[Fractures of the Triquetrum - a Frequent Cause of Post-Traumatic Ulnar-Sided Wrist Pain].

Fractures of the triquetrum are the second most common carpal fractures after scaphoid fractures and represent a frequent cause of post-traumatic ulnar-sided wrist pain. Dorsal shear fractures account for approximately 93% of all triquetral fractures and are therefore by far the most common fracture type. Two pathomechanisms have been proposed: avulsion by the dorsal ligamentous attachments to the triquetrum and impaction of the dorsal cortex by a prominent ulnar styloid. The dorsal shear/ avulsion fracture is commonly associate with a localized tenderness to palpation over the dorsal aspect o

Double Trouble: Malunited Distal Radius Fracture and Scapholunate Ligament Injury.

Background  Combinations of distal radius fractures with scapholunate ligament (SL) injuries are hard to diagnose, and amalunited fractures with ligament damage are rarely discussed. Materials and Methods  Here is presented a case of a 40-year-old man with history of a malunited distal radius fracture and a scapholunate rupture with a time difference of over a decade between assumed traumatic incidents. We present the results of a simultaneous treatment of both conditions with corrective osteotomy and osteosynthesis of the distal radius combined with SL tenodesis. Results  At 2 years after the

Total Wrist Arthrodesis: An Update on Indications, Technique and Outcomes.

Painful end-stage wrist osteoarthritis (OA) unresponsive to conservative treatment is frequently managed with total wrist arthrodesis (TWA), which might render pain alleviation and ameliorate function, pain, and grip strength. Usual indications for TWA include inflammatory arthritis, idiopathic degenerative OA and posttraumatic OA, Kienböck's illness, brachial plexus palsy, cerebral paralysis, infraclavicular brachial plexus blocks and other spastic and contracture base illnesses, scapholunate advanced collapse, scaphoid nonunion advanced collapse, and failure of other surgical techniques such

Chronic Isolated Dorsal Dislocation of the Lunate. A Rare Presentation of Carpal Instability.

UNLABELLED: Isolated dorsal lunate dislocation is a rare injury. Only one case has been reported previously in which the treatment was performed in the chronic stage. In this report, we present the case of a 49-year-old handworker male who presented a dorsal dislocation of the lunate after a traffic accident. He was referred to our clinic 2.5 months later due to an initial misdiagnosis. Surgical treatment was performed and consisted of an open reduction using a nerve-sparing dorsal approach. A complete rupture of the perilunate ligaments and a marked instability of the lunate were detected. St

What Is the Diagnostic Accuracy of Flat-panel Cone-beam CT Arthrography for Diagnosis of Scapholunate Ligament Tears?

BACKGROUND: Cone beam CT (CBCT) is a widely available technique with possible indications in carpal ligament injuries. The accuracy of CBCT arthrography in diagnosing traumatic tears of the scapholunate ligament has not been reported. QUESTIONS/PURPOSES: (1) What is the diagnostic accuracy of CBCT and how does it relate to the accuracy of multislice CT arthrography and conventional arthrography in diagnosing scapholunate ligament tears? (2) What is the estimated magnitude of skin radiation doses of each method? METHODS: This secondary analysis of a previous prospective study included 71 men

Arthroscopic proximal carpal row replacement by semitendinosus and gracilis graft (CArpus Row Plasty Using the Semitendinosus: CARPUS procedure). An anatomical study of 16 cases.

INTRODUCTION: Post-traumatic lesions of the carpus (scapholunate rupture, scaphoid non-union) frequently evolve into disabling osteoarthritis (scapholunate advanced collapse or scaphoid non-union advanced collapse: SLAC or SNAC wrist). Proximal row carpectomy (PRC) is a reliable option but with poorer prognosis in case of osteoarthritis of the distal radius or capitate head. In such situations, radiocarpal arthrodesis may be necessary, sacrificing wrist motion. To circumvent this limitation, we propose a new procedure consisting in arthroscopic PRC and replacing the proximal row by tendon graf

Stabilization for acute distal radioulnar instability: A novel surgical technique.

INTRODUCTION: Instability of the distal radioulnar joint (DRUJ) commonly results from traumatic disruption of the distal radioulnar ligaments of the triangular fibrocartilage complex (TFCC). Treatment of this rupture typically requires immobilization of the wrist and elbow for a period of 6 to 8 weeks. This study evaluated the hypothesis that treatment of DRUJ instability with dynamic stabilization would allow early mobilization of both the radiocarpal and distal radioulnar joints by the first postoperative week without compromising restoration of TFCC integrity. MATERIALS AND METHODS: Betwee

Elbow Common Flexor Tendon Repair Technique.

Medial epicondylitis, also known as "golfer's elbow," is a common orthopaedic condition that typically results from overuse of the flexor pronator mass. Repetitive eccentric loading of the muscles responsible for wrist flexion and forearm pronation leads to microtrauma and subsequent degeneration of the flexor pronator tendon. Patients with medial epicondylitis typically present in the fourth to sixth decade of life and have an insidious onset of medial elbow pain. Occasionally, medial epicondylitis may result from an acute traumatic event, such as an acute avulsion of the common flexor tendon

[Suture techniques for TFCC].

Traumatic ruptures of the triangular fibrocartilaginous complex (TFCC) may cause ulnar-sided wrist pain. These ruptures frequently lead to instability of the distal radioulnar joint (DRUJ) because the ulnoradial ligaments, which are interwoven with the TFCC, are avulsed from the fovea of the ulnar head or-very rarely-from the radius. Conservative treatment may be sufficient, and some instability may be tolerated over time. In the case of persistent pain, operative repair by reattachment of the TFCC and, thereby, the ligaments to the previously debrided bone in the fovea is effective in reestab

Braces and Splints for Common Musculoskeletal Conditions.

