Predicting the Presence of Compressive Conditions After Distal Radius Fractures.

INTRODUCTION: Distal radius fractures are among the most common orthopedic injuries. Despite adequate fracture management, some patients experience delayed onset of compressive conditions, increasing the overall treatment burden. Identifying risk factors for these complications is important to improve patient outcomes. We sought to identify independent preoperative factors associated with the delayed development of compressive conditions following distal radius fracture management. METHODS: A retrospective review was conducted of patients with distal radius fractures over a decade. Data colle

Ultra High-Resolution Ultrasound Features of Carpal Tunnel Syndrome in Transthyretin Amyloidosis: A Cross-Sectional Study.

INTRODUCTION/AIMS: Transthyretin amyloidosis (ATTR), including hereditary (hATTR) and wild-type (wtATTR), often presents initially as carpal tunnel syndrome (CTS), often preceding systemic symptoms by several years. Ultra high-resolution ultrasound (UHRUS) offers detailed visualization of peripheral nerve morphology, but its application in ATTR-related CTS is unexplored. This study evaluated UHRUS features in ATTR-related CTS. METHODS: This cross-sectional study enrolled 20 individuals with hATTR or wtATTR confirmed by genetic testing, biopsy, or cardiac pyrophosphate scan. Clinical evaluatio

Carpal Tunnel Syndrome Caused by Parosteal Lipoma of Carpal Bones: A Case Report.

The aetiology of carpal tunnel syndrome is often idiopathic, and it is seldom caused by a space-occupying lesion. Herein, we report our experience with a case of parosteal lipoma causing carpal tunnel syndrome. A 66-year-old woman with a history of hyperthyroidism and hypertension presented with numbness and pain on the ulnar side of the left index finger without any triggers. A positive Tinel sign in the left carpal tunnel and presence of a space-occupying lesion with fat concentration extending from the carpal tunnel to the central subcutaneous palmar region on computed tomography led to the

Postoperative pain after carpal tunnel or trigger digit release using local anaesthesia with tourniquet or WALANT.

INTRODUCTION: Local anaesthesia with tourniquet (LA-T) and wide-awake local anaesthesia no tourniquet (WALANT) are both commonly used in hand surgery. However, the differences in postoperative pain between these methods in various procedures are unclear. METHODS: We conducted a retrospective single-surgeon case series of 356 procedures (184 LA-T; 172 WALANT) between 2015 and 2019. Included cases were unilateral carpal tunnel release and single-digit A1 pulley release. Postoperative pain was assessed on days 1-3 and at 2 weeks using patient-reported pain scores, sleep disturbance, hospital vis

Surgical Management of Wrist Arthritis Is Linked to Increased Carpal Tunnel Syndrome/Carpal Tunnel Release Risk: Rethinking Preoperative Evaluation.

PURPOSE: This study aimed to determine whether patients with wrist arthritis undergoing corrective surgeries, such as 4-corner fusion (4CF), proximal row carpectomy (PRC), wrist fusion, or total wrist arthroplasty, have a higher incidence of carpal tunnel syndrome (CTS) and subsequent carpal tunnel release (CTR) compared with those managed nonsurgically. METHODS: The PearlDiver database was used to identify two patient groups: the surgical group (patient with wrist arthritis who underwent corrective surgery) and the conservative group (patients with wrist arthritis who did not have surgery).

Wrist Splinting versus a Placebo Soft Bandage for Carpal Tunnel Syndrome.

BACKGROUND: Wrist splinting is a common treatment for carpal tunnel syndrome (CTS), but there is little evidence supporting its efficacy. METHODS: In this parallel-group clinical trial, patients with primary idiopathic CTS with symptoms for at least 1 month and no previous treatment were randomly assigned to wear either a rigid wrist splint or placebo soft bandage (allowed full wrist motion) for 6 weeks. If symptoms persisted after 6 weeks, treatment was continued for 4 more weeks. If symptoms persisted after 12 weeks, surgery was considered. Co-primary outcomes were change in the six-item CT

MRI Assessment of Median Nerve Size in Patients with Proximate Electrodiagnostic Studies.

BACKGROUND: Carpal tunnel syndrome (CTS) diagnosis has traditionally relied on electrodiagnosis (EDX) to confirm the diagnosis and to assess severity. Ultrasound has shown potential in measuring median nerve cross-sectional area (CSA) for CTS diagnosis, and magnetic resonance imaging (MRI) can be used for wrist soft-tissue evaluation. This study explored the correlation between CTS diagnosis and median nerve CSA measured on MRI at different wrist levels. METHODS: A retrospective review of an electronic medical record database identified patients who underwent both wrist MRI and EDX within a 9

From trauma to fibrosis: a case of ulnar nerve entrapment in Guyon’s canal

Guyon's canal syndrome is an uncommon distal ulnar neuropathy caused by compression within the wrist. We report a case of delayed ulnar nerve entrapment secondary to post-traumatic fibrosis, diagnosed nine years after wrist injury and surgery. High-resolution ultrasonography demonstrated focal nerve compression within Guyon's canal adjacent to fibrotic tissue. The patient presented with chronic sensory deficits and hypothenar atrophy. Conservative treatment led to partial symptom control. This case highlights delayed post-surgical fibrosis as a potential cause of Guyon's canal syndrome and und

The Risk of Acute Carpal Tunnel Syndrome in Lunate and Perilunate Dislocations.

PURPOSE: Lunate and perilunate dislocations are uncommon wrist injuries that have a high reported association with acute median neuropathy, frequently referred to as acute carpal tunnel syndrome (ACTS). Previous investigations into the incidence of lunate and perilunate dislocation and acute median neuropathy have often been limited by small sample sizes. The purpose of our study was to report a relatively large cohort examining the incidence of ACTS in lunate and perilunate dislocations and assess predictive risk factors. METHODS: A retrospective review was performed of all lunate and perilu

Secondary Carpal Tunnel Syndrome Due to Flexor Tenosynovitis in a Diabetic Patient.

Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy of the upper limb. Although many cases are idiopathic, structural or inflammatory pathology of the flexor tendon sheath can result in secondary median nerve compression. Diabetes mellitus predisposes to such changes due to chronic connective tissue alterations, but secondary CTS arising solely from noninfective flexor tenosynovitis remains uncommon. A 49-year-old woman with recently diagnosed type 2 diabetes presented with a 2-month history of progressive right-wrist pain, paresthesia, and volar swelling. Magnetic resonance
