Assessment of bone mass by image analysis of metacarpal bone roentgenograms: a quantitative digital image processing (DIP) method.

a digital processing (DIP) method for assessing bone mass was developed on the basis of image analysis of roentgenograms. Linearity between DIP values and the actual calcium carbonate content was scarcely affected even if roentgenograms were made with bone phantoms placed in different depths of water or by altering the voltage of X-ray generation. In clinical studies, coefficients of variation (CV) for various measurements were lower than 2.4%. When the correlations between the DIP values and the bone mineral densities in the distal one-third of the radius, and the 2nd to 4th lumbar vertebrae

Indexes of bone mineral content on second metacarpal bone roentgenogram analyzed by digital image processing: a comparison with other bone mass quantifying methods.

A new photodensitometric technique (digital image processing, DIP) was performed to assess the bone mineral content of the metacarpal bone as a screening test for bone loss in 340 healthy or osteoporotic women as well as 35 steroid-dosed patients and 46 patients with chronic renal failure on hemodialysis. This procedure was compared with single photon absorptiometry and dual photon absorptiometry in terms of clinical value. Two bone mineral indexes, metacarpal index and sigma GS/D (the integrated bone mineral density of cortical and trabecular bone on a cross section in the middle of the secon

Long-term follow-up of partial limb amputation in 13 horses.

Thirteen cases of partial limb amputation in horses were reviewed. Nine horses (69%) survived 18 to 111 months (mean, 41 months). Four unsuccessfully treated horses were euthanatized within 21 days of surgery. The horses' ages at amputation were 4 to 13 years (mean, 7.3 years). There was no predilection for the left or right limb nor for fore or hind limb involvement. Osteomyelitis in six horses (46%) was the most common cause for amputation. Severe, open, comminuted fractures of the third metacarpal bone that were impossible to stabilize by any currently available technique in four horses (31

Long incomplete longitudinal fractures of the third metacarpal and metatarsal bone in horses.

Five horses with long incomplete longitudinal fractures of the third metacarpal bone and three horses with similar fractures of the third metatarsal bone were examined. In three of the metacarpal fractures a long incomplete fissure extended proximomedially, in association with the more common lateral condylar fracture of the third metacarpal bone, and in the other two cases the fracture originated from the medial aspect of the distal articular surface. In the three horses with fractures of the third metatarsal bone the fractures had a consistent spiral configuration.

Metacarpal fractures in the athlete.

Fifty-six fractures of the metacarpal occurring in 53 athletes were studied from September 1985 to December 1986, regarding mechanism of injury, type of fracture, type of treatment, and time lost from sport. Age range of the patients was 8 to 28 years with greater than 77% being in the 14 to 18 year age range, the high school athlete. Twenty-nine of the fractures occurred in football, 14 in basketball, and the remainder were divided between various other sports. The most common mechanism of injury involved falls or hitting an object such as a helmet or another player. Fractures were evenly div

[Use of a rod apparatus for external fixation in the treatment of hand injuries].

The authors describe an external fixation rod apparatus and the results of its application in 28 patients with various injuries. The apparatus was used in the patients with fractures of phalanges of the fingers and metacarpal bones for extra-articular fixation after the elimination of contractures and dislocations in the joints, in cases of combined epidermatoplasty, arthrodesis, etc. The method of treatment had to be changed only in 2 patients. In all other cases the effect of the rod apparatus was positive.

Fractures of the body of the trapezium.

Fractures of the body of the trapezium are uncommon. Ten such cases are reported. They may be associated with other carpal or metacarpal fractures and may require special radiographic views for diagnosis. There does not appear to be a consistent mechanism of injury. A classification system is proposed based on the five fracture configurations identified. Open reduction with internal fixation of the displaced articular surface is preferred.

Biomechanical and clinical evaluation of the expandable intramedullary fixation device.

The use of an expandable intramedullary device for internal fixation of metacarpal or phalangeal bones is described. The device is applicable to transverse fractures, short oblique fractures, or transverse osteotomies. The device consists of a cylindrical apparatus made of titanium that allows collapse in the circumferential diameter. It is introduced into the medullary canal in its collapsed state, and on release expands to its normal diameter in the canal with the fracture reduced over it. A biomechanical evaluation compared the stability of this device with other commonly employed fixation

Management of a severely comminuted fracture of the third metacarpal bone in a horse.

A 4-year-old Standardbred stallion sustained a severely comminuted fracture involving the second, third, and fourth metacarpal bones. The fracture was repaired using two 14-hole broad dynamic compression plates positioned at 90 degrees to one another, allowing one plate to protect the other in the bending mode. An autologous cancellous bone graft obtained from the tuber coxae was added at the site of the defect in the mid- to upper third of the third metacarpal bone. Complications associated with the fixation included a Staphylococcus aureus infection 5 months after surgery, laminitis that dev

[Lengthening middle hand and finger segment stumps by external distraction devices following traumatic amputations and in congenital abnormalities].

