Biomechanical analysis of plate versus K-wire fixation for metacarpal shaft fractures with wedge-shaped bone defects.

BACKGROUND: Metacarpal shaft fracture is a common type of hand fracture. Numerous studies have explored fixing transverse fractures in the midshaft of the metacarpal bone. However, this section of the metacarpal bone is often susceptible to high-energy injury, resulting in comminuted fracture or bone loss. In such cases, wedge-shaped bone defects can develop in the metacarpal shaft, increasing the difficulty of performing fracture fixation. Notably, the research on this type of fracture fixation is limited. This study compared the abilities of four fixation methods to fix metacarpal shaft frac

Operative Treatment of Non-Thumb Carpometacarpal Joint Fracture Dislocations.

PURPOSE: To investigate the functional outcomes and complications of operatively managed carpometacarpal (CMC) fracture dislocations, excluding the thumb, in a young, active population. METHODS: A retrospective chart review of all patients undergoing surgical treatment for CMC joint injuries at a single institution over a 6-year period was performed. Patients were excluded if they had a first CMC joint injury, were under 18 years old, or had incomplete documentation. Injury radiographs were categorized as simple/extra-articular, partial articular, and complete articular. Electronic health rec

Recurrent Pediatric Thumb Carpometacarpal Joint Dislocation Due to Generalized Joint Laxity Successfully Treated With Ligament Reconstruction: A Case Report.

Acute traumatic dislocation without fractures of the thumb carpometacarpal (CMC) joint is extremely rare in children. Treatment options, such as closed reduction with casting or pinning and open reduction with primary ligament repair, remain controversial. Here, we report the first case of an 11-year-old boy with recurrent left thumb CMC joint dislocation due to idiopathic generalized hyperjoint laxity, even after primary open reduction with capsular ligament repair of the thumb CMC joint, eventually treated with Eaton-Littler's ligament reconstruction. Intraoperatively, a drill hole was made

Surgical Management of a Delayed Presentation of Floating First Metacarpal with Multiple Fracture Dislocations: A Case Report.

CASE: A 23-year-old man presented with a 2-month-old injury involving a volar base metacarpal fracture dislocation of carpometacarpal joint (CMC), metacarpophalangeal joint (MP) dislocation, floating metacarpal, and proximal phalanx base fracture. Treatment involved open reduction of volar base metacarpal's fracture and CMC dislocation, MP joint reduction, collateral ligament repair, and proximal phalanx fixation with plates and screws. A modified Eaton-Littler procedure with flexor carpi radialis tendon enhanced CMC joint stability. At 6 weeks, fractures had healed well, achieving good thumb

3D-Printed Metacarpal Prosthesis in the Treatment of Primary Osteosarcoma of the First Metacarpal: A Novel Surgical Technique.

OBJECTIVE: Osteosarcoma at the first metacarpal is extremely rare. Reconstructing the metacarpal after tumor resection is essential, as the thumb accounts for approximately 40%-50% of hand function. Although autografts, arthroplasty, and transposition have been reported as reconstruction options, their use is limited by complications such as secondary injury, nonunion, and displacement. In this study, we present a case of a patient with first metacarpal osteosarcoma who underwent tumor resection followed by reconstruction with a 3D-printed metacarpal prosthesis. We tend to introduce a novel st

Knotless Anchor with FiberTape® for Fixing Pure Distal Patellar Tendon Rupture. A Case Report.

INTRODUCTION: Perilunate dislocations are rare high-energy injuries which may often have the potential to cause lifelong disability of the wrist if not addressed optimally. Hence, early recognition, diagnosis, and intervention are of paramount importance in the restoring function and prevention of morbidity. Lunate dislocations are the fourth and last stage of perilunate dislocations being extremely rare, with volar dislocations representing <3% of perilunate dislocations. CASE REPORT: A 24-year-old man suffered from an alleged history of fall from a bike on an outstretched hand following whi

Clinical Outcomes of Closed Reduction and External Fixation for Isolated Second Metacarpal Base Fracture-Dislocations.

