Splicing Alterations Associated with Multiple Homozygous TNXB Variants in a Patient with Suspected Classical-Like Ehlers-Danlos Syndrome.

Ehlers-Danlos syndrome (EDS) comprises a heterogeneous group of heritable connective tissue disorders characterized by joint hypermobility, skin hyperextensibility, and tissue fragility. Classical-like EDS (clEDS) is typically associated with pathogenic biallelic variants in TNXB. The patient showed generalized joint hypermobility involving the knees, fingers, elbows, and feet, along with soft, pliable skin. She had a history of three dislocations of the right elbow and one dislocation of the left elbow. She also reported orthostatic dizziness and chronic constipation, suggesting possible auto

Single-Stage Hybrid Fixation of a Grade III Open, Unstable Trimalleolar Ankle Fracture with Syndesmotic Disruption and Posterior Talar Subluxation.

INTRODUCTION: Medial epicondyle fracture dislocations are common in the pediatric population; however, they often lead to favorable patient outcomes. Rarely, the median nerve (MN) can be trapped within the joint space or fracture upon reduction. If the nerve is trapped within the fracture, the MN slowly gets compressed as the fracture heals, leading to progressive deficits in motor function and sensation. These symptoms present as a mixture of pain, numbness, weakness, and paresthesia in the distribution of the MN. Therefore, careful monitoring of the patient post-reduction is crucial in makin

Closed reduction of multiple MCP joint dislocations.

We report a young man in his 20s who fell from the roof of his house and sustained injury to his left hand. Physical examination and radiographic evaluation revealed closed dislocations of the second, third, and fourth metacarpophalangeal (MCP) joint with fractures of the bases of the third and fourth metacarpals and the base of the proximal phalanx of the middle finger. The injury was managed with closed reduction and splintage with an uneventful outcome. Multiple MCP joint dislocations are rare, and there are only a few reports of closed reduction with a good outcome.

Osteochondral autograft from the second toe for complex proximal interphalangeal joint fracture-dislocations: a case report and literature review.

BACKGROUND: Managing complex fracture-dislocations of the proximal interphalangeal (PIP) joint poses significant clinical challenges, necessitating innovative treatment strategies. CASE DESCRIPTION: This case report highlights the successful treatment of a severe PIP joint fracture-dislocation in a 49-year-old male carpenter who sustained a traumatic laceration to the right index finger. The injury's complexity led to the pioneering use of an osteochondral autograft from the patient's second toe proximal phalanx, marking a novel approach in autologous tissue utilization for joint reconstructi

Complex Dorsal Dislocation of the Metacarpophalangeal Joint of the Fifth Finger Treated by Open Reduction With Volar Approach: A Case Report.

Complex dorsal dislocations of the metacarpophalangeal (MCP) joint are rare, with involvement of the fifth finger being exceedingly uncommon. These injuries typically result from hyperextension forces that entrap the volar plate, precluding successful closed reduction and necessitating surgical intervention. In this report, we present the case of a 20-year-old male patient who sustained multiple injuries, including craniofacial trauma and an open dorsal dislocation of the left fifth MCP joint, following a road traffic accident. Initial attempts at closed reduction under local anesthesia were u

Use of skeletal and soft tissue imaging to guide the surgical repair of misclassified Wassel type VI thumb duplication with convergent deformity.

Surgical treatment of Wassel type Ⅵ radial polydactyly with convergent deformity is technically challenging. We found that 17 of our cases exhibited cartilaginous connections between the bases of the radial and ulnar thumb's metacarpals. To better assess the anatomic features and guide surgical design, we used X-ray and MRI imaging to characterize the adduction of the ulnar metacarpal, radial subluxation of the metacarpophalangeal joint, narrowness of the first web, and thenar muscle distribution. Ulnar deviation of the metacarpal bases was 36.18±11.27˚, metacarpophalangeal joint radial deviat

Isolated Carpometacarpal Dislocation Without Associated Fracture: A Case Report of High-Energy Wrist Trauma.

Carpometacarpal (CMC) joint dislocations without fractures are rare and often missed due to subtle clinical and radiographic signs. We report a case of a 28-year-old male patient involved in a motorbike rollover who sustained open comminuted radius and ulna fractures, an ulnar styloid fracture, and dorsal dislocation of multiple CMC joints without metacarpal fractures. He presented with absent distal pulses and finger numbness, which resolved after emergency reduction. CT angiography ruled out arterial injury. He underwent internal fixation of forearm fractures and Kirschner wire stabilization

A Novel Technique for Collateral Ligament Reconstruction at the Proximal Interphalangeal Joint.

Chronic proximal interphalangeal (PIP) joint instability is a rare and difficult problem to treat, with the main options including collateral ligament reconstruction and joint arthrodesis. To date, there are minimal described techniques for collateral ligament reconstruction. We describe a novel technique utilizing a slip of the flexor digitorum superficialis (FDS), harvested through a single minimal incision. This technique can be used for collateral ligament reconstruction with multiple applications, including chronic recurrent PIP joint dislocations and instability during PIP joint arthropl

Early Postoperative Outcomes of Surgical Fixation of Proximal Phalanx Fractures With Intramedullary Nails Versus Kirschner Wires.

BACKGROUND: Kirschner wire (K-wire) and intramedullary (IM) screw fixation are accepted techniques for treatment of unstable proximal phalanx fractures, but comparative reports are lacking. This study aimed to evaluate early clinical outcomes following treatment with K-wire or IM fixation. METHODS: A retrospective review of all proximal phalanx fractures treated surgically at a single center by multiple surgeons was performed from May 1, 2019 to March 1, 2024. Inclusion criteria consisted of patients above 18 years of age with transverse or short-oblique proximal phalanx fractures, without as

First Clinical Evaluation of a Modular Lumboperitoneal Shunt System With a Gravitational Unit for Communicating Hydrocephalus: A Retrospective Case Series.

