A unique case of exploded hand syndrome with hand ischemia and multiple phalangeal neck fractures of all fingers: Case report.

INTRODUCTION: Although exploded hand syndrome is well described in the literature, only a few cases have been reported. A sudden compressive force to the hand leads to extrusion of the intrinsic muscles through burst lacerations in the skin. This case report has been reported in line with the SCARE checklist. CASE REPORT: A 45-year-old male industrial worker sustained a crush injury to his right hand when a compressor plate hit the hand at the factory. Besides extrusion of the intrinsic muscles and multiple metacarpal fractures, two unique features were seen: fractures of the phalangeal necks

Enhanced Diagnostic Accuracy of Fast-Field Echo Resembling a Computed Tomography Using Restricted Echo-Spacing for Hand Tendon Injuries: A Comparative Study.

PURPOSE: Fast-field echo resembling a computed tomography using restricted echo-spacing (FRACTURE), a magnetic resonance imaging (MRI) sequence (Philips Healthcare) providing computed tomography-like bone contrast, enables excellent visualization of bones and tendons. Volume rendering (VR) images created from this sequence show anatomical details of a patient's actual structures. This study evaluated the diagnostic performance of FRACTURE and its VR images for visualizing wrist tendons on MRI images from patients and healthy individuals. METHODS: Twenty subjects were included, 10 patients wit

Restricted thumb extension due to flexor pollicis longus tendon adhesion following volar plate fixation with bone substitute use for distal radius fracture: A case report and literature review.

RATIONALE: Tendon adhesion is a relatively uncommon complication after distal radius fracture surgery. PATIENT CONCERNS: A 51-year-old woman underwent open reduction and volar plate fixation for a distal radius fracture at another hospital approximately 4 weeks ago. Upon presentation to our hospital, active flexion of the thumb interphalangeal (IP) joint was preserved, but active extension was absent; additionally, passive extension of the thumb IP joint could not be achieved by the examiner. DIAGNOSES: Plain radiography and computed tomography demonstrated that 1 distal screw of the volar p

Treatment of Metacarpal and Phalangeal Fractures: Intramedullary Screw Technique.

BACKGROUND: Metacarpal and phalangeal fractures are among the most common fractures that upper-extremity surgeons encounter, accounting for 30% of all hand fractures1,2. These particular fractures can be treated either operatively or nonoperatively, according to the amounts of displacement, malrotation, and shortening1. Operative treatment includes the use of Kirschner wire fixation, intramedullary screws, plate-and-screw constructs, or interfragmentary screws. Multiple studies have demonstrated superior biomechanical strength and early active range of motion with use of intramedullary screws

Efficacy of biomarkers in the endochondral phase of fracture repair and healing in long bones: A clinical observational studys.

BACKGROUND: Clinical determination of bone fracture healing remains qualitative, typically determined through the combination of plain film radiographs, clinical assessment, and patient-reported pain. Importantly, standard radiographs can only detect bone formation in the fracture site after sufficient tissue mineralization has occurred, restricting utility to the later stages of bone repair. A more rigorous method for determining fracture healing progression could significantly improve patient care. Quantitative biomarkers have gained diagnostic value in many clinical indications. Multiple bo

Revision of Multiple Revised Hinge Total Knee Replacement -a Rare Case Report.

INTRODUCTION: Enchondroma of the hand is a common lesion with a recurrence rate of up to 13.3% after curettage and bone grafting. When hand enchondroma is suspected, less common conditions, such as multiple enchondromatosis syndromes and benign and malignant lesions, should be ruled out. Pathologic fractures often occur. Post-operative complications are typically joint stiffness and soft-tissue-related deformities, whereas recurrence and malignant degeneration of solitary lesions are much less common. Most patients return to full function after surgery. CASE REPORT: We present a case of encho

Morbidity and Unplanned Secondary Surgery Following All-Inside Transosseous Flexor Tendon Repairs in Jersey Finger Injuries.

Background: We hypothesised that the complication rate is correlated with the technique and the type of suture in all-inside transosseous flexor digitorum profundus (FDP) tendon repairs. Methods: A retrospective review of our database identified zone 1 injuries of FDP or flexor pollicis longus (FPL) that were repaired using the transosseous technique. In this method, two limbs of the nonabsorbable sutures are passed from volar to dorsal via transosseous tunnels and tied just proximal to the germinal matrix over the proximal aspect of the distal phalanx (dorsal to the terminal band). Isolated Carpometacarpal Dislocation Without Associated Fracture: A Case Report of High-Energy Wrist Trauma.

Carpometacarpal (CMC) joint dislocations without fractures are rare and often missed due to subtle clinical and radiographic signs. We report a case of a 28-year-old male patient involved in a motorbike rollover who sustained open comminuted radius and ulna fractures, an ulnar styloid fracture, and dorsal dislocation of multiple CMC joints without metacarpal fractures. He presented with absent distal pulses and finger numbness, which resolved after emergency reduction. CT angiography ruled out arterial injury. He underwent internal fixation of forearm fractures and Kirschner wire stabilization

Early Postoperative Outcomes of Surgical Fixation of Proximal Phalanx Fractures With Intramedullary Nails Versus Kirschner Wires.

BACKGROUND: Kirschner wire (K-wire) and intramedullary (IM) screw fixation are accepted techniques for treatment of unstable proximal phalanx fractures, but comparative reports are lacking. This study aimed to evaluate early clinical outcomes following treatment with K-wire or IM fixation. METHODS: A retrospective review of all proximal phalanx fractures treated surgically at a single center by multiple surgeons was performed from May 1, 2019 to March 1, 2024. Inclusion criteria consisted of patients above 18 years of age with transverse or short-oblique proximal phalanx fractures, without as

Fix the phalanx: A meta-analysis comparing intramedullary screws, pinning, plates, and lag screws for closed extra-articular proximal phalanx fractures.