Since November 1984, lengthening of metacarpals and digital phalanges by a distraction apparatus has been performed in six metacarpal bones and four phalangeal bones in nine patients (four females, five males). The age ranged from 14 to 59 years (average 36). The follow-up period was from two to 28 months (average 16). Lengthening was performed by two kinds of external distraction devices, the Ikuta external fixateur for metacarpals and the mini-Hoffmann-device for phalanges. The achieved elongation of the metacarpal bones varied from 26 mm to 30 mm (average 28.8 mm), and of the digital phalan

Metacarpal fractures.

We have treated 1,080 fractured metacarpals between January 1972 and December 1982. Supported by ten years of experience and this homogenous series, we explain our choices of treatment. With early mobilization of hand and fingers a constant preoccupation to prevent joint stiffening, two main points stand out: active immediate mobilization, considered not as a dismissal but as a treatment, is managed by a specialized physiotherapist and supervised by the surgeon. We applied it in 32% of our cases. Internal fixation is sometimes tricky and always done with great attention to detail. If so, it en

Treatment of metacarpal and phalangeal fractures in noncompliant patients.

Uncooperative patients are a problem in any practice setting; however, this is particularly true in large urban county hospitals. Treatment of these patients was unsatisfactory unless special measures were undertaken. Behavior characteristics of noncompliant patients must be identified. Prospective identification of these individuals was found to be the key element in successful treatment. Special, heavy duty splints using proper hand posture were found to be most effective. If surgical treatment was required, then rigid fracture fixation was more beneficial than other forms of internal immobi

Use of a minicondylar plate for metacarpal and phalangeal periarticular injuries.

The minicondylar plate fixation system is a new method for stable internal metaphyseal bone fixation about the metacarpophalangeal (MP) and proximal interphalangeal (PIP) joints. Indications for its use are rigid stabilization of fracture, osteotomy, and the bony components of composite tissue transfer. The device should be used in deference to other techniques when early active range of motion is pursued. Its best use is in the face of combined injury or osteotomy with tenolyses/capsulotomy. This implant permits stabilization of small epiphyseal fragments with minimal interference of joint fu

Treatment of unstable metacarpal and phalangeal fractures with tension band wiring techniques.

The techniques of tension band fixation are applied in the treatment of metacarpal and phalangeal fractures. The technique uses standard, readily available materials. The variability of the fixation techniques allows for secure fixation of virtually any long bone fracture in the hand, even when other methods are not applicable. Nonunion, malunion, loss of fixation, infection, or tendon rupture have not occurred. The technique allows for early active motion and return to full activity usually within for to six weeks. Most patients achieve full painless motion in that time.

Osteoporosis: radiologic and nuclear medicine procedures.

A number of radiographic and nuclear medicine techniques are available for the assessment of patients suspected of having osteoporosis or at risk for the development of osteoporosis. Spinal radiographs are insensitive indicators of osteoporosis. They can document the presence of metastatic tumors or other lesions that may produce compression fracture. The Singh index assesses the trabecular pattern of the proximal femur. As bone loss occurs, the trabeculae disappear in a definite sequence. Radiogrammetry refers to the measurement of bone and cortical widths in the peripheral skeleton, usually

Nonunion of metacarpal extraarticular fractures in children: report of two cases and review of the literature.

Extraarticular metacarpal fractures are common during childhood, and osseous healing is the rule. We have treated two patients with delayed and nonunion of three metacarpal shafts. One patient sustained closed second and third metacarpal fractures, and the other sustained an open second metacarpal fracture. Both injuries were crush-type of significant force. Although initial treatment seemed appropriate, delayed union was not appreciated. The children were referred for cosmetic deformity. One of our patients required open reduction/internal fixation with bone grafting, whereas the other was ob

Rigid fixation of phalangeal and metacarpal fractures.

Rational use of rigid fixation in hand surgery requires awareness of the advantages as well as the potential pitfalls of this relatively complex method of fracture management. A rational decision also necessitates familiarity with closed techniques of internal fixation which, in most cases, provide superior alternatives for the phalanges and metacarpals. The optimal application of screws, plates, and tension-band wires to small bone fractures can be ascertained only by critical assessment of large series of cases employing these devices and comparisons with similar series using other technique

[Fractures of the long metacarpals. Long-term results].

Sixty-seven of long metacarpal fractures have been followed for more than one year. Digital mobility and strength of grasp of the traumatized hand were measured and then compared with the healthy hand, the result being modified, according to the patient's own appreciation. This clinical examination enabled a medium and long term assessment of functional results. Control radiograms helped us draw up a chart of the various types of fractures, their topographical distribution and eventual anatomical modifications according to displacement. When comparing clinical and radiographic results, a corre

Comparison of internal fixation techniques in metacarpal fractures.

A biomechanical study assessed quantitative differences in the stability that was obtained by five commonly used types of internal fixation employed in metacarpal fractures. The techniques included dorsal plating, dorsal plating combined with an interfragmentary lag screw, crossed Kirschner wires, a single intraosseous wire combined with a single oblique Kirschner wire, and a single intraosseous wire alone. Rigidity and strength in torsion and bending were determined after transverse osteotomy and fixation of the metacarpal were performed. The failure modes for each fixation technique were als
