External fixation is an accepted method of treatment for many injuries, however, only a limited number of reports concern its use in open, comminuted fractures of the hand. Ten patients with eleven fractures of the proximal phalanx were treated using the AO/ASIF small external fixator. Five fractures involved the interphalangeal joint (IPJ), two the metacarpophalangeal joint (MPJ), and four the shaft. Nine fractures were caused by small caliber gunshot wounds, one was due to a power saw injury, and one occurred during a three wheeler accident. The patients with interphalangeal joint involvemen

We have studied the layers of the muscular coat of the guinea-pig small intestine after enzymatic and chemical removal of extracellular connective tissue. The cells of the longitudinal muscle layer are wider, have rougher surfaces, more finger-like processes and more complex terminations, but fewer intercellular junctions than cells in the circular muscle layer. A special layer of wide, flat cells with a dense innervation exists at the inner margin of the circular muscle layer, facing the submucosa. The ganglia of the myenteric and submucosal plexuses are covered by a smooth basal lamina, a de

A prospective study of women's flag football injuries was carried out over a 4 year period at Mississippi State University. Data was gathered on 114 injuries. Organized intramural flag football activities caused 93% of the injuries observed (7% were injured in pick-up games). The finger accounted for 39%, knee for 16%, and ankle for 8% of these injuries. The type of injury was closely distributed among sprains, fractures, contusions, and strains. Collisions, with other players and objects, resulted in 64% of these injuries. Offensive ball handlers (running backs and receivers) had the greatest

We have treated 1,080 fractured metacarpals between January 1972 and December 1982. Supported by ten years of experience and this homogenous series, we explain our choices of treatment. With early mobilization of hand and fingers a constant preoccupation to prevent joint stiffening, two main points stand out: active immediate mobilization, considered not as a dismissal but as a treatment, is managed by a specialized physiotherapist and supervised by the surgeon. We applied it in 32% of our cases. Internal fixation is sometimes tricky and always done with great attention to detail. If so, it en

Uncooperative patients are a problem in any practice setting; however, this is particularly true in large urban county hospitals. Treatment of these patients was unsatisfactory unless special measures were undertaken. Behavior characteristics of noncompliant patients must be identified. Prospective identification of these individuals was found to be the key element in successful treatment. Special, heavy duty splints using proper hand posture were found to be most effective. If surgical treatment was required, then rigid fracture fixation was more beneficial than other forms of internal immobi

The minicondylar plate fixation system is a new method for stable internal metaphyseal bone fixation about the metacarpophalangeal (MP) and proximal interphalangeal (PIP) joints. Indications for its use are rigid stabilization of fracture, osteotomy, and the bony components of composite tissue transfer. The device should be used in deference to other techniques when early active range of motion is pursued. Its best use is in the face of combined injury or osteotomy with tenolyses/capsulotomy. This implant permits stabilization of small epiphyseal fragments with minimal interference of joint fu

The techniques of tension band fixation are applied in the treatment of metacarpal and phalangeal fractures. The technique uses standard, readily available materials. The variability of the fixation techniques allows for secure fixation of virtually any long bone fracture in the hand, even when other methods are not applicable. Nonunion, malunion, loss of fixation, infection, or tendon rupture have not occurred. The technique allows for early active motion and return to full activity usually within for to six weeks. Most patients achieve full painless motion in that time.

Both authors have dedicated most of their time since 1948 to the treatment of fractures in children. On the basis of their experience they are therefore submitting to the medical public those methods and results of the conservative treatment of fractures and dislocations in children which stood the test of time. The experiences of both authors as well as successful therapeutic methods of other surgeons are discussed in this book. Differences are stated between fractures in adults and in children which may be attributed to the growth factor of children's bones and their enormous biological driv

