Dynamic Ultrasound Imaging of Extensor Pollicis Brevis Hypertrophy in Proximal Intersection Syndrome: A Case Report and Literature Review.

This case report presents an atypical presentation of proximal intersection syndrome and provides ultrasound-based evidence of extensor pollicis brevis muscle belly hypertrophy with associated edema in a non-athletic population. While occupationally induced PIS has been previously documented, the patient's symptoms in this case were linked to repetitive lifting tasks involving prolonged extension of the first metacarpophalangeal joint with a wide grip. Dynamic ultrasound assessment revealed radial displacement of the second compartment tendons by the extensor pollicis brevis muscle during the

Most Common Injuries in Resistance Training: Mechanisms, Therapeutic Interventions, and Preventive Strategies.

Resistance training is a structured exercise modality aimed at enhancing strength, hypertrophy, and overall fitness. However, it carries an inherent risk of musculoskeletal injuries, particularly in the shoulder, lower back, knee, and wrist/hand. The most common injury mechanisms include muscle strains, tendon tears, tendinitis, and sprains, with acute injuries predominating. Key contributing factors are improper technique, excessive loading, insufficient recovery, and anabolic steroid use. While most injuries are mild and resolve with conservative management, severe cases may require surgical

Ultrasound-guided release of carpal tunnel syndrome - Anterograde technique.

Compression of the median nerve in the carpal tunnel is a frequent pathology with severe functional impact. An ultrasound-guided technique was developed to preserve structures, diminish scar fibrosis and enable fast return to full activity. Its advantages are safety and low additional cost. Ultrasound-guided carpal tunnel release is indicated in moderate and severe carpal tunnel syndrome persisting after conservative treatment. A preoperative ultrasound scan is recommended to evaluate anatomy. Recurrent carpal tunnel syndrome, secondary carpal tunnel syndrome and anatomic variants compromising

Diagnosis and Management of Carpal Tunnel Syndrome During Pregnancy.

Carpal tunnel syndrome (CTS) represents a constellation of symptoms that can occur as a result of compression of the median nerve as it traverses through a constrained space at the level of the wrist. It is the most common compressive mononeuropathy in the human body. Patients frequently present with similar history and physical examination findings, most commonly consisting of numbness of the hand that is worse at nighttime. Although CTS is one of the more common conditions seen by hand, orthopedic, or plastic surgeons, patients often first report symptoms to their primary or obstetric care c

Solid Waste and Associated Environmental Impact of WALANT Carpal Tunnel Release Performed Across Three Clinical Settings.

PURPOSE: Carpal tunnel release (CTR) remains the most common hand and wrist surgery in the United States, with an estimated 500,000 procedures per year. The introduction of wide-awake local anesthetic, no tourniquet (WALANT) technique and use of a minor sterile field has allowed CTR procedures to be performed in a clinic setting. Our study aimed to quantify the solid waste and related emissions associated with WALANT CTR procedures performed in a hospital operating room, ambulatory surgical center, and clinic. METHODS: All WALANT unilateral CTRs performed by three fellowship-trained hand surg

Ganglion Cyst in Zone 2 of Guyon's Canal Causing Ulnar Neuropathy With Claw Hand Deformity: A Case Report and Literature Review.

Guyon's canal, a fibro-osseous anatomical tunnel, sometimes harbors underlying pathologies such as a ganglion cyst, leading to symptoms including dysesthesia, paresthesia, and motor weakness in the ulnar nerve distribution. Surgical excision is often the recommended approach to restore normal hand function. Ulnar nerve palsy caused by mechanical compression can benefit from surgical decompression; however, a timely diagnosis is often difficult. In this report, we describe the successful tumor excision and nerve decompression of a ganglion cyst in Guyon's canal. The observed intraoperative find

Morphological analysis of the median nerve in the carpal tunnel during wrist movements, finger pinching and palm loading.

The aim of this study was to analyze the morphology of the median nerve (MN) in the carpal tunnel during hand motion and palmar load in healthy participants using ultrasound. Twenty healthy participants (10 men and 10 women) were enrolled in the study. Wrist flexion angle is negatively correlated with cross-sectional area (CSA) and perimeter, and positively correlated with circularity, whereas wrist extension angle is negatively correlated with MNCSA and circularity. At 15°, 45°, and 60° wrist flexion and extension, both MNCSA and perimeter were significantly smaller than at neutral (all P < 0
