[Criteria for amputation, reconstruction and replantation of extremities in multiple trauma patients].

The paper defines indications for amputation, reconstruction and replantation of severely injured extremities of multiple traumatised patients using the trauma score PTS (Hannover Polytrauma Score) and the fracture grading for open and closed fractures.

[Fractures of the distal end of the radius].

Attitudes to the treatment of distal radial fracture are now more critical than previously, especially where intraarticular fracture is concerned. Inadequate primary reduction and such complications as redislocation or dystrophic problems lead to poor results. Quantification of fracture instability is becoming increasingly important in fracture classification, determination of the indications for the different treatment modalities and management. Accurate anatomical reduction also improves later function. The quality of alignment attained during the treatment depends on the quality of reductio

Multiple flexor tendon ruptures following Colles' fracture: a case report.

Flexor tendon ruptures are a very rare complication of Colles' fracture. Only ten cases have been previously reported in the literature. We describe a case of multiple sequential flexor tendon rupture as a delayed consequence of a malunited Colles' fracture.

Bone injuries: treatment and rehabilitation.

Solid bony union with full range of painless motion is accomplished by correct early diagnosis, adequate reduction, stable fixation, prevention of edema, and institution of early motion programs. The degree of bony injury and involvement of multiple tissues will dictate the type of fixation and length of immobilization. Restoration of a stable skeleton with a functional range of joint motion must always be accomplished.

Rigid fixation of phalangeal and metacarpal fractures.

Rational use of rigid fixation in hand surgery requires awareness of the advantages as well as the potential pitfalls of this relatively complex method of fracture management. A rational decision also necessitates familiarity with closed techniques of internal fixation which, in most cases, provide superior alternatives for the phalanges and metacarpals. The optimal application of screws, plates, and tension-band wires to small bone fractures can be ascertained only by critical assessment of large series of cases employing these devices and comparisons with similar series using other technique

Geode development and multiple fractures in rheumatoid arthritis.

The radiological development from normal bone of geodes and subsequent fractures in phalanges of two adjacent fingers is described in a patient with classical rheumatoid arthritis. Presentation was as a septic, discharging focus, but infection was excluded; the pathology is described.

[Results of surgical treatment of fractures of the metacarpus and fingers].

Ninety-one patients with 108 fractures of the metacarpal and/or finger bones were treated by surgery between January 1974 and June 1982. 70 patients (76.9%) with 81 surgically treated fractures could be checked up after 3 3/4 years on an average. 24.3% out of these patients suffered from associated soft tissue leasions, 25.7% suffered from multiple fractures. 41 out of the 81 surgical interventions were performed for fractures of the metacarpus and 40 for fractures of the finger bones. The results were classified according to an evaluation scheme established for this purpose and containing the

[Injuries caused by accidents with chain saws].

Accident reports and medical statements (a total of 719) concerning chain-saw injuries were evaluated. Chain-saw wounds can be found in all parts of the body, especially the thighs and fingers. The typical chain-saw wound is a multiple superficial laceration, soft tissue wounds are usually heavily bruised and jagged , and bone lesions are rare and superficial. Among 188 finger injuries, only 15 slight bone lesions were seen. The frequency of all injuries decreases as one progresses from the thumb to the little finger and from the finger tip to the finger. The more proximal the isolated finger

The first infant died at 14 days of respiratory failure and the following findings characteristic of the syndrome: craniosynostosis of coronal and lambdoid sutures, brachycephaly, frontal bossing, severe midface hypoplasia with proptosis and choanal stenosis/atresia, humero-radial synostosis, medial bowing of ulnae, long slender fingers with camptodactyly, narrow iliac wings, anterior bowing of femora, cardiac and renal malformations. Acute osteolytic lesions following pancreatitis in a dialysis patient.

A 42-year-old man undergoing maintenance hemodialysis suffered an attack of acute pancreatitis. Convential treatment resulted in quick recovery. 10 days after its onset his hands and feet became swollen, hot, red and painful. Multiple intramedullary osteolytic lesions of the metatarsals, metacarpals and phalanges, with cortical destruction and a number of fractures were found. These lesions subsided over many weeks and did not recur. We believe that such acute osteolytic lesions following pancreatitis are not 'renal osteodystrophy' as such, but should be recognized as a possible complication i

Haemophiliac hands--a three year follow-up study.

The total absence of literature regarding the affects of haemophilia on the hand prompted this three year study at the Israel National Haemophilia Centre. The hands of all haemophiliacs were examined and X-rayed at the time of their annual check-up. Apart from a number of established Volkmann's contractures in elderly patients and multiple bone cysts of the carpus, surprisingly little pathology was found. When considering the extensive damage done to the musculoskeletal system by this haemarthritic pathogenesis it is astounding how the hand, with its multiple small joints, constant movement an

Intraosseous wiring of complex hand fractures.

Our results with the use of intraosseous wires for 72 complex, open fractures are reported. The technique is described, and attention to detail is emphasized. The technique is indicated especially for articular or comminuted fractures with multiple small fragments. It also provides solid, anatomic fixation for other types of fractures and for replants. A K-wire may be added if necessary and is usually used if there has been some bone loss. Complications have been few, most the result of displacement at the fracture site due to improper use of the technique. A major advantage of the solid fixat

Hand and wrist injuries in the athlete.

Injuries to the hand and wrist are probably among the most common injuries incurred by the athlete. Neglect of such injuries may result in irreparable damage to the hand or wrist. This summary of the more common injuries to these areas seen in the athlete, with a description of the recognition and treatment, gives the reader insight into the multiple considerations the athlete and coach must have. Even seemingly minor injuries must be carefully evaluated and treated by a physician. Early diagnosis, accurate precise treatment, and proper rehabilitation are extremely important to regain optimal

Multiple loose bodies in rheumatoid arthritis.

Intra-articular loose bodies are usually secondary to osteochondral fractures, osteochondritis dissecans, synovial osteochondromatosis, and degenerative arthritis. The authors report four patients with longstanding rheumatoid arthritis and multiple loose bodies in a variety of joints. All patients have remained active despite their disease, suggesting that the loose body production was secondary to trauma of continued activity superimposed on joints containing pannus and eroded cartilage.

Primary care of the injured hand, part 2.

Nerve injuries in the forearm and hand are common, especially with concomitant tendon lacerations. The optimal time for repair of a clean sharp laceration is within the first 24 to 48 hours using magnification to achieve precise surgical reapproximation. Transfer to a facility equipped to perform replantation is indicated for multiple digit amputations, any thumb amputation, transmetacarpal amputation, wrist and arm amputation, and major amputations in children. Acute paronychial infections are treated with warm soaks, antibiotics, elevation, and immobilization. Surgical drainage is often indi

Multiple volar carpometacarpal joint dislocation. Case report of traumatic volar dislocation of the medial four carpometacarpal joint in a child and review of the literature.

Traumatic dislocation of the medial four carpometacarpal joints in a child is an unusual injury. Descriptions of this injury in the literature have been reviewed and a case is presented to illustrate the pertinent anatomy, biomechanical aspects, and details of management. Specifically, the importance of collateral circulation to the hand and an effective technique of pin fixation are described.
