The frequency of calcifications in vessels and soft tissue was 39% in 184 patients with chronic renal failure. Non-dialyzed patients and dialyzed patients at the start of dialysis treatment had most frequently calcifications of the aorta and larger vessels. Renal transplant recipients, who developed osteonecrosis or spontaneous fractures after transplantation, showed at the time of transplantation a higher frequency of calcifications compared to renal transplant recipients, who did not develop these complications. The vascular-pedicle "wrap-around" free flap was used in five patients for reconstruction in amputations distal to the metacarpophalangeal joint. In five other patients who had normal thumbs but had lost all fingers, the technique was expanded to include the second toe with great toe wrap-around flap. All patients did well, but significant bone peg resorption occurred in six patients and in three others the bone peg fractured.

Osseous rupture of the ulnar lateral ligament of the thumb is a strain fracture;l hence, it is important for stability, but it is also equally important for the articular surface. Anatomic reposition and stable fixation are essential for best possible rehabilitation. In analogy to other strain fractures, traction belting is the therapy of choice. The present article reports on 12 cases.

This report on fractures and refractures in intercollegiate athletes covers an 11-year period from the early Fall of 1968 through the Spring of 1979. Two hundred thirty-one fractures occurred in 219 athletes. Of these athletes, 185 were male and 34 were female. Fractures occurred in 18 sports. Report about two cases of severe injuries to the thumb. One severe crush injury was treated primarily; in the other case of complete amputation a late reconstruction was performed. Methods and modern concepts for these problems are discussed. The importance of long vein grafts in microsurgery is pointed out. The advantage of reconstructing all structures primarily is stressed. Good functional results are presented to support these concepts.

Salivary calculi were formed by a laminated mixture of inorganic and organic substancs. However, there was no regular pattern for the growth of stones, as laminal thickness and the arrangement of the laminal component were not uniform. Finger-like radial projections, longitudinal pavements or amorphous structures were seen on the fractured surface of the calculi. No inert foreign body or microorganism was found in the calculi. The subject dealt with is the prognosis of residual axis deviations following finger fractures in the growing phase. By means of individual examples it is shown that the axis deviation in the frontal plane does not undergo any correction during further growth, whereas the axis deviation in the movement plane - the sagittal plane - can still be completely corrected even in cases of premature joins. Our results with the use of intraosseous wires for 72 complex, open fractures are reported. The technique is described, and attention to detail is emphasized. Patients with Colles' fractures have serious complications more frequently than is generally appreciated. A study of 565 fractures revealed 177 (31 per cent) with such complications as persistent neuropathies of the median, ulnar, or radial nerves (forty-five cases), radiocarpal or radio-ulnar arthrosis (thirty-seven cases), and malposition-malunion (thirty cases). Experience with 85 consecutive patients treated at Bellevue Hospital for hepatic trauma over the past two years has established the importance of several principles of management. Simple liver injuries can be treated by superficial suture and drainage. Using this approach in 57 patients there were no deaths and no postoperative abscesses. The closed as well as the operative treatment of fractures of the skeleton of the hand during growth must take into consideration the growth potential of the bones. It is particularly important in its potential to correct shortening as well as axial deviation and angulation (except rotational deformity). There are, however, limits to this potential as shown in epiphyseal fractures. Modern internal fixation has, therefore, an important -- though very restricted -- place in the treatment of these epiphyseal fractures. Other rare indications are mentioned.

One hundred cases of osteosynthesis done by the use of intraosseous wiring are presented.
