Efficacy of Pull-In Suture for Tendon Avulsion Injury Associated With Severe Forearm Open Fracture, Forearm Amputation, and Hand Amputation.

BACKGROUND: The pull-in suture technique is a method of primary tendon repair that involves pulling the distal stump of the ruptured tendon into the proximal muscle belly and suturing it to the intact muscle fascia for avulsion injuries at the musculotendinous junctions associated with severe open forearm trauma. The aim of this study is to report the clinical outcomes of the pull-in suture technique for musculotendinous avulsion injuries associated with severe open forearm trauma and to evaluate its effectiveness. METHODS: This study included patients who underwent pull-in suture repair for

From Devastation to Function: A Comprehensive Reconstructive Approach to High-Energy Hand Trauma with Multiple Amputated Digits.

This case report describes the successful multistage reconstruction of a severely mangled hand in a 15-year-old boy following a firecracker explosion. The injury resulted in multiple digit amputations, complex fractures, extensive soft tissue loss, and damage to critical anatomic structures, sparing only the little finger. Initial management focused on debridement, skeletal stabilization, and soft tissue coverage, followed by a carefully planned reconstructive pathway. Key surgical interventions included a medial plantar free flap for glabrous, sensate coverage of the thumb, a second toe trans

Attempted definitive revision amputations in emergency department vs operating room for traumatic finger injuries are associated with a high rate of revision surgery.

BACKGROUND: Revision amputation is a common treatment in the emergency department (ED) for traumatic finger injuries, yet there is limited data on outcomes for procedures completed in the emergency room versus the operating room. This study aims to assess outcome differences between ED revision amputation and delayed OR management. METHODS: 103 consecutive patients with traumatic finger(s) amputations were identified from a single tertiary care center. Patients were evaluated by the on-call hand team and staffed with a fellowship-trained hand attending. ED revision amputations were performed

Successful Salvage of a Mutilating Hand Injury With Multiple Digital Crush and Open Fractures: A Case Report.

Mutilating hand injuries involving multiple digits present significant reconstructive challenges, particularly in the setting of severe crush mechanisms and vascular compromise. We report the case of an 18-year-old right-hand dominant male manual laborer who sustained a devastating crush injury to his right hand after entrapment under a concrete block. The injury involved near-amputations of the index, middle, and ring fingers, with complete disruption of digital neurovascular bundles and associated open fractures. Given the non-viability of distal segments and extensive soft tissue damage, re

Clinical Outcomes and Complications of Intramedullary Proximal Phalanx Fixation.

BACKGROUND: Proximal phalanx fractures are common hand injuries that can result in significant functional impairment if inadequately treated. While multiple fixation strategies exist, the optimal approach remains debated. Intramedullary headless screw fixation has gained interest due to its minimally invasive nature and ability to allow early mobilization. This study evaluates clinical, radiographic, and functional outcomes following fixation with a single intramedullary device. METHODS: A retrospective review was performed of patients undergoing proximal phalanx fracture fixation with the Ex

Kirschner Wire Skeletal Fixation in the Hand and Wrist: Risk of Infection and Predictors of Progression to Major Complications.

PURPOSE: Kirschner wire (K-wire) infections continue to be a challenge in hand surgery. Our purpose was to quantify the frequency of K-wire site infections of the hand and wrist, to tabulate major and minor infectious manifestations, to identify factors that may predispose a patient to major complications, and to generate an updated algorithm for prognostication and treatment of K-wire site infections. METHODS: Patients treated with K-wires in the fingers, hand, and wrist who developed infections between 2018 and 2024 at our institution were identified. Information regarding patient factors,

A retrospective cohort study of distal radius fracture manipulation under different traction methods and different analgesic modalities and its impact on surgical rates.

PURPOSE: Distal radius fractures commonly present to emergency departments. Manipulation methods include manual traction (MT) and finger trap traction (FTT), combined with haematoma block, Bier’s block or entonox. Whilst previous studies assessed outcomes via radiological parameters, pain, and complications, none evaluated surgical rates as a measure of successful reduction. This study compares manipulation methods and their impact on surgical intervention rates. METHODS: Retrospective data were collected between December 2019 and December 2024. Inclusion criteria were patients aged 18–65 year

Pediatric Both-Bone Forearm Fracture with Flexor Digitorum Profundus Entrapment Causing Pseudo-Volkmann Contracture: A Case Report.

CASE: A 10-year-old boy presented with mechanical flexion contractures of the little, ring, and middle fingers after a both-bone forearm fracture surgery. Despite initial rehabilitation, he was unable to extend the fingers actively or passively in a neutral or extended wrist position, although extension was possible with wrist flexion, suggesting tendon entrapment. Surgical exploration revealed flexor digitorum profundus (FDP) entrapment at the radial fracture site. Surgical release provided immediate resolution of finger contractures. CONCLUSION: This case demonstrates that FDP entrapment ca

Complete Recovery of Iatrogenic Radial Nerve Palsy in a Child: A Case Report.

Supracondylar humerus fractures are among the most common paediatric injuries. Although neurologic complications are often seen in these fractures, iatrogenic nerve injuries following closed reduction and percutaneous pinning remain relatively uncommon. Radial nerve palsy is particularly rare in this context, and most cases represent neuropraxic injuries that recover spontaneously. This report describes a rare case of iatrogenic high radial nerve palsy following closed reduction and K-wire fixation of a comminuted supracondylar humerus fracture in a child, with complete functional recovery und

Surgically treated metacarpal fractures in adults: a study with 3286 cases based on the Swedish National Quality Registry for Hand Surgery.