Braces and splints can immobilize and protect joints, reduce pain, decrease swelling, and facilitate healing of acute injuries. They are also used for injury prevention and chronic pain reduction, and to alter the function of a joint. The medial unloading (valgus) knee brace is an option for patients with medial knee osteoarthritis, but evidence of long-term benefit is limited. The patellar stabilizing brace helps maintain proper patellar alignment but has mixed results in treating patellofemoral pain syndrome. The patellar tendon strap is effective in treating pain from patellar tendinopathy.

Accuracy of simple plain radiographic signs and measures to diagnose acute scapholunate ligament injuries of the wrist.

OBJECTIVE: To determine the accuracy of common radiological indices for diagnosing ruptures of the scapholunate (SL) ligament, the most relevant soft tissue injury of the wrist. METHODS: This was a prospective diagnostic accuracy study with independent verification of index test findings by a reference standard (wrist arthroscopy). Bilateral digital radiographs in posteroanterior (pa), lateral and Stecher's projection were evaluated by two independent expert readers. Diagnostic accuracy of radiological signs was expressed as sensitivity, specificity, positive (PPV) and negative (NPV) predicti

Arthroscopic treatment for ulnar-sided TFCC-tears.

OBJECTIVE: Ruptures of ulnar-sided triangular fibrocartilaginous complex (TFCC) often occur in cases of trauma. Golden standard for diagnosis is the arthroscopy of the wrist. TFCC lesions are classified according to their location if traumatic in origin or if degenerative according to their severity. MATERIALS AND METHODS: Recent literature has focused on the ruptures of ulnar-sided triangular fibrocartilaginous complex. This article describes conservative, operative and arthroscopic surgical techniques to reconstruct the triangular fibrocartilaginous complex and restore distal radioulnar joi

Fluoroquinolone-associated bilateral patellar tendon rupture: a case report and review of the literature.

Single series case reports have described bilateral traumatic patellar tendon ruptures, and less frequently, with little or no trauma. Patellar tendon rupture most often occurs in patients younger than 40 years old and is commonly precipitated by a sudden, significant eccentric contraction. Patellar tendon ruptures are uncommon in those older than 40 years of age, but when they occur, may indicate an underlying systemic disorder. Corticosteroid injections, rheumatic disease, metabolic disorders, and fluoroquinolone use have all been associated with increased risk of tendon rupture. While the A

[Arthroscopic refixation of traumatic 1B-lesions of the TFCC--a retrospective study].

AIM: The aim of the study was to investigate the postoperative outcome of the arthroscopic sutures of traumatic 1B-lesions of triangular fibrocartilage complex (TFCC) tears. METHODS: Thirty-six patients who underwent an arthroscopic repair of traumatic 1B-lesions of the TFCC without a dislocation of the ulnar styloid process were included in this study. The average age of the 18 male and 18 female patients was 43.8 (range: 22-70) years. In 16 cases, we operated the left and in 20 cases the right TFCC. On average, the follow-up examination was performed after 7.9 years (range: 3.2-14.3) years.

[Bulge of the scapholunate ligament: an arthro-CT sign of traumatic scapholunate instability].

Tears of the scapholunate ligament are usually diagnosed during an arthro-CT or arthro-MRI examination. Typically, the contrast agent passes from one joint to the other. In certain cases, fibrous scar tissue devoid of any mechanical effect may block the passage and lead to an erroneous interpretation of the images. We reviewed retrospectively patients who underwent arthroscopic treatment for scapholunate tears and who had normal arthro-CT images. We searched for specific aspects suggestive of ligament injury. Analysis of the arthro-CT images demonstrated that a bulge of the scapholunate ligame

[Trauma to the triangular fibrocartilaginous complex (TFCC)].

PURPOSE OF THE STUDY: The triangular fibrocartilaginous complex (TFCC) can be injured either due to trauma or by chronic strain. Based on these findings, Palmer devised a classification system distinguishing traumatic (I) and degenerative (II) TFCC lesions. Traumatic TFCC injury may be single or involved in a combined injury of the distal radius. The aim of this study was to evaluate the results of surgical treatment in patients with traumatic TFCC injury at six-month follow-up. MATERIAL AND METHODS: In the years 2000 to 2004, 23 patients with injury to the wrist were treated. The group compr

Diagnostic accuracy of plain radiographs and cineradiography in diagnosing traumatic scapholunate dissociation.

OBJECTIVE: Missed tears of the scapholunate ligament (SLL) and scapholunate dissociation (SLD) after wrist injuries pose a high risk of post-traumatic osteoarthritis of the carpus. Plain X-rays and dynamic radiographic studies are frequently used for initial diagnostic work-up. Given the limited evidence of their accuracy, we retrospectively compared the radiographic findings of patients with suspected traumatic SLD with wrist arthroscopy as the accepted reference standard. DESIGN AND METHODS: During a 6-year period, plain radiographs and DSA cineradiography scans were obtained from 198 patie

Three-ligament tenodesis for the treatment of scapholunate dissociation: indications and surgical technique.

Different surgical techniques have been proposed to treat traumatic scapholunate instability. Deciding which treatment is best for each individual case is not easy. In this article we report an algorithm of treatment based on a number of prognostic factors that may help in this matter. We also report on the promising results obtained using a new technique, the 3-ligament tenodesis, for the treatment of nonrepairable complete scapholunate ligament rupture, causing a reducible carpal malalignment without secondary osteoarthritis. This technique incorporates features from 3 previously described t