The synovial membrane in the carpal tunnel of twenty-nine patients suffering from the typical clinical picture of carpal tunnel syndrome was examined microscopically over an eighteen month period with a follow-up of six months to two years. Unusual causes such as tendon sheath tumours and post-Colles fractures were excluded. Three micropathological pictures emerged. Oedema was found in twenty-four patients, chronic perivascular inflammation in three and fibrinous synovitis in two. No difference was detected in the clinical presentation in the three pathological appearances. The authors believe

Rational use of rigid fixation in hand surgery requires awareness of the advantages as well as the potential pitfalls of this relatively complex method of fracture management. A rational decision also necessitates familiarity with closed techniques of internal fixation which, in most cases, provide superior alternatives for the phalanges and metacarpals. The optimal application of screws, plates, and tension-band wires to small bone fractures can be ascertained only by critical assessment of large series of cases employing these devices and comparisons with similar series using other technique

The authors describe one case of a severely displaced Smith's fracture which was originally immobilised by external fixation. Three month's later the fracture had not united and active extension of the thumb was absent. At operation, the extensor pollicis longus tendon was found to be dislocated into the fracture site. The tendon was repaired and the non-union was treated by plating. A review of the literature discovered only one other similar case.

A new type of endoprosthesis for reconstruction of the metacarpophalangeal (MCP) joint has been tested. The novel arthroplasty consists of a joint mechanism with two screw-shaped fixtures for bone anchorage. These fixtures, of commercially pure titanium, are gently screwed into the anchoring bone on each side of the diseased joint, the aim being to achieve osseointegration, i.e. a direct contact between implant and bone without interposed soft tissue layers. The hand was immobilized in a dorsal splint for 8-10 days post-operatively after which a carefully controlled mobilization program was in

A survey conducted among 224 volleyball players in the German Federal League revealed a high number of serious injuries. The upper ankle joint (55.1% of all serious injuries) and the fingers (22.4%) were most commonly affected. The principal cause of the accidents was a collision with an opponent at the net. There is a close relationship between the player's position, the part he plays in the game and the frequency of injury. Aside from other measures a change of one of the international volleyball rules would represent a suitable method of preventing such injuries.

The healing process of osteochondral fracture was studied experimentally and clinically, using fibrin adhesive system (FAS) containing highly concentrated and freeze-dried fibrinogen, thrombin and factor XIII. Osteochondral fractures were made in the femoral condyle of the rabbit. Most of the adhered fibrin was gradually absorbed at the fracture site and at the same time was replaced with granulation tissue containing abundant capillaries, fibroblasts and osteoblasts, by the 10th day. Complete fracture healing was achieved histologically in one month. Fifteen cases of intraarticular osteochond

106 liver resections have been performed at the Department of Surgery. 42 of these were liver segmentectomies and other minor liver resections. Such kinds of resective procedure have been adopted in 40 patients affected by benign or malignant lesions in whom the normal liver parenchyma had to be preserved. Indications and technical aspects of hepatic tissue preservation in liver resection are discussed. Liver segmentectomy required a transparenchymal approach, by means of finger fracture technique, through scissural planes to the deeply located segmental pedicle. It should be noted that blood

Since January 1979 we have treated 14 patients at the 1st University Clinic of Traumatology in Vienna with the MFE described by Jaquet. The MFE was used mainly in severe open fractures of fingers and metacarpal bones. All 14 patients were examined for follow-up after a mean interval of 20 months. This method of treatment warrants minimal strain on the soft tissues while it ensures stability and optimal care of the wounds. Osseous consolidation was achieved in 13 of 14 patients; although one patient developed pseudarthrosis, he was, like 12 other patients, perfectly satisfied with the result of

Two cases of rupture of the flexor tendons of the little finger associated with a fracture of the hook of the hamate bone are presented. The preoperative diagnosis had not been made. Excision of the fractured hook and repair of the tendons by a short graft from the palmaris longus in one case and by the transfer of the superficial flexor of the ring finger in the other case gave a good final result. The authors remark on the rarity of reports of this fracture in the literature, the frequent occurrence of nonunion and the two complications which are exceptional; rupture of the flexor tendons an