BACKGROUND: Proximal phalanx fractures are common hand injuries with multiple fixation options. This meta-analysis compared outcomes of closed extra-articular fractures treated with intramedullary screws (IMS), percutaneous pinning (PCP), plating, or lag screws. METHODS: A systematic review of MEDLINE, Embase, CINAHL, and Cochrane databases (January 2010-May 2025) was conducted according to PRISMA guidelines. Primary outcomes were complication and revision rates; secondary outcomes included immobilization duration, total active motion [TAM] and grip strength. Data were pooled using random- or

Replantation of multiple fingertip amputations using super microsurgery: A case report and literature review.

BACKGROUND: The fingertip amputation is an amputation type of the finger beyond the proximal nail fold. There is no vein available for anastomoses on the dorsal side of the finger, and the palmar vein of the finger is small and tightly attached to the skin. Therefore, it is relatively difficult to implement surgical anastomoses, which poses challenges to the clinical treatment of fingertip amputations. CASE REPORT: A 29-year-old male was admitted to the hospital due to "the amputation of the fingertips of the right index, middle, and ring fingers caused by a heavy object compression 3 h ago".

Functional and radiological outcomes of Bennett's fractures treated by Iselin's technique: About 29 cases with 6-year (2-14) follow-up.

INTRODUCTION: The percutaneous pinning method described by Iselin is one of the techniques used for treating Bennett fractures at the base of the thumb metacarpal. There is little published data on the medium- and long-term outcomes of this treatment, with most studies having a mean follow-up of 4 years. The primary objective of this study was to evaluate the functional and radiological outcomes of percutaneous pinning by Iselin's method with a mean follow-up of 6 years. The secondary objective was to look for evidence of radiographic carpometacarpal (CMC) arthritis. HYPOTHESES: 1) The functi

Successful Triple Flap Procedure for Thumb Reconstruction in Severe Hand Crush Injury.

BACKGROUND The thumb is crucial for the aesthetic and functional aspects of the upper extremity. A crushed thumb injury can be particularly challenging, especially for individuals with high demands. Currently, there is no consensus on the best approach for treating a crushed thumb. The scapular flap, vascularized by the subscapular artery system, shows great potential as a free flap option. This report aims to highlight the use of a triple flap procedure to preserve the function of the crushed thumb. CASE REPORT A 47-year-old man had a severe injury to his left hand, resulting in significant s

Trapeziometacarpal joint imaging: Normal high-resolution MRI, US and CT compared with cadaveric specimens and pathological imaging findings.

INTRODUCTION: The trapeziometacarpal (TMC) joint is a complex joint, whose anatomy and function are different from the metacarpophalangeal joints of the long fingers. The stability of this joint is ensured at three levels by multiple structures: osteochondral, capsulo-ligamentous, and musculo-tendinous. The anatomical and biomechanical structures ensuring the stability of the TMC joint are perfectly evaluated on magnetic resonance imaging (MRI), with a high degree of confidence. All described ligaments are anatomically visible and perfectly assessed on MRI and ultrasound (US): the dorsoradial

Single-Stage Press-Fit Osseointegration of the Radius and Ulna for Rehabilitation After Trans-Forearm Amputation.

BACKGROUND: Upper limb (UL) amputation is disabling. ULs are necessary for many domains of life1, and few effective motor and sensory replacements are accessible2. Approximately 41,000 people in the United States have UL amputation proximal to the fingers3, two-thirds of (all) traumatic amputations are UL4, and 80% of UL amputations are performed for trauma-related etiologies5. Socket prosthesis (SP) abandonment remains high because of the lack of sensation, limited prosthesis control, perceived weight, and difficulty comfortably wearing the SP6. Necrotizing Soft Tissue Infection Following a Closed Distal Radius Fracture: A Case Report and Literature Review.

Necrotizing soft tissue infections (NSTIs), characterized by extensive soft tissue destruction, are rare but life-threatening. We present a case of a NSTI in a healthy 65-year-old woman following a closed distal radius fracture. The patient presented with severe pain, fever, and lethargy 4 days after her index injury, with physical examination of the right upper limb revealing erythema and swelling to the mid-humeral level and blisters of the fingers and hand. Multiple surgical debridements were required to control the infection, which was caused by Streptococcus pyogenes. This case highlights

Proximal Interphalangeal Joint Fractures.

Multiple fracture patterns can occur around the proximal interphalangeal joint and require surgeons to have a thorough understanding of the anatomy, clinical and radiographic examination, common fracture patterns, surgical and nonsurgical treatment options, and potential complications. Proximal phalangeal condylar fractures are typically managed surgically, because even nondisplaced fractures have a propensity for displacement. Middle phalangeal base fractures most commonly present as a volar lip fracture with or without dorsal subluxation or dislocation. Treatment options include extension bl

CURRENT CONCEPTS IN THE MANAGEMENT OF BOXER'S FRACTURE.

Fractures of the metacarpal particularly the 5th metacarpal is quite common among all hand fractures and has a high incidence in male adult population. Proper management of these fractures plays a key role in rehabilitation and early return to work thus reducing the economic burden. Treatment of these injuries depends on the type of injury: whether it is a closed/open fracture, degree of angulation at the fracture site and also mal-rotation and shortening of the finger. Non-operative management is suitable for fractures which are closed, non-displaced and without angulation or rotation. Open f