BACKGROUND AND OBJECTIVES: In Europe and North America, communicating hydrocephalus (HC) is commonly treated with ventriculoperitoneal shunts, whereas lumboperitoneal shunts (LPS) are preferred in Japan and other Asian countries. However, no LPS valve with an integrated gravitational unit had been available until recently, leaving overdrainage as a persistent complication. The objective of this study was to report initial clinical experiences with a novel modular LPS valve system that incorporates a gravitational unit and to assess its safety, feasibility, and early clinical outcomes across va

Trapeziometacarpal joint imaging: Normal high-resolution MRI, US and CT compared with cadaveric specimens and pathological imaging findings.

INTRODUCTION: The trapeziometacarpal (TMC) joint is a complex joint, whose anatomy and function are different from the metacarpophalangeal joints of the long fingers. The stability of this joint is ensured at three levels by multiple structures: osteochondral, capsulo-ligamentous, and musculo-tendinous. The anatomical and biomechanical structures ensuring the stability of the TMC joint are perfectly evaluated on magnetic resonance imaging (MRI), with a high degree of confidence. All described ligaments are anatomically visible and perfectly assessed on MRI and ultrasound (US): the dorsoradial

Individual Joint Distractions Using Ilizarov Mini-Fixator Device for Chronic Simultaneous Dislocation of the Distal and Proximal Interphalangeal Joints in the Same Finger.

Simultaneous dislocation of the distal and proximal interphalangeal joints is an uncommon injury. Treating chronic cases proves challenging due to severe contractures affecting both joints and surrounding soft tissues. Here, we report an unusual chronic contracture case in a 46-year-old man who suffered a dorsal dislocation of the distal and proximal interphalangeal joints of the little finger. The patient underwent primary joint distraction followed by open reduction, leading to stable radiographic restoration and a functional range of motion in the affected finger 6 months postoperatively. Bilateral Hemi-Hamate Autograft for Two Proximal Interphalangeal Joint Fracture Dislocations: A Case Report.

A 27-year-old man sustained right ring and little finger proximal interphalangeal joint fracture dislocations. He subsequently underwent bilateral hemi-hamate autograft to treat these intra-articular injuries. The patient was followed periodically and demonstrated return of normal grip strength and radiographic union of the hemi-hamate reconstruction arthroplasties. This is the first reported case of a patient who underwent bilateral hamates as donor grafts for two proximal interphalangeal joint fracture dislocations. When patients present with multiple unrepairable proximal interphalangeal jo

Proximal Interphalangeal Joint Fractures.

Multiple fracture patterns can occur around the proximal interphalangeal joint and require surgeons to have a thorough understanding of the anatomy, clinical and radiographic examination, common fracture patterns, surgical and nonsurgical treatment options, and potential complications. Proximal phalangeal condylar fractures are typically managed surgically, because even nondisplaced fractures have a propensity for displacement. Middle phalangeal base fractures most commonly present as a volar lip fracture with or without dorsal subluxation or dislocation. Treatment options include extension bl

Ensemble detection of hand joint ankylosis and subluxation in radiographic images using deep neural networks.

The modified total Sharp score (mTSS) is often used as an evaluation index for joint destruction caused by rheumatoid arthritis. In this study, special findings (ankylosis, subluxation, and dislocation) are detected to estimate the efficacy of mTSS by using deep neural networks (DNNs). The proposed method detects and classifies finger joint regions using an ensemble mechanism. This integrates multiple DNN detection models, specifically single shot multibox detectors, using different training data for each special finding. For the learning phase, we prepared a total of 260 hand X-ray images, in

Case report: A complete lower cervical fracture dislocation without permanent neurological impairment.

BACKGROUND: Complete fractures and dislocations of the lower cervical spine are usually associated with severe spinal cord injury. However, a very small number of patients do not have severe spinal cord injury symptoms, patients with normal muscle strength or only partial nerve root symptoms, known as "lucky fracture dislocation". The diagnosis and treatment of such patients is very difficult. Recently, we successfully treated one such patient. CASE PRESENTATION: A 73-year-old male patient had multiple neck and body aches after trauma, but there was sensory movement in his limbs. However, pre

Complex Metacarpophalangeal Joint Dislocation (Kaplan's Lesion) of the Index Finger in a 5-year-old Patient: A Case Report.

INTRODUCTION: This report highlights a rare case of complex metacarpophalangeal (MCP) joint dislocation, commonly referred to as Kaplan's lesion. This type of dislocation typically results from a hyperextension injury and requires surgical intervention as closed maneuvers are unsuccessful in reducing the dislocation. Surgical reduction for complex MCP joint dislocations is commonly done through classic volar or dorsal approaches. This report presents a case of a complex MCP joint dislocation and the surgical approach taken to address the issue. CASE REPORT: We present a case of a complex MCP

Dorsal and Volar Approaches for Proximal Interphalangeal Joint Replacement: Comparing Outcomes Through Systematic Review and Meta-Analysis.

BACKGROUND: The 2 primary surgical approaches for proximal interphalangeal joint (PIPJ) arthroplasty, dorsal or volar, have been extensively described in the literature. However, the ongoing debate regarding which approach offers superior results or is associated with fewer complications persists. This systematic review aims to compare the outcomes of PIPJ arthroplasty between the dorsal and volar approaches. METHODS: A comprehensive search of multiple databases was conducted, and studies meeting predetermined criteria were included. Data extraction, assessment of bias risk, and statistical a