BACKGROUND: No international consensus regarding optimal non-surgical or surgical treatment of metacarpal fractures exists. Surgical outcome using patient-reported outcome measurements (PROMs) from national registers with large surgically treated population have rarely been reported. We present demographics and patient-reported outcomes in relation to fixation methods for surgically treated metacarpal fractures, using data from the Swedish National Quality Registry for Hand Surgery (HAKIR). METHODS: In a retrospective analysis of a prospective collected dataset, patients aged ≥ 18 years with

Non-union rates and complications after elective interphalangeal joint arthrodesis of the hand: a cohort study of 366 arthrodeses in 289 patients.

BACKGROUND AND PURPOSE: Interphalangeal joint arthrodesis of the hand is a common procedure, but data on outcomes and complications is limited. We aimed to determine the non-union rate, evaluate risk factors for non-union, and investigate the frequency and distribution of complications across different fixation methods. METHODS: We retrospectively reviewed all interphalangeal joint arthrodeses performed between 2014 and 2022. Union and non-union were defined clinically and/or radiographically. Potential risk factors for non-union were evaluated by logistic regression with cluster-robust stand

Adult Case of Stüve-Wiedemann Syndrome.

Stüve-Wiedemann Syndrome (SWS) is a rare autosomal recessive skeletal dysplasia caused by variations in the leukemia inhibitory factor receptor (LIFR) gene. Due to its high rate of mortality in early life, current literature is largely limited to children with SWS. We present a case report of a 35-year-old female with SWS, one of the few documented adults with SWS. She was diagnosed at age 34 after clinical exome sequencing identified homozygous pathogenic variants in LIFR at c.756dup (p.Lys253*), a known pathogenic variant associated with SWS. Her past medical history consists of feeding and

Subcondylar Fossa Reconstruction in Pediatric Patients Following Malunion of Proximal Phalangeal Neck Fractures: Technique and Case Series.

The subcondylar fossa is the bony depression between the articulating surface of the proximal and middle phalanges of the fingers that allows for full flexion at the proximal interphalangeal joint. A bony block may form in the subcondylar fossa secondary to the malunion of a proximal phalangeal neck fracture. If there is a substantial loss of flexion, surgical correction could be indicated. Corrective osteotomy is technically challenging in the proximal interphalangeal joint area as the phalangeal head is small, creating fixation challenges and carrying the risk of avascular necrosis and joint

Forensic Evaluation of Multiple Gunshot Injuries: A Case Report.

Firearm injuries, particularly those caused by shotguns, present complex clinical and forensic challenges due to the variable nature of wound patterns and retained projectiles. Shotguns inflict varying injury patterns based on range and ammunition type, while high-velocity bullets cause extensive cavitation injuries, especially in dense or fluid-filled organs. Understanding firearm mechanisms, wound ballistics, and associated forensic evidence like gunpowder residue and blood patterns is essential for accurate forensic investigation, trauma management, and preventing long-term complications fr

More Amputations and Open Fractures: Pediatric Utility Task Vehicle (UTV) Injuries Are More Severe Than All-terrain Vehicle (ATV) Injuries.

BACKGROUND: Utility task vehicles (UTVs) differ from all-terrain vehicles (ATVs) in that they are heavier, have larger engines, and have rollover protective structures. Despite their growing popularity for recreational use, there is emerging evidence that mutilating injuries may be higher in UTVs than ATVs, but this has not been evaluated in a pediatric-only population. This study seeks to determine differences in injury characteristics between vehicle types. METHODS: A retrospective cohort study was performed at a level 1 pediatric trauma center, evaluating all patients <18 years old who pre

Closed reduction of multiple MCP joint dislocations.

We report a young man in his 20s who fell from the roof of his house and sustained injury to his left hand. Physical examination and radiographic evaluation revealed closed dislocations of the second, third, and fourth metacarpophalangeal (MCP) joint with fractures of the bases of the third and fourth metacarpals and the base of the proximal phalanx of the middle finger. The injury was managed with closed reduction and splintage with an uneventful outcome. Multiple MCP joint dislocations are rare, and there are only a few reports of closed reduction with a good outcome.

Staged Reconstruction and Salvage of a Small Finger With a Corticocancellous Distal Radius Autograft: A Case Report.

Finger reconstruction in the aftermath of severe trauma poses significant challenges in restorative surgery, particularly when addressing extensive soft tissue damage, comminuted fractures, and simultaneous neurovascular complications. This study presents a case of a patient with a complicated proximal phalangeal fracture who pursued digit salvage. A 31-year-old Hispanic woman sustained severe injury to her right fifth digit during an all-terrain vehicle (ATV) rollover resulting in an exposed, comminuted fracture of the proximal phalanx with extensive structural and neurovascular compromise. Outcome of Non-thumb Metacarpal Shaft Fractures Treated by Two Different Techniques - K Wire and Herbert Screw Fixation: A Comparative Single Centre Study.

INTRODUCTION: Metacarpal fractures are one of the most commonly encountered musculoskeletal injuries in the hand often caused by road traffic accidents, trivial fall or workplace injuries. It can lead to severe disability of the hand if left untreated or improperly treated. About 88% of all metacarpal fractures are non-thumb metacarpals, with the fifth finger being most frequently affected. Hence, achieving optimal function will be the primary goal, though it remains challenging. MATERIALS AND METHODS: A prospective study done at Government Thoothukudi Medical College for a period of 3 years