OBJECTIVE: The isolated second metacarpal base fracture-dislocations in adults are rare, and the traditional main treatments include closed reduction with the plaster or splint and open reduction with internal fixation (ORIF). However, closed reduction with the plaster or splint is not solid enough and ORIF can damage the surrounding important tissues. The purpose of this study was to explore the clinical outcomes of closed reduction and external fixation (CREF) for the treatment of isolated second metacarpal base fracture-dislocations. METHODS: Ten patients who suffered isolated second metac

The floating first metacarpal: A rare case.

A floating metacarpal is defined as the simultaneous dislocation of the metacarpophalangeal (MCP) and carpometacarpal (CMC) joints. Metacarpal dislocations are rare, and floating metacarpals with double dislocations are extremely rare. In this article, we present a very rare case of floating metacarpal in which the first MCP and CMC dislocations were found simultaneously and successfully treated with closed reduction, open reduction, and Kirschner wire fixation methods.

Comparative Bending Strength of Metacarpal Neck Fractures Fixed with Two Types of Intramedullary Screws.

OBJECTIVES: Intramedullary (IM) screw fixation of metacarpal fractures is a technique, which has gained in popularity owing to its simplicity, speedy rehabilitation, and good functional outcomes. A new, larger diameter, non-compression screw designed specifically for IM metacarpal fixation was recently introduced which could provide better fracture stability and reduce the risk of hardware failure. Our goal was to evaluate the strength of this screw compared to a first-generation screw. METHODS: This mechanical study was designed to compare a 4.5 mm metacarpal headless screw (MCHS) to data fr

Outcome and racing performance following standing fracture repair in 245 horses.

BACKGROUND: Repair of sagittal proximal phalanx (P1) and parasagittal metacarpal/metatarsal III (MC/MTIII) fractures has evolved over recent decades from a procedure carried out solely under general anaesthesia, to one commonly performed under standing sedation. To date, standing fracture repair has not been evaluated for large cohorts. OBJECTIVES: To determine short-term (survival to discharge) and long-term (return to racing) outcomes of horses undergoing standing repair of MC/MTIII and P1 fractures, and to compare pre-surgical and post-surgical racing performance. STUDY DESIGN: Single-cen

The stacking concept: cannulated screws for extra-articular metacarpal base fracture of the thumb.

An alternative technique to treat extra-articular fractures of the base of the first metacarpal with intramedullary canulated headless screws is presented. The principle is creating an internal fixator within the medullary canal by introducing multiple retrograde screws until they have jammed.

Intramedullary fixation of metacarpal and phalangeal bone fractures with bioabsorbable Mg K-wire in 20 cases.

PURPOSE: The treatment of irreducible or severely displaced metacarpal and phalangeal bone fractures is still much debated. The recent development of the bioabsorbable magnesium K-wire is thought to allow effective treatment upon insertion via intramedullary fixation by minimizing articular cartilage injuries without discomfort until pin removal and drawbacks, such as pin track infection and metal plate removal. Therefore, this study investigated and reported the effects of intramedullary fixation with the bioabsorbable magnesium K-wire in unstable metacarpal and phalangeal bone fractures. ME

Preventable hand injuries: A national audit.

Little is known of the scale of avoidable injuries presenting to medical services on a national level in the UK. This study aimed to assess the type and incidence of preventable wrist and hand injuries (as defined by the core research team) at a national level in the UK. 28 UK hospitals undertook a service evaluation of all hand trauma cases presenting to their units over a 2 week period in early 2021 identifying demographical and aetiological information about injuries sustained. 1909 patients were included (184 children) with a median age of 40 (IQR 25-59) years. The commonest five types of

Treatment of Fourth and Fifth Carpometacarpal Fracture-Dislocations in Punching Injuries With Motion-Sparing Hamate Fixation Spring Plates: "Rabbit Ears" Plating.

Punching injuries to the hand are frequent and can result in fourth and fifth carpometacarpal (CMC) fracture-dislocations. Fourth and fifth CMC fracture-dislocations are unstable, and dorsal metacarpal dislocations are most common. Operative management for maintaining reduction of the unstable fracture-dislocation was closed reduction and percutaneous pinning; however, in delayed fractures, open reduction is necessary. We report on a plating technique used for acute and delayed, unstable fourth and/or fifth CMC fracture-dislocations. This method of plating is novel and allows for physiological
