Introduction Injuries sustained by professional athletes can severely impact seasonal performance and future abilities. The aim of this study was to characterize the impact of phalanx, thumb, metacarpal, and carpal injuries on the performance of athletes in the National Football League (NFL). Analyzing outcomes for football players is significant because the functional demands of each position vary widely. Methods NFL players who sustained phalanx, thumb, metacarpal, or carpal fractures from the 2012-2013 to 2022-2023 seasons were identified using the public database Pro Sports Transactions. P

PURPOSE: Phalangeal fractures of the hand are the second most common upper-extremity fracture in the United States. Numerous studies have reported on the biomechanical efficacy of intramedullary (IM) hand nail fixation of the phalanges and increased utilization of IM hand nails. During IM fixation, the nail often penetrates the articular cartilage during insertion. This study seeks to quantify the percentage of cartilage involvement created by anterograde and retrograde intramedullary fixation of proximal and middle phalangeal fractures with 3.0 mm and 3.5 mm IM threaded hand nails.

PURPOSE: Intramedullary screws in single- or double-screw configurations are increasingly used for proximal phalanx (P1) fractures. Our purpose was to assess the biomechanical rotational stability of antegrade one- versus two-screw constructs in the fixation of simulated transverse mid-diaphyseal P1 fractures.
METHODS: We used matched pairs of fresh-frozen cadaver upper extremities. We harvested the P1s of all fingers and made transverse mid-diaphyseal osteotomies. Across each matched pair, we alternated between one- and two-screw antegrade fixation constructs on matched P1s. We placed one-sc

Regional anesthesia has been proven useful in hand surgery after trauma, most commonly via brachial plexus blocks (BPB), with widely established benefits. Distal nerve blocks at the elbow are used much less frequently. A 15-year-old boy was admitted because of traumatic amputation of the 4th digit on his right hand. Difficult intubation was suspected. History was indicative of obstructive sleep apnea, and nil per os status was confirmed, with no apparent risk factors for regurgitation. Ultrasound-guided blocks of the median, radial, and ulnar nerves at the elbow were performed with 2% Lidocain

Ring avulsion injuries, though rare, represent a uniquely devastating subset of hand trauma, with circumferential damage to skin, tendon, bone, and neurovascular structures. Historically, these injuries were almost universally treated with amputation; however, advances in microsurgical repair have transformed management and opened the possibility of finger salvage. We review the epidemiology, classification systems, treatment options, and biomechanical studies that have prompted innovations in ring manufacturing. Over time, classification systems have shifted treatment emphasis from survival t

Extensor tendon rupture following plate fixation for distal ulna fractures is extremely rare. We present a case of delayed closed rupture of the extensor tendons of the ring and little fingers following distal ulna plate fixation. A 71-year-old woman developed progressive swelling and loss of active extension of the ring and little fingers eight years after surgery for distal radius and ulna fractures. Computed tomography demonstrated dorsal penetration of the most distal ulnar screw. Surgical exploration confirmed rupture of the extensor digitorum communis tendons to the ring and little finge

PURPOSE: To evaluate the biomechanical properties of four fixation methods for proximal phalanx base fractures, single antegrade cannulated screw (control), V-pattern, Y-pattern, and K-wire configurations, using a matched pair cadaveric model.
METHODS: Forty-eight fingers (index through little) from six matched cadaveric hand pairs were included. A standardized transverse base fracture was created in the proximal phalanx. One side of each pair received a single antegrade cannulated screw. Contralateral digits were treated with either a V-pattern (group 1), Y-pattern (group 2), or K-wire confi

Production of excess undesirable brine, is one of the severe problems in highly heterogeneous and fractured oil reservoirs which are under water based enhanced oil recovery (EOR) methods. Recently, preformed particle gels (PPGs) are proposed and used as a diverting agent to control the conductivity of the fractures. Although numerous studies in the literature report improved structural strength and rheological properties of PPGs in the presence of silica nanoparticles (SNP), the possible effect of SNP concentration on such results have not been investigated in details. In addition the effect o

