From trauma to fibrosis: a case of ulnar nerve entrapment in Guyon’s canal

Guyon's canal syndrome is an uncommon distal ulnar neuropathy caused by compression within the wrist. We report a case of delayed ulnar nerve entrapment secondary to post-traumatic fibrosis, diagnosed nine years after wrist injury and surgery. High-resolution ultrasonography demonstrated focal nerve compression within Guyon's canal adjacent to fibrotic tissue. The patient presented with chronic sensory deficits and hypothenar atrophy. Conservative treatment led to partial symptom control. This case highlights delayed post-surgical fibrosis as a potential cause of Guyon's canal syndrome and und

The Risk of Acute Carpal Tunnel Syndrome in Lunate and Perilunate Dislocations.

PURPOSE: Lunate and perilunate dislocations are uncommon wrist injuries that have a high reported association with acute median neuropathy, frequently referred to as acute carpal tunnel syndrome (ACTS). Previous investigations into the incidence of lunate and perilunate dislocation and acute median neuropathy have often been limited by small sample sizes. The purpose of our study was to report a relatively large cohort examining the incidence of ACTS in lunate and perilunate dislocations and assess predictive risk factors. METHODS: A retrospective review was performed of all lunate and perilu

Secondary Carpal Tunnel Syndrome Due to Flexor Tenosynovitis in a Diabetic Patient.

Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy of the upper limb. Although many cases are idiopathic, structural or inflammatory pathology of the flexor tendon sheath can result in secondary median nerve compression. Diabetes mellitus predisposes to such changes due to chronic connective tissue alterations, but secondary CTS arising solely from noninfective flexor tenosynovitis remains uncommon. A 49-year-old woman with recently diagnosed type 2 diabetes presented with a 2-month history of progressive right-wrist pain, paresthesia, and volar swelling. Magnetic resonance

Evaluating median nerve stiffness in patients with mild to moderate-severe idiopathic carpal tunnel syndrome receiving OMT and conservative therapy: a pilot study.

CONTEXT: Osteopathic manipulative treatment (OMT) has been recognized as a conservative management option for patients with carpal tunnel syndrome (CTS), although limited research exists to validate its ability to effect posttreatment changes in the median nerve or the surrounding soft tissues. OBJECTIVES: The objectives of this study are to evaluate and quantify changes in the elasticity of the median nerve, transverse carpal ligament (TCL), and intracarpal tunnel soft tissues in patients treated for CTS with traditional conservative therapy (e.g., steroid injection and splinting), OMT, or O

Ulnar tunnel syndrome caused by two anomalous muscle bellies: a rare etiology of dynamic ulnar nerve compression.

Ulnar tunnel syndrome, or Guyon's canal syndrome, is an uncommon compressive neuropathy of the ulnar nerve at the wrist. While most cases result from trauma or ganglion cysts, anomalous muscles are rare. This case describes an ulnar tunnel syndrome caused by two anomalous muscle bellies crossing the ulnar neurovascular bundle in a 64-year-old man with recurrent ulnar neuropathy and intrinsic hand atrophy. Surgical excision and decompression resulted in durable symptom resolution at 5-year follow-up. This case highlights the importance of considering anatomical variations in persistent or recur

The role of TWEAK/Fn14 Axis in Carpal tunnel syndrome and evaluation of its relationship with electrophysiological stage.

BACKGROUND: Carpal tunnel syndrome (CTS) is an entrapment neuropathy resulting from compression of the median nerve at the wrist level. The aim of the study was to investigate the efficacy of tumor necrosis factor-like weak apoptosis inducer (TWEAK) and its receptor, inducible fibroblast growth factor 14 (Fn14) axis in CTS and to conduct a preliminary study on the usability of TWEAK/Fn14 as a new pharmacological treatment target in CTS. METHODS: A total of 93 participants were included in the study: 48 individuals in the CTS group and 45 individuals in the control group. CTS patients were ele

Anterior interosseous to ulnar nerve transfer with an ultrasound-guided short-incision.

INTRODUCTION: The anterior interosseous nerve (AIN) to ulnar nerve transfer is an established procedure for restoring intrinsic hand function in high ulnar nerve injury and severe cubital tunnel syndrome. Conventional techniques require extensive dissection along the distal forearm and across the wrist flexion crease, which may increase wound complications and delay recovery. METHODS: A modified ultrasound-guided, short-incision technique is described. Preoperative ultrasound is used to localize the AIN and the dorsal sensory branch of the ulnar nerve, enabling a targeted 4-cm incision proxim

Vibration dose and carpal tunnel syndrome in sandstone quarry workers: A cross-sectional study from Thailand.

Carpal tunnel syndrome (CTS) is common in vibration-exposed occupations, but evidence from informal sandstone workers is limited. This cross-sectional study examined hand-wrist symptoms and the association between cumulative hand-arm vibration exposure and CTS severity among 92 sandstone workers. Data were collected through structured interviews and ergonomic observation, and CTS severity was assessed using the Boston Carpal Tunnel Questionnaire-Symptom Severity Scale (BCTQ-SSS). CTS (mild or greater severity) was observed in 58.7% of workers. Compared with the lowest exposure quartile (Q1), w

Surgical outcome for type I Guyon's canal syndrome with claw hand deformity: a review of 11 patients.

