the bone into two distinct fragments, characterized by a smaller fragment and a larger counterpart. Rolando fracture is associated with a fracture of the base of the first metacarpal bone, typically divided into thr