Suture tapes are widely used in orthopedic procedures, providing superior tissue repair through higher tensile strength than traditional round sutures. Their broad, flat profile makes them especially well-suited for cerclage fixation, distributing pressure over a larger surface area and enhancing overall stability. Cerclage fixation remains a widely utilized technique in orthopedic practice, serving to reinforce structurally compromised tissue through circumferential mechanical support. Suture-based cerclage fixation has emerged as an alternative to traditional metal wires or cables for the ma

BACKGROUND: Infection rate after pediatric hand CRPP has been reported to be 2% in a small cohort, while infection rate after adult hand CRPP ranges from 7% to 16%. We are unaware of other established baseline rates of infection for this common pediatric surgery.
METHODS: This is a retrospective study of 734 pediatric patients treated at 6 pediatric institutions who underwent closed reduction percutaneous pinning (CRPP) for a closed finger/hand fracture. The primary goal was to establish infection rates in this patient population, with a secondary objective of comparing infection rates betwee

Isolated finger compartment syndrome is a rare disorder and is sparsely recorded in the literature. Fascial compartment pressure builds up, reducing perfusion to the finger and potentially leading to irremediable injury. We present a case of a man with severe pain who arrived at the hospital nine hours after he wounded his right ring finger in a high-pressure machine. The fingertip was pale and swollen, with a small lesion on the volar side. Capillary refill test and the pinprick test were abnormal, and radiographs showed no fracture. Finger fasciotomy was performed immediately through a radia

Volar proximal interphalangeal (PIP) joint fracture-dislocations presenting more than two weeks after injury pose significant management challenges, as progressive soft tissue fibrosis and capsular contracture frequently necessitate open reduction. Reports of successful delayed closed reduction with long-term follow-up in this setting remain limited in the literature. We report an 18-year-old man who presented 16 days after injury with a neglected volar PIP fracture-dislocation of the left ring finger, managed with delayed closed reduction under sedation and fluoroscopic guidance the following

Perioperative antibiotic prophylaxis (PAP) is a key component of infection prevention in trauma surgery and orthopedics. The new German S3 guidelines recommend a single-shot PAP dose for most procedures, particularly joint arthroplasty, osteosynthesis of closed fractures and other elective operations. Open fractures require pre-emptive antibiotic treatment for a maximum of 24 h in type I and type II fractures and for a maximum of 72 h in type III open fractures. Patients with an open distal phalanx fracture of the finger should not receive PAP. In knee arthroscopy PAP is recommended when forei

Mutilating hand injuries involving multiple digits present significant reconstructive challenges, particularly in the setting of severe crush mechanisms and vascular compromise. We report the case of an 18-year-old right-hand dominant male manual laborer who sustained a devastating crush injury to his right hand after entrapment under a concrete block. The injury involved near-amputations of the index, middle, and ring fingers, with complete disruption of digital neurovascular bundles and associated open fractures. Given the non-viability of distal segments and extensive soft tissue damage, re

BACKGROUND: Police work is associated with high physical demands and an increased risk of occupational injury, particularly among specialized tactical units exposed to intense operational activity. Musculoskeletal injuries are common in law enforcement personnel and frequently result in work incapacity, but data specifically addressing upper limb injuries in intervention units remain limited. The aim of this study was to characterize the clinical profile of upper limb injuries reported as duty-related accidents in a Portuguese police intervention unit and to analyze their association with surg

BACKGROUND: Hip resurfacing arthroplasty remains an appealing option for young and active patients1, but its use has declined because of technical challenges, risks of femoral neck notching, and adverse reactions associated with metal-on-metal bearings, raising concerns regarding long-term implant survival2. Accurate preparation of the femoral head and acetabulum is critical for successful outcomes3. In the present video article, we describe a reproducible technique utilizing preoperative 3D computed tomography (CT)-based planning and patient-specific 3D-printed jigs to optimize femoral head p