BACKGROUND: Guyon's canal syndrome (GCS) is an uncommon compressive ulnar neuropathy at the wrist, and claw hand deformity could be seen in severe Tpye I cases. Data on whether claw hand deformity could be reversed by surgical treatment is lacking, and we aimed to evaluate the prognosis of these patients. METHODS: 11 patients with claw hand deformity were enrolled in this retrospective study, and the minimum follow-up was set at 1 year. All patients underwent exploration of the ulnar nerve, decompression of Guyon's canal, and removal of potentially concomitant space-occupying lesions. Clinica

Ultrasonographic parameters comparison for pre- and postoperative carpal tunnel syndrome release: A single-center prospective study.

Carpal tunnel syndrome (CTS) is a prevalent condition characterized by median nerve compression, impacting a significant portion of the population worldwide. While neurophysiological testing has been traditionally employed for diagnosis, recent advancements in ultrasonography have shown promise in offering a noninvasive alternative. In this study, we aimed to evaluate the mid-term postoperative statistical changes of the median nerve and flexor retinaculum, share our ultrasonographic measurement methodologies for standardized reporting, and suggest a set of ultrasonographic parameters for post

Repeated Measurements of Ultrasound Median Nerve Cross-Sectional Area Are Associated with Symptomatic and Functional Improvement after Carpal Tunnel Release.

Background: The potential for ultrasound (US) to be used as an objective measurement of improvement after carpal tunnel release is poorly understood. The purpose of this study is to evaluate whether measurements of US cross-sectional area (CSA) of the median nerve in the postoperative recovery period are associated with improvement of Boston Carpal Tunnel Questionnaire (BCTQ) scores for the Symptom Severity Scale (SSS) and Functional Severity Scale (FSS). Methods: A total of 167 wrists from 137 patients who underwent CTR with availability of US and BCTQ at 2-weeks, 6-weeks, 6-months and 1-year

Bilateral wrist masses as first manifestation of wild-type transthyretin amyloidosis in a case with recurrent carpal tunnel syndrome: a case report.

BACKGROUND: Carpal tunnel syndrome (CTS) is the most common peripheral neuropathy in the general population. Recurrent bilateral CTS could be the first manifestation of a more serious systemic disease, such as amyloidosis, which would help with early diagnosis and prevent mortality and morbidity associated with this illness. CASE PRESENTATION: We present a rare case of a 75-year-old Middle Eastern man suffering from bilateral wrist edema with weakness and numbness of both hands and a history of recurrent bilateral CTS. The bilateral wrist masses were revealed at the MRI investigation. He was

A multimodal imaging approach to predict carpal tunnel syndrome after distal radius fractures.

OBJECTIVE: Distal radius fractures are commonly associated with the development of carpal tunnel syndrome (CTS), yet current evidence primarily focuses on radiographic alignment parameters. The role of carpal tunnel morphology, particularly total carpal tunnel cross-sectional area (CSA) measured on computed tomography (CT), remains insufficiently investigated. This study aimed to evaluate the relationship between CSA, volar tilt, and the development of CTS in conservatively managed distal radius fractures, and to assess the predictive performance of these parameters when used in combination.

Carpal Tunnel Release: Technical Considerations for the Orthopaedic Trauma Surgeon.

Emergent carpal tunnel release is indicated for acute carpal tunnel syndrome, most commonly secondary to distal radius fracture and may be performed in the setting of forearm/hand compartment syndrome along with forearm and/or hand fasciotomies if median nerve compression is suspected. Carpal tunnel release can safely and efficiently be performed concurrent to fracture fixation and/or compartment release, generally with excellent results. This abstract is intended to serve as a succinct review for orthopaedic traumatologists who do not routinely perform carpal tunnel release on an elective bas

Complications and risk factors following volar locking plate fixation of distal radius fractures: A 2-institution retrospective study.

Distal radius fractures are among the most common skeletal injuries and are increasingly treated with volar locking plate (VLP) fixation. Despite its widespread use, postoperative complications remain a significant concern. This study aimed to determine the incidence and spectrum of complications following VLP fixation and to identify independent predictors in a 2-institution cohort. Adult patients who underwent VLP fixation for acute distal radius fractures at 2 tertiary centers between January 2022 and June 2024 were retrospectively reviewed. Inclusion criteria involved operation within 3 we

Examining the role of insulin-like growth factor 1 in the pathophysiology of carpal tunnel syndrome: a Mendelian randomization study.

BACKGROUND: Carpal tunnel syndrome (CTS) is a common and disabling condition of the hand, affecting up to 10% of the population, caused by compression of the median nerve at the wrist. Conventional observational epidemiological studies have demonstrated associations between CTS and elevated plasma insulin-like growth factor 1 (IGF-1), as well as several metabolic factors, such as obesity and type 2 diabetes (T2DM). METHODS: Here, we use two-sample univariable and multivariable Mendelian randomization to investigate causality in the relationships between IGF-1, metabolic factors related to IGF

Hand Pain and Sensory Deficits: Carpal Tunnel Syndrome: 2026 Revision. Using the Evidence to Guide Musculoskeletal Rehabilitation Practice.

SYNOPSIS: Carpal tunnel syndrome is common in women, manual laborers, and during pregnancy; it is often associated with medical conditions such as obesity and diabetes. The musculoskeletal rehabilitation clinician's approach to managing hand pain and sensory deficits associated with carpal tunnel syndrome should include prescribing a neutral-positioned wrist orthosis plus instructing the patient on how to modify activities, adjust ergonomics, and reduce exposure to risk factors. Here, we present for clinicians the most up-to-date information to guide their work in managing hand pain and sensor
